2.Multiple subcutaneous Rosai-Dorfman disease involved nasal and pharyngeal cavity: a case report.

Although Rosai-Dorfman disease is a rare disorder characterized by proliferation of distinctive histiocytes within lymph node sinuses and lymphatics, sometimes involving extranodal sites, the nasal cavity and pharyngeal cavity are rarely involved additional. In this report, a 22-year old man with multiple subcutaneous lumps and submucosal lumps in bilateral nasal cavity and pharyngeal cavity is presented, which was diagnosed by nasopharyngeal endoscope,computed tomography scan and biopsy. The subcutaneous lumps were ablated and prednisone was accepted for three months as preventive therapy.

4.Cutaneous and subcutaneous metastases of adenocarcinoma of the colon and rectum.

INTRODUCTIONThe interesting topic of cutaneous and subcutaneous metastasis from rectal carcinoma is discussed using 3 cases.
CLINICAL PICTUREThe first case was a 70-year-old man with T3N2M0 rectal mucinous adenocarcinoma, who developed an inflammatory subcutaneous metastasis at the left scapula 2 years after anterior resection. The second case was a 51-year-old man with T4N2M0 splenic flexure mucinous adenocarcinoma, who developed metastatic disease including a subcutaneous secondary to the back. The third case was a 53-year-old woman who developed vulval recurrence 10 months after abdomino-perineal resection for a low T3N1M0 rectal adenocarcinoma.
TREATMENTAll underwent wide resection.
CONCLUSIONThis entity is rare and usually signifies disseminated disease if found remote from the resection site and warrants a thorough metastatic work up. A high index of suspicion is recommended when encountered with unresolving skin lesions in cancer patients.

5.Frozen Section: Application in the Surgical Pathology.

The frozen section technique is a means of intraoperative pathological diagnosis, and a procedure of great value to the surgeon. This method should be accurate, rapid and reliable. This method serves useful purposes, such as determining the presence of tumor, its type (especially whether it is benign or malignant), the adequacy of a biopsy of a suspected lesion, and the conditions of the surgical margins. But, it bears many disadvantages, the most of which is the danger of incorrect diagnosis. We studied the indications, the limitations, and the accuracy of the frozen section method and the materials studies was total of frozen section diagnosis of 809 cases was 98.1% with 0.5% of false negative, 0% of false positive 0.5% of incorrect histological diagnosis or grading errors, and 0.9% of deferred cases. The tissues submitted were lymph node, gastrointestinal tract, skin subcutaneous tissues in decreasing oder of frequency. The false positive case is not present, while the false negative cases were 4.

6.One cases of scalp teratoma.

The patient, female, 27 years old. The scalp neoplasm had been found at her right temple 10 years ago. At first, the size of the tumor was just like a soybean, and did not grow obviously in the past nine and a half years. But the tumor increased fast in these six months, almost as large as the pigeon egg now. The physical examination showed that: A spherical tumor locates in the right temple near the right ear; the size of the tumor is 3 cm X 4 cm X 3 cm; the surface is smooth, no tenderness, no obvious mobile base, skull defect not touched. Primary diagnosis: the scalp cyst or lipoma. Under the local anesthesia we remove the neoplasm, during the operation we found the lesion located in the subcutaneous tissue layer, involved the galea aponeurosis layer, the skull surface are smooth and complete. The tumor was completely removed. We cut the mass and found a cystic cavity sized 3 cmX4 cm, which contained gray jerry-built materials, contained oil and a mass of hair, the capsule wall was complete. Postoperative pathological report: The tumor is cystic look throught the microscope, the capsule wall is squamous epithelium, containing keratinized material and hair. The pathological diagnosis is teratoma. The incision healed primarily, no recurrence found for about a year.

7.Endoscopic removal of foreign body in nasal apex: a case report.

We present a rare case report of foreign body of the nasal apex in a 22-year man who were hospitalized because of a bit of metal rebounded. There was slight bleeding at wound site with a feeling of pain and swelling. On physical examination, the apex lied in the median position with a bleeding cut about 3 millimeter in length. There was no visible stump on the cut or rupture in the nasal vestibular. Computed tomographic scan showed the abnormal high-density shadow in the nasal apex while the foreign body was located in the subcutaneous tissue of the antero-upper part of septal cartilage. The admitting diagnosis was foreign body in nasal apex. Endoscopic surgery was adopted to remove the foreign body.

Myolipoma is a rare benign tumor, composed of irregularly admixed adipose tissue and smooth muscle fibers. Few literature studies have described the radiologic appearance of myolipoma, especially in the soft tissue. No MRI findings in subcutaneous myolipoma of an extremity have been reported. Here, we report on the case of a 34-year-old woman with myolipoma in the subcutaneous tissue of the ankle and describe MRI features of the lesion.

9.Surgical Correction of Congenital Epiblepharon: Lower Eyelid Crease Reforming Technique.

Epiblepharon is a commonly encountered congenital anomaly in Asian infants and children. It causes symptoms of ocular irritation and inferior punctate corneal epithelial erosion. Surgical correction may be needed if ocular irritation symptom and corneal pathology persist with age. A series of 185 lower eyelid epiblepharon in 98 Korean children underwent lower eyelid crease reforming technique. Surgical treatment included excision of redundant skin and pretarsal orbicularis muscle with lid everting suture by anastomosis of the subcutaneous tissue and inferior tarsal border including capsulopalpebral fascia. With a minimum follow-up of 12 months, a total of 37 [20%]eyelids developed recurrence of cilia touch, and then 9 [4.8%]eye-lids needed further surgery.

10.A Case of Clostridium sordellii Bacteremia in a Patient with Osteosarcoma.

Clostridium sordellii is a gram-positive spore forming anaerobic bacillus, rarely encountered in human infection. It produces several exotoxins which contribute to myonecrosis, pulmonary infection, obstetric infection and bacteremia. C. sordellii bacteremia is very rare and has been reported in few patients with malignancy or immunosuppression. To date, there has been no report in Korea. Mortality is high when deep site infection exists regardless of underlying pathology and immune status. In contrast, superficial skin and subcutaneous tissue infection has a favorable prognosis. We present a case of C. sordellii bacteremia in a patient with osteosarcoma. The patient was admitted for the treatment of the infection at the ulcerative osteosarcoma lesions of the leg, and C. sordellii bacteremia was demonstrated. He was successfully treated with intravenous ampicillin/sulbactam for two weeks.
