2.A Case of Pulmonary Infarction Probably Related with Henoch-Schönlein Purpura.

We report a case of pulmonary infarction probably related with Henoch-Schönlein purpura, which presented with purpura on both lower extremities in a 27-year-old woman. The purpura had developed 4 days previously and it had no itching or tend'erness. She complained of mild fatigue, chest tightness, but no dyspnea or hemoptysis. On routine examination, chest radiography showed a poorly defined nodular opacity on the left lung field, and it was diagnosed by a follow up chest CT as a pulmonary infarction secondary to pulmonary vasculitis.

3.Surgical Extent of Thyroidectomy for Papillary Thyroid Microcarcinoma.

The incidence of PTMC (papillary thyroid microcarcinoma) has rapidly increased recently due to the application of ultrasonography to the thyroid. The good prognosis of PTMC is well known with a mortality rate of less than 1%. However, there is controversy about the surgical extent of thyroidectomy for PTMC patients between surgeons and endocrinologists due to differences in understanding the clinical properties of PTMC, while having a difference in basic concepts in the treatment and follow up strategy for PTMC patients. Total thyroidectomy is recommended for PTMC patients because there is no major difference in the rate of lymph node metastasis, extrathyroidal extension, multiplicity between the PTMC and PTC over 1 cm in size and although rare, occasional distant metastasis and mortality cases could be developed. However, there is no evidence of benefit of total thyroidectomy for the survival rate of PTMC patients. The microscopic lymph node metastasis and extrathyroidal extension are not prognostic factors for the survival or recurrence in PTMC. The clinical lateral neck lymph node metastasis and multiplicity has been proposed as valuable prognostic factors in micropapillary carcinoma and these factors could be assessed accurately by ultrasonography preoperatively. A decision on the proper extent of thyroidectomy could be possible in most PTMC patients. This article summarizes available data and concludes that routine total thyroidectomy for PTMC patients is not rational.

4.Three Cases of Bee-sting Granuloma.

Most insects including the honeybee cause erythema and edema at the site of a sting in non-allergic individuals. This reaction usually resolves within a few days. However, in certain cases, the persistence of inciting materials in the cutaneous tissue may promote a local inflammatory foreign body reaction. Localized nodular lesions were seen in our two patients and erythemstous patches were present in one patient due to rarely encountered complications induced by retained sting materials. In these cases histopathology showed multiple foci of foreign body granuloma containing yellow refraetile material in the center surrounded by histiocytes and foreign body giant cells. The granulomatous skin lesions in these patients were treated with intralesional injections of triamcinolone acetonide and partial excisions of the nodular masses were performed.

5.A Case of Tubulovillous Adenoma Involving Ampulla of Vater, which Recurred after Local Excision.

The villous adenoma of the duodenum is a rare disease and a considerable portion of the cases are known to be associated with malignancy. Although the diagnosis can be made with duodenoscopy, some cases showed false negative rate for malignancy detection with endoscopic biopsy only. So Whipple's operation is preferred than local excision. We experienced a case of tubulovillous adenoma involving ampulla of Vater, which recurred after local excision. So we report a case of tubulovillous adenoma involving ampulla of Vater with relevant literature.

6.A Case of Plasma Cell Balanitis Controlled by Fusidic Acid Cream.

In 1952, Zoon described eight cases of benign circumscribed chronic balanitis characterized by an extensive infiltration of plasma cells with no evidence of dysplasia of the overlying epidermis. Plasma cell balanitis can often be confused clinically with other conditions, such as erythroplasia of Queyrat, fixed drug eruptions, secondary syphilis, candidiasis and Reiters disease. We report a case of plasma cell balanitis in a 65-year-old man. He complained of a single, red, shiny and smooth patch involving the glans penis and adjacent prepuce. This patch was unresponsive to systemic and topical steroid treatment. Laboratory studies were negative or within the normal range. Histopathological findings showed a band-like mainly plasmacytic inflammatory infiltrate of the upper dermis. This patient was treated once daily with 2% fusidic acid cream topically for 5 weeks. The lesions resolved and no recurrence was observed during 2 years of follow-up.

7.2 Cases of Surgical Experience of Secondary Hyperparathyroidism.

Secondary hyperparathyroidism is the condition is which parathyroid hormone(PTH) is over secreted to compensate for a chronically low serum calcium level and chronic renal failure is the most common cause. In 1934, Albreight reported an association between hyperparathyroidism and the chronic renal failure and in 1960, Stanbury first reported subtotal parathyroidectomy for the treatment of secondary hyperparathyroidism. The physiologic mechanism leading to secondary hyperparathyroidism in the chronic renal failure are well known and relatively well controled with medical management, but sometimes may necessitate surgical intervention in medically intractable cases. In Korea, the surgery for secondry hyperparathyroidism is not frequently performed yet as in western countries. We experienced two cases of secondary hyperparathyroidism recently and report its results of subtotal parathyrodiectomy.

8.The role of mammography on breast cancer patients in a hospital setting.

From Oct. 1, 1983 to May 31, 1988, 6198 mammograms were taken and 443 mastectomies were performed at the Yonsei University Hospital, Seoul, Korea. Of these, 191 cases had preoperative mammography performed prior to surgery (study group) and 252 cases underwent mastectomies without preoperative mammography (control group). There were no significant differences between the two groups in pathological stages, axillary lymph node metastasis and size of the tumor, suggesting that little benefit is derived from the preoperative mammography in the hospital setting. However, there were a number of other findings, including a 14% incidence of multiple foci and localization of non-palpable cancer in 3% of cases, that could help us to make therapeutic plans and strategies for these patients.

9.Ectopic ACTH Syndrome Due to Pheochromocytoma: A case report.

A 28-year old female was diagnosed preoperatively to have a pheochromocytoma producing ACTH at other hospital. However the operation was delayed due to poor general condition and abnormal liver function. She revealed severe hypercortisolism, hypokalemic metabolic alkalosis, glucose intolerance and showed rapid downhill course. She was transferred to our hospital for operation but septic complications were strarted already. She was not recovered in spite of desperate operation and died just after moribund discharge. ACTH producing pheochromocytoma is uncommon and literature review indicated high mortality rates due to improper preoperative diagnosis. Proper preoperative recognition and early surgical management is mandatory for better results.

10.Chylous Fistula after Surgery for Papillary Thyroid Carcinoma.

PURPOSE: Most of postoperative chylous fistula in the neck occur after lateral neck lymph node dissection. However we experienced chylous fistulas in the central neck as well as lateral neck after surgery for papillary thyroid carcinoma. Herein we reviewed our experience of chylous fistula and tried to make guideline for the decision of optimal treatment in the early period of chylous fistula. METHODS: We retrospectively reviewed our thyroidectomy cases for the papillary thyroid carcinoma with central neck node dissection (n: 1220) and left neck node dissection (n: 149) over a period of 6years. In 17 patients, a chylous fistula was occurred, 8 in the lateral neck, 9 in the central neck. The treatment method, daily output, and the hospital course of the chylous fistula were analysed. RESULTS: The incidence of chylous fistula after lymph node dissection in the central neck and lateral neck was 0.7% and 5.4% respectively. All 9 central neck fistulas were successfully treated with conservative treatment . 6 lateral neck fistulas were also treated successfully with conservative treatment including medium chain triglyceride treatment and compression dressing. In 2 lateral neck fistulas, operative management was required, one due to poor response to conservative management and metabolic derrangement, another one due to large amount of daily output in the early post operative days. The maximal daily output of conservative management group and operative management group were below 250 cc/day and over 1,800 cc/day respectively. CONCLUSION: The chylous fistula in the neck could be occurred not only after lateral neck dissection but also after central neck dissection, although the clinical course of central neck fistula was relatively benign. Most of chylous fistulas could be treated conservatively. However, in the early high output fistula (over 1,800 cc/day) cases, prompt operative management should be considered for the prevention of metabolic derrangement and shortening the hospital course.
