3.Competitive exclusion against Salmonella gallinarum of Salmonella enteritidis infected chickens.

To evaluate the degree of competitive exclusion against Salmonella gallinarum(S. gallinarum) of Salmonella enteritidis(S. enteritidis) infected chickens, fifty-six, 4-week old Hyline layer suspected of S. enteritidis infection were challenged with S. gallinarum. All chickens were tested for S. enteritidis isolation using cloacal swabs and serum plate agglutination test using S. enteritidis Ag. before challenge and classified into four groups(SE isolated, SE nonisolated, SE seropositive and SE seronegative). None of the SE isolated and the SE seropositive groups died after challenge and the average weight gains were 245.5g and 254.6g, respectively. But in the SE nonisolated and the SE seronegative groups, mortality was 18.2% and 20.6% and the average weight gains were 150.1g and 111.2g. The incidence of reisolation of S. gallinarum of the SE isolated and the SE seropositive groups were 41.7% and 47.6% from liver, 33.3% and 47.6% from spleen and 8.3% and 14.3% from cecum, respectively, and the SE nonisolated and the SE seronegative group were 63.6% and 64.7% from liver, 84.1% and 88.2% from spleen and 47.7% and 52.9% from cecum. The serological response of the SE isolated and the SE seropositive groups hardly changed from 75.0 and 81.8% before challenge to 75.0 and 85.7% after. But, the other two groups were found to be significantly higher after challenge and increased from 0 and 18.2% to 100%. Consequently, S. enteritidis preinfected chickens were found to be significant different in terms of mortality, weight gain, reisolation of S. gallinarum and serological response compared to noninfected chickens. Moreover, our study shows that S. enteritidis infected chickens appear strong competitive exclusion against the colonization of S. gallinarum.

5.Comparison of Dietary Behaviors and Blood Clinical Indices in Underweight, Normal Weight, Normal Weight Obese and Obese Female College Students

OBJECTIVES: Normal weight obesity (NWO) is defined as excessive body fat in the context of a normal body mass index (BMI). This condition carries a greater risk of developing noncommunicable chronic disease and has been associated with early inflammation. This study was conducted to compare the anthropometric measurements, eating behaviors, and blood clinical indices among four groups: underweight, normal, normal weight obesity and obesity. METHODS: The subjects included 215 female college students. A questionnaire was administered regarding general characteristics, dietary behaviors, food consumption frequency. Anthropometric measurements and blood clinical indices were also investigated. RESULTS: The average BMI, body fat percentage, waist circumference, fat-free mass, and muscle mass were highest in the obesity group (p < 0.05). Most subjects had tried to lose weight and perceived that their health was worse than before they became college students. The ratio of students in the NWO group who thought their health was very poor was significantly higher than in the other three groups (p < 0.05). The obesity and NWO groups seemed to eat more and their eating speed was significantly faster than the other groups (pv0.001). The consumption frequency of caffeinated beverages was significantly higher in the NWO group than in the other three groups (p < 0.01). WBC was significantly higher in the obesity group (p < 0.05). Serum levels of TG and total cholesterol were also significantly higher in the obesity group (p < 0.05). Serum GPT was significantly higher in the obesity group (p < 0.05) while BUN level was highest in the NWO group (p < 0.05). CONCLUSIONS: The obesity group showed the most health problems while the NWO group seemed relatively healthy. However, NWO can lead to problems such as metabolic syndrome and cardiovascular disease in later life if poor dietary habits are maintained. Therefore, education in appropriate eating habits is needed for these subjects.

7.Comparison of Extended-Spectrum beta-Lactamase-Producing Escherichia coli and Klebsiella pneumoniae Bloodstream Infection Epidemiology.

BACKGROUND: Extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli and Klebsiella pneumoniae are main pathogens of bloodstream infection. We compared the epidemiology of ESBL-producing E. coli and K. pneumoniae. METHODS: From January 2003 through March 2007, we retrospectively investigated the clinical characteristics and comorbidities of patients with bloodstream infection by ESBL-producing E. coli and K. pneumoniae. RESULTS: During the study period, 79 patients (54 with E. coli and 25 with K. pneumoniae) with blood cultures positive for ESBL-producing E. coli or K. pneumoniae were studied. When comparing the source of bloodstream infections, urinary tract infections (UTIs) were more commonly caused by E. coli (24% vs. 0; P=0.007), and respiratory tract and soft tissue infection (36% vs. 15%; P=0.04, 12% vs. 0; P=0.03, respectively) were more commonly associated with K. pneumoniae. Among hospital-acquired bloodstream infection, third-generation cephalosporin was more commonly used in patients with E. coli than patients with K. pneumoniae (81% vs. 24%, P<0.001). Medical devices (central venous catheter and urinary catheter) were more commonly used in patients with K. pneumoniae. CONCLUSION: ESBL-producing E. coli bloodstream infection is more common in urinary tract infections. ESBL-producing K. pneumoniae is more common in respiratory tract infections and in skin and soft tissue infections. In hospital-acquired infections, ESBL-producing E. coli bloodstream infection is more common in biliary tract infection. ESBL-producing E. coli was more commonly associated with prior frequent antibiotics use and K. pneumoniae was more commonly associated with use of medical devices.

8.A Case of Severe Hypoglycemic Encephalopathy with Extensive Brain Lesions in Non-diabetics and Alcoholism.

Hypoglycemic encephalopathy is a rare problem among diabetic patients who are receiving treatment with insulin or other glucose-lowering drugs. The MRIs of patients with hypoglycemic encephalopathy commonly show scattered lesions in the cerebral cortex, hippocampus and basal ganglia, but lesions in the cerebellum or brain stem are extremely rare. A 44-year-old alcoholic woman without diabetes was admitted with a semicomatose mentality and seizure with severe hypoglycemic encephalopathy with extensive brain lesions seen on MRI at the middle cerebellar peduncle and midbrain, as well as in the other brain areas.

9.Treatment of Massive Pulmonary Embolism with Urokinase during Surgery for Femur Fracture.

A massive pulmonary embolism occurring during surgery is a fatal complication. Therefore, early detection and subsequent treatment of pulmonary emboli are important clinical challenges. In this report, an acute pulmonary embolism associated with cardiac arrest occurred during the surgical reduction of a femur fracture. It was diagnosed by echocardiography and treated with urokinase successfully. Therefore, we recommend the use of urokinase for the treatment of a intraoperative massive pulmonary embolism.

10.The Childbirth Experience and Life Satisfaction among Women with Disabilities

Purpose:
The aim of this study was to show the association between the childbirth experience and life satisfaction among women with disabilities and estimate the moderating effect of family strengths and social support.
Methods:
The target sample included disabled married women aged 20-49 years. We used the Disability and Life Dynamics Panel 2018. The total number of participants was 220. Three groups were defined based on the childbirth experience: women who had never given birth (13.6%), those who gave birth before the onset of disability (73.2%), and those who gave birth after the onset of disability (13.2%). We identified differences in general characteristics, family strengths, social support, and life satisfaction across the 3 groups and examined the moderating effect of family strengths and social support for the childbirth experience and life satisfaction.
Results:
Women who gave birth after the onset of disability were in better health, exhibiting longer disability retention periods. The multiple regression analysis revealed that among women who gave birth after the onset of disability, life satisfaction was significantly higher than that of women without the childbirth experience because of the moderating effect of family strengths. However, social support had no significant effect on the childbirth experience and life satisfaction.
Conclusion
We empirically analyzed the status of childbirth before and after the onset of disability using representative survey data. The findings indicate a positive moderating effect of family strengths on the level of life satisfaction among women with disabilities.
