1.A Case of Acne Conglobata.

A case of acne conglobata associated with hidradenitis suppurativa, dissecting cellulitis and disseminated superficial actinic porokeratosis in 44 year-old man is presented with brief review of literature. The patient had acne vulgaris for a few years during his highschool days, about 5 years prior to the development of such a typical lesions of acne conglobata as multiple comedones, papules, pustules, abscesses, cysts and draining sinnses and subsquent hypertrophic scars and keloids. We are ps,rticularly interested in association of hidradenitis suppurativa and dissecting cellulitis with ance conglobata that comprise follicular occlusion triad in tbis teint. The pstient was treztei with systemic antibiotics and corticosteroids injections and surgical interventions with considerable improvement.

2.Change of Dynamic Gastrocnemius Length after the Block of Spastic Gastrocnemius Muscle in Cerebral Palsy.

OBJECTIVE: To evaluate the clinical utility of the dynamic gastrocnemius length, calculated with gait analysis after phenol or botulinum toxin block in spastic cerebral palsy. METHOD: Gastrocnemius muscles were injected with phenol or botulinum toxin. Kinematic gait parameters including dynamic gastrocnemius length were surveyed with 3-dimensional gait analysis system before and after the procedure. RESULTS: The dynamic gastrocnemius lengths improved significantly after block of calf muscles, except 3 cases which showed severe genu recurvatum. The vertical displacement of the center of gravities and the maximal ankle dorsiflexion angles after the block were not significantly different from those before the block. CONCLUSION: Dynamic gastrocnemius length calculated with gait analysis can be used as a tool to determine the efficacy of spastic calf muscle block, in the absence of severe genu recurvatum. In case of associated severe genu recurvatum, other parameters may be substituted.

3.The Effect of Smoking on Penile Hemodynamics in Erectile Dysfunction.

PURPOSE: Smoking is a known risk factor for lung cancer, cardiovascular diseases, and atherosclerosis. The link between smoking and erectile dysfunction(ED) has been supported from in vitro studies, animal studies, and epidemiological evidence. We evaluated the effect of smoking on penile hemodynamics in ED. MATERIALS AND METHODS: Patients aged 40~80 years who visited our urological institute for ED were enrolled. We performed a standard ED evaluation that included medical history, physical examination, and measurements of serum glucose, testosterone, and prolactin. We excluded ED patients with abnormal androgen profiles and patients with ED risk factors other than smoking. A total of 108 ED patients entered the study, including 51 current smokers, 24 non-smokers, and 33 ex-smokers. All patients then underwent extensive evaluation, including nocturnal penile tumescence and rigidity(NPTR) monitoring with Rigiscan(R), and pharmacopenile Doppler ultrasonography for peak systolic velocity(PSV), and end diastolic velocity(EDV). RESULTS: The average ages of the current smokers, non-smokers, and ex-smokers were 55.7+/-4.6, 53.7+/-3.9, and 56.5+/-4.3 years, respectively(p=0.07). Of current smokers, 78% had abnormal NPTR testing, compared with 64% of non-smokers and 67% of ex-smokers(p=0.04; current vs non-smokers, p=0.04; current vs ex-smokers). The average PSV for current smokers, non-smokers, and ex-smokers was 26.8+/-3.2, 31.2+/-4.5, and 29.7+/-4.4 cm/s, respectively(p=0.06; current vs non-smokers, p=0.07; current vs ex-smokers). The average EDV for current smokers, non-smokers, and ex-smokers was 5.9+/-2.7, 4.4+/-1.6, and 4.2+/-1.5 cm/s, respectively(p=0.04; current vs non-smokers, p=0.03; current vs ex-smokers). In the currently-smoking patients, there was no significant correlation between NPTR, PSV, or EDV and the amount or duration of smoking. CONCLUSIONS: We concluded that dysfunction of penile veno-occlusive mechanism plays a substantial role in the development of ED in smokers.

4.Effect of Chronic Obstructive Pulmonary Disease on Swallowing Function in Stroke Patients.

OBJECTIVE: To investigate the prevalence of chronic obstructive pulmonary disease (COPD) in stroke patients, and to assess the difference in swallowing function between stroke patients with COPD (COPD group) and stroke patients without COPD (control group). METHODS: The subjects included 103 stroke patients. They underwent the pulmonary function test and were assigned to either the COPD group or the control group. Their penetration-aspiration scale (PAS) scores and functional dysphagia scale scores were compared by performing a videofluoroscopic swallowing study. The intergroup differences in lip closure, bolus formation, mastication, and the oral transit time, laryngeal elevation, cricopharyngeal dysfunction, oronasal regurgitation, residue in pyriform sinus and vallecula, pharyngeal transit time, aspiration, and esophageal relaxation were also compared. RESULTS: Thirty patients were diagnosed with COPD. The COPD group showed statistically higher PAS scores (4.67+/-2.15) compared to the control group (2.89+/-1.71). Moreover, aspiration occurred more frequently in the COPD group with statistical significance (p<0.05). The COPD group also showed higher occurrence of cricopharyngeal dysfunction, albeit without statistical significance. CONCLUSION: This study shows that a considerable number of stroke patients had COPD, and stroke patients with COPD had higher risk of aspiration than stroke patients without COPD.

5.Subvalvular Aortic Stenosis Developed after Patch Closure of VSD in A Child: Case Report.

Subvalvular aortic stenosis developed after patch closure of perimembranous VSD is rarely reported. A 18-month-old, 8 kg child with this complication after VSD closure 8 months ago in other hospital has been treated medically and was admitted to this hospital because of severe cardiomegaly and sign of heart failure. Cardiac catheterization revealed 55 mmHg of pressure gradient between aorta and LV cavity. We report one successful redo case of surgically relieved subvalvular aortic stenosis in a child after patch closure of perimembranous VSD.

6.Three-dimensional symmetry and parallelism of the skeletal and soft-tissue poria in patients with facial asymmetry.

OBJECTIVE: The purpose of this study was to examine the symmetry and parallelism of the skeletal and soft-tissue poria by three-dimensional (3D) computed tomographic (CT) imaging. METHODS: The locations of the bilateral skeletal and soft-tissue poria in 29 patients with facial asymmetry (asymmetric group) and 29 patients without facial asymmetry (symmetric group) were measured in 3D reconstructed models of CT images by using a 3D coordinate system. The mean intergroup differences in the anteroposterior and vertical angular deviations of the poria and their anteroposterior and vertical parallelism were statistically analyzed. RESULTS: The symmetric and asymmetric groups showed significant anteroposterior angular differences in both the skeletal and the soft-tissue poria (p = 0.007 and 0.037, respectively; Mann-Whitney U-test). No significant differences in the anteroposterior and vertical parallelism of the poria were noted (p < or = 0.05; Wilcoxon signed-rank test). CONCLUSIONS: In general, the skeletal poria are parallel to the soft-tissue poria. However, patients with facial asymmetry tend to have asymmetric poria.

7.Laparoscopic Nephrectomy for Ectopic Dysplastic Kidney.

A single dysplastic ectopic kidney associated with ectopic ureter or ureterocele defies detection even with repeated examinations using conventional imaging modalities and cystovaginoscopy under anesthesia, thus leading to significant delay in diagnosis and appropriate treatment. Laparoscopy can be a useful modality for the diagnosis, and also the treatment, of small ectopic kidney not found with conventional diagnostic method. We report three cases of transperitoneal laparoscopic nephrectomy for ectopic dysplastic kidney.

8.Thoracoscopic Sympathectomy for Facial Hyperhidrosis: Three Cases Report.

Facial hyperhidrosis has a symptom of excessive sweating on the face with or without underlying disease. It can be surgically treated by video-assisted thoracic surgery (VATS). We encountered three cases of facial hyperhidrosis which we treated by VATS, which was performed by resection of the lower third of stellate ganglion and T2-T3 sympathetic ganglia with chains. Postoperative symptom was improved in all cases. There were no postoperative complications such as Horner's syndrome or postsympathectomy neuralgia.

9.Delayed Spontaneous Rupture of an Orthotopic Ileal Neobladder.

Orthotopic bladder replacement using detubularized ileum has become a popular form of continent urinary diversion after performing radical cystectomy for treating invasive bladder cancer. Delayed spontaneous rupture of an orthotopic ileal bladder has rarely been reported in the English literature and it has never been reported in Korea. We describe here a case of delayed spontaneous rupture of an ileal orthotopic neobladder secondary to overdistension, and this was successfully managed nonoperatively.

10.Retroperitoneal Seminoma with the 'Burned out' Phenomenon in the Testis.

The rare 'burned out' phenomenon in germ cell tumors is known as the presence of an extragonadal germ cell tumor with a spontaneously regressed testicular tumor found in common metastatic sites, including the retroperitoneal, mediastinal, supraclavicular, cervical, and axillary lymph nodes; lung; and liver. We report a patient who presented with a retroperitoneal extragonadal germ cell tumor with a spontaneously regressed testicular tumor.
