1.Malignant Brenner Tumor: Report of a case.

Brenner tumors constitute about l.5~2.5% of all primary ovarian neoplasms and are almost always benign. It appears to derive from the surface epithelium of the ovary which undergoes metap1asia to form the urothelial-like components. we experienced a case of malignant Brenner tumor with adenocarcinoma and squamous cell carcinoma patterns in a 57-year-old woman. It was partly cystic tumor and contained a 4cm-sized gray yellow, lobulated or papillary solid mass, projecting from the cystic wall. Ultrastructurally, the solid mass was composed of malignant urothelial-like cells with focal glandular differentiation.

2.Angiosarcoma of Jejunum Following Therapeutic Irradiation: A case report.

We report a case of angiosarcoma of the jejunum in a 65-year-old female. This angiosarcoma developed 20 years after postoperative irradiation for cervical carcinoma of the uterus. Grossly, the resected jejunum showed a 8 cm long segmental hemorrhagic lesion with multiple, small hemorrhagic nodules or cysts on its serosa and mesentery. Microscopically, the wall of jejunum was infiltrated by epithelioid or polygonal tumor cells arranged in solid nests or lining irregular vascular spaces. The case is of interest that the tumor occurred in the very unusal site for angiosarcoma and the patient had a past history of irradiation for uterine cervical carcinoma 20 years ago. In addition, the resected intestine showed histologic changes of chronic irradiation effect. Therefore, this case supports the view that there is cause and effect relationship between irradiation adn angiosarcoma.

3.Malignant Lymphoma of Thyroid Associated with Chronic Lymphocytic Thyroiditis and Occult Sclerosing Carcinoma: A case report.

Although the reported number of malignant lymphoma of the thyroid has been increased, primary malignant lymphoma of the thyroid associated with chronic lymphocytic thyroiditis has been sporadically reported since its first description by Graham in 1931. The apparent coexistence of these two conditions has undoubtedly been noted by other observers. And they suggested that malignant lymphoma of the thyroid might arise from the lymphoid tissue ina wide variety of preexisting thyroidal disease. This report was made to record a case of primary thyroid lymphoma recently seen in this department with special emphasis on the associated thyroiditic changes in the uninvolved portion of the gland, which suggest the antecedent presence of chronic lymphocytic thyroiditis.
4.Fibrocalcific Nodule in the Liver Capsule Caused by Ascaris Eggs: A case report.

Ascariasis is probably the most common helminthic infestation of man, but it seldom causes severe illness. Pathologic conditions of Ascaris may be caused by adult worms, eggs or larvae. We describe a case of Ascaris egg granulomas that were found incidentally on the surface of the liver in a 75-year-old woman who had undergone a segmentectomy for an intrahepatic stone. Grossly, there were several yellowish calcific nodules of 0.4 cm in diameter on the lateral surface of the left lobe of the liver. Microscopically, the lesions were located in the hepatic capsule and consisted of fibrocalific nodules with many eggs. The eggs were round to oval, thick-shelled and measured 50~75x30~50 um. Most of the morphologically preserved eggs were fertilized eggs, but they had smooth shells without external protein coats. This case is of interest for the unusual location of the lesion, the presence of eggs without mammillation, and the association with the intrahepatic stone.

5.A Case of Giant Cell Tumor of the Tendon Sheath Developing on the Dorsum of the Foot.

We report a case of giant cell tumor of the tendon sheath involving the foot, in a 21-year-old female patient, who presented with an asymptomatic tumor on the dorsum of her right foot. Histopathologically the tumor is surrounded by thin fibrous connective tissue. The characteristic findings of the excised specimen revealed a typical mixture of abundunt round or polygonal histocyte-like cells with varying portions of spindle-shaped fibroblast-like cells and multinucleated giant cells. Variable amounts of hyalinized fibrous stromal tissue were also present. After excision, local recurrence has not been observed for 3 months.

6.Pathological Findings of the Femoral Head in Avasular Necrosis after Failed Core - Decompression Surgery.

Core decompression is still widely used in avascular necrosis of the femoral head but the results are unpredictable and the indications are not settled yet. The reparative process of the decompressed femoral head is poorly understood. Seven cases in 5 patients were undertaken THRA following failed decompression and these were studied for the radiological and pathological changes of the core tracts. The lesions of failed cases were involved more than 1/3 of femoral head on MRI and all cases were stage II A or B. The extent of the necrotic area in MRI was enlarged with crack, sclerosis and sometimes gas collection. Depression of the subchondral plates were also observed. Capillary ingrowth or neovascularization was not found at all and there were only fibrosis, inflammatory response and foreign body reactions.

7.Rotator Cuff Injury: Fat Suppression MR Image.

PURPOSE: We performed the study prospectively to evaluate the advantage of fat suppression MR in the diagnosis of rotator cuff injury. MATERIALS AND METHODS: Ten symptomatic patients were studied with both conventional T2WI and FST2WI using chemical shift technique. Each image was analyzed for the assessnent of injuries, conspicuity of the lesion, the presence of effusion in subacromial bursae and joint space, and presence of humeral head injury. Arthroscopy was done in 4 patients following MRI. RESULTS: We could made presumptive diagnoses on FSMR as identical as on conventional MR in six cases(1 normal, 2 tendinitis, 2 partial thickness tear, 1 full thickness tear), two of them were confirmed by arthroscopic procedures. Two cases of partial thickness tear proved by arthroscopy were detected on FST2WI, whereas they were considered tendinitis on conventional T2Wl. There were another 2 cases who showed tendinitis on FSMR, but normal on conventional T2Wl. They, however, were not confirmed by either arthroscopy or surgical procedure. CONCLUSION: We found the FSMR were superior to conventional T2Wl in the conspicuity of lesions and detection of joint effusion and abnormalities on the humeral head. We think FSMR of the shoulder could have significant diagnostic advantages over the conventional spin-echo MR imaging.

8.Sputum induction method for studying total IgE levels in atopics asthamtic patients.

BACKGROUND: IgE antibodies have been considered to play an important role in the pathogenesis of atapic asthma. However, there have been only few studies on the role of IgE in airway secretion in the pathogenesis of bronchial asthma. This might be partly due to difficulty in sampling of airway seceretion from asthmatic patients. Recently, sputum induction method by inhalation of nebulized hypertonic saline was developed, and proved to be valid and useful method for obtaining airway secretion from asthmatic patients for studying airway inflammation. OBJECTIVE AND METHOD: To evaluate the usefulness of sputum induction method for studying IgE antibodies in airway secretion from atopic asthmatic patients, total IgE levels in induced sputum from 54 atopic asthmatics were measured by enzyme-linked immunosorbent assay(ELISA) and tried to find an association with sputum eosinophilia. RESULT: Total IgE levels in induced sputum were significantly higher in atopic asthmatic patients(1.27+82.066 IU/ml) than in controls(0.203+0.291 IUgmP)(p<0.05). In atopic asthmatic patients, total IgE levels in induced sputum were not significantly different between patients with and without sputum eosinophilia(>5% of 200 counted leukocytes). There was a significant correlation of total IgE levels between induced sputum and serum in atopic asthmatic patients(r=0.60, p<0.05). Total IgE levels in induced sputum(1.278+ 2.066) were significantly higher than saliva sample(0.504 + 1.111 IU/ml) from atopic asthmatic patients(p<0.05). CONCLUSION: These results suggest that total IgE levels are increased in the induced sputum of atopic asthmat,ic patients and sputum induction method is a useful tool for studying IgE antibodies in airway secretion from asthmatic patients.

9.The Risk of Malnutrition, Depression, and the Perceived Health Status of Older Adults.

PURPOSE: The purpose of this study was to investigate the risk of malnutrition and its relationship with depression and perceived health status. METHODS: A total number of 154 elderly over 60 years participated in the study through a community elderly center. The risk of malnutrition was measured by NSI (Nutritional Screening Initiative), depression by CES-D, and health status by a self-rated Likert scale. RESULTS: About one fourth (22.7%) of the subjects had a high risk, and 31.2% had a moderate risk of getting malnutrition. Regarding depression, 34.4% (53 elderly) of the subjects had a high risk. Overall health status had a mean of 3.46 within the range of 1 to 5. In relation to demographic factors, female elderly (chi-square=6.68, p= .04), aged younger than 75 years old (chi-square=8.60, p= .01), and having co-morbidity (F=9.81, p= .001) were significantly related to a high risk of malnutrition. Having a higher depression score, higher number of co-morbidity, and lower perceived health status were significantly related to a higher risk of becoming malnourished. CONCLUSION: The elderly's risk of getting malnutrition was significantly related to their depression and perceived health status. With these findings nursing interventions focusing on these factors should be developed in order to improve the elderly's multidimensional well-being.

10.Thrown in a Different World: The Later Lives of Korean Elderly in an American Nursing Home.

PURPOSE: Increasing numbers of Koreans have immigrated to the United States since the late 1960s. The first generation of Korean immigrants or their parents become old and institutionalized in American nursing home setting. Although the Korean elders would experience many cultural differences in the nursing home, no study to date has investigated their everyday lives on how they live through their later lives within a different cultural environment from their own. METHODS: Using ethnographic methodology, the purpose of this paper was to illustrate Korean residents' experiences and daily lives in a nursing home located in an east coastal city in the U.S. Participant observation, filed notes, semi-structured interviews were utilized by means of data collection. Eighteen Korean residents were observed, and five of them and two nurses participated in informal qualitative interviews. RESULTS: The overriding theme from the findings is "thrown in a different world." Three sub-themes include "constant struggles in making themselves understood", "dealing with culturally inappropriate nursing care," and "maintaining their own ways of life". CONCLUSIONS: The discovered themes reflect culturally isolated lives of the participants and open a venue for designing a culturally congruent nursing care for Korean elders living in the U.S. nursing homes.
