Objective To investigate the efficacy of oxaliplatin plus S-1 versus the XELOX(oxaliplatin + capecitabine) program in the adjuvant chemotherapy in the patients with postoperative gastric cancer .Methods A total of 81 cases with postoperative gas-tric cancer were randomly divided into the treatment group and the control group .The treatment group (41 cases) received the chemotherapy of oxaliplatin +S-1 ,while control group was given the chemotherapy of XEOLX program .Results The 1-year and 2-year recurrence-free survival (RFS) of the treatment group and the control group were 51 .3% vs .61 .5% ,25 .6% vs .20 .5% ,there were no statistically significant differences between the two groups (P=0 .361 ,P=0 .591) .The 1-year and 2-year overall survival period of the treatment group and the control group were 64 .1% vs .69 .2% ,30 .8% vs .25 .6% ,respectively ,there were also no sta-tistically significant differences between two groups(P=0 .631 ,P=0 .615) .The mainly adverse reactions of the two groups were myelosuppression ,gastrointestinal reactions ,hand-foot syndrome ,mucositis ,peripheral nerve toxicity ,hepatic and renal function damage .The occurrence rate of hand-foot syndrome in the control group was significantly higher than that in the treatment group (P=0 .001) ,the adverse reactions were improved after symptomatic treatment .Conclusion Oxaliplatin plus S-1 and oxaliplatin plus capecitabine are therapeutic equivalence and well tolerated in the patients with postoperative gastric cancer .

Objective To observe the long-term effects of maternal mild hyperglycemia on the growing development and glucose and lipid metabolism in intergenerational rat offspring.Methods Streptozotocin (25 mg/kg) was administered to 14 pregnant Wistar rats intraperitoneally on the first day of gestation to induce an animal model of mild intrauterine hyperglycemia,which was successful in nine rats.Seven pregnant rats without streptozotocin intervention served as controls.Female first-generation offspring (F1) rats were intercrossed with normal male rats to obtain the second-generation offspring (F2) rats.F1 and F2 rats were divided into 8 groups:female group (group F1-G-♀) or male group (group F1-G-♂) exposed to intrauterine hyperglycemia;female group (group F1-C-♀) or male group (group F1-C-♂) exposed to intrauterine euglycemia;transgenerational female group (group F2-G-♀) or male group (group F2-G ♂)exposed to intrauterine hyperglycemia;and transgenerational female group (group F2-C-♀) or male group (group F2-C-♂) exposed to intrauterine euglycemia.Body weight of the offspring was recorded.At the age of 28 weeks,the weight of pancreas and visceral fat,fasting plasma glucose (FPG),fasting insulin,total triglyceride,total cholesterol,high-density lipoprotein-cholesterol,low-density lipoprotein-cholesterol were measured in all groups.Two sample t test was used for statistical analysis.Results (1) At 4,24 and 28 weeks old,the body weight of group F2-G-♀ [(177.3± 12.3),(314.5± 13.9) and (322.3± 16.2) g] was remarkably increased compared with group F2-C-♀ [(164.3±6.0),(290.2± 18.3) and (300.2± 16.2) g,t=-2.324,-2.584and-2.359,all P ＜ 0.05].(2) At 28 weeks,the pancreas weight and the ratio of pancreas weight to body weight of group F1-G-♀ [(0.53 ±0.05) g and (0.17±0.02)％] were decreased significantly compared with group F1-C-♀ [(0.65±0.04) g and (0.21±0.02)％,t=4.159 and 2.483,both P ＜ 0.05].The pancreas weight,ratio of pancreas weight to body weight,visceral fat weight and fat weight of mass ratio in groups F2-G-♂ and F2-G-♀ were not remarkably different from groups F2-C-♂ and F2-C-♀,respectively (all P ＞ 0.05).(3)The level of FPG at three weeks of age in F2-G-♀ group was higher than in F2-C-♀ group [(6.5±0.8) vs (4.9±1.2) mmol/L,t=-2.786,P ＜ 0.05],and it was also higher in F2-G-♂ group than in F2-C-♂ group [(6.3±0.8) vs (4.7± 1.0) mmol/L,t=-2.696,P ＜ 0.05].At 28 weeks,the FPG level was (6.7±0.6) and (8.4±2.2) mmol/L in F2-G-♀ and F2-G-♂ groups,being higher than in F2-C-♀ and F2-C-♂ groups,respectively [(5.8±0.5) and (6.2± 1.0) mmol/L,t=-2.695 and-2.337,both P ＜ 0.05].Compared with F1-C-♀ group [(0.61 ±0.10) mmol/L],the high-density lipoprotein-cholesterol level was lower in F1-G-♀ group [(0.47±0.05) mmol/L,t=2.433,P ＜ 0.05].(4) In groups F1-G-♀,F2-G-♀ and F2-G-♂,the structure of the islet cells was obviously atrophic and disordered,and β-cells were slightly decreased and distributed unevenly,but group F1-G-♂ had significantly increased β-cells with hypertrophic islet size compared with control group.Conclusions F 1 rat offsprings exposed to mild intrauterine hyperglycemia experience excessive weight gain after birth,impaired structure of the islet,metabolic abnormality,increased visceral fat weight and glucose and lipid metabolism disorder,which show intergenerational inheritance and gender differences.

Objective To evaluate the safety of laparoscopic complete mesocolic excision (CME) in radical resection for right colon cancer. Methods From January 2012 to June 2013, laparoscopic CME was performed in 40 patients with right colon cancer. Another group of 38 patients underwent traditional radical resection from January 2011 to December 2011.Surgical outcomes were compared between the two groups . Results The number of lymph nodes retrieved in the laparoscopic group ( 22.1 ±7.8 ) was significantly more than that in the traditional group (18.6 ±4.3, t =2.436, P =0.017).In patients with stage Ⅲ cancer, the laparoscopic group was associated with higher lymph node counts (23.0 ±6.0 vs.18.2 ±5.1, t=2.699, P=0.000), however, there were no significant differences in those with stage Ⅱ cancer between the two groups (t=0.758, P=0.454).There were no differences in operation time and intraoperative blood loss between the two groups (t=0.716, P=0.476;t=-1.547, P=0.126), but in the laparoscopic group the time to pelvic drainage tube removal , time to first flatus, time to liquid diet intake , duration of hospital stay, and postoperative complications were significantly better than those in the traditional group (t=-2.950, P=0.004;t=-5.767, P=0.000; t =-7.817, P =0.000; t =-6.065, P =0.000; χ2 =4.504, P =0.034). Conclusions CME technique in laparoscopic radical right hemicolectomy is feasible , safe, and effective.CME improves the surgical quality with keeping the integrity of mesocolon , more harvested lymph nodes , and not increasing surgical risks and postoperative complications .

Aim To study the protective effect of caffeic acid on brain damage induced by chronic aluminum overload in rats.Methods The brain injury model of rats was established via intragastric administration of aluminum gluconate(element aluminum 200 mg?kg-1) once a day,5 d a week for 20 weeks.The 5-LO inhibitor caffeic acid(30 mg?kg-1,10 mg?kg-1) was intragastrically administered 1 h after aluminum administration.Pathomorphological changes of hippocampal neuron and the spatial learning and memory function were observed.The MAO-B,AChE and SOD activities,and the MDA contents of brain tissue in rats were measured.Results Chronic aluminum overload could markedly induce spatial learning and memory function impairment,and hippocampal neuronal injury in rats.The MAO-B and AChE activities,and the MDA content significantly increased,while SOD activities significantly decreased in chronic aluminum overload rats.The administration of the caffeic acid can significantly prevent rats from learning and memory function impairments and hippocampal neuronal death,and blunted the decrease of SOD activities and increased MDA contents induced by chronic aluminum overload in rats.Caffeic acid also obviously decreased the MAO-B activities and AChE activities of chronic aluminum overload rats.Conclusion Administration of caffeic acid can prevent the rat brain from damage induced by chronic aluminum overload.

Objective To investigate the corelationship between proton pump inhibitors (PPI) and the incidence of hospital-acquired pneumonia (HAP) in abdominal surgical patients. Methods A total of 283 cases admitted from 2010 to 2014 were divided into PPI group (n = 326) and HRA group (n = 92). In PPI group, patients were divided into omeprazole subgroup (subgroup A, n = 146), lansoprazole subgroup (subgroup B, n = 102) and pantoprazole subgroup (subgroup C, n = 78) by the types of PPI. In HRA group, patients were divided into famotidine subgroup (subgroup D, n = 58) and ranitidine subgroup (subgroup E, n=34) by the types of histamine-2 receptor antagonist (H2RA). The incidence of stress ulcer bleeding (SUB) and HAP were calculated, while mechanical ventilation period (MV), ICU stay and 28-day-mortality were compared. Results There were lower incidences of SUB and HAP, shorter MV and ICU stay and lower 28-day-mortality in PPI group than those in HRA group (P < 0.05). In three subgroups of PPI group, there were no significant difference on incidence rate of SUB and HAP, as well as MV, ICU stay and 28-day-mortality. Conclusion The incidence rates of SUB and HAP are lower by using PPI than H2RA in abdominal surgical patients, as well as MV, ICU stay and 28-day-mortality. Different types of PPI have no effect on the incidence rate of HAP in abdominal surgical patients.

Objective To investigate the effect of Tongmai oral liquid on the early diabetic nephropathy. Methods Diabetic nephropathy rats were induced by excision of the right kidney and a single intraperitoneal injection of streptozotocin (STZ). Forty male Wistar rats were divided randomly into 3 groups:Tongmai group, nephrotic model group, and normal group. At the end of the 8th week, the rats general states and some parameters such as kidney hypertrophy index, 24 h urine volume, blood glucose, 24 h urinary microalbumin (24 h UMA), renal function [serum creatinine (Cr), blood urea nitrogen (BUN)], serum lipid, and the change of nephritic pathologic lesion were measured. Objective: To study the antiemetic effect of the ethyl acetate extract with ethanol extraction of Galangal (hereinafter referred to as galangal extract) in allotriophagic Kaolin model rats.Methods: The rats were administered respectively with chemical drug cisplatin and apomorphine, and received the rotation stimulation as well.Three types of allotriophagic Kaolin models were established, and apomorphine, ondansetron and metoclopramide were used as the control drugs.The experimental rats were divided into 3 groups at low, medium and high dose of Galangal extract with prophylactic effect.The antiemetic effect was observed after the treatment.Results: Chemical drug cisplatin (3.0 mg·kg-1, intraperitoneal injection) and apomorphine (3.2 mg·kg-1,subcutaneous injection) and the rotation stimulation (centrifugal force of 3.4×g,60min) could induce allotriophagic vomiting in rats.The galangal extract at low (1 500 mg·kg-1), medium (4 500 mg·kg-1) and high (9 000 mg·kg-1) dose all fail to effectively inhibit the allotriophagic Kaolin behavior of rats caused by cisplatin, apomorphine and rotation(P>0.05).Conclusion: Galangal extract can not effectively inhibit the vomiting of allotriophagic Kaolin model rats.Probably, there is no correlation between the pharmacological effects of galangal extract and substance P, 5-hydroxytryptamine, dopamine as well as receptive zone of chemical vomiting inhibition.

BACKGROUND: Studies on the reparation of peripheral nervous system are not only a key focus in medical research, but also a major topic for biologists, biochemical engineers and materialogy specialists. OBJECTIVE: To investigate the individual difference of ideal muscle bridging neurologic defect configuration on nerve regeneration. DESIGN, TIME AND SETTING: The control experiment was performed at the Shenyang Medical College from June 2004 to September 2005. MATERIALS: Ten common dogs weighing 20-30 kg were used in this study. METHODS: After intravenous anesthesia, muscle strips in the caudal right sartorius and middle musculus flexor digitorum superficialis of dogs were sliced into two segments. One was observed by dissection under a stereoscope. Another received Haematoxylin & Eosin (HE) Staining and transverse area of muscle fiber of distal and proximal ends was analyzed by image analysis. The same muscle strip was sutured at 2 cm neurologic defect region of autologous left and right limbs. Number of medullated nerve fibers was measured after osmic acid staining. MAIN OUTCOME MEASURES: Morphological difference of muscle bridge, transverse area of muscle fiber of distal and proximal ends, and number of medullated nerve fibers at bridging femoral nerve defect of left and right limbs. RESULTS: The muscle fiber in sartorius paralleled the longitudinal axis of muscle strip. The angle between muscle fiber in musculus flexor digitorum superficialis and the longitudinal axis of muscle strip was big. 80% muscle fiber in sartorius was equa1 to the length of the muscle bridge. Muscle fiber in the musculus flexor digitorum superficialis was shorter than the length of muscle strip. Muscle fiber in the musculus flexor digitorum superficialis was thick, whereas muscle fiber in sartorius was thin. Objective To investigate whether area under the curve (AUC) of oral glucose tolerance test (OGTT) could work as a predictor of outcomes of gestational diabetes mellitus (GDM) on condition that blood glucose is controlled. Methods A total of 1 796 women who had a standard antenatal care in Peking University First Hospital and gave single live births from July 1, 2011 to December 31,2 013 were included.They should be diagnosed of GDM by the diagnosis criteria of gestational diabetes published by the Ministry of Health of PRC and diabetes pre-pregnancy excluded. Data were analyzed with SPSS 17.0, grouping by AUC. Results (1)Women with higher AUC had a rising trend of age and a downward trend of gestational weight gain, however, not statistically significant [specifically, in the four group of less than 15.00 mmol·L-1·h-1, 15.00 to 16.79 mmol · L-1 · h-1, 16.80 to 17.99 mmol · L-1 · h-1 and 18.00 mmol · L-1 · h-1 or more, gestational weight gain was (15.3±5.2), (14.1±4.8), (13.5±4.7) and (13.1±4.8) kg]. The prevalence of macrosomia raised while AUC increased. Those with an AUC of lower than 15.00 (mmol·L-1·h-1) had a lower risk of macrosomia (P=0.04) . But those with an AUC of 18.00 (mmol·L-1·h-1) or more had a higher risk of macrosomia (P=0.02). There was a rising trend in premature birth and preeclampsia with AUC increasing but not significant (the prevalence of premature birth was 4.38%, 5.36%, 7.71%and 7.94%while that of preeclampsia was 2.85%, 4.69%, 4.67% and 5.08% in these four groups).(2)The prevalence of macrosomia was 12.76% (54/423) when overweight pre-pregnancy ,significantly higher compared with 5.87%(65/1 107) in normal group. The prevalence of preeclampsia was 5.91%(25/423) and 3.34%(37/1 107) in those two groups, which was also significantly different. The obese group had a statistically highest prevalence of preeclampsia of 9.23%(12/130). (3)AUC (P<0.05, OR=1.113, 95%CI:1.008-1.218), as well as gestational weight gain (P<0.05, OR=1.520, 95%CI:1.279-1.806) and pre-pregnancy BMI (P<0.05, OR=1.183, 95%CI:1.125-1.243) made a difference in the prevalence of macrosomia. Meanwhile, pre-pregnancy BMI made sense in the prevalence of premature labor (P<0.05, OR=1.059, 95%CI:1.003-1.119) and preeclampsia (P<0.01, OR=1.202, 95%CI:1.123-1.286). Conclusions AUC, as well as pre-pregnancy BMI and gestational weight gain have a significant impact on outcomes of GDM, macrosomia especially, though blood glucose is controlled. Meanwhile, AUC might be considered as a predictor of macrosomia.
