1.Effect observation of sodium nitroprusside and dopamine injected by micro pump for ;the treatment of refractory heart failure

Objective: To observe the clinical effect of sodium nitroprusside and dopamine injected by micro pump for the treatment of refractory heart failure. Methods:One hundred and six cases of patients with refractory heart failure were chosen as the research object, which were cardiac functionⅢ~Ⅳlevel. All patients were treated with sodium nitroprusside and dopamine injected by micro pump. Starting dose of sodium nitroprusside was 6.25 μg/min. At same time pumping dopamine, starting dose of 2 g/kg·min, the dose was increased gradually until blood pressure dropped to 90~100 mm Hg, continuous 7~10 d, the signs and symptoms after treatment and parameters of cardiac function and adverse reactions were observed. Results: 1)In 106 cases of patients with refractory heart failure after treatment, the total effective rate 85.85%(91/106). 2)The heart rate, blood pressure, cardiothoracic ratio, left ventricular short axis reduced rate,cardiac output, each index of cardiac,indexes of left ventricular ejection fraction were improved significantly after treatment. Conclusion:The clinical effect for the treatment of refractory heart failure by using the micro pump injection of small doses of sodium nitroprusside combined dopamine would be obvious, have less adverse reactions, worthy of clinical popularization and application.
2.Construction of human immunoglobulin combinatorial library and screening of phage antibodies to platelet

Objective Human immunoglobulin combinatorial library was generated by using phage surface display expression system, and phage antibodies (Fabs) to platelet were screened.Methods:PBMC were separated from a patient who Were transfusion refractory to platelet. mRNA was isolated and cDNA was synthesized by reverse transcriptase.The immunoglobulin heavy chain Fd and the light chain ? genes were amplified by half nested PCR and the PCR products were cloned into the expression vector pCome3 respectively.The immunoglobulin combinatorial library was constructed and screened by 3 rounds of affinity selection.Results Human Immunoglobulin Combinatorial Library was successfully constructed.The specific phage antibodies were highly enriched after 3 rounds of biopanning selection against platelet membrane proteins.Conclusion The antibody library and human monoclonal antibodies to platelet may be useful as molecular tools to study the anti platelet drugs, platelet related diseases, epitope of human platelet antigens.
3.IL-1?,IL-6 levels and SOCS-3 gene expression in hypothalamus of BALB/c mice with brain asymmetry

Objective:To investigate the effects of brain asymmetry on levels of IL-1?,IL-6 and suppressor of cytokine signaling-3(SOCS-3) in hypothalamus,to get better understanding of relationship between brain asymmetry and neuro-immuno-endocrine network at the molecular level.Methods:By using paw preference test,right-pawed,left-pawed,and ambidextrous mice model was established.After decapitation of the mice,hypothalamus was separated rapidly.Some samples were homogenized and used for detection of levels of interleukin-1?(IL-1?),interleukin-6(IL-6) by Enzyme-Linked-Immunosorbent Assay(ELISA).The other samples were applied to detect SOCS-3 expression level by RT-PCR.Results:(1)The level of IL-6 in hypothalamus:left-pawed were higher than right-pawed mice,there was significant difference in statistics( P 0.05).(3)The expression level of SOCS-3:the SOCS-3 expression of right-pawed showed higher than left-pawed mice in hypothalamus,revealing a significant difference( P
4.Arthroscopic reconstruction of anterior cruciate ligament with hamstring tendons and mini-plates

ObjectiveTo study the clinical and pathological characteristics of tubulointerstitial nephropathy associated with Chinese herb patent Guan Xin Su He Wan(GXSHW-TIN).Methods15 patients with GXSHW-TIN were studied.Clinical and pathological data were semi-quantitatively assessed.Relationships between medication and the incidence,clinical characteristics and the outcome of the disease were analyzed.ResultsAll the patients had chronic renal failure when GXSHW-TIN was diagnosed.They all got the disease after long-term taking of GXSHW in routine dosage.Most of the patients presented gastrointestinal dysfunction,abnormal urine analysis and mild to moderate anemia as onset symptoms.The pathological characteristics were similar to those of Guanmutong(Aristolochia manshuriensis Kom) induced chronic aristolochic acid nephropathy(AAN).ConclusionLong-term taking of GXSHW,which contains Radix Aristolochiae,might induce AAN.It indicates that GXSHW should be causious for clinical use,the ban of Radix Aristolochiae in the pharmaceutical market needs to be considered for prevention of AAN.

Objective To evaluate whether the predictive value of admission hyperglycemia for mortality differs between diabetics and non-diabetics with acute coronary syndrome (ACS).Methods A total of 1534 consecutive patients admitted with ACS to Central Control Unit of Beijing Friendship Hospital were analyzed.Among these patients,1332 patients who had a venous plasma glucose record at admission were retrospectively enrolled and were stratified according to admission glucose levels with or without diabetes.The primary end point was in-hospital and 2-year all-cause mortality.Results In patients without a history of diabetes,649 patients had glucose level ≤7.8 mmol/L(group A),204 patients with 7.8-11.0 mmol/L (group B) and 142 patients > 11.0 mmol/L (group C).Of those with diabetes,124 patients were in group A,142 in group B and 126 in group C.By Cox regression analysis,admission hyperglycemia (group B,HR 1.48,P =0.05 ; group C,HR 2.78,P < 0.001) was an independent predictor of long-term all-cause mortality in patients without diabetes.In patients with diabetes,blood glucose > 11.0 mmol/L (group C,HR 2.68,P < 0.001) also independently predicted long-term all-cause mortality.Conclusions Compared with the diabetic patients,elevation of blood glucose at admission is common in ACS patients without definite history of diabetes.Admission hyperglycemia should be considered as a risk factor strongly correlated with in-hospital and 2-year all-cause mortality in patients with ACS.

Department of rheumatology and immunology is a compulsory training department for the standardization training of the residents. However, it has been noted that the residents often ignore the importance of rheumatology. They are usually short of communication skill with the patients and theoretical knowledge. They often have high error rate in the clinical work and training their clini-cal skills and thinking ability is very difficult in the actual work. All of these problems may decrease the training quality and make the training a mere formality. Therefore, in order to carefully implement resident standardization training, a set of specific training programs has been developed, involving in theory of knowledge, practice standardization and comprehensive quality training. With these training programs, the quality and efficiency of the residents' training has greatly improved.
