1.Studies on the Internal Relationship between Traditional Identification Term in Chinese Medicine and Phar-maceutical Botany

OBJECTIVE:To enrich the identification diversity of traditional Chinese medicine(TCM),and provide theoretical guidance for the quality evaluation of TCM. METHODS:According to literature references and traditional identification experienc-es,characteristics including medicinal shape,size,color and lustre,surface,texture,section,odor and other aspects were identi-fied by sense organs such as eyes,hands,nose and mouth. The vivid traditional identification term were obtained through systemat-ic summarization in order to explore the internal relationship with pharmaceutical botany. RESULTS&CONCLUSIONS:As the sim-plest identification method,traditional identification method can rapidly identify the species and quality of TCM,evaluate the quali-ty,and has great significance to solve the security issues of clinical medication and health care in daily life. There was a correlation between the traditional identification and botanical research,which could be able to provide theoretical guidance to characters identi-fication and quality evaluation of TCM.
2.Multislice spiral CT of congenital bridging bronchus

Objective To assess the diagnostic value of MSCT in congenital bridging bronchus (BB).Methods Three-dimensional (3D) reconstructed CT images including MPR,MinIP,MIP,VR were respectively analyzed in 12 patients with congenital bridging bronchus on postprocessing workstation.Results Six of 12 BB patients were type Ⅰ bridging bronchus which originated from the left main bronchus at the level of fourth and fifth thoracic vertebral body,while the right bronchus was originated from the normal carina at the level of second and third thoracic vertebral body.The average angle of normal carina was about 59.2°,and the average angle of pseudocarina where BB originated from the left main bronchus was about 108.8°.The left main bronchus proximal to the origin of BB showed significant stenosis,with 1-2 mm width and 17 mm length in the involved segment.Six cases were type Ⅱ bridging bronchus,the right bronchus was absent in normal carina,BB originated at the level of fourth and fifth thoracic vertebral body,the average angle of pseudocarina was about 131°,the whole right lung was supplied by BB.The lower portion of trachea had stenosis in all 6 patients with 2-3 mm width and 30 mm length in the involved segment The lower portion of the trachea was found moving to the left in 4 patients and the left pulmonary artery sling was found in 2 patients.Conclusion MSCT can afford a definite diagnosis of BB by showing the morphology of trachea,bronchus,carina and relationship with surrounding organs with 3D reconstructions.
3.Thermolability in the process of safflower infusion solution by HPLC fingerprint

AIM:To study incorporate changes of compound in safflower infusion solution before and after heating by HPLC fingerprint, in order to provide reference of drug’s safety evaluation. METHODS: Hydroxysafflor yellow A was used as reference substance, safflower infusion solution was detected by HPLC fingerprint in the whole manufacturing process from original raw material、intermediate to finished product.The detector temperature was set at room temperature、100 ℃、115 ℃,and then the change in the peak’ disappearance and appearance was observed and the peak area’ decrease and increase showed by characteristic absorption peak of every sample product at different temperature. RESULTS: As the temperature rose to 105 ℃,new peaks appeared at the retention time of 7、26-27、44 minutes,original peaks would disappear at the time 8、36minutes.As the temperature continued to rise to 115 ℃,the peak at the time of 7、10 minutes disappeared again.the peak area increased during the temperature rising at retention the time of 31-32、44 minutes and decreased at the retention time of 37 minutes. CONCLUSION: It indicates that,heating safflower infusion solution would produce a new compound. As the temperature continues to rise,the new compound would be destroyed.
4.Study on the Protein Extraction Method in Angelicae sinensis Seed and Its SDS-PAGE Technical System

OBJECTIVE:To establish the technical system that is suitable for protein extracting in Angelicae sinensis seed and sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE),and provide technical support for detecting the protein quality and variety purity. METHODS:Using protein content and number of electrophoretic bands as indexes,8 methods,includ-ing voncentrated gel method,salt-soluble protein method,electrode buffer method,dimercaptosyl alcohol (DTT) method,urea method,mercaptoethanol method,trimethylolaminomethane (Tris) method,and acetone precipitation method,were conducted to extract the protein in A. sinensis seed and screen the optimal extraction method. Then based on optimal extraction method,effects of different materials-lipid ratios,sample dilution times(sample volume)and separate gel concentration on SDS-PAGE were investi-gated. RESULTS:Mercaptoethanol method extracted the highest protein contents(29.931 mg/g),with many electrophoretic bands and clear background. When mercaptoethanol method was used as optimal extraction method,electrophoresis effects were the best in the conditions of materials-lipid ratio of 1:10,sample volume of 5 times,separate gel concentration of 15%,which obtained 18 bands totally. CONCLUSIONS:Established protein extraction method and SDS-PAGE technical system are suitable for detecting the purity of A. sinensis seed.
5.Development of an ELISA to quantitatively determine the nucleoprotein of rabies virus

Objective To develop a double-antibody sandwich ELISA for determining the concentration of nucleoprotein (NP) of rabies virus in various products of rabies vaccine.Methods The purified rabies antibodies from a rabbit were used to coat microwell plates. Horseradish peroxidase-conjugated anti-NP monoclonal antibody was used to probe the NP bound to coated antibodies.The assay was used to quantitatively determine the concentration of NP of rabies virus.Results The results showed that the coefficient of linear correlation was higher than 0.97.The optimal linear range was 0.000625~0.01 IU/ml and the detection limit was 0.000625 IU/ml. The recovery rate was 102~109% and the coefficient of variation was only 7.2%~9.4%.No cross reactions were observed with bovine serum,bovine serum albumin,ovalbumin,refined solution of influenza vaccine,encephalitis B vaccine,and hepatitis A vaccine.Conclusions The results indicated that the assay is specific,sensitive,accurate,reproducible,and stable,and could be suitable for quantitative determination of different rabies vaccine's processes products.
6.Predictive value of Downes score on respiratory failure in term neonates

Objective To explore the clinical significance of Downes score in predicting respiratory failure (RF) in term neonates.Methods From June 1, 2011 to December 31, 2013, 243 hospitalized term neonates with dyspnea, who were born at 37 to 42 gestational age and admitted to the hospital within 24 h after birth, were selected from Neonatal Medical Center of Huai'an Maternity and Child Healthcare.Downes scores were evaluated based on five indicators including respiratory rate, skin color, three depressions sign, moaning and lung breath sounds at the time of admission.RF was diagnosed based on clinical menifestations and radial arterial blood gas analysis.Receiver operating characteristic (ROC) curve analysis was performed to evaluate the predictive value of Downes score on RF.Results The average Downes score of the 243 cases was 6.57±0.86 and the RF rate was 69.6% (169/243).The RF rate was 93.4% (169/181) when Downes score ≥ 4.5, with the sensitivity, specificity, misdiagnostic rate and missed diagnositic rate of 100.0%, 83.8%, 16.2% and 0.0%, respectively.While, the RF rate was 80.7% (130/161) when Downes score ≥ 5.5, with the sensitivity, specificity, misdiagnostic rate and missed diagnositic rate of 76.9%, 58.1%, 41.9% and 23.1%, respectively.When Downes score ≥ 6.5, the incidence of RF was 97.6% (123/126), with the sensitivity, specificity, misdiagnositic rate and missed diagnositic rate of 72.8%, 95.9%, 4.1% and 27.2%, respectively.The cutoff value was 6.5 according to the Youden Index, which with lower rate of misdiagnosis and missed diagnosis.When Downes score ≥ 7.5, the incidence of RF was 100.0%(22/22), with the sensitivity, specificity, misdiagnositic rate and missed diagnosis rate of 13.1%, 100.0%, 0.0% and 87.0%, respectively.The area under the ROC curve was 0.906 (95%CI: 0.866-0.955).The higher the Downes score, the greater the possibility of RF (P=0.000).Conclusions Dynamic Downes scores in term neonates with dyspnea could predict the risk of RF.The higher the Downes score, the greater the possibility of RF.
7.Study on Physiological Mechanisms in Frozen Storage to Reduce Early Bolting of Angelica Sinensis

This article was aimed to study the physiological mechanisms in frozen storage to reduce early bolting of Angelica sinensis. By analyzing the root soluble sugar, soluble protein, MDA content changes before and after -10oC processing of the Angelica sinensis seedlings of different size from different sources , the physiological mechanisms in frozen storage reducing early bolting of A . sinensis had been studied. The results indicated that the greater seedlings , the higher content of soluble sugar , lower protein content and higher MDA . There were no obvious differences on the soluble sugar , soluble protein and MDA content of the same size seedlings from different sources . After -10oC freezing , the content of soluble sugar and protein in the seedlings with the same size were decreased. Meanwhile, the content of MDA was increased. It was concluded that the content of soluble sugar, soluble protein and MDA were obviously related to the size of Angelica sinensis seedlings from the same source before and after low temperature process .
8.Effect of humanistic care for patients in the intensive care unit

Objective To analyze the effect of the application of the concept of humane care in the ICU patient care. Methods A total of 126 patients in our hospital ICU from December 2013 to February 2015 were treated for analysis objects, and they were divided into two groups, the control group and observation group, and the control group was given routine care, observation group was given humane care treatments, including environmental, psychological and physio-logical care, The nursing satisfaction, physilolgical discomfort and adverse psychological reactions of two groups were compared. Results The satisfaction rate of observation group was 98.41%, the control group was 88.89%;The incidence of physiological discomfort of patients in the observation group was lower than the control group, there was a significant difference(P<0.05);The adverse mental reaction rate of observation group was significantly lower than the control group (P<0.05). Conclusion Application in the ICU nursing care and humane care philosophy helps to improve results, should be widely used.
9.Clinical effects of Hybrid knife versus triangular tip on peroral endoscopic myotomy for patients with achalasia: a retrospective cohort study

Objective To compare the clinical efficacy and safety of Hybrid knife versus triangular tip during peroral endoscopic myotomy(POEM) for patients with achalasia of cardia. Methods Data of patients with achalasia of cardia who received POEM in Nanfang Hospital, Southern Medical University from June 2012 to July 2014 were collected and divided into the Hybrid knife group ( using Hybrid knife) and triangular tip group ( using injection needle and triangular tip). Procedure-related parameters, symptom relief, and adverse events were compared between the two groups. Results A total of 57 patients were selected, including 25 patients in the Hybrid knife group and 32 in the triangular tip group. There were no significant differences on baseline characteristics between the two groups ( P>0. 05). The mean procedure time was shorter in the Hybrid knife group than that in the triangular tip group (55. 3±17. 7 min VS 69. 5± 9. 4 min, P=0. 038). The mean frequency of devices exchange was less in the Hybrid knife group than that in the triangular tip group (4. 5±1. 5 VS 10. 7±1. 7, P=0. 000). No serious complications occurred during operation and periodical follow-up in both groups. At one-year follow-up, the treatment success rate was 92. 0%(23/25) in the Hybrid knife group and 96. 9%(31/32) in the triangular tip group (P=0. 576). Conclusion Using Hybrid knife in POEM can shorten procedural time and achieve similar treatment success rate compared to triangular tip.
10.Training can promote the recovery of gastrointestinal function after radical cystectomy for elderly bladder cancer patients

Objective:To explore the effect of comprehensive gastrointestinal training on the recovery of gastrointestinal function after radical cystectomy (Bricker′s operation).Methods:Seventy-one elderly bladder cancer patients who had undergone Bricker′s operation were randomly divided into a control group of 34 and an observation group of 37. Both groups were given routine treatment, while the observation group was additionally provided with comprehensive gastrointestinal training immediately after the operation. Plasma gastrin was measured before the operation and on the 1st and 3rd day afterward. The return of peristaltic sounds, the exhausting and defecation time, the incidence of intestinal obstruction and the average hospital stay were recorded. On the 7th, 14th, 21st and 28th day after the operation, both groups′ gastrointestinal functioning was scored by using Gastrointestinal Symptom Rating Scale (GSRS).Results:The levels of plasma gastrin in the observation group were significantly higher than before the surgery and significantly higher than those in the control group on the 1st and 3rd day after the surgery, though there had been no significant difference between the two groups before the operation. Compared with the control group, peristaltic sounds returned significantly earlier in the observation group (after 1.22±0.15d), and the group′s exhaust time (1.88±0.22d) and first defecation time (2.95±0.19d) were also better. The incidence of intestinal obstruction (23.53%) was significantly lower in the observation group as well. The observation group recorded lower average GSRS values than the control group throughout the observation and follow-up periods, and their average hospital stay was significantly shorter.Conclusion:Comprehensive gastrointestinal training can effectively promote recovery after radical bladder cystectomy.
