2.Determination of High Density Lipoprotein Cholesterol in Psoriasis Patients.

In orper to measure the levels of high density lipoprotein cholesterol in psoriasis, 60 psoriatic patients and 30 healthy subjects were included in this study. Lopez-Virells methos was applied for measuring the serum level of high density lipoprotein cholesterol. The results were obtained as follows. The level of serurn HDL cholesterol was 58. 39+17. 40 mg/dl in psoratcs and 50.3+0.31 mg/dl in healthy subjects and 50. 43+ 10. 31 mg/dl in healthy subjects. No significant differences were noted between psoriatics & healthy subjects. 2. The level of serum HDL cholesterol was 56. 40+19. 10 mg/dl in male group of psoriatics and 60. 00+15.47 mg/dl in female group of psoriatics and 48.3+9. 50 mg/dl in healthy male group and 52.60 -10.59 mg/dl in healthy female group. No significant differences of serum HDL cholesteol levels were noted in both sexes.3. The mean value of serum HDL cholesterol by age groups of 10, 20, 30, 40, and 50 years old with psoriasis were 50. 70 mg/dl, 61. 97 mg/dl, 57. 44 mg/dl, 49. 11 mg/dl and 70. 36 mg/dl, respectively and those of healthy groups were 57. 25 mg/ dl, 45. 17 rng/dl, 50. 97 rng/dl, 48. 07 mg/dl and 46. 98 mg/dl, respectively.

3.Treatment of Unstable Fracture of the Thoraco

The spinal injuries were classified into bursting fracture, fracture-dislocation, seat-belt injury, compression fracture according to the three column theory by Denis. The bursting fracture and fracture-dislocation required the most careful planning. So, myelography, computerized tomography enabled us to diagnose the spinal fracture including retropulsed bony fragment into the spinal canal. There was much controversy as to appropriate treatment af unstable thoraco-lumbar fractures. The frequent surgical treatment of thoraco-lumbar spinal fractures was still posterior spinal instrumentation including Harrington rod system. Since 1964, the use of anterior spinal instrumentation had been started by Dwyer, Dunn, Kostrik, Slot, and Zielke used anterior spinal instrumentation in unstable thoraco-lumbar spinal fractures, but not popular. This study suggested the experience to accomplish the decompression of the neural elements and stabilization of the spine by using Kandea device in unstable thoraco-lumbar spinal fractures. 12 patients treated with this Kaneda device in unstable thoraco-lumbar spinal fractures were analyzed from Dec. 1988 to May, 1989 at the Department of Orthopaedic Surgery, Ewha Womans University Hospital. We obtained the following conclusions. The results were as follows. 1. The common injury mechanism was falling down in 5 cases, the frequent injury site was 1st lumbar vertebra. 2. According to Denis classification, the bursting fractures were in 8 cases(68% ), the fracture-dislocations were in 4 cases(33%). 3. The average preoperative kyphotic angulation was 24.5 degrees(range 8 to 45) and postoperative angulation was 5 degrees(range 2 to 15), the correction rate was 79.6% and correction degree was average 19.5 degrees. 4. The advantages were the fixation of one level above and below the injury site, sufficient neural decompression, firm spinal stabilization, early mobilization with a brace and elimination of the 2nd posterior procedure. 5. The fixation of vertebral plate was difficult due to the invariable size of vertebral plates, especially, in upper thoracic spinal fracture and children's fracture.

5.Statistical Observation on Inpatient (1961-1965).

A statistical survey was made on 729 inpatients during the period of 4 years and 9 months, 1961-1965, in the Department of Urology, The First Army Hospital, in reference to age distribution, monthly distribution on various diseases of the genitourinary tracts and operative procedures.The tables in the text represent the result of the observation and are summarized as follows: The majority of the patients (85%) was distributed between the ages 21 and 26. Of 729 in patients, there were 270 cases (37%) of genito-urinary tuberculosis, 84 cases (11.5%) of urinary calculus,69 cases of trauma and 60 cases of non-specific urinary tract infection. There were 379 operative procedures including 91 nephrectomies, and 108 epididiymectomies.

6.Cerebello-Pontine Angle Lipoma: Case Report and Review of the Literature .

Intracranial lipomas located in the cerebellopontine angle are extremely rare. These tumors are maldevelopmental lesions which can cause slowly progessive neurological symptoms. The clinical management of these tumours differs significantly from other lesions in this region. A 45 year old man presented with a 3-month history of vertigo and tinnitus in the left ear. In T1-weighted magnetic resonance imaging, the lesion was hyperintense and did not enhance after application of gadolinium. The patient was treated with medical therapy with good response. We present a review of the literature, with particular regard to management. Conservative treatment is recommended and limited surgery is usually indicated if the patients suffer from disabling neurological symptoms and signs e.g., vertigo, nausea, trigeminal neuralgia, facial weakness or facial spasm.

7.Comparative Analysis between Natural Evacuation and Irrigation Technique in Patients with Colostomy.

Sixty-three patients with permanent sigmoid colostomy were surveyed to evaluate their satisfaction and complications with the "irrigation technique" and the "atural evacuation" of the colostomy management (irrigation technique; 32 patients, natural evacuation; 31 patients). All patients had colostomy for at least 12 months without disease recurrence. Each patient was interviewed in addition to standard questionnaire. The irrigation was not associated with any major complication including colonic perforation. The irrigation was used younger age group than the natural evacuation (53+/-10 vs. 62+/-12, P=0.01). The frequency of bowel movement was lower in the irrigation than in the natural evacuation (5.1+/-2.5/wk vs. 10.8+/-9.0/wk, P=0.04). Five patients (16%) of the irrigation experienced spontaneous bowel action but only one patient suffered from it. The time spent for managing irrigation was 59+/-13 minutes. Twenty-three patients (74%) of the natural evacuation suffered from one or moreproblems such as expensive apparatus, leakage, skin irritation or odor. Sixteen patients (52%) of the natural evacuation and 2 patients (6%) of the irrigation were restricted in social activity (P=0.0001). Thirteen patients (42%) of the natural evacuation and 1 patient (3%) of the irrigation were dissatisfied with colostomy management (P=0.002). Thus, the irrigation technique seems to be more effective and satisfactory method for managing colostomy without compromising patient's social activity when it is performed in appropriately selected patients.

8.A Case of Mucosal Melanoma of the Lip Metastasized to the Brain and Lung.

Mucasal melanoma is rare and it is one of the diseases of the worst prgnosis. We report a case of metastatic mucosal melanoma of the lip in 50-yearold male with a peanut sized, round, black colored macule on the lower lip, which was metaetasized to the brain and lung for 8 years. Histologically, typical findings of malignant melanoma showed. Chest P-A showing nodular high densities on the right upper and lower lobes and left lower lobe. Brain C,T. scan revealed round nodular densities on the right and left frontal and parietal lobes. In spite of chemical and radiation therapy, he was dead.

9.A Case of Childhood Systemic Lupus Erythematosus Showing Erythema Multiforme-like Eruption.

We present a case of childhood systemic lupus erythematosus(SLE) with renal disease in a 10-year-old girl with multiple, scattered, erythema multiforme-like eruptions on the face, trunk, forearms and lower legs. Serologic abnormalities included hypocomplementemia, high ANA & anti DNA titers, and LE cell test was positive. The histologic finding of the lesion showed leukocytoclastic vasculitis and direct immunofluorescent test revealed linear deposit of IgM at the dermoepiderrnal junction. Electron microscopy showed glomerular subepithelial depasition of eletron dense material in the kidney.
