The Journal of Practical Medicine 2014;(12):1955-1958
Objective To study the distribution and its clinical significance of hepatitis B virus genotypes and subtypes in HBV-infected patients in Hefei. Methods 192 serum samples from HBV-infected patients were collected to extract HBV DNA. HBV the genotypes and subtypes of HBV were detected by PCR. Results Among 192 HBV-infected patients, 82 had genotype B, 99 had genotype C, and 11 had B/C. In genotype B, 57 were subtype Ba, 8 were subtype Bj, and the remaining 17 were neither Ba nor Bj. In genotype C, 8 were subtype C1, 56 were subtype C2, and the other 35 were neither C1 nor C2. In genotype B/C, 6 were Ba/C2, 2 Bj/C2, 2 Ba/neither C1 nor C2, and one neither Ba nor Bj/C2. Tthe distribution of HBV genotypes and subtypes exhibited significant differences in the various clinical types; and it showed that genotype C was predominant among patients with liver cirrhosis and hepatocellular carcinoma , while genotype B was predominant in asymptomatic carriers. In addition, there were no differences in HBV genotype distribution among patient wih different gender; but the genotype distribution differed significantly between patients positive with HBeAg and those negative with HBeAg. Conclusions Subtypes Ba and C2 are predominant in HBV-infected patients in Hefei. Genotype C may be associated with the development of liver cirrhosis and hepatocellular carcinoma.

Objective To investigate the blood pressure circadian rhythm in patients with IgA nephropathy by ambulatory blood pressure monitoring and explore its role in the disease progression. Methods A cross sectional study was carried out.Blood pressure rhythm was studied by ambulatory 24-hour monitoring with a portable oscillometric recorder in selected patients with primary IgA nephropathy. The term dipper was described as blood pressure during night dropped at least 10% below daytime blood pressure. The term non-dipper referred to those in whom the nocturnal decline in blood pressure was less than 10%.Clinicopathological indices between dipper and non-dipper groups were compared. Results Ninety-three patients completed ambulatory blood pressure monitoring among whom 68 (73%) patients were non-dipper.The frequency of non-dipper was 70%,70% and 81% in the patients at chronic kidney disease stage 1,2 and 3 or more. The frequency did not differ among these three group patients (P=-0.587).77% of patients with hypertension and 69% of patients with normotension were non-dipper (P=0.373).The disappearance of blood pressure circadian rhythm in IgA nephropathy was not influenced by age, gender, blood pressure, proteinuria, renal function and renal pathology lesions.Among the patients who were followed up regularly for more than 12 months (n=54),patients in the dipper group had a trend of slower eGFR decline rate than those in non-dipper group albeit the difference was not significant (P=0.329).Subgroup analysis revealed that in patients with hypertension and non-dipper (n=29),the eGFR decline rate was much faster than that in dipper group[(-6.79±11.58 )vs (-0.34±1.74) ml ·min-1 ·(1.73 m2)-1·year-1,P=0.019]. Conclusions Most patients with IgA nephropathy present disappearance of blood pressure circadian rhythm, even among those at an early stage or without hypertension. The loss of blood pressure rhythm may be associated with a rapid renal function decline rate in those with hypertension.

Background and purpose:Currently, subjective questionaire is the most frequently used methods to evaluate swallowing dysfunctions after radiotherapy in nasopharyngeal carcinoma patients, while lacking of effective objective examinations. This study aimed to explore effective methods to evaluate swallowing dysfunctions after radiotherapy in nasopharyngeal carcinoma patients, and gain knowledge of the incidence and severity of swallowing dysfunctions. Methods: From Oct. 2013 to Dec. 2013, 128 consecutive outpatients with previously treated nasopharyngeal carcinoma received esophageal barium lfuoroscopy examination at there regularly follow-ups to evaluate swallowing function. Among these patients, 89 were primary treated with intensity modulated radiation therapy (IMRT) and 39 with conventional radiotherapy (CRT). In this study, each patient received esophageal barium lfuoroscopy examination for 3 times with thin, thick and pasty barium and were dynamically observed using X-ray fluoroscopy from front and lateral direction. Swallowing dysfunctions were defined as follows:①The bolus could not be swallowed and blocked in the mouth;②The dilute barium diverted to the glottis or trachea;③Residual barium delayed in the pyriform sinus and vallecula;④The movement of the hyoid bone or epiglottis were restricted;⑤Bolus prolong through the pharynx;⑥Barium slowed down when went though the esophageal entrance. Results:Of the 128 patients, incidence of dysphagia was 60.2%for the entire cohort, 52.8%for IMRT group and 76.9%for CRT group. Incidence of dysphagia for IMRT group was signiifcantly lower than CRT group (P=0.018). Dysphagia incidence within 1 year, 1 to 2 years and more than 2 years after RT were 63.1%, 33.3%and 69.0%, respectively (P=0.019). Conclusion:There was a high incidence of swallowing dysfunction for the nasopharyngeal carcinoma patients treated with radiotherapy and dysphagia incidence decreased when treated with IMRT. Esophageal barium lfuoroscopy examination is objective method to evaluate the incidence and severity of the swallowing dysfunction.

BACKGROUND:Mechanical properties of the blood vessels include axial and radial tension which is important to guarantee the tissue perfusion and its pressure. OBJECTIVE:To test and analyze stretching and puncture parameters that can ful y characterize the performance of blood vessels, so as to predict the health condition of the vessels transplanted into recipients. METHODS:Mechanical properties as circumferential tension, axial tension and puncture were tested on the porcine aorta, and then pathological and staining analysis was done to explore whether freeze-dried vessels can withstand the blood pressure in al directions after transplanted into the body. RESULTS AND CONCLUSION:Specific trend on the curves of freeze-dried vessels during circumferential tension, axial tension and puncture process was closer to that of fresh vessel curves, suggesting that structural components and arrangement of the freeze-dried vessels were not changed greatly. Meanwhile, the microscopic observations of slice and tensile section were taken into account so that the reason was studied and explained for the mechanical properties of the experimental data and results. The macroscopic mechanical result was analyzed based on the change of microstructure. These findings indicate that the vacuum freeze-drying is a better method to maintain the performance of blood vessels.

Fifty five patients with alcohol induced-mental disorder (study group) and 43 local inhabitants without history of alcohol abuse (control group) were surveyed with family environment scale (FES-CV) and generic quality of life inventory-74 (GQOLI-74). The total score and the scores of all dimensions except material life in GQOLI-74 of study group were significantly lower than those of control group(P <0. 05). Compared with control group, the scores in FES of study group were lower for factors of cohesion, expressiveness, active-recreational orientation, moral-religious emphasis and organization in the patient's family, while the scores for conflict and control were higher( P < 0. 05 or P < 0. 01 ). The results indicate that family environment is closely correlated with quality of life in patients with alcohol-induced mental disorder, and family therapy would improve their quality of life.
