Graft vs Host Disease*

We have analyzed 27 cases(38.8%) of scapho-lunate(SL) dissociation which were the most com- mon type of total 72 cases of post-traumatic wrist instabilities since 1985. As a result, SL dissociations associated with wrist fracture were 23 cases, more common than simple dynamic SL dissociation(4 cases). Early diagnosis and treatment influenced on their prognosis, and both wrist PA roentgenograms were useful for the confirmative diagnosis of SL dissociation. In 16 cases with early diagnosis, closed reduction, 2 or 3 K-wires fixation under C-arm image and short arm cast for 8 weeks were effective. But in 11 cases with neglected diagnosis and with wrist fractures showed poor results. Excessive wrist ligamentous reconstruction in chronic cases should be avoided because of osteoporosis and wrist stiffness. Conclusively, clinical wrist symptoms are more important than radiological changes in wrist insta- bilites. So if the symptoms are not so severe, we must consider the patient's age, occupation and their social activities etc. to decide the proper way of treatment.

BACKGROUND: It is well known that atrial fibrillation is common cardiac arrythmia in old age and poses a definitive risk factor of cerebral infarction. Therefore, effective treatment of atrial fibrillation is very important in the prevention of cerebral infarction. However, oral anticoagulant medication for the prevention of embolic ischemic stroke may be dangerous due to cerebral hemorrhage side effects. METHODS: This is a controlled case study designed to identify the risk factors in a large numbers of stroke and atrial fibrillation patients and to assess those patients with atrial fibrillation as a high risk group for cerebral infarction. All patient discharged from Chonnam National University Hospital were identified over a 42 month period who met our case standards of atrial fibrillation and ischemic stroke (n=62), and compared them with the control groups who were discharged with atrial fibrillation without stroke(n=68). We excluded the atrial fibrillation due to valvular heart disease, ischemic heart disease and congestive heart failure during the selection of subjects. RESULTS: Subjects and the controls were characteristically similar with common past medical histories of diabets, smoking, and hyperlipidemia. Sex, familial history, left ventricular hypertrophy in 2D-echocardiogram were also similar in both groups, however differences did exist. Subjects were significantly older than controls(68.9 : 63.9, p<0.001) and more likely to have a history of hypertension(56.5% : 23.5%, p<0.001) and left atrial enlargement(>40mm)(52.6% : 29.0%, p<0.001). Each of these 3 factors were assinged a measure of 1 point as a risk score, ischemic embolic stroke was found in 4 out of 28 patients(14.3%) with a risk score of 0, in 16 out of 39 patients(41.0%) with a risk score of 1, in 29 out of 41 patients(70.7%) with a risk score of 2, in 8 out of 11 patients(72.7%) with a risk score of 3. CONCLUSIONS: Based on the above results, a subject having more than two risk factors should be regarded as a high risk group for cerebral infarction and the long term anticoagulant therapy for the prevention of stroke may also be necessary even though some complications are present.

To compare the specific information of each imaging modality of US (7.5MHz, water path system) and galactography, and to appreciate clinical applicability of US as a primary diagnostic method in breast tumor causing nipple discharge, we performed a comparative study of each method in 15 cases of the ductal tumor of breast. To evaluate the demonstrable smallest anatomic unit of the lactiferous system by US, 30 normal lactating breasts were examined prospectively. The visible smallest lactating unit in the normal lactating breast by US is terminal duct lobular unit(TDLU). US is superior to galactography for detection of the mass less than 0.5cm in size with proximal ductal dilatation, and for evaluation of the multiple ductal lesions, in contrast, inferior to galactography for systemic visualization of involving ducts and for the detection of microcalcification. Sonographic diagnosis is limited in detection of small retromammillary masses, peripheral small masses without ductal dilatation and masses in excessively fatty breast. In conclusion US can be a primary diagnostic tool in suspicious breast tumor causing nipple discharge as a non-invasive method but galactography should be combined in the indicated cases.

Pheochromocytoma originate from chromaffin cells and occur in the adrenal medulla in majority. About 10 per cent of pheochromocytomas can occur in the ectopic or extra-adrenal sites, anywhere along the fetal route of the chromaffin tissues. Pheochromocytoma of the bladder is a rare neoplasm that often occurs with the unique symptom complex of micturitional attacks, such as headache, palpitation, hypertension of sweating due to increased catecholamine secretion during detrusor activity. We report a very rare case of nonfunctioning pheochromocytoma of the bladder in a 67- year-old woman without typical symptoms. Preoperatively elevated urine catecholamines and their metabolites returned to normal after removal of the tumor.

We report a case of congenital giant pigmented nevus with malignant melanoma of brain in a 14-year-old male patient. He had giant pigmented nevus on the back and neck, and multiple satellite lesions over the whole body since birth. One year prior to visit to our hospital, the patient suffered from various neurologie symptoms including headache, nausea, vomiting, seizure and right side motor weakness. Flistologic findings of skin lesions were benign nevocytic nevi. Computed tomogram of brain demonstrated increased densities in the both fronto-parietal leptameninges and brain parenchyme. Histologic findings of brain parenchyme by stereotaxic long needle biopsy showed the infiltration of melanin containing atypical melanocytes. There was no evidence of malignant melanoma at other organs. All of these findings suggested that origin of malignant melanoma of brain parenchyme was leptomeninges rather than skin.

BACKGROUND: It is well known that chronic hepatitis B virus infection can commonly cause membranous nephropathy (MN), membranoproliferative glomerulonephritis type I (MPGN I), and rarely, membranoproliferative glomerulonephritis type III (MPGN III), which has the combined pathologic findings of MN and MPGN I. We report the clinical findings of MPGN III in patients with positive serum hepatitis B virus surface antigen. METHODS: The clinical findings of six MPGN III patients were investigated retrospectively to compare with those of eight MN and nineteen MPGN I patients with positive serum hepatitis B virus antigen. RESULTS: The mean age of MPGN III cases (29.7 years) was younger than that of MN (43.7 years) and MPGN I cases (44.4 years)(p<0.05). Males were predominant in all groups and the male/female ratio was 6:0 in MPGN III, 6:2 in MN, 17:2 in MPGN I group. The cases of nephrotic range proteinuria (4 out of 6 (66.7%) in MPGN III and 6 out of 8 (75%) in MN group) were more frequent than the 5 out of 19 (26.3%) found in the MPGN I group (p<0.05). The cases of HBe antigenemia were 5 out of 6 (83.3%) in MPGN III, 16 out of 19 (84.2%) in MPGN I and 5 out of 8 (62.5%) in the MN group. The cases of increased SGOT level were 5 out of 6 (83.3%) in MPGN III, 14 out of 19 (73.7%) in MPGN I and 4 out of 8 (50%) in the MN group. The cases of increased SGPT level were 5 out of 6 (83.3%) in MPGN III, 10 out of 19 (52.6%) in MPGN I and 2 out of 8 (25%) in the MN group. The cases of decreased serum C3 level were 4 out of 6 (66.7%) in MPGN III, 13 out of 19 (68.4%) in MPGN I and 3 out of 8 (37.5%) in the MN group. CONCLUSION: Patients with MPGN III show similar clinical findings to both the nephrotic proteinuria of MN patients and the decreased serum C3 level and increased SGOT/SGPT of MPGN I patients.
