2.The Studies on the Residual Accommodation of Koreans II. The Residual Accommodation under 0.5% Scopolamine and l% Cyclogyl Cycloplegia.

This study was conducted to determine how much of a residual accommodation remained under 0.5% scopolamine and 1% cyclogyl in young Koreans ranged from 6 years to 20 years. The amount of the residual accommodation was measured by the blur point method, and the following results were obtained. 1) The average residual accommodation for 150 eyes was 0.99 D under 0.5% scopolamine and for 96 eyes was 1.53 D under 1% cyclogyl. 2) In comparing the cycloplegic agents, 0.5% scopolamine was almost parallel with 1% atropine (0.96D) and 1% cyclogyl was almost parallel with 5% homatropine (1.42 D). 3) The residual accommodation under 0.5% scopolamine and 1% cyclogyl showed a gradual decrease with increasing age as Erst reported for 1% atropine and 5% homatropine. 4) In comparing with foreign countries, the residual accommodation in Koreans was at a similar level with white people.

This study was conducted to determine how much of a residua1 accommodation remained one hour after three instillations of atropine or homatropine in 384 eyes of younger Koreans. The amount of residual accommodation was measured by the blur point method, and the following results were obtained. 1) The average amount of the residual accommodation was 0.96 D under l% atropine and 1.42 D under 5% homatropine. 2) In comparing the two cycloplegic agents, 1% atropine was found to be more effective than 5% homatropine. 3) Residual accommodation under l% atropine and 5% homatropine showed gradually decrease in older patients. 4) No sex difference was found.

5.12 Cases of Retinal detachmant by the scleral buckling procedure.

12 cases of retinal detachment treated with scleral buckling Procedure of 4392 visitors the department of ophthalmology. Yonsei University medical school from Feb. 28th 1958 to April 30th 1959. 30 cases of retinal detachment were found 12 cases out of 30 received the scleral buckling procedure, and exhibited the following results. 1) a. Successful, b. Fair 3(25%), c. Failure 2(16.6%). 2) The shorter the duration of retinal detachment existed the better the resutls of the operation was. However in 2 cases of old retinal detachment lasted more than 7 months the results of the operation were also successful. 3) Results of the surgery were more favourable in the cases of having no definite hole. 4) In one case, retinal detachment was recurred a year after the first procedure and a secondary procedure was performed with fair results.

8.THE APPROACH OF SKULL BASE LESIONS IN THE VIEW POINT OF PLASTIC SURGERY.

Skull base surgery has developed through the evolution of imaging, anatomic research, surgical approach and reconstructive techniques. The basic disciplines of approaching skull base lesions are provide direct vision, minimizing brain retraction, excellent exposure and minimal blood loss. The focus of this report is to review the advantages of skull base approach in our cases and suggest some indications. We experienced 20 cases of skull base surgery by a team approach consisting of a neurosurgeon and plastic surgeon. The surgical approach were supraorbital osteotomy(5 case), orbitozygomatic osteotomy(8 case), orbitozygomaticoglenoid osteotomy (5 case ) and orbitozygomaticoglenoidocondylar osteotomy (2 case). In our experience, these approaches provided excellent exposure of the lesion, direct access to lesions and minimal brain retraction thereby better outcome.

10.Evaluation of Residual Hepatocellular Carcinoma after Transcatheter Arterial Chemoembolization: Usefulness ofContrast Enhanced Power Doppler Ultrasonography - Preliminary Report.

PURPOSE: To determine the usefulness of microbubble contrast enhanced power Doppler ultrasonography (PDUS)for the detection of residual tumor in hepatocellular carcinomas (HCCs) treated by transcatheter arterialchemoembolization (TACE). MATERIALS AND METHODS: Fourteen nodular HCCs (size range: 1 - 7.3 cm, mean: 3.5) intwelve patients treated by TACE, and on the basis of follow-up liver CT, thought to have a residual tumor, wereincluded in this study. Between July 1997 and April 1998, PDUS examinations were performed with a 2-4 MHz convextransducer before and after intravenous injection of a microbubble contrast agent (Levovist(, Schering AG, Berlin,Germany). Real-time power Doppler ultrasonographic images were recorded on videotape and representative imageswere color-printed. Tumor vascularity was analyzed on real-time images with regard to its presence or absence, andchanges, and two observers reached a consensus. The results were compared with those of other diagnostic tests(three-phase helical CT, conventional angiography, percutaneous biopsy, and/or surgical pathology). RESULTS: Contrast-enhanced PDUS revealed intratumoral vascularity in ten of 14 tumors, none of which showed vascularity onunenhanced PDUS. In the remaining four tumors, both unenhanced and enhanced PDUS showed intratumoral tumorvascularity, which in all cases was more pronounced on enhanced than on unenhanced PDUS. Other diagnostic testsrevealed residual tumors in eleven lesions. CONCLUSION: Microbubble contrast-enhanced PDUS was more sensitivethan non-enhanced PDUS in depicting vascularity within a residual tumor and could be a useful method for thedetection of residual tumor in HCCs treated by TACE.
