1.Tuberculous Lesion after Local Steroid Injections

Local steroid injection is applied for the various problems in orthopaedic fields. Its not well known whether local steroid injection can induce or aggrevate the tuberculous lesion of the injection site. Tuberculous lesion of the shoulder and posterior compartment of the lumbar spine is rare, but rather commonly noted in the steroid injection group. Fifteen patients of tuberculous lesion(8 in shoulder and 7 in posterior compartment of the lumbar spine) after local steroid injection were treated surgically at the Department of the Orthopaedic Surgery, College of Medicine, Seoul National University, from Jan. 1980 to Agu. 1986, and following results were obtained; 1. The highest incidence was in the 6th decade and male to female ratio was 2: 13. 2. The average number of the injection was 16 times and the average duration was 6 months. 3. The duration from the last steroid injection to the first sign of the tuberculous lesion was from 2 weeks to 3 months and average 2.4 months. 4. Of the 15 cases, active tuberculous lesion at other sites were associated in 6 patients. 5. All cases were treated by complete curettage. It took 5.2 months for the primary wound healing. 6. Local steroid injection could induce or aggrevate the local tuberculous lesion. Radical extirpation of the lesion is the treatment of choice, but with high recurrence rate.

2.Expression of TRAIL Receptors in Cervical Cancer.

Apoptosis is an intrinsic and fundamental biological process that plays a critical role in the normal development of multicellular organisms and in maintaining tissue homeostasis. Some of the well known regulators of apoptosis are cytokines of the tumor necrosis factor(TNF) ligand family, such as Fas ligand(Fas L) and TNF, which induce apoptosis by activation of their corresponding receptors, Fas and TNFR-1. Recently, a new member of the TNF family known as TRAIL (TNF-related apoptosis-inducing ligand) was identified and shown to induce p53-independent apoptosis in a variety of tumor cell lines but not in normal cells, Four human receptors for TRAIL were also recently identified and designated TRAIL-R1, -R2, -R3, and -R4. The aim of this study is to examine whether TRAIL and TRAIL receptots(-R1, -R2, -R3) are expressed in uterine cervical cancer and whether it is correlated with apoptosis, TRAIL and TRAIL receptors. The subjects were 20 patients who were diagnosed with cervical cancer. Western blotting was performed in 9 cases, immunohistochemical staining for TRAIL and TRAIL receptors(-R1, -R2, -R3) and TUNEL method for detection of apoptosis in 11 cases. There were proteins for TRAIL, TRAIL-R1, -R2, and -R3 in tissues from cervical cancer. All TRAIL receptors were expressed in both normal cervical epithelium and tumor cells, and TRAIL-Rl and -R2 were more strongly expressed in tumor cells than normal epithelium(p<0.05). Apoptosis correlated with expression of TRAIL-Rl and -R2(p<0.05). This study suggests that TRAIL induces apoptosis in cervical cancer through its receptors.

3.The Clinical Application of Radionuclide Angiography(RNA)

Angiography has been playing important roles in diagnosis and treatment in the field of orthopaedic surgery. Conventional angiography is the most reliable and widely used method in diagnosis of peripheral arterial disease. But the clinical use of the conventional angiography has been limited by the risk of possible complications and time-consuming procedures. Radionuclide sngiography is rapidly performed, rapidly interpretable and time-saving procedure for the visualization of arterial tree prior to vascular intervention in the critically ill patient. We have analyzed and compared the 22 cases who had taken radionuclide angiography and conventional angiography simultaneously from November, 1986 to August, 1987 in Department of Orthopaedic Surgery, Hanysng University Hospital. The results were as follow 1. Radionuclide angiography is simple, non-invasive, accurste, reproducible method. It eliminstes the discomfort and morbidity of conventional angiography and can be done on an outpatient basis at a much lower cost. It is useful in the patients who may be allergic to the contrast medium. 2. In cases of arterial occlusion, radionuclide angiogrpahy is also useful in the evsluation of obstruction of major artery and can be substituted for conventionsl angiography. 3. For the purpose of evaluation of success in the cases of vascular reconstruction, radionuclide angiogrphy also demonstrate the vasculsr blood flow without any risk. 4. Although rsdionuclide angiography has limitstion in snatomic detsil, it could be psrtially substituted for conventionsl angiography in the diagnosis of vascular anomalies or tumors.

5.Evaluation of Reliability of Traditional and Computerized Neurobehavioral Tests.

OBJECTIVES: Thls study was conducted to select more reliable neurobehavioral tests that are compatible with the characteristlcs of Korean workers and their health management system. METHODS: To compare the reliability of neurobehavioral tests, test and retest of five computerized and five traditional neurobehavioral tests administered to 85 medical school students and 35 hospital workers. The computerized tests include Simple reaction time, Addition, Symbol digit, Digit span, and Finger tapping speed, while the traditional tests include the trenton visual retention test, Digit symbol, Digit span, Pursuit aiming, and Pegboard. RESULTS: Computerized Addition was found to have the highest test-retest reliability (0.90) of and followed by Finger tapping speed (nondominant hand, 0.89, dominant hand, 0.85), Symbol digit(0.82), and Digit span(0.74). Only two traditional tests, Digit symbol(0.86), and Pursuit aiming(0.72), showed test-retest reliability coefficient greater than 0.70. CONCLUSIONS: These results suggest that the computerized Additions, Symbol digit, Fingel tapping speed, and traditional Digit symbol are more satisfactory for our purposes. These results may allow a reasonable selection of the most appropriate tests for periodical evaluation of central nervous system of workers exposed to neurotoxic substances in Korea.

7.Treatment of Unstable thoracic and Lumbar Spine Fracture with Harrington Segmental Spinal Instrumentation (SSI)

This is a retrospective clinical and roentgenographic study to measure the correction of deformity and rigidity of Harrington SSI in the stabilization of unstable thoracic and lumbar spine fractures. 35 patients with unstable thoracic and lumbar spine fracture were treated with Harrington SSI from Feb. 1985 to Mar. 1987 in SNUH and 29 patients were followed up for more than 1 year, average 15.6 months. At final follow up of these 29 patients, 73.1% of patients gained neurologic improvement. Measurement of correction of anterior, middle and posterior column height, local kyphosis and anteroposterior offset were 29.7%, 5.2%, 31.2%, 12.8°and 5.3mm and loss of correction of these were 6.5%, 0.1%, 5.8%, 3.1°and 1.1 mm.

8.A Case of Bilateral Pneumothorax In a Scoliosis with Marfan's Syndrome

Scoliosis has been reported in 40 to 60 percent of patients with Marfsn's syndrome. The tensile strength of tissues in which the collagen is a major component has been to be reduced in this disease. An abnormality of cross-linking in the α2(I) peptide chain of collagen is the simplest explanation for this observation. We experienced a case of scolisis with Marfan's syndrome in whom Cotrel-Dubousset instrumentation was csrried out successfully and bilateral pneumothorax was observed in 10 days postoperatively. The occurrence of this complication, which has not been previously reported in the literature, suggests that excessive distractional and rotational force during procedure may be harzsrdous in the patient with intrinsic collagen weakness.
