3.Evaluation of the Efficacy of Sodium Hyaluronate to Degerative Osteoarthritis of the Knee.

OBJECTIVE: To obtain a effect of intraarticular injection of Sodium Hyaluronate in the patient with degenerative osteoarthritis without restriction of activities of daily living. METHOD: Twenty-five patients were participated in this study. These patients are diagnosed as degenerative osteoarthritis by clinical symtoms and radiographic findings. Sodium hyaluronate, 2.5 ml, 3 mg/ampule, were injected intraarticulary without local anesthesia once a week for 5 times consecutively. For evaluation of the effectiveness of sodium hyaluronate, we assess the parameters for subjective and objective symtoms scored from 0 to 3 on 3 items, and for activities of daily living scored from 0 to 4 on 4 items. And then, we compared these data between pre-injection and at post-injection 2, 4, and 5 weeks. Changes of subjective and objective symtoms, and activities of daily living are assessed using Kellgren's X-ray grading of degenerative osteoarthritis. Improvement of the subjective pain is recorded by visual analogue scale. RESULTS: 1. Subjective and objective symtoms, and activities of daily living with time progression were significantly increased at post-injection 4, 5 weeks compared with pre-injection status (P<0.05). 2. Subjective and objective symtoms, and activities of daily living according to Kellgren's X-ray classification were significantly increased at stage II and III (P<0.05). 3. Visual analogue scale is significantly decreased after injection (P<0.05). CONCLUSION: Intraarticular injection of sodium hyaluronate showed improvement of patient's subjective and objective symtoms, and activities of daily living. The improvement was pronounced in the cases of high grade of degenerative osteoarthritis (stage II, III) according to Kellgren's X-ray classification as well as low grade (Stage I).

4.The Characteristics of Suicide Attempts and Psychosocial Risk Factors in Correctional Institutions.

OBJECTIVES: Suicide is one of the most common causes of deaths in correctional institutions. This study aimed to examine the characteristics of suicide attempts and its related psycho-social factors in correctional institutions. METHODS: This study examined the characteristics of 101 suicide attempts from 2006 to 2007 in the two regional correctional headquarters. Thirty-seven male inmates (43 suicide attempts) and 40 matched controls were included in interviews and reviews of personal records. Psychiatric illnesses were examined using the Structured Clinical Interview for DSM-IV and medical outcomes of their suicide attempts with the Lethality Scale of Diagnostic Interview for Genetic Studies. RESULTS: Over a half of suicide attempts occurred in solitary cells and the most common method was hanging. Medically, 70% of the suicide attempts were more than severe. Poor social support, a lifetime history of suicide attempts, and incarceration were associated with suicide attempts. Additionally, psychiatric illnesses were more likely to increase the risk of suicide. CONCLUSION: This study implies that mental health issues and monitoring systems are important to reduce suicides in the correctional system. Regular checks and management of suicide risk and mental illness are crucial to prevent suicide in correctional institutions.

Odontogenic keratocysts are benign intraosseous tumors of odontogenic origin that occur most commonly in the jaw. In particular, they have a predilection for the angle and ascending ramus of the mandible. In contrast, odontogenic keratocysts arising in the maxillary sinus are relatively rare. Two such cases are reported herein. In addition, the English literature that concerns odontogenic keratocysts of the maxillary sinus is reviewed.

6.A Case of Sebaceous Trichofolliculoma.

Sebaceous trichofolliculoma, which is a variant of trichofolliculoma, is a rare disease that clinically show a centrally depressed lesion usually singly on the nose, and histologically a centrally located cavity, lined by squamous epithelium, with numerous sebaceous lobules connected to them. We describe a case of sebaceous trichofolliculoma in a 27-year-old woman who had several brown papules on the nose. We think that this is the first report of sebaceous trichofolliculloma in Korean literature.

7.A Study on the Relationship between Subjective Symptoms and Psychosocial Well-being Status of VDT Operators.

This study was undertaken to identify the relationship between the subjective symptoms and the psychosocial well-being status of VDT operators. The study subjects of this study were 89 female telephone operators in Korea Telecom. The mean age of study subjects was 37.7 (s.d.; 3.1), and 91% of them were the married and 9% were the single. The mean values of psychosocial well-being status by general characteristics were not statistically significant. Of the 89 operators, 98.9% felt musculoskeletal subjective symptoms in shoulder, 91.2% in neck, 89.9% in hand, 89.9% in lowback, 88.9% in arm, 87.8% in back, and 85.6% in leg, respectively. The mean scores of the psychosocial well-being status by three categories of subjective symptoms were significant in shoulder, neck, arm, hand, and leg, but were not significant in back and lowback. Correlations between the subjective symptoms and the psychosocial well-being status were significant in discomfort scale, leg, arm, neck, shoulder, lowback, and hand, but were not significant in back. Multiple regression analysis were used to determine whether the independent variables (age, discomfort, and subjective symptoms) contributed to explaining the psychosocial well-being status. Discomfort and the subjective symptoms in leg were a correlate of the psychosocial well-being status.

8.A case of squamous cell carcinoma arising from ovarian mature cystic teratoma treated with adjuvant chemoradiation.

Malignant transformation of mature cystic teratoma is rare and difficult to diagnose preoperatively. Therefore, in the suspected case considering the age, tumor size and imaging findings, it is important to diagnose intraoperatively through a frozen section biopsy. It is clear that aggressive optimal surgical resection should be done in the squamous cell carcinoma arising from mature cystic teratoma. However it is controversy what postoperative adjuvant therapy is optimal in the locally advanced case because of its rarity. We report a case of a 59-year-old woman in the menopause with locally advanced squamous cell carcinoma arising from left ovarian mature cystic teratoma treated with optimal cytoreductive surgery followed by concurrent chemoradiation.

10.Effects of Cigarette Smoking on Psychosicial Distress and Occupational Risks.

The effects of cigarette smoking on the psychosocial distress, the absenteeism, the occupational injuries, and on the prevalence for the accidents were assessed in 795 male workers who had been employed since March 1994. The results show that the prevalence of current smokers were higher in young men, lower educational level, lower income, single men(unmarried or divorced), alcohol drinkers, and blue collar workers. In the bivariate analyses, the workers with the high job demand and low work control were more likely to smoke, although the relationship was not strong. Those who reported lower satisfaction on his job tended to smoke more. Mean scores of psychosocial well-being Index(PWI) were higher in current smokers than nonsmokers. For the hierarchical multiple regression analyses, interaction terms between cigarette smoking and job stressors(job demand and work control) were not significant. For smokers, the odds ratios for the occupational injuries, and the accidents were 1.40(95% confidence interval 0.77-2.57) and 1.96 (95% confidence interval 0.75-5.09), respectively. The mean absent day were 4.13 for smokers, 3.65 for nonsmokers, although the differences were not statistically significant. It seems that cigarette smoking had not caused any crucial effects on the psychosocial well-being status and the occupational risks. Some considerations for the further research on the relationship of cigarette smoking on the mental health status and the occupational risks were also discussed.
