Supracondylar fracture is the most frequent injury of the elbow and frequently accompanies complications in children. Seventy two cases of supracondylar fractures have been treated and managed at Department of Orthopaedic Surgery, Catholic Medical College during the period from Jan. 1969 to Dec. 1972. The evaluation of fracture was done according to the classification of Holmberg and Hoyer. Among 72 cases of the supracondylar fracture, the most common type is 29 cases of Grade 2: (40.2%). Most cases were treated conservatively, and various method of treatment were adopted according to the condition of fracture. Residual angular deformities after successful manipulative reductiion can be accurately evaluated roentgenographically by the Baumanns method. Good results were obtained in 62.6%, fair results in 33.3%, and poor results in 4.1%. The incidence of cubitus varus deformity is 37.5%. There were no instance of ischemic contracture and no permanent neurological deficit in fracture cases.

Access to the craniofacial skeleton and skull base requires osteotomies. An adequate exposure of neoplasms occupying the skull base and the oral cavity and oropharynx, especially retromolar trigone, is very important for precise tumor ablation. The demonstration by Tessier of the capacity for large segments of bone to survive removal and replacement has enabled skull base tumor surgery to grow into a subspeciality. Through the refined craniofacial exposure osteotomies, the surgery is safer, the operation shorter, and the result better both oncologically and reconstructively. We experienced 24 cases of exposure osteotomies consisting of 12 cases transzygomatic approaches, 1 case tranglabellar osteotomy, 1 case lateral transmaxillary approach, 1 case Le Fort II & midline splitting, and 9 cases mandibulotomy. There was no significant complication except a local wound infection in transzygomatic approach and one case of minimal malocclusion after parasymphyseal mandibulotomy. The orthotopic bone graft and the rigid fixation enables the postoperative morbidity to decrease. It seems that the previously inaccessible craniofacial tumors can be treated through the various exposure osteotomies.

We report a case of pulmonary infarction probably related with Henoch-Schönlein purpura, which presented with purpura on both lower extremities in a 27-year-old woman. The purpura had developed 4 days previously and it had no itching or tend'erness. She complained of mild fatigue, chest tightness, but no dyspnea or hemoptysis. On routine examination, chest radiography showed a poorly defined nodular opacity on the left lung field, and it was diagnosed by a follow up chest CT as a pulmonary infarction secondary to pulmonary vasculitis.

Eighteen cases of double outlet right ventricle were observed during past six years at the department of pediatrics, Seoul National University Hospital. 1. Therw were 13males and 5 females. Sex ratio showed male predominance with the ratio of 2, 6:1. 2. According to the Sondheimer's classification, 6 cases were in group l, none in group ll, 10cases in group lll and 2 cases in group lV. 3. Presenting symtoms and signs were cyanosis(18/18), dyspnea(17/18), growth retardtion(10/18), and cyanotic spell(6/18). 4. The range of hemoglonin concentration was 14 to 21 gm%(mean 18.2gm%). Simple chest X-ray showed cardiomegaly in 10 patients. On routine ECG examination, RVH were noted in all 18 cases. 5. In group l patients, mean oxygen saturation of the aorta was 74.5%, mean oxygen saturation of the pulmonary artery was 86.7%. But on group lll, the results were variable. And in guoup lV, oxygen saturation difference were negligible in both great arteries. 6. Pulmonic outflow obstruction were noted in 12 cases in which 8 cases were combined type. 7. Operation was done in 8 cases. Out of the 6 cases who were receive corrective surgery, 3 cases died of the right heart failure. 8. Associated anomalies were as follows : bilateral SVC in 6 cases, PDA in 5 cases, ASD in 4 cases and dextrocardia in 4 cases.

6.A Case of Vesiculobullous Darier' Disease.
Korean Journal of Dermatology 2005;43(3):428-430
Vesiculobullous Darier's disease is a rare variant of Darier's disease which is characterized by vesicles and bullae, usually distributed on the sun-exposed areas of the body. Blistering may be percipitated by factors such as high humidity, ultraviolet radiation, etretinate treatment, surgery or physical stress. Widespread vesiculation or bullous lesions are unusual in the absence of cutaneous bacterial or viral infections. Herein, we report an unusual case of vesiculobullous Darier's disease which developed on the scalp in a 25-year-old man.

It is established that intracranial hemorrhage is relatively common benign intracranial diseases in adult. This is very rare in children reports are limited to a few cases or to those cited in reviews of series of intracranial hemorrhage in adult group. We wish to report three cases of large intracranial hematoma in three infants. The diagnosis was greatly facilitated by the CT scan and the treatment did by the microsurgical methods.

Journal of the Korean Radiological Society 1994;30(4):757-762
PURPOSE: We performed the study prospectively to evaluate the advantage of fat suppression MR in the diagnosis of rotator cuff injury. MATERIALS AND METHODS: Ten symptomatic patients were studied with both conventional T2WI and FST2WI using chemical shift technique. Each image was analyzed for the assessnent of injuries, conspicuity of the lesion, the presence of effusion in subacromial bursae and joint space, and presence of humeral head injury. Arthroscopy was done in 4 patients following MRI. RESULTS: We could made presumptive diagnoses on FSMR as identical as on conventional MR in six cases(1 normal, 2 tendinitis, 2 partial thickness tear, 1 full thickness tear), two of them were confirmed by arthroscopic procedures. Two cases of partial thickness tear proved by arthroscopy were detected on FST2WI, whereas they were considered tendinitis on conventional T2Wl. There were another 2 cases who showed tendinitis on FSMR, but normal on conventional T2Wl. They, however, were not confirmed by either arthroscopy or surgical procedure. CONCLUSION: We found the FSMR were superior to conventional T2Wl in the conspicuity of lesions and detection of joint effusion and abnormalities on the humeral head. We think FSMR of the shoulder could have significant diagnostic advantages over the conventional spin-echo MR imaging.

10.Two Cases of Sclerotic Fibroma.
Korean Journal of Dermatology 2004;42(11):1466-1469
Sclerotic fibroma is an uncommom fibrotic neoplasm that can occur as solitary lesions in otherwise healthy individuals. Multiple sclerotic fibromas of the skin are considered a cutaneous marker of multiple hamartoma syndrome or Cowden's disease. Sclerotic fibroma was first described by Weary et al. in the tongue of a patient with Cowden's disease in 1972. Rapini and Golitz described the sporadic occurrence of histologically identical tumors, and they suggested the names `hypocellular fibroma' and `sclerotic fibromas of the skin' for these lesions in 1990. The characteristics of sclerotic fibromas include hypocellularity and stromal sclerosis with eosinophilic, hyalinized collagen bundles in a parallel or whorled arrangement. We report two cases of solitary sclerotic fibroma of the skin not associated with Cowden's disease.
