3.Mitomycin C Single Soaking during Surgery for Primary Pterygium.

Complications of mitomycin C soaking method during pterygium surgery and its effect on recurrence rate were compared with postoperative use of topical mitomycin C. Sixty three patients(seventy three eyes) with primary pterygium received surgery with the bared sclera method. Thirty nine eyes randomly chosen were treated with cotton applicator soaked with 0.2mg/ml mitomycin C for 5 minutes after the excision of pterygium(soaking group). Thirty four eyes underwent the same surgical procedure and received topical 0.2mg/ml mitomycin C postoperatively, twice a day for five days(eye drop group). During the mean follow-up period of 13 months, recurrence rate was 15.4%(6 eyes) in soaking group and 20.6%(7 eyes) in eye drop group respectively. This difference was not statistically significant(p>0.5). There were no specific complications related to mitomycin C treatment in both groups during the follow-up periods. The soaking method of 0.2mg/ml mitomycin C was found as effective and safe as postoperative use of topical 0.2mg/ml mitomycin C for prevention of the recurrence of primary pterygium.

4.Correlation of Tc-diphosphonate bone scintigraphic findings and histologic grade in prostatic adenocinoma.

Prostatic adenocarcinoma is a common genitourinary malignancy which frequently is assiciated with bony metastasis at initial diagnosis. The detection of bony metastasis is important because it often alters the therapeutic strategy. The radionuclide one scintigraphy is known to be the most sensitive diagnostic method in detecting bony metastasis from prostatic adenocarcinoma. In the histologic grading of prostatic adenocarcinoma, the Gleason's grading system is the most widely employed one which is based on the degree of glandular differentiation and the growth patterns of tumor in relation to the prostatic stroma. The purpose of this study is to know the correlation of Gleason's grade and bone scintigraphic findings in prostatic carcinoma. We analyzed 99mTc-diphosphonate bone scintigraphies of 19 cases with prostatic adenocarcinoma confirmed by transurethral prostatic biopsy. Ten of the 19 cases showed asymmetrical multiple radioisotope uptake suggesting bony metastases and their Gleason's grade were 4 or 5 in seven cases. The remaining three cases were Gleason's grade 2 or 3. Another nine cases showed symmetrical radioisotope uptake with normal renal imaging suggesting no bony metastases and their Gleason's grade were 1,2 or 3 in seven cases. The remaining two cases of these nine cases were Gleason's grade 4 and 5. As the results, there was a correlation between the Gleason's grade and bone scintigraphic findings. The asymmetrical multiple skeletal uptake of radioisotope may suggest that the Gleason's grade is likely to be higher.

5.Prediction of Normal Values of Systolic Time Intervals.

In order to establish the predicted normal values of the systolic time intervals the duration of the systolic time intervals measured from simultaneous recordings of the electrocardiogram, the phonocardiogram and the carotid pulse tracing. The subjects studied were 160 healthy males and 160 females. The mean ages of males and females were 29 and 31 years old, respectively. The transformation period was not closely related to heart rate, and its mean values for males and females were 58 and 56 msec., respectively, and the mean for males and females combined was 57 msec. The remainder of the systolic time intervals, however, showed a significant linear and inverse relation to heart rate. Thus, based upon these data regression equations for the prediction of the normal values of electromechanical systole, left ventricular ejection time, mechinical systole, precjection period andisovolumiccontraction time for males, females, and males and females combined were obtained.

6.ST Segment Elevation in Lead V1on Treadmill Exercise Test in the Patients with Angina : A Predictor of Coronary Artery Disease and It's Location.

OBJECTIVES: To assess the relationship between ST segment elevation during exercise testing and coronary artery disease. METHODS: Treadmill exercise electrocardiography and coronary angiography were performed in 137 patients with angina with no Q waves. The J point elevation of > 0.03mV(0.3mm) measured from baseline, the elevation of ST segment at 80msec. from J point(ST80) measured larger in lead V1than in aV1, or both the elevation of ST segment at J point and at ST80 developed earlier than or concomitantly with ST depression were considered as abnormal responses. RESULTS: 1) There was no clinical significance of ST elevation in lead aV1. 2) Exercise-induced ST elevation in lead V1occurred in 29 of 137 patients with angina. 3) Abnormal St elevation in V1was detected in 26 of 86 patients with coronary artery disease, for a specificity of 98%. The patients with left anterior descending coronary artery or left main coronary artery stenosis were observed in 3 of 5(60%) patients with single-vessel disease, in 7 of 9(79%) with two-vessel disease, and in 9 of 10(90%) with three-vessel disease. 4) The standard ST depression yielded a specificity of 55% in the patients with isolated or predominant ST depression in inferior leads(II. III, and aVF) and of 81% in the patients with isolated or predominat ST depressin in precordial leads(p<0.001). 5) Abnormal exercise-induced ST elevation in V1was detected in 13 of 39(33%) patients with isolated or predominat ST depression in inferior leads and yielded a specificity of 94% for coronary artery disease. CONCLUSION: Abnormal ST segment elevation in V1may increase the specificity in isolated or predominat ST segment depression in inferior leads and may predict left anterior descending or left main coronary artery disease.

Change in heart rate, mean arterial pressure, PaO2 and PaCO2 Caused by enflurane and halothane anesthesia were investigated in patients premedicated with diazepam and glycop-yrrolate. Enflurane caused a slightly(10.1%) increase in heart rate and not change in mean arter-ial pressure(0.1%), Halothane depressed heart slightly(10.16%) and arterial pressure was also (12.2%). The authors conclude that enflurane possesses a positive chronotropic effect.

8.Computed Tomography of Spinal Tuberculosis.

Twenty-nine patients with spinal tuberculosis were evaluated by computed tomography (CT). The contiguous involvemnt of the vertibral bodies as well as the detailed localization of the destruction were analyzed. Our result disclosed that there was a higher incidence of the destruction of pedicles(68 percent) and an involve-ment of posterior elements (13 percent). Frequent patterns were fragmentation, sequestrae, disc space narrowing reactive sclerosis, paravertebral mass (abscess) and calcification. CT also p--vided precise information about the rim, density and size of the soft tissue mass, particularly following intravenous contrast infusion. CT was found to be helpful in the evaluation of the extent of osseous and soft tissue involvement as well as the destructive pattern. We conclude that CT can be used to detect the extent of osseous and paravertebral soft tissue involvement and to differentiate spinal tuberculosis from a neoplastic lesion.

9.A Case of Childhood Ataxia with Diffuse Central Nervous System Hypomyelination(CACH) Syndrome.

Childhood ataxia with diffuse central nervous system hypomyelination(CACH) syndrome is a recently described leukodystrophy of unknown etiology. The patients show normal development until the age from 1.5 to 5 years, and sudden deterioration of all motor abilities with irritability is presented after a viral infection or minor head trauma. Brain magnetic resonance imaging(MRI) shows generalized hypointensity of the white matter in T1-weighted image, which turns hyperintense in T2-weighted image, and Proton Magnetic Resonance Spectroscopic Imaging(1H-MRSI) shows reduced signal of N- acetylaspartate, choline, and creatine only in white matter. Dementia is not present and peripheral nerves are normal. We report a case of CACH syndrome who was born with no perinatal problem, and showed normal development until the age of 16 months. She suddenly lost all motor abilities after exanthem subitum who recovered fully over two months. At the age of 18 months she experienced similar attack after chicken pox, and developed seizures at age of 18 months.

10.A Case of Vocal Cord Dysfunction Masqueraded as Exercised-Induced Asthma.

Vocal cord dysfunction (VCD) is respiratory disorder characterized by paradoxical closure of the vocal cord during the respiratory cycle leading to obstructive airway symptoms. The clinical presentation of VCD is often dr amatic and its misdiagnosis as asthma of exercise-induced brochospasm(EIB) has led to inappropriate treatment including high dose corticosteroids, intubation, and tracheostomy. Many VCD patients are asympromatic at rest and require exercise challenge to elicit symptoms and vocal cord abnormalities. The "gold standard" for the diagnosis of VCD remains laryngoscopy of bronchoscopy with direct visualization of paradoxical adduction of the vocal cords. We report a case of exercise-induced Vocal cord masqueraded as exercise-induced asthma unresponsive to corticosteroids. And bronchodilator confirmed by typical bronchoscopic findings with paradoxial adduction of the vocal cords.
