2.A clinical Study on the Fractures of the Femoral Neck in Children

Most obviuos will be the several anatomic differences between the proximal femur of the child and the adult. Fractures of the femoral neck in the children are extremely rsre and usually are the result of severe trauma in 80% of the cases. This accounts for the high frequency of serious associated injuries. In this injury the vascular anatomy changing with age, continuing growth potential and the effect of deforming forces play critical roles in high complication rate and these make treatment difficult. But still any secure guidelines for the treatment of this injury are not established and this is a field which requires more active research and further progress in future. 26 cases treated at Paik Hospital from Jan. 1979 to June 1988 were analyzed in clinical and radiological aspect and the following results were obtained. 1. The commonest type was Cervicotrochanteric fracture in 13 cases(50%),displaced one 21 cases. 2. In Transepiphyseal, Transcervical and Cervicotrochanteric fracture by early reduction and internal fixation, and in Intertrochanteric fracture by conservative treatment we gained satisfactory results. 3. We experienced 2 cases of coxa vara in nonthreaded pin used cases, 4 cases of premature epiphseal closure and 4 cases of avascular necrosis in threaded pin used ones. 4. Complications were developed in 13 cases (50%). These were 5 cases of coxa vara(19.2%) 4 cases of premature epiphyseal closure (15.4%) and 4 cases of avascul necrosis (15.4%). 5. Avascykar necrosis was developed in displaced fracture of Transcervical or Cervico- trochanteric fracture beyond postoperative 1 year. So. we recommend periodic long follow up for evaluation of avascular necrosis.

3.Determinants of White Coat Effect in Essential Hypertension.

BACKGROUND: White coat effect (WCE) and white coat hypertension (WCH) are relatively prevalent in clinical situation (20 - 57% of WCH in the hypertensive population). The aim of this study was to analyze the determinant factors of WCE. METHODS: A prospective study was carried out in outpatient clinic in a consecutive hypertensives without receiving pharmacologic treatment. Twenty-four hour ambulatory blood pressure monitoring (ABPM) was performed following more than two resting blood pressure (BP) determinations carried out with the interval of 1 - 2 weeks by mercury sphygmomanometer. WCE was calculated for systolic and diastolic BP as the difference between a clinic BP (CBP) and an average daytime ambulatory BP (ABP). WCH was defined as;BP in the clinic > or = 140/90 mmHg with a mean daytime BP by ABPM < or =137/< or =89 mmHg. RESULTS: 1) Two hundred thirty-five patients (mean age 49.7 years, females 74%) were studied. Thirty-seven percent in male and forty-six percent in female patients fulfilled WCH criteria. 2) CBP was significantly correlated to daytime ABP (systolic BP;r=.47, p<.001 and diastolic BP;r=.65, p<.001). 3) The magnitude of WCE was greater in the group of WCH (28.9+14.6/19.3+6.9 mmHg) than ambulatory hypertensives (15.1+15.7/13.0+8.2 mmHg) (p<.001). 4) The magnitude of WCE is significantly correlated with female (r2=.12, p<.001) and the stage of CBP according to JNC-V (r2=.23, p<.001) in systolic BP, the stage (r2=.08, p<.001) in diastolic BP, and the stage (r2=.09, p<.001) and weight (r2=.15, p<.01) in mean BP. CONCLUSIONS: The magnitude of WCE in essential hypertension diagnosed at the clinic is significantly correlated with female, the magnitude of clinic BP, and weight.

9.Confusions and Controversies about Asperger Syndrome.

Asperger syndrome (AS) is a disorder of early childhood characterized by impaired social interaction, communication difficulties, play and imagination deficits and a range of routines and rituals. A review of the literature explores current issues concerning the diagnosis of AS. Despite its increasing popularity as a distinct condition, it seems to be still controversial to what extent AS differs from high-functioning autism. Especially in European perspectives, AS is a variant of autism typically occurring in high-functioning individuals, and not a separate disorder. However, the validity of AS as a different diagnostic entity has been recognized through several lines of evidence in the US. AS and nonverbal learning disabilities are often confused with some authors and practitioners not differentiating at all, others perceiving as different points along a continuum, and still others seeing them as different, albeit overlapping, disorders. The major difference is that AS is a pervasive developmental disorder, with chronically restricted repetitive and stereotyped patterns of behavior, interests, and activities, which are far beyond the narrow range of activities, social awkwardness, and slightly eccentric behaviors that are sometimes found individuals with nonverbal learning disabilities.
