BACKGROUND: Primary multidrug-resistant tuberculosis is defined as Mycobacterium tuberculosis isolates that are resistant to at least isoniazid and rifampin in never-been-treated tuberculosis patients, and this malady is caused by the transmission of a resistant strain from one patient, who is infected with a resistant Mycobacterium tuberculosis strain, to another patient. The prevalence of primary multidrug-resistant tuberculosis could be a good indicator of the performance of tuberculosis control programs in recent years. We conducted a case-control study to identify the risk factors for primary multidrug-resistant tuberculosis. METHODS: From January 1, 2001 to, June 30, 2003, by conducting prospective laboratory-based surveillance, we identified 29 hospitalized patients with P-MDRTB and these patients constituted a case group in this study. The controls were represented by all the patients with culture-confirmed drug susceptible tuberculosis who were admitted to National Masan Hospital during the same study period. The odds ratios for the patients with primary multidrug-resistant tuberculosis, as compared with those of the patients with drug susceptible tuberculosis, were calculated for each categorical variable with 95% confidence intervals. RESULTS: Multivariate logistic regression showed that the presence of diabetes mellitus (odds ratio 2.68; 95% confidence interval, 1.05-6.86) was independently associated with having primary multidrug-resistant tuberculosis. CONCLUSION: This study has shown that diabetes mellitus might be one of the risk factors for primary multidrug-resistant tuberculosis.

OBJECTIVES: The aim of study was to examine the prevalence of night eating syndrome(NES) and its correlates in schizophrenic outpatients. METHODS: The 14 items of self-reported night eating questionnaire(NEQ) was administered to 201 schizophrenic patients in psychiatric outpatient clinic. We examined demographic and clinical characteristics, body mass index(BMI), subjective measures of mood, sleep, binge eating, and weight-related quality of life using Beck's Depression Inventory(BDI), Pittsburgh Sleep Quality Index(PSQI), Binge Eating Scale(BES) and Korean version of Obesity-Related Quality of Life Scale(KOQoL), respectively. RESULTS: The prevalence of night eaters in schizophrenic outpatients was 10.4%(21 of 201). Comparisons between NES group and non-NES group revealed no significant differences in sociodemographic characteristics, clinical status and BMI. Compared to non-NES, patients with NES reported significantly greater depressed mood and sleep disturbance, more binge eating pattern, and decreased weight-related quality of life. While 'morning anorexia' and 'delayed morning meal'(2 of 5 NES core components in NEQ) were not differed between groups, 'nocturnal ingestions', 'evening hyperphagia', and 'mood/sleep' were more impaired in NES group. CONCLUSION: These findings are the first to describe the prevalence and its correlates of night eaters in schizophrenic outpatients. These results suggest that NES has negative mental health implications, although it was not associated with obesity. Further study to generalize these results is required.

OBJECTIVES: Outpatient treatment orders refer to a mandatory social program in which mentally ill persons are ordered by the court to participate in specified outpatient treatment programs. This study aimed to investigate the factors that affect outpatient treatment orders and adherence to outpatient treatment in mental health patients. METHODS: A survey on outpatient treatment orders and adherence to outpatient treatment was conducted on 60 psychiatrists between October and November 2016. The questionnaire items were drafted based on a literature review, and they were then evaluated by 3 psychiatrists and 1 law school professor before being finalized. Answers from the respondents were analyzed using descriptive statistics, and the median, maximum, and minimum values of the effectiveness scores of outpatient treatment orders were calculated. RESULTS: Among the 60 psychiatrists, 45(75.0%) were aware of outpatient treatment orders; however, only 2 out of the 45(4.0%) had actually used the program in the last 12 months. The subjective effectiveness was very low, with only 40 points out of 100. Furthermore, of the readmitted patients, 37.7% had received continued outpatient treatment, whereas 53.1% chose to quit the outpatient treatment programs, meaning that the number of dropouts was higher. Among the discharged patients, approximately two-thirds were receiving continued treatment. With regard to follow-up for dropouts, majority of the responses were either “Not taking any action”(n=27) or “Not following up”(n=15). Only two respondents answered “Contact the community mental health promotion center,” meaning that this response was very rare. Meanwhile, when asked about efficient measures to be implemented for dropouts, a vast majority of the respondents(n=30) selected the answer “Work with the community mental health promotion center.” CONCLUSIONS: The outpatient treatment orders currently being administered were found to be ineffective, and the associated adherence to outpatient treatment was also found to be extremely poor. Hence, the effectiveness of the therapeutic interventions could benefit from institutional as well as administrative improvements. Community mental health promotion centers are expected to have an important role in the future.

BACKGROUND: This study examined how mental health status influenced suicidal ideation among community-dwelling elderly at senior citizen centers in Korea.METHODS: Data were obtained from a survey on elderly mental health in Jeollanam-do (a southwest province in Korea) conducted by the Jeollanam-do Provincial Mental Health and Welfare Center. In total, 4,113 people were recruited from all 22 cities in Jeollanam-do. We evaluated sociodemographic factors and mental health status using self-reported questionnaires, namely, the Suicidal Ideation Scale, Geriatric Depression Scale-Short Form Korean Version, Multidimensional Scale of Perceived Social Support, Korean version of the General Health Questionale-12, and Satisfaction with Life Scale, to assess psychosocial factors affecting suicidal ideation. Multivariate logistic regression was performed to examine the factors associated with suicidal ideation.RESULTS: Among the 4,113 subjects, 325 (7.9%) reported recent suicidal ideation. Multivariate logistic regression analysis revealed that depression (p<0.001), low-level social support and life satisfaction (p=0.006), poor general mental health (p<0.001), physical disease (p=0.009), and poor self-perceived health status (p=0.039) were significantlyassociated with suicidal ideation.CONCLUSION: The presence of physical disease, poor self-perceived health status, depression, and poor general mental health conditions increase the risk of suicide ideation among the elderly. Social support and life satisfaction affected their suicidal ideation independently of depression.

Purpose: Scoliosis is a common musculoskeletal problem in adolescents. This study aimed to identify the prevalence of adolescent idiopathic scoliosis (AIS) and its associated factors among Korean adolescents. The prevalence of thin individuals among students with AIS was compared based on body mass index (BMI) classifications. Methods: This study was a secondary data analysis and used the 2016 Korean National Health Examination for School Students data. Data from 16,412 students were analyzed using descriptive statistics, Chi-square tests, and logistic regression analysis. Results: The prevalence of AIS was higher in women (3.8%) than in men (1.6%), and a higher school year was a risk factor for AIS in both sexes. In woman adolescents, scoliosis was associated with thinness; however, the risk of AIS was inversely associated with overweight/obesity in both sexes. The prevalence of thin woman students with scoliosis differed based on the criteria used: 3.3% by the World Health Organization criteria and 14.3% by the International Obesity Task Force criteria. Conclusion The prevalence of thin students with scoliosis could increase by up to four times depending on the BMI criteria. For early screening of thin people at risk of AIS among female students, the criterion of International Obesity Task Force should be used as it is more permissive of thinness. This is also because of the underestimation of AIS prevalence when using the BMI Z score of the World Health Organization cutoff.

Background: Depression is increasing among college students in general. Moreover, almost one-third of college students have been reported to suffer from depression. Thus, this study aimed to assess differences in stress coping strategies and resilience between depressed and normal-mood groups among college students. Methods: A total of 3,306 college students participated in this study. The students responded to a questionnaire that included questions of the Center for Epidemiologic Studies Depression (CES-D) scale, stress coping scale (SCS), and brief resilience scale. Logistic regression analysis was conducted to evaluate the impact of variables on depression. Results: Using the CES-D (cutoff score ≥21), 423 (12.8%) college students were classified as depressed. Adjusting for individual demographic factors, the SCS results of the students with depression showed significantly higher scores in emotion-focused coping (p<0.001), wishful thinking (p<0.001), and lower problem-focused coping (p<0.001) than the normal-mood group. Moreover, they presented lower resilience scores. Students who had emotion-focused coping (odds ratio [OR], 1.11; p<0.001) and lower resilience scores (OR, 0.76; p<0.001) were associated with higher CES-D scores. Conclusion The study findings revealed significant differences between the depressed and normal-mood groups in terms of stress coping skills and resilience, suggesting the need for promoting stress coping strategies and resilience to lower depression-related problems among college students.

Background: Alcohol drinking among college students is socially permissible in Korea. However, this population’s tendency to consume alcohol excessively results in many alcohol-related problems, including psychiatric problems.This study aimed to identify the sociodemographic characteristics and psychiatric comorbidities associated with hazardous alcohol drinking among college students. Methods: In total, 2,571 college students participated in the study. Data were collected using the Korean version of the Alcohol Use Disorders Identification Test (AUDIT-K), the Mood Disorder Questionnaire, the Center for Epidemiologic Studies Depression Scale, a modified Korean version of the 16-item Prodromal Questionnaire, the Adult Attention-Deficit/Hyperactivity Disorder Self-Report Scale-Version 1.1, and a stress-coping scale. Logistic regression analysis was performed on variables significantly correlated with hazardous alcohol drinking. Results: In total, 633 students were grouped into the hazardous alcohol drinking group (AUDIT-K, ≥12). The associ-ated variables were age (odds ratio [OR], 0.95; p<0.05), smoking (OR, 4.00; p<0.001), bipolar disorder (OR, 2.45; p<0.05), depressive disorder (OR, 1.35; p<0.05), attention-deficit/hyperactivity disorder (ADHD; OR, 1.44; p<0.05), and problem-focused stress coping (OR, 0.97; p<0.05). Conclusion In this study, hazardous alcohol drinking was associated with smoking, mood disorders, and ADHD. We suggest that alcohol use among college students be carefully monitored and managed in terms of its psychiatric comorbidities.

Background: The workload of organ transplant coordinators is increasing as administrative tasks become more diverse with changing laws and regulations. These changes have heightened the demand for organ transplant coordinators with expertise in the field. This study aimed to determine the status of human resources of organ transplant coordinators and conduct job analysis using the Developing A Curriculum (DACUM) method. Methods: We conducted a questionnaire survey with 107 transplant coordinators employed at medical centers across Korea. The questionnaire gathered data on general and job-related characteristics of organ transplant coordinators and assessed the importance, difficulty, and frequency of their task elements. Results: The job of organ transplantation was categorized into five duties, 14 tasks, and 97 task elements. These duties included recipient management, donor management, organ donation activation management, organ transplantation administration, and professional capability development. We noted statistically significant differences in the importance scores of organ donation activation based on age, as well as in the difficulty scores for recipient management and administrative tasks based on work experience. Furthermore, the frequency of task performance varied significantly according to the number of coworkers and the total number of transplants conducted. Conclusions This study confirmed the current status of the workforce and task performance of organ transplant coordinators. The findings will serve as basic data to enhance the expertise of coordinators in the future.
