1.Hemodynamic Study of Patients with Budd-Chiari Syndrome by Duplex Doppler Ultrasound

The hemodynamic features of the involved blood vessels in 11 patients with Budd-Chiari syndrome were studied by duplex Doppler SODography.The results showed that there were abnomalities in the direction,velocity,nature and spectral patterns of the blood flows of the involved blood vessels,It is suggested that duplex Doppler ultrasound examination can nonmvasively obtain the hemodynarnic data of inferior vena cava,hepatic veins and portal vein and is very valuable in the clinical analysis,diagnosis,treatment and evaluation of the theurapeutic effect of Budd-Chiari syndrome.

OBJECTIVE:To improve the quality of pharmaceutical care and to provide bases for outpatients and medical staff in drug informatin inquiry.METHODS:Information about drug package inserts wre collected and the drug-inquiry system was designed using the database management function of Access.RESULTS & CONCLUSIONS:The drug-inquiry system is easy to operate,clear in interface,humanization in operation,and it faciliats individualized drug information inquiry through daily mantinance and satisfies the requirments for outpatient drug information inquiry.
3.Effects of EPO on expression of TGF-β1 and ColⅠin myocardium of liver cirrhosis rats

Aim To observe the effects of liver cirrho-sis on the expression of transforming growth factor-β1 ( TGF-β1 ) and ColⅠin rat myocardium and interven-tion of erythropoietin ( EPO ) . Methods Thirty-six male Sprague-Dasley rats were randomly divided into three groups:control group, liver cirrhosis group and EPO group, then the cardic hemodynamic parameters in vivo and levels of serum lactate dehydrogenase ( LDH ) as well as creatine kinase isoenzyme ( CK-MB) were measured. With Masson′s trichrome stain, changes of collagen formation of myocardial tissue in different groups were observed. Also the mRNA ex-pressions of TGF-β1 and ColⅠin myocardium were de-tected by RT-PCR. Results In contrast to control group, rats in liver cirrhosis group showed a decline in systolic and diastolic function of left ventricule, rising myocardial enzyme, a distinct increase of cardiac colla-gen deposition, as well as an elevation of TGF-β1 and ColⅠmRNA expressions. In contrast to liver cirrhosis group, rats in EPO group demonstrated an improve-ment in systolic and diastolic function of left ventricule as well as in cardiac collagen deposition, and a de-crease in both myocardial enzyme and TGF-β1 and ColⅠmRNA expressions. Conclusion Liver cirrhosis can lead to the changes of myocardial structure and function in rats,and it can accelerate myocardial inter-stitial fibrosis; EPO can protect the myocardial injury in liver cirrhosis rats.
4.Reliability and validity of Chinese version of housebound scale in Chinese elderly people

Objective To study the reliability and validity of Chinese version of housebound scale in Chinese elderly people in community.Methods The housebound scale was amended and translated according to the Procedure of the European organization for Research and Treatment of Cancer (EORTC) Quality of Life Group.Include two scales,which is whether is housebound and the reason of housebound.The reliability and validity of Chinese version of housebound scale were tested in a sample of 860 Chinese elderly people in two communities among 127 communities in Tangshan from October 2012 to March 2013.Reliability was evaluated using the splithalf reliability and internal consistency.Validity was assessed by content validity,discriminate validity and construct validity.Results The Cronbach’s α coefficient of the first subscales was 0.743,the split-half reliability was 0.856,the convergent validity ratio was 0.960,and the scale had good discriminate validity (P < 0.01).The Cronbach's αcoefficient of the second subscales was 0.853,the split-half reliability was 0.816,the convergent validity ratio was 0.910.Four factors body and mental factors,environmental factors,psychological and social factors,listening factors were extracted by a factor analysis,which explained 48.37% of the total variance.The loading value of each items in their respective common factor ranged from 0.42 to 0.82.Conclusion The housebound scale has better reliability and validity,which is available for the evaluation of housebound amnong elderly people.
5.Changes of sarcoplasmic reticulum calcium ATPase, titin, and nebulin expressions in the diaphragm of rats with liver cirrhosis.

OBJECTIVETo investigate the molecular mechanisms of diaphragm injury in rats with liver cirrhosis.
METHODSThirty adult male Sprague-Dawley rats were randomized into control group (n=10) and carbon tetrachloride-induced liver cirrhosis group (LC group, n=20). In the 9th week, the rat body weight and diaphragm to body weight ratio were measured, and the parameters of diaphragm contractility including peak twitch tension (Pt), maximum tetanic tension (Po), time to peak contraction (CT), half relaxation time (1/2RT), and force-frequency curve were assessed using a Medlab-U/4C biological signal collecting system. The activities of superoxide dismutase (SOD), succinic dehydrogenase (SDH) and myeloperoxidase (MPO) and malondiadehyde (MDA) content in the diaphragm were detected. The mRNA expression levels of sarcoplasmic reticulum calcium ATPase (SERCA) and cytoskeletal proteins (titin and nebulin) in the diaphragm were detected by RT-PCR, and the diaphragm ultrastructure was examined with electron microscopy.
RESULTSCompared with those in the control group, body weight, diaphragm to body weight ratio, Pt, Po, and tetanic force under the stimulus frequency of 10, 20, 40, 60, 100 Hz were all significantly decreased (P<0.01), while CT and 1/2RT were significantly prolonged in LC group (P<0.01). SOD and SDH activities were significantly lowered (P<0.01) while the contents of MDA and MPO activity were significantly increased in LC group (P<0.01) with significantly decreased SERCA, titin and nebulin mRNA expressions in the diaphragm (P<0.01). Electron microscopy of the diaphragm in LC group revealed myofibrillar degeneration, absence of the Z line, and mitochondria swelling and edema.
CONCLUSIONLiver cirrhosis increases free radicals and aggravates inflammatory response and lipid peroxidation in the diaphragm, thus leading to mitochondrial damages and decreased expressions of cytoskeletal proteins and SERCA to cause diaphragmatic dysfunction.

6.Outcome of allogeneic hematopoietic stem cell transplantation for relapsed/refractory acute myeloid leukemia in NR status

Objective To explore the outcome of allogeneic hematopoietic stem cell transplantation (allo-HSCT) for relapsed/refractory acute myeloid leukemia (AML) patients in nonremission (NR) status and its related risk factors.Methods Thirty-nine relapsed/refractory AML patients in NR status who received allo-HSCT between Jan.2006 and Dec.2016 were retrospectively analyzed.Major end points of study included overall survival (OS),disease free survival (DFS),and relapse rate.Results All patients achieved hematopoietic reconstitution.Median time to neutrophil and platelet engraftment was 13 (11 20) and 16 (10-58) days,respectively.Acute graft-versus-host disease (aGVHD) occurred in 25 (64.1%) patients,and 11 (31.4%) patients developed chronic GVHD.During a median follow-up period of 9.1 (1.6-93.8) months,11 (28.9%) cases survived,10(26.3%) survived without leukemia,and 21 (53.8%) relapsed.The estimated 2 year OS and DFS were 30.0% ± 8.0% and 26.7% ± 7.7%,and cumulative incidence of relapse and transplantation related mortality at 2 years was 56.63% (95% CI 37.84%-71.71%),19.7% (95% CI 8.3%-34.5%),respectively.The multivariate analysis revealed that the number of bone marrow blasts≥25% or any level of blasts in peripheral blood was significantly associated with worse OS (HR =11.91,P=0.003),DFS (HR =10.75,P =0.002) and higher rate of relapse (70.83% versus 20.22%,P =0.002).In addition,the development of grade Ⅱ-Ⅳ aGVHD also predicted an inferior OS (HR =3.18,P =0.039).Conclusion Salvage therapy with allo-HSCT can induce long-term survival in part refractory/relapsed AML patients.Decrease in the pre-transplant disease burden is the key to reduce relapse and improve survival.
7.Effect of pre-transplantation iron overload on the outcomes of allogeneic hematopoietic stem cell transplantation patients with non-hepatitis-related severe aplastic anemia

Objective To explore the effect of iron overload on the outcomes of allogeneic hematopoietic stem cell transplantation (allo-HSCT) patiems with non-hepatitis related severe aplastic anemia (SAA).Methods The clinical data were retrospectively analyzed for 98 patients with non-hepatitis related SAA undergoing allo-HSCT from July 2012 to late July 2018 at a single center.Serum ferritin (SF) was measured within 2 momhs before HSCT.They were divided into iron overload (SF>800.0 ng/ml,n =49) and control (SF<800.0 ng/ml,n =49) groups according to SF level.Overall survival (OS),hematopoietic reconstitution and common complications after allo-HSCT were analyzed.Results The median pre-transplantation SF value was 798.7 (52.0-11060.01) ng/ml.Patients with pre-transplantation iron overload had a higher incidence of cytomegaloviremia (P =0.041),delayed recovery of neutrophil/platelet (P =0.001,P =0.005 respectively) and transfusion-dependence in donor-recipient blood group-matched patients (P =0.043) after allo-HSCT.The 3-year OS was (65.1 ± 7.1) % in iron overload group and (93.3 ± 3.7) % in control group (P =0.001).Multivariate analysis indicated that 3-year OS was independently correlated with pre-transplantation iron overload (P =0.022),blood group of donor & recipient (P =0.015),early bacteremia (P=0.003) and cytomegaloviremia (P =0.003).Conclusions Iron overload is common in patients with non-hepatitis-related SAA before transplantation.Pre-transplantation iron overload has a significant impact on OS,hematopoietic reconstitution and cytomegaloviremia after allo
8.Establishment of a method for the evaluation of emergency granulopoiesis in mouse bone marrow with EdU

Objective To label granulocytes in a state of differentiation in mouse bone marrow (BM) with EdU (5-ethynyl-2′-deoxyuridine) for further understanding the changes in granulocyte produc-tion at different stages of differentiation during inflammation. Methods C57BL/6 mice were intraperitoneal-ly (i.p.) injected with EdU and heat-inactivated Escherichia coli(HI E.coli). BM cells were harvested at different time points after HI E.coli injection and then stained with fluorescent-conjugated antibodies(Abs). Myeloblasts,promyelocytes,myelocytes, metamyelocytes and band and segmented neutrophils were identi-fied by fluorescence-activated cell sorting(FACS). The percentage of EdU-positive cells in each population was recorded. Results The percentage of EdU-positive myeloblasts in mice increased by 10.0% at 24 h af-ter intraperitoneal injection with HI E.coli,but decreased by 75.0% and 23.0% at 48 h and 72 h,respec-tively. The percentage of EdU-positive promyelocytes declined by 23.0%,54.5%,64.3% and 77.8% at 24 h,48 h,72 h and 96 h,respectively. The percentage of EdU-positive myelocytes increased by 60.0% and 10.0% at 24 h and 48 h,but decreased by 80.0% and 90.0% at 72 h and 96 h. The percentage of EdU-positive metamyelocytes increased by 50.0% at 24 h,but decreased by 33.3%,61.5% and 66.7% at 48 h,72 h and 96 h. The percentage of EdU-positive band and segmented cells increased by 14.0% at 24 h,but decreased by 50.0%, 77.8% and 88.0% at 48 h, 72 h and 96 h. Conclusion Emergency granulopoiesis occurred 24 h after the establishment of HI E.coli-induced model of acute peritonitis, which meant that the proliferation of myeloid precursor cells,especially that of myelocytes and metamyelocytes,was accelerated and resulted in increasing number of mature neutrophils immigrating to sites of inflammation.
