1.A case of lipodystrophy centrifugalis abdominalis infantilis.

We report a case of lipodystrophia centrifugalis abdomina]is infantilis in a 2 year 5 month male. At age 7 months, his parents noticed a bluish macule in the right inguinal region, which gradually became depressed and increased in size over a year to involve the abdomen and flank bilaterally. Regional lymph nodes were enlarged. Biopsy showed absence of subcutaneous fat in the affected area.

2.A case of fixed sporotrichosis treated with itraconazole.

We report a case of fixed sporotrichosis in a 9-year-old boy, who showed;i well-cle fineci erythematous plaque on right periorbital area for 4 months. Histopathologically, chronic granulomatous inflammation with pseudoepitheliomatous hyperplasi was oh served. Fungal cultures grew out the typical moist wrinkled colonies. Therapy was initiated on with 100mg of itraconazole daily with almost complete resolution of the skin lesion 2 months after treatment. I3uring the 4 month follow up period, there was no recurrence.

3.Metastatic Squamous Cell Carcinoma of the Stomach from the Uterine Cervix: A case report.

Secondary tumor of the stomach is rare when metastatic involvement is defined as an extragastric tumor invading the muscularis, submucosa or mucosa and the only serosal implant is excluded. Recently, authors experienced a case of metastatic squamous cell carcinoma of the stomach in the pyloric antrum from the uterine cervix. The patient was a 57-year-old female who received a simplified Wertheim's operation followed by irradiation for squamous cell carcinoma of uterine cervix in stage IIb, and 2-1/2 years later she presented gastric symptoms, for which subtotal gastrectomy was carried out. The gastric lesion simulated primary adenocarcinoma of Borrmann type III, but differed in the following macroscopic characteristics. Firstly, Bormann classification is not applicable; secondly, mucosal ulcerations appear irregular and multicentric; thirdly, flaskshaped ulceration and broad based penetration are evidet; fourthly, submucosal tumor-growth pattern is conspicuous in bulk of areas; and fifthly, circumferential/semicircumferential growth is manifested. The above features should also be differentiated from those of malignant lymphoma and leiomyosarcoma.

4.Primary Squamous Cell and Adenosquamous Carcinomas of the Stomach: Clinico-pathologic analysis of 6 cases.

Squamous cell carcinomas and adenosquamous carcinomas are rarely originated from the stomach, thus the limited and sporadic informations about their clinico-pathologic characteristics as well as histogenesis have been elucidated. Six cases of primary squamous cell carcinoma (3 cases) and adenosquamous carcinoma(3 cases) in the stomach were examined during a period of 4 years from 1981 to 1984. After pertinent review of both clinical data and morphology through extended histotopographic study performed on 4 cases, their clinicopathologic characteristics were described. Male to female sex ratio was 4 : 2 and average age was 58.6 years. Clinical presentations or physical findings were not different from those of adenocarcinoma. Tumor masses of all cases were located in the body or phloric antrum. In all cases except for the early gastric carcinoma, each tumor was larger than 8 cm in long axis, and showed expansile and intraluminal endophytic growing tendencies with sharp margin. Simple Borrmann's macroscopic classification was applicable to none of 6 cases because of irregular and multicentric ulceration pattern and occasional submucosal growth which were reminiscent of those in submucosal tumors such as malignant lymphoma and leiomyosarcoma. Virtually four out of all six cases were diagnosed as submucosal tumor at the time of radiological examination. Microscopically three cases were composed of pure squamous cell carcinoma and remaining three contained both adeno and squamous components. Metastatic regional lymph nodes revealed only squamous element in case of pure squamous cell carcinoam, while both components were identified in adenosquamous carcinomas. Surrounding non-tumors mucosa exhibited moderate to severe degree of intestinal metaplasia accompanied by multifocal mucin pool formation. Growth pattern and surrounding mucosal changes as with older age prevalence of these tumors might express the biological behavior of intestinal type of gastric carcinoma.

5.Giant cell tumor in long bones: The significance of marginal sclerosis for the differential diagnosis.

Plain radrographs of thirty nine patients with giant cell tumor of long bone and CT scans of twenty patients among the thirty patients were reviewed retrospectively to evaluate the frequency and significance of sclerosis of the tumor margin. The sclerosis of the tumor margin was observed on plain radiographs in thirteen patients(33.3%) and they were located either on epiphyseal or on both epiphyseal or metaphyseal portion of the tumor. The authors concluded that the giant cell tumor should not be excluded from the differential entities eventhough the tumor has the marginal sclerosis.

8.Quality Assurance of Intraoperative Consultation Review Analysis of 2,392 frozen sections.

A retrospective quality assurance study of intraoperative consultation (frozen section) was carried out to assess the accuracy and to determine the reasons of discordance. Of 14,977 surgical pathology cases accessioned over a 6-month period in Asan Medical Center, frozen sections were done on 1,270 (8.5%) patients and 2,392 frozen sections (1.88 frozen sections/case) were performed. Discordance was noted in 106 cases (4.4%) and diagnosis was deferred in 26 cases (1.1%). All deferred cases were reviewed with the result of 53.8% justified and 46.2% unjustified. The discordant cases were divided into three categories as to their clinical significances: category A (no affect on patient care) 61.3%, B (minimal affect) 9.4%, and C (major affect) 29.2%. Of 31 category C cases, 7 cases were false positive and 24 cases were false negative. Misinterpretation (70.8%) was the leading cause of discordance, followed by sampling error (15.1%), failure to identify lesion (8.5%), and technical problem (5.7%). More than one-third (35.8%) of all discordances were of central nervous system cases. Total central nervous system cases were 403 (16.8%) with a significantly higher disordance rate (9.8%) and deferral rate (2.5%) in comparison to the other cases with 3.4% discordance rate and 0.8% deferral rate. There were 43 colorectal cancer cases of intraoperative consultation for adequacy of resectional margins. The surgical margins were between 0.4 cm and 28 cm (mean: 6.7 cm) away from the tumor and there was no tumor-positive case. The study indicates surgical pathology should 1) promote interpretative skills in cases involving minute fragments of neurosurgical cases, 2) defer the diagnosis and ask for more tissue on inadequate or inappropriate specimens and 3) give only gross opinions without unnecessary frozen section procedures in the event of simple, clear-cut cases.

9.Tinea Capitis of Adults in Taegu City for 11 Years ( 1978 ~ 1988 ).

Epiderniologic description of the tinea capitis of adults was made for the cases detected in Taegu city for 11 years from January 1978 to December 1988. 1. Totalnumber of adult patients(20 years or older) were 41(2.0%) out of 2,029 cases of all tiena capitis. Among 41 patients, 34 cases were detected between 1983 and 1988. 2. Clinically 24 patients were superficial tinea capitis and 17 patients were kerion celsi. 3. Age of the patients ranged from 21 to 85 years with an average of 58.5 years and mode interval of 61-70 years. Patients over 50 years of age accounted for 70.7% of all csses. 4. Sex distribution of the cases was 39 females(95.1%) and 2 males(4.9%). 5. Causative agents isolated from the patients were Microsporum canis in 36 cases, Trichophyton verrucosum in 2 cases, T. mentagrophytes, T. ferrugineum and T. rubrum in 1 case, respectively. 6. Solitary lesion was most common and alopecia was not prominent in most of the cases. The most frequent site of involvement was frontal and posterior scalp. At initial exarnination, 5 cases were misdiagnosed as seborreic dermatitis and 1 case as multiple folliculitis. 7. Sources of infection were presurned to be cat in ll cases, cattle in 2 cases and the rests were unidentified. Interfarnilial infection was observed in 5 cases.

10.Effects of Inspiratory Pressure Preset on Alveolar Gas Exchange Using Anesthetic Ventilator.

The study was undertaken to determine the most adequate tidal volume when used volume preset ventilator during anesthesia. The thirty patients were received controlled mechanical ventilation with constant inspiratory pressure of 10 cmH2O and respiratory frequency of 12/minute. The results were as follows: 1) The PH was 7.39±0.01 and it is within normal limit. 2) The PaCO2 was 34.0±0.6 mmHg and it is a slightly hyperventilatory state. 3) The PaO2 was 228.0±8.2 mmHg. 4) The Buffer base was 20.7±0.3 mEq/L and it is a slightly buffer base deficient state. From the above results. We concluded that if patients were fully relaxed during general anesthesia, it is desirable to maintain the inspiratory pressure of anesthetic mechanical ventilator to 10 cmH2O for adequate alveolar ventilation.
