1.Perioperative nursing care to patients with open angle glaucoma undergoing EX-PRESS drainage device implantation

Objective To investigate the key points in the care of patients with open angle glaucoma undergoing EX-PRESS drainage device implantation during perioperative period.Method Eighteen patients with open angle glaucoma underwent EX-PRESS glaucoma drainage device implantation and received individualized nursing care during perioperative period.Results Drainage device implantation was successfully performed in 18 patients.The mean operation time was(30.0±5.0)mins,ranged 25 to 45 mins.The postoperative intraocular pressure was stable.Conclusions EX-PRESS glaucoma drainage device implantation is an innovative and effective strategy in the treatment of open angle glaucoma.Comprehensive nursing during perioperative period is important to ensure the clinical efficacy.
2.Comparison of two in vitro gonadotrophin priming protocols in maturation treatment of patients with polycystic ovary syndrome

Objective To compare the therapeutic effect of two Methodsof in vitro oocyte maturation (IVM) technology with polycystic ovary syndrome (PCOS) patients in infertility treatment.MethodsRetrospective analysis was performed on patients with PCOS infertility who were stimulated with two different kinds of stimulating method of the IVM cycle in our center during April 2010 and August 2010.There were 20 patients in the temporary FSH stimulating group (group A) and 31 patients in the HCG stimulating group (group B).The number of oocytes maturation rate,fertilization rate,cleavage rate,excellent embryo rate,implantation rate,pregnancy rate per oocyte recycle,pregnancy rate per transfer and abortion rate of transfer were compared.ResultsThere was no significant difference between the two groups of age,duration of infertility,body mass index,based sex hormone levels by t test(P>0.05).There was no significant difference between the two groups of the number of oocytes retrieved,oocyte maturation rate,cleavage rate,and abortion rate of transfer(P>0.05).In group B,the fertilization rate was 80.3%,good-quality-embryo rate was 49.8%,implantation rate was 31.5%,pregnancy rate per oocyte recycle was 54.8%,which was significantly higher than in group A (73.9%,35.4%,12.5% and 25.0%,respectively).ConclusionsIf IVM was used to treat PCOS patients with infertility,HCG stimulating will be better than FSH stimulating,and it could result in a higher clinical pregnancy rate.
3.Construction and application of clinical nursing quality evaluation index system in patients of ophthalmology

Objective To investigate the scientific and objective quality management methods of ophthalmic nursing to improve the quality of ophthalmic clinical nursing. Methods According to the implemention details of review standards of tertiary special eye hospital (2011) and the management specification of clinical nursing of Guangdong province, combining with the characteristics of ophthalmic nursing to formulate the 21 indicators. The indicators had been applied in 12 wards to evaluate the nursing quality since 2013 each quarter. Using the Plato analysis method to analyze the data dynamically since 2013, then controlling and improving the nursing quality in view of the analysis result. The clinical nursing quality in the first half of 2013 was compared with the second half of 2013. Results The ophthalmic nursing indicators of 12 wards in the second half of 2013 were higher than the first half of 2013. Significant statistical difference was found in the implementation rate of periocular clean, cut fingernails preoperative, infusion system, identification, send medicine to the mouth, patients at high risk of falling drop of bed evaluation, wrist band awareness, patients′ satisfaction between two groups (χ2=4.115-12.308, P < 0.05 or 0.01). Conclusions Establishing and implementing the nursing quality evaluation indicators system of ophthalmology can evaluate the clinical nursing quality objectively and promote the continuous improvement of clinical nursing quality.
4.SIMPP analysis on learning diligence level of students

Academic diligence level can not only directly affect the academic performance of students, but also affect the individual's emotional and physical health. In this study, through a question-naire survey of university students, with the method of SIMPP and the concept of the micro system engi-neering in higher education teaching reform, the related factors of diligence and the college students' behavior mode with respect to diligence are systematically analyzed. Analysis shows that, students' diligence level is affected not only by inherent factors such as clear learning objectives, learning state and after-school learning time, but also by the school ethos, the influence factors of the curriculum and parents' re-quirements.
5.Nursing of patients undergoing cataract extraction and intraocular lens implantation of a multiple focus lens

Objective To investigate the key nursing points during and after phacoemulsification and intraocular implantation of multiple focus lens.Method Pre-and post-operative nursing was done to the patients undergoing cataract surgery for 42 eyes of 42 patients.Result All surgeries were well done without complication.The patients’visual acuity had significant improvement after operation as compared to the pre-operation(P<0.05).Conclusions Intraocular implantation with a multiple focus lens can markedly improve the patients’vision and enhance their life quality.It is important for a nurse to conduct mental care,health education and complete preoperative examination. After surgery,the disease conditions should be carefully monitored and instructs on eye protection should be done to the patients for the purpose of promoting their recovery.
6.Factors Associated with Postvitrectomy Endophthalmitis

OBJECTIVE To investigate factors associated with postvitrectomy endophthalmitis. METHODS Cases with postvitrectomy endophthalmitis in our hospital between 2003 and 2007 were collected and retrospectively reviewed. RESULTS All 14011 cases (eyes) underwent pars plana vitrectomy and postoperative endophthalmitis happened to seven cases (eyes). The incidence of infection was 0.047%. Confirmed microbiology was demonstrated in 4 of 7 cases (57.0%),in which Gram-positive cocci were found 75.0%. 71.4% Of cases with postvitrectomy endophthalmitis were older than 60. The infection was controlled in 6 cases after antibiotic treatment. One case with diabetes and culture confirmation of Gram-negative bacilli deteriorated even after the second vitrectomy,and evisceration was finally performed. CONCLUSIONS The predominant organism is Gram-positive cocci in postvitrectomy endophthalmitis. The risk factors include increasing age,presence of systemic diseases and immunodeficiency. Timely and effective antibiotic therapy is important to restore visual acuity for postvitrectomy endophthalmitis.
7.Primary breast diffuse large B-cell lymphoma:a clinicopathologic study and prognosis analy-sis of seven cases

Purpose To discuss the clinical pathological features, diagnosis and differential diagnosis, treatment and prognosis of pri-mary breast diffuse large B-cell lymphoma ( DLBCL) . Methods 7 cases of primary breast DLBCL were collected, their clinical path-ological characteristics and immunophenotypes were also observed, and the treatment and prognostic factors were discussed. Results All of the 7 patients were women, aged 28~75 years, with the median 51 years. 5 cases involved left breast, 2 cases were located in the right breast. Painless mammary mass and ipsilateral axillary lymphadenopathy were the commonest clinical manifestations. On mi-croscopic observation, tumor cells were large to medium-sized which characterized as diffuse infiltration between the lobules of mamma-ry gland, around the duct, interstitial and fat tissue, some were single file cord pattern. 1 case was ALK-1 positive DLBCL, 6 cases were DLBCL of the non special type. The immunophenotype showed 5 cases were of non-GCB, 2 cases were of GCB type. Ki-67 index were from 60% to 95%. According to Ann Arbor staging, 2 cases were stage I EA, 5 cases were stage II EA. IPI score:4 cases with 0 score, 3 cases with 1 score. Of 7 patients, one case was modified radical mastectomy, 4 cases were lumpectomy, 2 cases were diag-nosed by core needle biopsy. 2 patients died without chemotherapy, 5 patients were chemotherapy or radiotherapy for 4 to 6 cycles ( CHOP/R-CHOP) , 4 patients received complete response, one died. Patients were followed up from 1 to 114 months, the median fol-low-up time was 18 months. The 1 year OS rate was 57. 1%, 5 years OS rate was 14. 3%. Conclusion The diagnosis of primary breast DLBCL is confirmed by pathological biopsy and immunohistochemical markers. The immunophenotype was mainly non-GCB type. Comprehensive treatment including chemotherapy and radiotherapy is appropriate. The prognosis should be comprehensively eval-uated by multiple factors.
8.Jugular vein thrombosis, subclavian vein thrombosis and right brachiocephalic vein thrombosis after in vitro fertilization and embryo transfer: a case report.

To discuss the diagnosis and treatment of jugular vein thrombosis, subclavian vein thrombosis and the right brachiocephalic vein thrombosis after in vitro fertilization and embryo transfer (IVF-ET)cycles in clinical practice. The clinical data regarding a case of jugular vein thrombosis, subclavian vein and the right brachiocephalic vein thrombosis in IVF-ET were reviewed. Clinical characteristics, prevention and treatment of jugular vein thrombosis, subclavian vein and the right brachiocephalic vein thrombosis in IVF-ET were discussed. A woman with secondary infertility underwent an IVF cycle with prolonged protocol controlled ovarian hyperstimulation. The oestradial concentration was 2 495 pg/mL on the day of human chorionic goeadotrophin (hCG). Fifteen occytes were retrieved and 2 embryos were transferred. Nine days after the embryos were transferred, the patient had ascites,hydrothorax and fluid of pelvic cavity accumulating, and was hospitalized. The patient underwent volume expansion and paracentesis, and left the hospital 30 days after the embryo transfer. Her right neck had pain 43 days after the embryo transfer. B ultrasound showed jugular vein thrombosis, subclavian vein and the right brachiocephalic vein thrombosis. The patient underwent low molecular weight heparin anticoagulation and low molecular weight dextran expansion, and left hospital with symptoms improved. She had Caesarean section and had a healthy baby girl. The thrombosis in the IVF-ET was a rare and serious complication. Prevention of ovarian hyperstimulation syndrome (OHSS) may reduce the incidence. The patients had local pain, swelling, skin temperature increased, headache, neck pain, and had to be checked to determine whether there were blood clots. The main treatment was low molecular weight heparin anticoagulation and low molecular weight dextran expansion. Timely Cesarean section is recommended to ensure the safety of perinatal mother and child.
