3.Early Developed Candida Endophthalmitis Following Intracapsular Cataract Extraction.

Candida albicans endophthalmitis following cataract surgery is fortunately rare complication, but once the infection is established, its course is unrelenting and the prognosis for a good visual result is usually very guarded. The authors experienced a case of culture-proved Candida albicans endophthalmitis following intracapsular cataract extraction in a 74-year old man. Endophthalmitis was resolved after treatment with intravitreal injection of amphotericin B, intravenous amphotericin B, oral Ketoconazole and 2% of topical Ketoconazole.

4.Chronic Hydroxyurea-induced Dermatomyositis-like Eruption Showing Epidermal Dysmaturation.

Hydroxyurea is an effective treatment for a variety of myeloproliferative disodrers. A distinct cutaneous reaction to long-term administration of hydroxyurea has been characterized and designated hydroxyurea dermopathy. Epidermal dysmaturation refers to histologic changes that may be observed in the epidermis after any significant cytoreductive therapy. We report a patient with hydroxyurea-induced dermatomyositis-like eruption showing epidermal dysmaturation who developed an erythematous scaly patches on the dorsal aspects of the hands while on long-term administration of hydroxyurea for chronic myelogenous leukemia.

5.Osteochondrodysplasia Pathologic study of 29 autopsy cases.

Osteochondrodysplasia is a heterogeneous group of disorders appearing short limbed dwarfism. Because many of these entities are lethal and hereditary, an accurate diagnosis is mandatory. The purpose of this study is to define the clinicopathologic features and radiologic findings of osteochondrodysplasia. We reviewed 29 autopsy cases of congenital short limbed dwarfism, consisting of thanatophoric dysplasia (TD) (12 cases), osteogenesis imperfecta (OI) (12 cases), asphyxiating thoracic dysplasia (ATD) (3 cases), short-rib-polydactyly syndrome (SRPS) (1 case) and hypochondrogenesis (1 case). The gestational age ranged from 16 to 41 weeks. Of 6 fetuses that were born alive, 3 were ATD, 2 were TD and 1 was hypochondrogenesis. TD was frequently complicated by hydramnios. Of 8 cases studied chromosomally, only 1 showed chromosomal abnormality -46XY, inv 9. Intrauterine growth retardation was frequently associated with OI. Pulmonary hypoplasia was present in 23 cases (79%), including all cases of ATD, SRPS and hypochondrogenesis, 11 in TD and 7 in OI. Other associated anomalies were present in 17 cases (59%).

6.Comparison of Chlamydia Trachomatis Antigen Detection Rate between Endourethral Swab and Urine Examination.

Chlamydia trachomatis is known as a major causative microorganism in non-gonococcal urethritis(NGU) in men. We examined two kinds of specimens simultaneously, endourethral swab and urine, in each patient to determine the reliability of these two different specimens for the detection of the Chlamydia trachomatis antigen in male NGU patients using ELISA method. Tota1 273 patients entered this study were divided into two groups according to sampling order of urine and endourethral swab. In group A(141 patients), we took endourethral swab first, then first portion of voided urine was caught. In group B(132 patients), endourethral swab was performed after urine sampling. Twenty five out of 273 patients(9.2%) showed Chlamydia trachomatis antigen positive in endourethral swab sample and 1 out of 273 patients(0.4%) was positive in urine sample. There was no significant difference of antigen positive rate of endourethral swab examination between group A and group B. Our data suggest that examination of endourethral swab is more reliable method for the detection of the Chlamydia trachomatis antigen than urine examination in male NGU patient.

7.A Case of Mucosal Melanoma of the Lip Metastasized to the Brain and Lung.

Mucasal melanoma is rare and it is one of the diseases of the worst prgnosis. We report a case of metastatic mucosal melanoma of the lip in 50-yearold male with a peanut sized, round, black colored macule on the lower lip, which was metaetasized to the brain and lung for 8 years. Histologically, typical findings of malignant melanoma showed. Chest P-A showing nodular high densities on the right upper and lower lobes and left lower lobe. Brain C,T. scan revealed round nodular densities on the right and left frontal and parietal lobes. In spite of chemical and radiation therapy, he was dead.

8.Percutaneous Antegrade Pyelography Guided by Ultrasound

The authors performed percutaneous antegrade pyelography guided by ultrasound on 33 patients, from J une 1982 to October 1984, at the department of radiology, Busan National University Hospital. The results obtained were as follows: 1. Of the 31 cases,17 cases (5 1.5%) were female and 16 cases (48.5 %)were male,and age distribution was nearly even, but most prevalent age group was third decade. 2. Comparing intravenous pyelographic findings with ultrasonographic findings, pyelographically non. visualized kidney 15 cases (45 .5%) were hydronephrosis 12 cases, multiple cysts 2 cases, and intrarenal cystic mass 1 case, ultrasonographically. Pyelographically hydronephrosis 9 cases (27.3%) were all hydronephrosis, ultrasonographically. Intrarenal mass 5 cases (15.2%) were all intrarenal cystic mass, NVK with air in kidney 1 case (3.0%) was air in perirenal space, partial NVK 1 case (3.0%) was per. irenal fluid , suprarenal mass 1 case (3 .0%) was suprarenal intrarenal and huge perirenal cystic masses, ultraso nograp h ically. 3. On technical reliability of antegrade pyelography under ultrasound gUide, 31 cases (93 .9%) could be done fluid aspiration and visualization, and 2 cases (6.1 %) could be only done fluid aspiration but failed visualization . 31 successful cases were visualization of collecting systems 23 cases, visualization of cyst 6 cases, i!nd visualization of perirenal space 2 cases. 2 partial successful cases were perirenal injection 1 case and parenchymal injection 1 case. 4. On fluid aspiration, 22 cases (66.7%) were clear, but 11 cases (33.3%) were not clear, which were pus 7 cases, turbid urine 2 cases, bloody urine 1 case, and bloody pus and air 1 case. 5. Comparing ultrasonographic findings with antegrade pyelographic findings, ultrasonographically hydronephrosis 21 cases revealed obstruction in 16 cases, antegrade pyelographically, which were consisted of ureteral stricture 14 cases, ureteral stone 1 case, and ureteral mass 1 case, non-obstruction in 4 cases, which were consisted of pyonephrosis 2 cases, posterior urethral valve 1 case, and megaureter 1 case, and other 1 case was visualization failure. Ultrasonographically intrarenal cystic mass 6 cases were simple renal cyst 4 cases, and infected renal cyst 2 cases, antegrade pyelographically. Multiple cysts 2 cases were lobulated huge renal cyst 1 case, and visualization failure 1 case, which was multi.cystic kidney. Air in perirenal space 1 case was emphysematous pyelone. phritis, suprarenal cystic mass 1 case was complete duplication with ectopic ureteral orifice, perirenal fluid 1 case due to kidney fracture was perirenal fluid , and intrarenal and perirenal cystic mass was per irenal abscess, antegrade pyelographically. 6. On ana lysis of anteg rade pyelography result as next diagnostic step of ultrasound, 31 successful cases were 27 conclusive diagnostic cases (87.1%), and 4 heplful diagnostic cases (12.9%) with percutaneous antegrade pyelography guided by ultrasound . 7. Antegrade pyelography provides significant diagnostic information on the nature of the obstructive lesion and can be performed as an adjunct to retrograde study or as an alterative to a pyelogram. 8. Ultrasonographic examination could be performed easiJy in diagnosis of renal and perirenal diseases as non.invasive method without risk of radiation hazard , and was not influenced by renal function. 9. Ultrasound is considered a most advantageous aid to the performance of antegrade pyelography and has yie lded valuab le diagnostic information in patients with obstructive hydronephrosis.

9.ProMACE-CytaBOM Combination Chemotherapy in Stage II/III/IV Intermediate- and High-Grade Non-Hodgkin's Lymphoma.

BACKGROUND: CHOP regimen is four-drug, first-generation combination chemotherapy that has been the standard treatment for patients with aggressive non-Hodgkin's lymphoma for the last 20 years. The second- and third-generation regimens have been developed in an attempt to improve the treatment outcome, but recent randomized studies failed to demonstrate an advantage of these regimens compare to the first-generation CHOP regimen. So, we performed clinical study of 8-drug combination chemotherapy, ProMACE-CytaBOM regimen for patients with non-Hodgkin's lymphoma to evaluate the response rate, overall survival, and toxicity. METHODS: Between November 1992 and Feb. 1997, previously untreated stage II/III/IV intermediate- and high-grade non-Hodgkin's lymphoma patients were treated with a ProMACE- CytaBOM regimen including cyclophosphamide 650 mg/m2 IV on day 1, adriamycin 25 mg/m2 IV on day 1, etoposide 120 mg/m2 IV on day 1, cytosine arabinoside 300 mg/m2 IV on day 8, bleomycin 5 mg/m2 IV on day 8, vincristine 1.4 mg/m2 IV on day 8, methotrexate 120 mg/m2 IV on day 8, and prednisone 60 mg/m2 PO on day 1 to day 14 with 3 weeks interval. RESULTS: Twenty eight patients (87.5%) were evaluable. Patient characteristics include: Median age 57.5 years (17-75) and 15 patients were 60 years or old; clinical stage II in 12 patients (37.5%), stage III in 7 patients (21.9%), and stage IV in 13 patients (40.6%). Objective response were 22 CR, 4 PR, 2 PD with 92.8% response rate. The complete response rate was associated with the International Prognostic Index (low risk 100%; low intermediate risk 70%; high and high intermediate risk 50%, P<0.05), but age did not influence the response rate. The median survival time was 23.6 months, and 1-year, 3-year, 5-year total survival and relapse-free survival rate were 75.0% 52.0% 34.7%, 84.4% 84.4% 54.3%, respectively. International Prognostic Index was correlated well with survival time (1-year 3-year 5-year survival : Low risk 91.7% 91.7% 91.7% vs low intermediate risk 80.0% 41.1% 27.4% vs high and high intermediate 33.3% 16.6% 0%, P< 0.005), however age was negatively associated with survival time (1-year 3-year 5-year survival : Below the 60 years 81.3% 75.0% 37.5% vs age greater than 60 years 66.7% 34.3% 22.9%, P<0.05). Grade 3/4 toxicities were anemia in 28.6%, neutropenia in 64.3%, thrombocytopenia in 14.3%, vomiting in 35.7%, oral mucositis in 14.3%, alopecia in 25.0%, increased AST in 3.6% and generation CHOP regimen, with higher treatment-related death due to infection. Therefore, ProMACE-CytaBOM combination chemotherapy is not routinely recommended to treat the patients with non-Hodgkin's lymphoma. CONCLUSION: These data indicate that IDA+BHAC induction chemotherapy seems to be comparable to other combinations for induction chemotherapy in AML. More longer follow-up period is needed for assessing the impact of BHAC-containing regimen on the outcome of autologous and allogeneic hematopoietic stem cell transplantation.

10.Fracture of the Femoral Component in Total Knee Arthroplasty Using Whiteside Ortholoc Prosthesis with Cement - Metallurgical Study by SEM and EDS.

The present study reports two cases of femoral component fracture using Whiteside Ortholoc prosthesis(Dow Corning Wright, Arlington, Tennessee) with cement. The causes of the fracture were investigated by radiological findings, prosthetic design and metallurgical study using scanning electron microscope(SEM) and energy dispersive spectrometer(EDS). Respectively, the findings from this study are summarized as follows. In X-ray study, one case had valgus 3 mechanical axis(normal: valgus 7 axis) while the other case had 10 tibial anterior slope(normal: 10 posterior slope). The medial side of femoral component fractured in both cases. The fracture site was in the junction between the posterior bevel and distal surface of the medial side of femoral component. The analysis of the prosthetic design showed that the Whiteside Ortholoc femoral stem was thinner, and shorter at the site of posterior bevel surface of the femoral stem than other TKA(Total Knee Arthroplasty) system. The metallurgical study by SEM showed that the crack was developed initially near the inner surface of fractured femoral component and was extended to the outer surface. Carbon particles were detected in the fractured femoral component by EDS. The findings imply that operating technique is very important especially during bone cutting and soft tissue balance in TKA. It is also proposed from this study that a minor design modification such as a thicker and longer Whiteside Ortholoc femoral component would be better and that the cast-iron ware of TKA system has to he produced by careful metallurgical process, that precludes the incorporation of any foreign material such as carbon particles.
