1.Comparison Analysis of Drug Instructions between Hospital Directory and OTC Standard Model Instruc-tions in 2014

OBJECTIVE:To analyze the differences of drug instructions between hospital directory and OTC standard model in-structions,and to provide reference for enhancing instruction management and reducing the safety risk of clinical drug use. METH-ODS:1 324 drugs of hospital directory in a hospital in 2014 were compared with OTC directory from CFDA websites. The instruc-tion of drug types included in OTC directory were compared OTC model instruction. According to the degree of risks which the dif-ferences may bring,differences were divided into four levels for analysis as negligible,general,important and severe. RESULTS:244 drugs belonged to OTC,of which 32.38%were different from standard model instructions. The four risk levels rates of negligi-ble,general,important and severe accounted for 29.11%,34.18%,7.59% and 29.11%,respectively. Among important risk,the difference of“indication limit”occupied the highest proportion,being 50.00%. Among severe risk,the difference of“forbidden for special disease”and“forbidden for pregnant women”accounted for 43.48% and 39.13%. CONCLUSIONS:There are problems, such as the absence of important medication information,statement conflicts. The hospital and administration departments should en-hance the standard management of drug instruction to guarantee safe and rational drug use in the clinic.
2.Application of Quality Control Circle for reducing the radiation dose during chest CT scan

Objective To explore the effect of quality control circle (QCC) in reducing the radiation dose during chest CT scan. Methods QCC was founded, activity themes were selected, activity schedule was planned, the reasons of high radiation dose was analyzed, countermeasures were planned and implemented jointly by circle members, and then per capita radiation dose and image quality before and after improving was compared to confirm the effect of QCC activities. The subjects of chest CT scan before and after improvement in our center were selected, included totally 218 cases, their average age was (47.05 ± 8.58) years, 162 were male, 56 were female, 44 cases had CT scan before improvement, 174 cases after improvement, and the data were analyzed by T-test and Chi-square test. Results The radiation dose per capita has declined from 13.75 mGy to 3.45 mGy, The rate of standard was 102.08%, progress rate was 74.91%. The rate of high-quality image was 92.52%. Compared with per capita radiation dose before and after the activities, the differences were statistically significant(P<0.001). The rate of high-quality image was 93.18% before activities, the percentage remained steady at 92.52% after the activities, there was no statistically significant differences in image quality(P>0.05). Conclusions Application of QCC not only reduced the radiation dose of the client without image quality changes, but also improved thinking and learning capacity, cooperative consciousness of the circle members.
3.Pulmonary Infection Status, Drug-resistance and Risk Factors of Extended-spectrum ?-Lactamases-producing Bacteria

OBJECTIVE To survey the patient with pulmonary infection induced by extended-spectrum ?-lactamases-producing bacteria and their Enterobacteriaceae clinical characteristics, drug-resistance and countermeasure. METHODS Isolation, cultivation, identification, drug-sensitivity tests and confirmation of ESBLs-producing bacteria were done for the bacteria of sputum specimens collected from our hospital from Feb 2001 to Sept 2004. Susceptibility testing was performed by disk diffusion(K-B)method. RESULTS Totally 541 strains of Enterobacteriaceae were cultivated altogether and ESBLs-producing bacteria were 135 strains. The ESBLs- producing strains were sensitive to imipenem, and the resistance rates to it were 0.00% . The resistance rates of ESBLs-producing strains to cefoperazone/sulbactam and piperacillin/tazobactam were 31.11% and 44.44%, respectively . 4.Analysis of Leukemia incidence (1997-2007) in vicinity area of a nuclear installation

Objective To investigate the leukemia incidence among residents in the vicinity area of a nuclear installation.Methods The data were collected by the retrospective survey.Diagnosis is based on the medical records in all the relevant hospitals.The study area was those residential areas within 10 km of radium surrounding the installation.All residents who had lived in the study area for six months and over were recruited into the study group.Results The survey ascertained 643 newly diagnosed leukemia cases in the study area during 1997 to 2007.The crude rate was 2.51/100,000,and its standardized rate to national population structure was 2.53/100,000.The incidence rate in the study area was slightly lower than that in the whole city.No increase trend was detected during the period.Conclusions No significant difference of the leukemia incidence rate was found between the vicinity area of the nuclear installation and the whole city.
5.Design of traditional Chinese medicines with antihypertensive components based on medicinal property combination modes.

Multi-component traditional Chinese medicines are an innovative research mode for traditional Chinese medicines. Currently, there are many design methods for developing multi-component traditional Chinese medicines, but their common feature is the lack of effective connection of the traditional Chinese medicine theory. In this paper, the authors discussed the multi-component traditional Chinese medicine design methods based on medicinal property combination modes, provided the combination methods with the characteristics of traditional Chinese medicine for the prescription combinations, and proved its feasibly with hypertension cases.

6.The relationship between phosphatidylinositol-3-kinases/protein-serine-threonine kinase signaling pathway and orthodontic tooth movement.

OBJECTIVETo study the relationship between phosphatidylinositol-3-kinases(PI3K)/protein-serine-threonine kinase(AKt) signaling pathway and orthodontic tooth movement.
METHODSTwenty-four rabbits were chosen to establish rabbit models for the study. The right maxillary teeth of each animal treated by orthodontics were as the test side, and the untreated left teeth were as the control side. The animals were sacrificed at 3, 5, 7, 14 d, respectively. The prepared tissue specimens were processed for the study. The changes of the expression of PI3K, AKt in periodontal tissues were detected by real-time quantitative-polymerase chain reaction (RQ-PCR) and Western blot techniques.
RESULTSRQ-PCR showed that the expression of PI3K, AKt mRNA dramatically changed at 3 d. The expression of PI3K, AKt mRNA in the test side was higher than the control side, especially at 7 d, and then decreased. Compared with the control side, there was statistical significant difference in the test side(P < 0.05). The study obtained consistent conclusion from Western blot and RQ-PCR.
CONCLUSIONExpression of PI3K, AKt in rabbit periodontal tissues increase during orthodontic tooth movement, which prompts that PI3K/AKt signal pathways relate to orthodontic tooth movement and PI3K/AKt signal pathway involve in the periodontal tissue remodeling.

7.Analysis and assessment of Coptis chinensis for different parts, ages, and heights using Fourier transform infrared spectroscopy.

OBJECTIVETo study the infrared (IR) fingerprint spectra of Coptis chinensis for different parts, ages, and heights, and to analyze the integrate rules about the content of berberine component in Coptis chinensis for different parts, ages, and heights.
METHODThe Fourier transform infrared (FTIR) spectroscopy was applied to detect the infrared spectra of Coptis chinensis samples rapidly and non-separately.
RESULTSThe IR spectra of the root, leafstalk, and fibre of Coptis chinesis have their unique features. The content of berberine component in Coptis chinensis was different for different parts and the sequence: root > leafstalk > fibre. When Coptis chinensis grows, the content of berberine component in leafstalk also increases. The content of berberine component in leafstalk which planting in 1 200 m was less than that in 1 300, 1 400, and 1 500 m. The ages and heights provide no obvious influences on the content of berberine in the root of Coptis chinensis.
CONCLUSIONUsing FTIR spectroscopy, the quality of Coptis chinensis can be controlled, which provides a useful method for the standardized planting of Coptis chinensis.

8.Measurement of serum IgG4 levels by an established ELISA system and its clinical applications in autoimmune diseases.

IgG4-related disease (IgG4-RD) is a novel and rare autoimmune disease entity. Elevated serum IgG4 level is strongly suggestive of IgG4-RD. But it is still unknown whether serum IgG4 elevation commonly occurs in other autoimmune diseases. In this study, the serum IgG4 levels were detected by an established enzyme-linked immunosorbent assay (ELISA) in a variety of autoimmune diseases including systemic lupus erythematosus (SLE), Sjogren's syndrome (SS), polymyositis or dermatomyositis (PM/DM) and IgG4-RD. To evaluate the reliability of this ELISA system, some of our samples were sent to a lab in Kanazawa Medical University, Japan, and detected by using the nephelometric assay. The results showed that our findings were consistent with theirs. Moreover, it was found that the serum IgG4 levels were 0.23±0.16 g/L in 53 healthy controls, 0.16±0.15 g/L in 103 SLE patients, 0.22±0.18 g/L in 41 SS patients and 0.40±0.32 g/L in 21 PM/DM patients. No significant difference in the serum IgG4 level was observed among these groups (P>0.05). The serum IgG4 levels of two cases of IgG4-RD were 1.63 and 4.65 g/L respectively, and both decreased markedly after treatment with glucocorticoids. These data indicated that this established ELISA system can be used for detecting serum IgG4 levels. Elevated serum IgG4 levels help diagnose IgG4-RD and evaluate the curative effect of this condition rather than other autoimmune diseases.

9.Health investigation of 3 551 middle-aged and old males in Jiangsu Province.

OBJECTIVETo understand the state of health of the middle-aged and old males in Jiangsu Province.
METHODSA total number of 3 551 middle-aged and old males aged 46 approximately 69 years were randomly selected from 13 counties of Jiangsu Province. All of them received physical examinations, including laboratory tests of liver and kidney function, sugar and cholesterols, radioimmunoassay of testosterone and free testosterone levels in the serum, and B-ultrasonic examination of the prostate volume and remaining urine. They also underwent inquiries according to the International Index of Erectile Function (IIEF-5) and the questionnaire for PADAM (partial androgen deficiency of the aging male).
RESULTSThe prostate volume differed significantly (P < 0.05), while the testosterone level showed no significant difference in different age groups (P > 0.05). The level of the free testosterone in the serum descended with the increase of age. The incidence of ED and PADAM was also correlated with the increase of age.
CONCLUSIONIn the old and middle-aged population of males, with the increase of age, sexual function decreases, the prostate volume enlarges, and the incidence of ED and PADAM obviously increases (P < 0.001).

10.Effect of compound recipe Gengniankang on senile sexual hormone and expression of estrogen receptor in bone of climacteric female rats.

OBJECTIVETo compare the therapeutic effect of Compound Recipe Gengniankang ( GNK) with that of hormone replacement treatment (HRT) on climacteric female rats with osteoporosis, and to investigate the roles of estrogen and estrogen receptors in the mechanism of osteoporosis.
METHODSClimacteric female rats with osteoporosis were chosen and divided into three groups (GNK group, HRT group and control group). Apoptosis of ovarian granulose cells was measured by terminal-deoxynucleotidyl transferase mediated nick end labeling (TUNEL) assay. Serum level of estradiol (E(2)), follicle stimulating hormone (FSH), luteinizing hormone (LH) were determined by the method of radioimmunoassay (RIA). Reverse transcriptase polymerase chain reaction (RT-PCT) technology was used to evaluate the expression of estrogen receptor (ER) in bone. Bone mineral density (BMD) was measured by double energy X-ray absorption (DEXA).
RESULTSIn the climacteric rats, BMD, serum E(2), ER mRNA expression in bone decreased remarkably, and serum FSH, LH and apoptosis of ovarian granulose cells increased obviously. After treating with GNK, all the indexes were reversed except serum E(2). The increase of E(2) was not significant.
CONCLUSIONGNK is effective on climacteric osteoporosis female rats. Its role is performed not by increasing serum E(2) but by enhancing ER in the bone and inhibiting apoptosis of ovarian granulose cells. GNK can deter further exhaustion of ovarian function.
