1.Dental Anthropologic Study on the High School Students of Che-ju island.

This dental anthropological study was performed to analyse male and female students of Che-ju high school students. Cephalometric X-rays and dental casts were obtained of 60 male and 60 female students and the Mean and Standard Deviation were obtained. There was no difference in craniofacial morphology between male and female students but the size of craniofacial skeleton of the male is bigger than that of the female. The distance from NB line to pogonion is longer in the male group, and it meant that mandibular symphysis of the male is well developed than the female. The size of teeth of the male is larger than that of the female and is significant in upper and lower canine and first molar. The size of upper arch width, intercanine width, basal arch width and lower basal arch width of the male is larger than that of the female, but arch length of the male and the female showed no significant difference.

2.A Comparative Study of Positive Culture Rates Between Urethral Discharge and Urinay Sediment for the Diagnosing of Neisseria gonorrhoeae.

BACKGROUND: Although the culture of urethral discharge using a cutton swab is regarded as one of the most sensitive and reliable diagnostic methods of gonorrhea, it may give pain or diagnostic methods of gonorrhea, it may give pain or discomfort to patients, and its detection rate may be influenced by the examiner's technical ability. OBJECTIVE: The purpose of this study was to compare the positive culture rates between ure4thral discharge and urinary sediment for the diagnosing of Neisseria gonorrheae. METHODS: We compared this technique with the culture of urethral discharge using the specimens from 357 male patients with gonococcal infection at VD clinic of Jungku Pvblic Health center in Seoul from Jan 1991 to Jun 1993. RESULTS: The culture of urinary sediment was positive in 339 of 357(94.95), whereas the culture of urethral discharge was positive in 346 of 357(96.95). However, these data have no statistical significance(P<0.05). The obe observed sensitivity and specificity of culture of urinary sediment comjpared to culture of urethral discharge were 97.75 and 90.9%, respectively. The predictive value of positive and negative test compared to the culture of urethral discharge were 99.7% and 55.65, respectively. As our data showed low negative predictive value, a negative culture result of urinary sediment does not always imply a negative culture result of urethral discharge. CONCLUSION: Therefore, at present, the previous method of the culture of urethral discharge is more highly recommended and further studies for this diagnostic and/or new method of gonorrhea based on larger group of patients is encouraged.

3.A Case of Pulmonary Infarction Probably Related with Henoch-Schönlein Purpura.

We report a case of pulmonary infarction probably related with Henoch-Schönlein purpura, which presented with purpura on both lower extremities in a 27-year-old woman. The purpura had developed 4 days previously and it had no itching or tend'erness. She complained of mild fatigue, chest tightness, but no dyspnea or hemoptysis. On routine examination, chest radiography showed a poorly defined nodular opacity on the left lung field, and it was diagnosed by a follow up chest CT as a pulmonary infarction secondary to pulmonary vasculitis.

4.A Clinical Study of Supracondylar Fracture of the Humerus in Children

Supracondylar fracture of humerus is the most common fracture about the elbow in children and much has been written about its treatment and the prevention of both Volkmann's ischemia with contracture and cubitus varus deformity. In a retrospective survey of 155 children with supracondylar frscture of the humerus, 63 cases were found to have sufficient clinical and retrospective data to classify the fracture. 1. The average age was 6.5 years ranged from 1 year 3 months to 14 years and sex ratio was 2.2: 1(M:F) 2. 62 cases(98.4%) were extension type and 1 case(1.6%) was flexion type. In extension type, displacement was posteromedial 30 cases(47.6%), posterolatera117 cases(26.9%), posterior 10 cases(15.9%) and undisplaced 5 cases(7.9%). 3. Fracture level was divided into 3 Groups. Trans-olecranon fractures were 47 cases(74.6%), proximal to olecranon fractures were 11 cases(17.5%) and distal to olecranon fractures were 5 cases(7.9%). According to age distribution lower age group involves the distal part and higher age group involves proximal part. 4. Transverse fractures were 39 cases(61.9%) and oblique fractures were 24 cases(38.1%). In oblique fracture, postero-anterial oblique fractures were 19 cases(30.2%), medio-lateral oblique fractures 3 cases(4.8%), and latero-medial 2 cases(3.2%). In oblique fractures redisplacements were more common in P-A oblique csse. 5. In the treatment of fracture, 31 cases(49.2%) were treated with closed reduction and percutaneous pinning, 26 cases(41.3%) were treated with closed reduction and splint immobilization, 3 cases(4.8%) were treated with open reduction and K-wire fixation, 3 cases(4.8%) were treated with skeletal traction. 6. Functional results according to degree of initial displacement was worse in some displaced(Grade II) cases, rather than total displaced (Grade IV) or displaced with rotation (Grade III) cases. 7. Complications in splint immobilzation method were cubitus varus deformity 2 cases, nerve injury 2 case, and in k-wire fixation method, nerve injury 5 cases and pin migration 1 case.

6.The application of cranofacial osteotomies to surgical exposure of crantofacial tumors.

Access to the craniofacial skeleton and skull base requires osteotomies. An adequate exposure of neoplasms occupying the skull base and the oral cavity and oropharynx, especially retromolar trigone, is very important for precise tumor ablation. The demonstration by Tessier of the capacity for large segments of bone to survive removal and replacement has enabled skull base tumor surgery to grow into a subspeciality. Through the refined craniofacial exposure osteotomies, the surgery is safer, the operation shorter, and the result better both oncologically and reconstructively. We experienced 24 cases of exposure osteotomies consisting of 12 cases transzygomatic approaches, 1 case tranglabellar osteotomy, 1 case lateral transmaxillary approach, 1 case Le Fort II & midline splitting, and 9 cases mandibulotomy. There was no significant complication except a local wound infection in transzygomatic approach and one case of minimal malocclusion after parasymphyseal mandibulotomy. The orthotopic bone graft and the rigid fixation enables the postoperative morbidity to decrease. It seems that the previously inaccessible craniofacial tumors can be treated through the various exposure osteotomies.

7.Urine culture and serologic test for diagnosis of trichomoniasis in male patients with nongonococcal urethritis.

Trichomoniasis, a common and sometimes distressing condition in women, is caused by infection of the genitourinary tract by the pathogenic protozoan Trichomonris aginalis(T. vaginalis). Men may also be infected, although frequently without any symptoms, and most cases of trichomoniasis are sexually transmitted. The best evidence for an etiologic role for T. vaginalis is in nongonococcal urethritis(NGU). In epidemiologic studies, T,ginalis has been isolated from 1 to 68 percent in men with NGU. The nurnber of NGU is increasing rapidly than gonorrhea in deeloped nations, and in some developing countries, especially in Korea, In view of the difficulties in findng trichomonads by direct microsco by in men, urine culture and serodiagnosis plays an important role in the diagnostic work. Urine culture, indirect fluorescent antibody test(IFAT) and enzyme-linked immunosorbent assay (ELISA) were used to elecect T.vaginalis in 221 patients with male NGU. These patients visited the VD Clinic of ChoongKu Public Health Center in Seoul from May to August, 1989 and from March, 1990 to December, 1991. The results obtained were as follows.: 1. T. vaginalis was grown from only one of the 221 samples on TPS-1 meida. 2. In 208 NGU patients, there were 10(4.8%) positive reactions on IgG antibody in IFAT. 3. In 208 NGU patient, there were 14(6.7%) positive reactions to IgG antibody in ELISA. 4. No statistical significant correlation was found between then sults of IFAT and ELISA in individual patients. In conclusion, 1. Male trichomoniasis in patients with NGU was detected at a, significantly low rate. Due to the small number of patients in the test, further epidemiologic stidies of trichomoniasis in male NGU patient are necessary. 2. Serologic studies (IFAT and ELISA) have detected sorre trichomoniasis in male patients with NGU. These tests seem to be helpful in the diagnosis of male t-ichomoniasis.

The lesions of central nervous system in tuberous sclerosis often calcify, especially those present in the perivascular region, allowing the computed tomography to make early and definite diagnosis, even when patients intelligence was normal, seizures were absent, or clinical findings were not specific, We report three cases of tuberous sclerosis, which showed pathognomic subependymal paraventricular calcification in brain CT.

9.Malfunction of Ventriculo-Peritoneal Shunt due to Acute Appendicitis.

We have been experienced a case of malfunction of ventriculo-peritoneal shunt(Uni-shunt) due to acute appendicitis. The peritoneal tip seemed to be obstructed by inflammatory reaction around the diseased appendix. After appendectomy and reposition of the peritoneal tip to the left side, the shunting function returned to normal. We have reported the case and discussed the various causes of the malfunction of ventriculo-peritoneal shunt.

10.The Effects of Continuous Epidural Blockade in the Treatment of Postherpetic Neuralgia.

BACKGROUND: Postherpetic neuralgia is usually defined as pain persisting for more than one month after the onset of the eruption of herpes zoster, although there is a overlap between this and the pain of the acute zoster. Many approaches have been proposed to treat postherpetic neuralgia, however, it remains a source of frustration for both patients and physicians. OBJECTIVE: The purpose of this study was to determine more precisely the relationship between the effects of continuous epidural blockade and the time of treatment on postherpetic neuralgia. METHODS: We analyzed the results of 5 patients with postherpetic neuralgia for whom continuous epidural blockade was performed within 1 month after the onset of postherpetic neuralgia, and compared them with the results of 5 patients for whom continuous epidural blockade was performed after 1 month after the onset of postherpetic neuralgia. RESULTS: Continuous epidural blockade performed within 1 month after the onset of postherpetic neuralgia reduced pain significantly, compared to the results of continuous epidural blockade performed after 1 month after the onset of postherpetic neuralgia. CONCLUSION: Continuous epidural blockade performed within 1 month after the onset of postherpetic neuralgia may be the treatment of choice for the pain.
