1.Treatment of the Chronic Osteomyelitis Involving the Ilium

In the chronic osteomyelitis of the ilium the infection is often so diffuse that removing all sequestra and small cavities are practically impossible. Either the partial or total resection of the ilium has been thought as a standard method of treatment in these situations. According to authors experience, the partial resection is not sufficient for removing such diffuse lesions, and all the patients that underwent the total resection of the ilium have been suffering from the instability with severe Trendelenburg gait because of the loss of the abductor origin. These results are far from a satisfactory one in modern orthopedic field in which many reconstructive surgeries have been done for the disabled patients. Removal of only one cortex followed with the closed suction irrigation technique are applied in three cases. The result of this operation is as good as that removing the total ilium in controlling the infection, and there are no patients who are suffering from the gait disturbance. The inner or outer cortex could be safely and widely removed without affecting the hip joint of the affected side.

5.Subperiosteal Cortical Defect: Case Analysis

Subperiosteal Cortical Defect is a symptomless rarefaction of cortical bone which begins most often on the external surface of the medial and posterior cortical walls of the lower end of the femur in children between the ages of 3 and 6 years. The name of Subperiosteal Cortical Defect was first used by Aegerter who differentiated it from non-osteogenic fibroma. The authors found out 5 positive cases of incidental findings among 62 cases of both knee joint X-ray of which 3 was male and 2 was female. Also we experienced 3 cases which necessiat d a correct diagnosis and differential diagnosis. Because this is a common lesion, it frequently accompanies more serious bone conditions, and because this is self-limited disease, treatment is not necessary. It was emphasized that its nature should be understood by all those dealing with bone disease lest it be overdiagnosed and overtreated.

6.Rhabdomyosarcoma (Report of Five Cases)

Rhabdomyosarcoma which was first described by Weber has generally been considered an uncommon tumor. Recently it was classified as four types such as pleomorphic, embryonal, botryoid and alveolar type by Stout, Stobbe et al, Horn et al. and Riopelle et al. Five cases of rhabdomyosarcoma treated recently in Seoul National University Hospital were repored in this paper. 1. There were three pleomorphic types, one alveolar type and one undifferentiated malignant mesenchymal tumor. 2. Four cases were between 18-23 years old and one was 37 years old. 3. Three were male and two were female. 4. Time to be consulted to us was average 8 months. 5. Three were in lower extremity (two in thigh, one in calf) and two were in upper extremity (one in elbow, the other in forearm). 6. Follow-up study was done for three cases. One was exoired at,three months after hindquarter amputation, the other was expired at two years after supracondylar amputation and no recurrence and metastases were developed one year after local resection in another one case.

7.Treatment of the Spinal Tuberculosis with Severe Kyphosis and Paraplegia

Five cases of the spinal tuberculosis with severe kyphosis and paraplegia were treated with radical surgery at the Department of Orthopedic Surgery, Seoul National University Hospital. All the cases were involved at the thoracic spine and kyphosis were ranged 100 through 150 degrees. Severe fixed chest deformity and impaired pulmonary function were observed. By thoracotomy, aorta was seen to be adherent to the surrounding tissue and this was separated from the verterbral lesion with meticulous procedure. The focus was curetted radically exposing spinal cord which was decompressed with anterior transposition. Then the spine was stabilized with anterior interbody fusion. Postoperative intensive pulmonary care and antituberculous therapy are imperative for the good result. Spinal tuberculosi with severe kyphosis and paraplegia could be benefited with the radical surgery by the experienced surgeon.

8.Osteosarcoma, Arising after Resection and Irradiation of Synovioma: A Case Report

1. A Case of osteosarcoma is reported which arose 8 years after synovima of the knee had been locally excised. The patient is a 51-year-old housewife who had received irradiation therapy upon her knee for 30 days post-operatively. Following irradiation, asymptomatic latent period lasted 8 years when mass appeared on the site, which was biopsied and diagnosed as osteosarcoma one year later. 2. This case generally suffices the McKennas criterias for irradiation-induced osteosarcoma, 3. It seems to be worthwhile to keep an eye upon the prognosis of this possible case of irradiation- induced osteosarcoma, which is largely reported to be very grave.

9.Clinical Study of Functional-Below-Knee Total Contact Cast on Tibial Fractures

Functional-Below-Knee Total Contact Cast characterized by early weight bearing and free motion of affected knee joint during the cast immobilizaton has brought to significant result on trearment of tibial fractures, and is considered to take early bony union by establishment of almost physiological environment on fracture site during immobilization. For recent 2 years, writers have experienced many tibial fractures to treat with Functional Below-Knee Total Contact Cast. In them, clinical data and results of 17 cases which were studied well are reviewed, and method of cast application is illustrated.

10.Clinical Analysis on Ischemic Myosits associated with Carbon Monoxide Poisoning

A clinical review and analysis was carried out on 14 cases of ischemic myositis associated with carbon monoxide poisioning treated at S.N.U.H. during the 3 year period from 1968 to 1971 with following results. l. Among 14 patients, there were 8 males and 6 females. 8 cases were between the 20–30 years of age. None was observed under 20 years old. 2. Average duration of exposure to carbon monoxide gas was 9 hours, mostly between 8–12 hours. 3. Such systemic conditions are suggested as predisposing causes leading to ischemic myositis as pregnancy, postpartum, active pulmonary tuberculosis, obesity with hypertension, and hypnotic addiction. 4. In regard to the local factors, prolonged external pressure the body is subjected to in unconcious state as well as local burn is shown to be responsible for the myositis. And in this respect Korean mode of bed on On-Dol may deserve blame for myositis because of its hardness and much heat. 5. Complications associated with ischemic myositis were 3 cases of ischemic myocarditis, 2 cases of brain syndrome and 3 cases of acute renal failure. In 3 cases of renal failure, all terminated fatally inspite of the intensive care including hemodialysis. 6. Fasciotomy should be performed cautiously, as 4 cases of secandary infection were complicated after fasciotomy on 5 cases.
