1.Electrocardiographic Findings in Korean Students: Electrocardiographic Findings in Cardiomegaly by Chest X-Ray.

Mass screening of cardiomegaly by chest X-ray in 144,021 (male 55,491, female 88,530) students of primary, middle and high school (6~17 years of age) in Seoul was performed and electrocardiograms of 217 cases of cardiomegaly were studied. The results were as follows; 1) Cardiomegaly (cardiothoracic ratio over 0.5) was seen in 0.19% of each sex and it was higher in middle and high school ages (12~17 years) than primary school ages (6~11 years). 2) Incidences of abnormal electrocardiogram in cardiomegaly were 59.5% in male and 54.7% of female students. Major abnormal electrocardiograms in cardiomegaly were right ventricular hypertrophy (24.8%), left ventricular hypertrophy (11.6%), biventricular hypertrophy(4.8%), complete right bundle branch block (7.7%), incomplete right bundle branch block (8.7%), first degree A-V block (5.8%) and premature beat (4.3%). 3) Left ventficular hypertrophy was seen most frequently in high school ages (15~17 years) and decreased with decreasing age. Right ventricular hypertropy was seen most frequently in primary school ages (9~11 years) and decreased with increasing age. Biventricular hypertrophy was seen most frequently in primary school ages (6~8 years) and decreased with increasing age. 4) Complete and incomplete right vundle branch block were seen commonly in 12~17 years of age and first degree A-V block in 9~11 years of age.

3.The Significance of the Expression of p53, E-cadherin, nm23, CD44, and Tumor Angiogenesis in Colorectal Adenocarcinoma.

Many oncogenes and tumor supressor genes have been identified and studied in colorectal carcinoma. Among them, p53 is a tumor supressor gene and its mutation is frequently noted in human tumors. E-cadherin is a cell adhesion molecule and associated with tumor differentiation. CD44 is a cell surface glycoprotein that plays a role in cell migration and metastasis. nm23 is a gene known to lower metastatic potential of tumors and has been proposed to be a metastasis supressor gene. Tumor angiogenesis is required for the expansion of the primary tumor and metastasis and its degree is related to the potential of malignancy. We studied the expression of p53, E-cadherin, nm23, CD44 and tumor angiogenesis in 36 cases of colorectal adenocarcinomas. They were compared with previously known prognostic factors such as the stage, tumor size, depth of invasion, differentiation, presence of lymphatic or venous invasion, the lymph node and distant metastasis. The results were as follows. 1) The expression of p53 was not significantly associated with any prognostic factors. 2) The expression of E-cadherin was significantly associated with tumor differentiation. In the well differentiated adenocarcinomas, its expression was higher than in the poorly differentiated adenocarcinoma. 3) The expression of nm23 was also significantly associated with tumor differentiation. In carcinoma with lymph node metastasis, the expression of nm23 was reduced, but statistically it was not significant. 4) The expression of CD44 was higher in tumors with lymph node metastasis than in tumors without lymph node metastasis, but it was not statistically significant. 5) The degree of microvessel density was significantly associated with lymphatic invasion. According to the above results, the expression of E-cadherin and nm23 are related to the differentiation of the tumor and tumor angiogenesis is related to the lymphatic invasion of the colorectal adenocarcinoma.

4.Statistical Analysis of Twin Pregnancy.

Twin pregnancies represent a high risk obstetric population with perinatal morbidity and mortality much greater than that of singleton pregnancies. A retrospective dinical evaluation was attempted to analyse 211 cases of twin pregnancies in Chunchon Sacred Heart Hospital of Hallym University from Jan. l. 1988 to Dec. 31. 1997. The results were as follows. 1) The incidence of twin pregnancy was 1 in 93.9 deliveries. 2) The age of mother ranged from 19 to 42 years and predominant age group was 26-30 which covers 43.1% of all. 3) Nullipara was 61.1% and multipara was 38.9%. 4) The predominant gestational age group at delivery was 36-37 weeks, 33.2%. 5) In presentation, both cephalic combination was 46.4%, cephalic-breech was 26.1%. 6) The birth weight was less than 2500gm in 64.9% of the 1st babies and 70.6% of the 2nd babies, respectively. 7) The rate of cesarean section was 89.0%. The indications of cesarean section were malpresention (48.9%), elective cesarean section(38.8%), and toxemia(9.0%). 8) In 23 cases of vaginal delivery, the average duration of labor was 7.0 hours and 43.5% of 2nd baby was delivered within 5 min after 1st baby delivery, 9) The most frequent maternal complications during pregnancy were anemia(36.0%), preterm labor(34.6%) and preeclamsia(17.5%). 10) The perinatal mortality rate of the first and second twins was 10.4% and 12.3%, respectively. The main causes of perinatal death were prematurity(52.1%) and stillborn(22.9%). 11) Both female was 43.6%, both male was 39.3%, and opposite sex was 17.1% in infant.

6.Effects of Dementia Knowledge, Self-Efficacy and Depression on Dementia Preventive Behavior in Elderly Couples: Dyadic Data Analysis.

PURPOSE: This study was done to examine actor and partner effect of dementia knowledge, self-efficacy and depression on dementia preventive behavior in elderly couples. METHODS: Participants were 115 couples aged 60 years or over who met eligibility criteria. All measures were self-administered. Data were analyzed using SPSS 18.0 and AMOS 18.0 program. RESULTS: Dementia knowledge in elderly couples showed actor and partner effect on dementia preventive behavior. Self-efficacy in the wife did not have direct effects on dementia preventive behavior, but showed indirect effects through dementia knowledge. Self-efficacy in the husband showed direct effects on dementia preventive behavior and indirect effects through dementia knowledge. Wife's depression had direct actor effect on dementia preventive behavior and indirect effect through self-efficacy and dementia knowledge. Husband's depression did not have direct actor effect on dementia preventive behavior, but indirect effect through self-efficacy and dementia knowledge. Effect size of wives' dementia knowledge, self-efficacy and depression on dementia preventive behavior was larger than that of husbands'. Dementia preventive behavior, dementia knowledge and depression had a mutual effect. CONCLUSION: Results indicate that to promote dementia preventive activity in elderly couples, programs should be conducted for both of the couple, but focused differently for wife and husband.

7.A case of type IV hyperlipoproteinemia with xanthoma tuberosum and xanthoma striatum palmare.

We experienced a case of a 30 year-old male with hyperlipoproteinemia associated with xanthoma tuberosum and xanthoma striatum palmare. The laboratory findings showed increased serum cholesterol and triglyceride on lipid profile, and widening of prep band on elec1rophoresis of lipoprotein. The biopsy specimens of orange yellow colored nodules on the elbow and yellow streaks on the palrnar creases showed characteristic findings of xanthoma. He had been treated with gempibrozil(lopid) for 1 month.

8.A case of nevus sebaceous associated with sebaceous epithelioma and basal cell epithelioma.

We report a case of nevus sebaceus associated with sebaceous epithelioma and basal cell epithelioma in a 48-year-old male patient. A 2.6 x 2.0cm orange-yellow erosive tumor and 1.5 x 1.0cm, l.0 x 0.8cm dark blue-yellow varigated nodules were intimately associated with a 4 x 3cm sized yellowish verrucous plaque on the parieto-occipital area of the scalp. Biopsy revealed sebaccous epithelioma, basal cell epithelioma and nevus sebaceus, respectively.

10.A Case of Primary Choriocarcinoma of The Fallopian Tube.

Choriocarcinoma is a relatively rare malignancy of which characteristic is rapid metastasis to the other organs. It is related to the previous gestation or originated from the teratoma. Choriocarcinoma is mostly originated from the intrauterine chorionic villi, but it is rarely originated from the utreine cervix, fallopian tube, ovary, vagina and pelvic cavity. Primary choriocarcinoma of the fallopian tube is exceedingly rare and it is originated from ectopic tubal pregnancy, tubal migration from the intrauterine pregnancy or intratubal teratoma. 9 Symptoms and signs of the choriocarcinoma originated from the ectopic pregnancy are abdominal pain, vaginal bleeding, palpable adnexal mass, positive pregnancy test and amenorrhea. Thus it is difficult to distinguish choriocarcinoma from ectopic pregnancy on the basis of symptoms before the microscopic diagnosis presented.20 Effective treatment of choriocarcinoma is chemotherapy. Additional operation is possible. B-HCG is a useful measure for the follow up. We experienced a 36-year-old multigravida Korean woman who was diagnosed as the rupture of ectopic pregnancy after left salpingectomy in our hospital and then confirmed primary choriocarcinoma of the fallopian tube without metastasis on microscopic finding. Postoperative chemotherapy was performed with methotrexate. The follow up of disease is still on going at two month intervals and she has remained healthy, We report this case with review of literatures.
