The 6 patients with neglected rupture of the Achilles tendon were treated by end to end anastomosis using the V-Y tendinous flap originated by Abraham in 1975. A tendinous flap was developed over the tendon by making an inverted V incision, which was then repaired in a Y fashion. The postoperative results were satisfactory with no functional disability of the ankle and with recovery of muscle strength for periods of 8 months to 15 months, but slight limited dorsiflexion of the ankle in two cases were noticed. The advantages of this method are: 1. Optimum functional recovery. 2. Recovery of muscle strength and tendon contour. 3. Prevention of late stretching or possible rupture of the healed tendon at the rupture site.

Nine strains (6%) were confirmed as resistant (MIC=20ug/ml) to methicillin (MT) among 150 isolates of staphylococcus aureus. These MT-resistant staphylococcus aureus (MRSA) strains were also turned out to be more less resistant to other antibiotics tested, such as penicillin, chloramphenicol, tetracycline, and gentamicin except for cephaloridine. It was noteworthy, however, that strain MRSA 5 showed a relatively susceptible attitude toward most of the antibiotics tested, while being extremely resistant to MT only. Salt (NaC1) containing (5-3%) media did not have any significant effect on the degree of MT-resistance of MRSA strains. There was, however, such a tendency as that by the addition of salt the degree of MT-resistance was lowered in the highly MT-resistant strains while enhanced in MT-susceptible ones. It was confirmed that penicillinase did not have any effect on the MT-resistance while most of the penicillin- resistant strains produced the enzyme.

Journal of the Korean Radiological Society 1982;18(1):68-73
Pneumocystis carinii pneumonia is caused by Pneumocystis carinii. It usually occurs in premature or debilitated infants. Recently sporadic cases of human disease in patients who have been on long term steroid therapy, cytotoxic drug therapy, immunosuppressive drug were significantly increased. We recently experienced 35 cases of Pneumocystis carinii pneumonia in infants of an institution for foreign adoption in three epidemic period of Feb.1979, Mar. 1980, and Jan. 1980. The clinical review of 35 cases was made. Patients' age was between 1 to 4 months. Twenty-one cases (60%) occurred in 2-month-old infants. Many patients were included in poor weight gain and development. The common symptoms were tachypnea, cyanosis, restlessness, cough, diarrhea in order of frequency. The roentgenological findings were classified into three groups. normal finding, pulmonary emphysema only, and various forms of pneumonic infiltration. The roentgenological findings were somewhat characteristic. The most common finding (24 cases) showed streaky and mottled densities which began in both hill and were spreaded peripherally. The pneumonic infiltrations were spared peripheral lung, but progressed to total involvement. The prominence between alveolar and interstitial infiltration was almostly equal when patients were admitted. Nineteen cases (54%) showed pulmonary emphysema.

PURPOSE: The purpose of this study was to evaluate the effects of revision endoscopic endonasal dacryocystorhinostomy (EDCR) and the causes of postoperative failure after primary endoscopic DCR in patients with nasolacrimal duct obstruction. METHODS: This prospective study enrolled 32 patients (32 eyes) who had undergone revision of endoscopic DCR with the diagnosis of obstruction of nasolacrimal system after primary EDCR between October 1997 and February 2003. RESULTS: The most common cause of revision operation after primary EDCR was granuloma, followed by membranous obstruction and common canalicular obstruction. The success rate of the cases with granuloma was 69.2%, and that with membranous obstruction was 100%. CONCLUSIONS: Revision EDCR success rate can be improved by reducing the frequency of granuloma as this was the most common factor for revision EDCR. The effect of antimetabolites in revision EDCR success rate is to be further evaluated.

5.A Clinical Observation on Congenital Renal Polycystic Disease.
Korean Journal of Urology 1981;22(4):407-412
The renal polycystic disease is familial, almost always bilateral and often involves liver and pancreas as well. It is the most common form of cystic kidneys it humans. Adult renal polycystic disease usually presents in the 4th or 5th decade as an abdominal mass or with signs and symptoms of hematuria, infection, hypertension or renal failure. A clinical observation was made on 13 congenital renal polycystic disease patients who were admitted to our department from June, 1968 to May, 1980. The results obtained were as follows 1. Of 2252 cases hospitalized, 13 cases were congenital renal polycystic disease, giving a ratio of 173:1. The age ranged from 22 to 58 years, and most favorable age was over 35 years (62%). 2. The chief complaints were palpable abdominal mass in 51%, pain in 38% and gross hematuria in 15%. 3. Hypertension was found in 85%, hepatic cyst in 23% and renal stone in 15%. 4. Familial tendency was noted in 54%. 5. Laboratory study showed anemia in 38%, azotemia in 31%. hematuria in 62%, proteinuria in 46% and pyuria in 38%. 6. Ultrasonography appears to be superior to excretory urography in diagnosis of the renal polycystic disease. On pyelography, increased intercalyceal length was exhibited, 11.0 (1.00cm on left kidney and 10.6 ( 1. 97 cm on right kidney. The cortical thickness was also increased to 5.0 ( 1. 54 cm on left kidney and 4.6 ( 0. 86cm on right kidney 7. The patients were managed by medical treatment in 46%, nephrectomy 23% and marsupialization of the cyst in 15%.

Journal of the Korean Ophthalmological Society 1998;39(10):2385-2392
In the treatment of retinopathy of prematurity, it is necessary to coagulate retina including inner retinal layer to achieve regression. However, diode laser has long wavelength, thus damage to bruch`s membrane or choroid can occur when one attempts to coagulate whole retinal layers. We evaluated whether or not we could coagulate whole retinal layer without damage to Bruch`s membrane or choroid in rabbit eyes with indirect diode laser ophthalmoscopy. We palaced laser spots on retina of pigmented rabbits with various laser powers(150, 300, 500mW) and durations(0.3, 0.5, 1.0 second). Fundus photographs were taken immediately after photocoagulation. then we took fundus photographs and enucleated the eyes at one day, one week, 3 weeks, and 5 weeks after photocoagulation to perform pathologic examination. With laser setting of 150mW and 0.3 second, the coagulated retina revealed damaged outer layer but inner retinal layer was spared. We could coagulate whole retinal layer without any damage to Bruch`s membrane or choroid with laser power of 300mW and 0.3 second duration. In 300mW, 0.5 second laser burns and 500mW, 0.3 second burns, internal limiting membrane remained intact while Bruch`s membrane and whole retinal layers were damaged We found whole layer rabbit retina could be coagulated without damage to Bruch`s membrane and choriocapillaries with indirect diode laser opthalmoscopy. and we think it would be necessary to adjust laser power and duration in clinical situation to minimize possible deep tissue injury by diode laser.

10.Surgical management for stage 5 retinopathy of prematurity.
Korean Journal of Ophthalmology 1994;8(1):37-41
To determine the prognosis of advanced retinopathy of prematurity (ROP) after surgery, we reviewed retrospectively the results of vitrectomy performed between November 1987 and December 1993 in 38 eyes with stage 5 ROP. The patients' mean age at surgery was 11 months, mean follow-up period was 24.1 months. Anatomical success, defined as reattachment of posterior pole retina, was achieved in 11 eyes (29.0%) and functional success, defined as presence of fixation and following was achieved in 7 eyes (18.4%). Anatomical success was well correlated with preoperative configuration of retina, i.e. open-funnel type showed better results than closed-funnel type (44.4% compared to 15.0%). There were no differences between anatomical success and failure group according to gestational age, birth weight, age at the time of diagnosis of ROP, chronological and postconceptional age, and body weight at the time of surgery. For good prognosis, it is necessary to operate early in stage 5 ROP when retinal funnel is still open.
