1.Argyrophilic Nucleolar Organizer Region and Expression of Ki-67 in Malignant Lymphoma.

Nucleolar organizer regions (NORs) are loops of DNA which occur in the nucleoli of cells which possess ribosomal RNA genes. The numbers and areas of NORs have been thought to be related to cellular activities. We aimed to investigate the direct relationship between the parameters of AgNORs and cellular proliferative activity using immunohistochemical method with the monoclonal antibody Ki-67, which demonstrates proliferating nuclei. The sequential technique for the simultaneous Ki-67 immunostaining and NOR staining was applied to the same slides of a series of non-Hodgkin's lymphomas (NHL) of the low, intermediate, and high grade type. The number of AgNOR per cell was counted and mean NOR percentage nuclear area (NPNA) was measured by morphometry in both the Ki-67 positive and Ki-67 negative nuclei. The increased immunoreactivity for Ki-67 was found in the high grade than in the low grade non-Hodgkin's lymphoma. This was reflected in the two areas of the palatine tonsils and lymph nodes, the positive cell counts being higher in the follicle center nuclei than in those in the interfollicular compartment. In general the numbers and NPNA of AgNORs were higher in the Ki-67 positive nuclei than in those lacking the antigen in malignant lymphomas as well as in control. The AgNORs numbers and NPNA in controls were the highest in the Ki-67 positive cells in the follicular area and the lowest in the Ki-67 negative cells in the perifollicular area. In malignant lymphomas the numbers and NPNA of AgNORs tended to increase in proportion to their grade in both the Ki-67 positive and negative cells. The numbers of AgNORs of the high grade and the NPNA of the intermediate and the high grade were significantly higher in Ki-67 positive cells than in Ki-67 negative ones. With this double staining method it was now possible to confirm that numbers and NPNA of AgNORs were directly related to the cellular proliferative activity. In maligant lymphoma, among the several parameters of the AgNORs, NPNA in Ki-67 positive cells is proposed to be the most useful marker in determining the prognosis of the patient.

2.Expression of Matrix Metalloproteinase and Tissue Inhibitors of Matrix Metalloproteinase in Malignant Lymphoma.

Matrix metalloproteinases (MMPs) and their inhibitors (tissue inhibitors of matrix metalloproteinases, TIMPs) play essential roles in the remodelling of the extracellular matrix. The balance between MMPs and TIMPs is altered in neoplasia, contributing to the invasive and metastatic properties of malignant tumors. Although MMP and TIMP are believed to play an important role in invasion and metastasis in malignant solid tumors, little is known about their involvement in malignant lymphoma. Immunohistochemical stains for MMP-1, MMP-2, MMP-9, TIMP-1 and TIMP-2 were performed using 56 paraffin blocks of the malignant lymphoma and the results were analyzed by using the tumor grade by Working Formulation. The expression of MMP-9 was noted in 45.5% of low grade, 86.1% of intermediate grade, and 100% of high grade malignant lymphoma. The incidence of MMP-9 expression in tumor cells was positively correlated with the grade of the malignant lymphoma (P<0.025). In nodal lymphomas, the incidence of the MMP-9 expression of the tumor cells was higher in malignant lymphoma with extracapsular invasion than those without extracapsular invasion (P=0.008). The incidence of TIMP-1 expression in the tumor cells and fibroblasts was positively correlated with the grade of the malignant lymphoma (P<0.025). In nodal lymphoma, the incidence of the TIMP-1 expression of the tumor cells was higher in malignant lymphoma with extracapsular invasion than those without extracapsular invasion (P=0.009). The incidences of the MMP-1, MMP-2, and TIMP-2 expression in malignant lymphoma were neither increased in the malignant lymphoma with extracapsular tumor invasion nor correlated with the grade by working formulation. There was no significant difference in the expression rate of MMP-1, MMP-2, MMP-9, TIMP-1, and TIMP-2 in nodal- and extra-nodal malignant lymphoma. The above results suggest that the expressions of MMP-9 and TIMP-1 are positively correlated with the grade and the presence of extranodal tumor invasion in malignant lymphomas.

3.The Relationship between Parent's and Offspring's Personality and Offspring's Psychological Adaptation.

OBJECTIVES: The aim of this study was to examine the relationship between personality of parent and offspring, and offspring's psychological adaptation. METHODS: We examined temperament and character of 65 parents and their offspring (measured using the Junior Temperament and Character Inventory and Temperament and Character Inventory) and offspring's internalizing (anxiety, depression) and externalizing (delinquency, hyperactivity) problems. RESULTS: The internalizing problems of offspring were positively associated with harm avoidance (HA) of offspring and parent, and negatively associated with self-directedness (SD) of offspring and parent. The externalizing problems of offspring were positively associated with novelty seeking of offspring, and were predicted by SD of parent. The interaction between parent's HA and offspring's HA predicted higher levels of anxiety of offspring. CONCLUSION: The association between the offspring's personality and internalizing and externalizing problems can depend on the personality of the parent.

5.The postperative results and survival rate of extrahepatic bile duct cancer.

The extrahepatic bile duct cancers(EBDC) are rare and are found late stage in their course. Because of their location in close proximity to the liver, hepatic artery, and portal vein, they are often unresectable. However, high survival rates after an aggressive surgical resection has been reported lately. This report presents the results of 62 cases with EBDC operated at the Department of Surgery, Pusan National University Hospital, from 1988 to 1997. EBDC usually occurs in older age group. especially between 60~70 years of age, and the mean age was 56.6 years. Sex ratio(male:female) was 5.2:1 with male predominance. Frequent clinical manifestations in order of frequency were jaundice(82.3%), abdominal pain(69.4%), pruritus(35.5%), anorexia(33.9%), fever and chill(32.3%), weight loss(22.6%). The laboratory findings were increased alkaline phosphatase(93.5%), bilirubin(77.4%), SGOT(77.4%), SGPT(69.4%). Elevated CEA(>5ng/dl) was 28.6%. The preoperative diagnostic accuracy was 100.0% in ERCP and PTC, 96.4% in abdominal CT, and 95.3% in ultrasonogram. The most common site of the cancer was lower third of the bile duct(61.3%), followed by upper third(27.4%) and middle third(11.3%). Curative resection was possible in 33 cases(53.2%) and the other 29 cases were received palliative biliary decompression. Postoperative complications were bile leakage(30.6%), wound infection( 25.8%), intraabdominal hematoma(19.4%), cholangitis(17.7%), and UGI bleeding(11.3%). The dead cases were 5 cases(8.0%) of all 62 cases and the causes of death were sepsis in 3 cases(4.8%) and UGI bleeding 2 cases(3.2%). By TNM classification(AJCC, 1992) stage IV(64.5%) was most common and stage II(25.9%) was next in order. Among the 62 adenocarcinoma, 25 cases were poorly-differentiated lesion(40.3%), 19 cases were moderately-differentiated lesion(30.6%), and 18 cases were well-differentiated lesion(29.0%). In conclusion, the survival rates of EBDC were higher low staged, curative resected, well differentiated and lower third located groups than high staged, palliative resected, poorly differentiated and upper third located groups.

6.Miliary Tuberculosis: HRCT Findings in 14 Patientst.

PURPOSE: To evaluate high-resolution CT(HRCT) findings of the miliary tuberculosis and their significancy. MATERIAL AND METHOD: We retrospectively studied clinical records, HRCT and chest radiographs of 14 patients with miliary tuberculosis. RESULTS: On HRCT, nodules were seen in all 14 cases, 10 of them evenly, and 4 were irregularly distributed. The size of each nodule was less than 1 mm in 7 cases, 1 --2ram in 6 cases, and 3mm or more in 1 case. The ground - glass opacity was accompanied in 8 cases, and fine reticular opacity was also noted in 8 cases. Other associated findings were pleural effusion (n=4), hilar and mediastinal lymphadenopathy (n=3), consolidation of the exudative tuberculosis (n=4). CONCLUSION: HRCT findings of miliary tuberculosis are diffusely distributed micronodules of variable size, less than 5mm in diameter. The ground-glass opacity can be combined.

8.Echocardiographic Observation in Patients with Mitral Valve Prolapse.

Twenty-one cases of mitral valve prolapse (MVP) diagnosed by M-mode echocardiograms were studied in regard to the underlying or associated conditions, types of MVP, and dimensions of the cardiac structures and parameters reflecting cardiac performance determined by echocardiograms. In 9 cases, MVP was thought to be idiopathic origin. Associated conditions in the remainder were 5 cases of congenital heart disease, 2 cases of rheumatic myocarditis, 1 case each of aortic valve disease, ischemic heart disease and hyperthyroidism, and 2 cases of undiagnosed cardiac conditions. There were 2 cases of buckling type of MVP and in the remainder hammock type. In these 2 patients with hammock type of MVP, mitral valve echoes were seen in the left atrium during systole in 1 case, and diastolic anterior displacement of posterior mitral leaflet in the other. In 9 cases of idiopathic MVP, cardiac dimensions and parameters reflecting cardiac performance as determined from echocardiograms showed no significant differences from those of 27 normal controls.

9.Percutaneous catheter drainage of abdominal abscess associated with fistulas.

The authors retrospectively reviewed the efficacy of percutaneous catheter drainage in treatment of abdominal abscess associated with fistulas. One hundred sixty four consecutive patients with abdominal abscesses drained percutaneously since 1985 at department of Radiology, Wonkwang University Hospital were studied. Among these, 13 patients were found to have fistulous communications to the biliary duct, the intestinal tract, or the renal calyx. Eleven patients (85%) were successfully treated without surgical intervention while two patients (15%) needed surgical drainage and fistulectomy. The duration of drainage ranged from 7 days to 9 months. Initial drainage of abdominal abscess was performed in the hospital, but 5 of 13 patients were discharged with a tube in place and were followed up as outpatients. In conclusion, percutaneous catheter drainage is an effective and safe means of treating abdominal abscesses with fistulas.

10.Thyroid Disease in Childhood.

Diseases of the thyroid gland are manifested by either increase in gland size, alterations in hormonal secretion, or both. Changes in gland size and weight are associated with toxic or nontoxic goiter, adenomas, thyroiditis, or malignancies. Many reports can be found to study thyroid disease, but there is no report of thyroid disease in childhood. So the present studies have surveyed the record of patient with thyroid disorders seen at Severance h spital from 1969 to 1979. The following results were obtained. 1. Among 76 cases, 56 cases were fmeale and theoratio of male and female thyroid disease was 1:3.5. 2. In classification of thyroid diseases according to function, hyperthyroidism was 20 cases(26.3%), euthyroidism 38 cases(50%), and hypothyroidism 18 cases(23.7%). 3. Disease distribution according to age, congenital hypothyroidim comprises about 75% of thyroid disease below 10 year old, and adenoma 42%, hyperthyoidism 27% of thyroid disease from 10 to 18 years old. 4. The most common sign and symptom was goiter(75%) in hyperthyroidim, palpable mass(100%) in euthyroidism, distended abdomen(83.3%) in hypothyroidism. 5. Treatment of thyoid disease was medical treatment(44 cases), surgical treatment(23 cases), and observation(9 cases).
