1.Open Skin Grafting of War Wounds

During the period from April 1971 to September 1971, 30 young male patients with war wounds sustained in the conflict of Viet Nam were treated by open skin grafting of their wounds at the Orthopedic Servlce of Taegu Hospital. The majority of patients had multiple wounds and most had associated fractures of the long bones. The indication, preoperative preparation, operative technique, postoperative care and results have been described. We believe that this open skin grafting offers the following advantages over other techniques; 1) Graft taken more satisfactorily. 2) Remarkable decrease of operating time. 3) Simplicity of operative technique. 4) Continuous observation of wound healing. 5) Relative safety in using this treatment upon unstable injured limbs and cases with neurovascular deficit because the lesion is free from compression dressing. 6) The grafts tend to be “take” even if there is bleeding or drainage, because it can be carefully rolled out under the direct observation.

2.A Case of Atrophie Blanche.

Atrophie blanche is a rare vasculitis reported by Milian in 1929, that is characterizel by small erythematous, telangiectatic purpura which developes central hemorragic curst or painful ulceration and heals with atrophic white scar several months later. It is seen more commonly in middle-aged women and become worse in summer. It manifests primarly on the lower legs, ankles and dorsa of the feet, rarely on the elbows and dorse of the hands, where are the areas of the poor blood circulation. This case was 45 year-old woman who had suffered from periodic painful ulceration on the legs, ankles, dorsa of feet k hands and. elbows for about 18 years. Histopathologic findings showed fibrinoid degeneration R partial occlusion in the capillaries and mild perivascular chronic inflammatory cells infiltration in the upper corium. The lesion was not so improved by systemic k local corticosteroid therapy for about 3 months,

3.Clinical Experience of Etafenone (Baxacor(R)) in the Treatment of Angina Pectoris.

The clinical effect of etafenone on 12 cases of angina pectoris(9 males and 3 females with average age of 58.3 years) were studied and following results were obtained: 1. Anginal chest pains were improved after average 9 days (3~15 days) of medication and the dreug was very effective in 41.7%, moderately effective in 33.3% and slightly effective in 25.0% of cases. 2. The ST segment depression of E.C.G. were reduced in 50.0%, improvement of T waves inversion or enlargement of T waves were observed in 66.7% and E.C.G. improvement either ST segment or T wave observed in 75.0% of cases. 3. No side reactions such as hypotension, palpitation, dizziness, headache or gastrointestinal disturbances were seen during the treatement with etafenone.

5.Comparative study of STIR image with SE image in diagnosis of the orbital disease.

To evaluate the efficacy of STIR (short tau inversio recovery) image in the diagnosis of orbital diseases, we compared the STIR images with conventional SE(spin-echo) images in 17 patients. The fat signal was reliably nulled at inversion time of approximately 160-170 msec in all cases. We analyzed all the cases in the aspects of the detection of lesion and the lesion-to-back ground contrast retrospectively. Regarding lesion, the detection rates of STIR, T1 and T2-weighted images were 89, 88 and 83% respectively. In the aspect of lesion-to-background contrast, the contrast ratios of STIR and T1, T2-weighted image were 87% and 64, 56%, respectively. In conclusion, STIR images were similar to T1-and T2-weighted images in the detection of lesion, but superior in the aspect of lesion-to-background contrast. So the STIR images may be a useful adjunct to conventional SE images for the diagnosis of orbital disease.

Three cases of spondyIolysis were experienced and treated by anterior interbody fusion of the involved spines at the department of orthopaedic surgery in the Taegu Hospital One case was accompanied with severe lumbago with radiating pain and other two cases with lumbago significantly. 7.Functional MRI of The Supplementary Motor Area in Hand Motor Task: Comparison Study with The Primary Motor Area.

PURPOSE: To investigate the localization and functional lateralization of the supplementary motor area (SMA) in motor activation tests in comparison to that of the primary motor area. MATERIALS AND METHODS: Seven healthy volunteers obtained echoplanar imaging blood oxygen level dependent technique. This study was carried on 1.5T Siemens Magneton Vision system with the standard head coil. Parameters of EPI were followed as ; TR/TE; 1.0/66.0 msec. flip angle : 90degree, field of view : 22cmx22cm, matrix : 128x128, slice number/slice thickness/gap : 10/4mm/0.8mm with fat suppression technique. Motor task as finger opposition in each hand consisted of 3 sets of alternative rest and activation periods. Postprocessing were done on Stimulate 5.0 by using cross-correlation statistics. To compare the functional lateralization of the SMA in the right and left hand tests, each examination was evaluation for the percent change of signal intensity and the number of activated voxels both in the SMA and in the primary motor area. Hemispheric asymmetry was defined as difference of summation of the activated yokels between each hemisphere. RESULTS: Percent change of signal intensity in the SMA (2.49-3.06%) is lower than that of primary motor area(4.4-7.23%). Percent change of signal intensity including activated voxels were observed almost equally in the right and left SMA. As for summation of activated voxels primary motor area had significant difference between each hemisphere but not did the SMA. CONCLUSION: Preferred contralateral dominant hemisphere and hemispheric asymmetry were detected in the primary motor area but not in the SMA.

8.Arterial Oxygen Desaturation Rate Following Obstructive Apnea in Parturients .

Preoxygenation is a standard anesthetic technique for preventing a significant hypoxemia during the induction of anesthesia. Complete denitrogenation is especially important in clinical situations of difficult intubation or in patients with decreased functional residual capacity, and in situations where oxygen saturation is critical. Oxygen consumption in pregnancy is markedly increased at term as compaired to the nonpregnant stage. It is important to evaluate how long parturient women can withstand apneic hypoxemia during induction of general anesthesia. This study was carried out to measure the duration of time required to decrease the SaO2 to 90% After written informed consent was obtained from six healthy parturients who were to under go elective Cesarean section and six healthy non-pregnant women who were to have total abdominal hysterectomies. All subjects had a tight fitting anesthesia mask applied and breathed 100% oxygen, and a single isolated apnea was carried out. Arterial oxygen saturation and gas tensions were measured at a time SaO2 decreased to 90%, also blood gas data of 4 minutes after apnea in the non-pregnant women were obtained. From these, arterial oxyhemoglobin content was calculated, and mean desaturation rate from denitrogenation to the time SaO2 decreased to 90% was calculated. The mean time to obtain 90% saturation was 7.5+/-0.9 minutes in the nonpregnant women and 3.6+/-0.8 minutes in the parturient group. The mean slope of desaturation was steeper in the paturient(-3.336) than the nonpregnant (-1.52). The PaO2 inereased over 400mmHg in both the groups after denitrogenatio. After 4 minutes of apnea, the mean PaO2 decreased to 200mmHg in the non-pregnant women. The rate of rise of alveolar PCO2 during apnea were alower in the non-pregnant women(2.8+/-1.2mmHg/minute) than in the parturient women(6.8+/-1.8mmHg/minute). This study demonstrates that the rate of oxygen desaturation is faster in the parturients than the nonpregnant women. It is suggested that those results came out because of pregnancy-in-duced increase of oxygen consumption rate and decrease in FRC. The results of this study show the induction for term parturients should be speeded up with caution after full oxygenation in comparison with non-pregnant patients.

10.The Calretinin Immunoreactive Ganglion Cell Postsynaptic to the ON-Cholinergic Amacrine Cell in the Guinea Pig.

PURPOSE: To demonstrate the characterization calretinin-immunoreactive displaced amacrine cells in the ganglion cell layer using immunohistochemistry and electron microscopy. METHODS: For immunohistochemistry, sections from guinea pig retina were incubated with mouse monoclonal antibody directed against calretinin. For double label studies, sections were incuated in mixture of mouse monoclonal anti-calretinin or rabbit polyclonal anti-calretinin with following antibodies: goat polyclonal anti-ChAT, rabbit polyclonal anti-GABA, mouse monoclonal anti-GABAA receptor alpha1, beta2/3. Sections were analyzed using Bio-rad Radiance Plus confocal scanning microscope. Stained sections from three guinea pig were observed with transmission electron microscope. RESULTS: Calretinin immunoreactivity was present in displaced amacrine cells and ganglion cells gaving rise to processes ramified in the inner part of the inner plexiform layer in stratum 4. The same stratum was also occupied by the dendrites of ON-cholinergic amacrine cells. Double-labeling demonstrated that dendrites and cell bodies of displaced amacrine cells colocalized with ON-cholinergic amacrine cells and dendrites of ganglion cells directly overlapped with dendrites of ON-cholinergic amacrine cells. The synaptic connectivity was identified by electron microscopy. Ganglion cell dendrites received synaptic input from ON-cholinergic amacrine cell. GABAA receptor beta2/3 subunit bands cofaciculates the dendrites of displaced amacrine cell and ganglion cell that are juxtapose to the alpha1 subunit of GABAA receptor. CONCLUSIONS: These results indicate that ON-cholinergic amacrine cells modulate calretinin-labeled ganglion cell via GABAA receptor beta2/3 in the guinea pig retina.
