1.Diagnosis and treatment of post-orthotopic liver transplantation complications (a report of 7 cases)

Objective To investigate the experience of diagnonsis and treatment of post-orthotopic liver transplantation (OLT) complications. Methods The clinical data of diagnosis and treatment of post-OLT complications in 7 cases were analysed retropectively. Results Complications following OLT including intracranial hemorrhage (1/7), renal failure (1/7), intrabdominal hemorrhage (2/7), pulmonary infection and/or, pleurorrhea (5/7), adult respiratory distress syndrome (1/7), billirubinemia (5/7). Five patients survived while two died. Conclusions Proper prevention and management can effectively reduce post-OLT complications, Timely diagnosis and suitable therapy would improve the result of liver transplantation.
2. Effects of rosuvastatin on LDL-C concentration and incidence of cardiovascular events after percutaneous coronary intervention in patients with diabetes mellitus associated with mild -moderate renal insufficiency

Objective To explore the effects of short-term application of rosuvastatin on LDL-C concentration and the incidence of cardiovascular events after percutaneous coronary intervention (PCI) in patients with diabetes mellitus associated with mild-moderate renal insufficiency (MMRI). Methods From Dec. 2008 to Oct. 2011, 2998 patients were enrolled from 53 centers in China for a TRACK-D project. Of them, 1590 patients to receive PCI were selected for this trial and assigned to rosuvastatin group (n=808) and control group (n=782). Patients in rosuvastatin group were treated with 10mg rosuvastatin every evening for 2 days before and 3 days after intervention. Control patients did not receive statin treatment in the 2 days before and 3 days after the procedure. The levels of LDL-C in the serum were measured before and after 72 hours of interventional treatment. The cardiovascular event rate was recorded during hospitalization and follow-up of 30 days. Results The baseline data were not significantly different (2.56±0.81mmol/L vs 2.52±0.81mmol/L, P>0.05) between the two groups. At 72 hours after PCI, the LDL-C concentration was lower in rosuvastatin group than in control group (2.12±0.67mmol/L vs 2.61±0.67mmol/L, P<0.01). The difference in LDL-C between post- and pre-operation were -0.42±0.59mmol/L vs 0.09±0.54mmol/L in rosuvastatin group and control group respectively and statistically significant (P<0.01). During the hospitalization and follow-up period of 30 days, the incidence of cardiovascular events was lower in rosuvastatin group than in control group (3.3% vs 4.9%, P=0.127), without significant difference. Conclusion The short-term application of rosuvastatin during the perioperative period in the patients with diabetes mellitus associated with MMRI (stages 2-3) can reduce LDL-C concentration and the incidence of cardiovascular events.
3.Survey and Analysis on nurses'knowledge and attitude about HIV/AIDS

Objeetlve To identify the nurses’knowledge and attitude about HIV/AIDS,and to provide evidence for implementing the related education and training.Methods The questionnaire survey was used to collect the data of knowledge and attitude about HIV/AIDS in Sichuan,Yunnan and Guangxi Provinces.A total of 463 numes participated the study.Results Except HIV/AIDS training.training for nurses and hospital with HIV/AIDS patients.the other personnal information of nurses had significant difierence among the numes in the three areas(P<0.05).The mean score of uuraes’knowledge about HIV/AIDS Was(12.95±2.35).There were no significant difiefence between the mean scores in the three areas(P<0.05).The score of nurses’HIV/AIDS attitude Was(3.38 4±0.30),with no statistical meaning in the three areas(P>0.05).In the aspects of 18 items of work-related attitude.4 items were approved by over 80%nurses.There were signicanfly different meanings in 6 itents among the 3 area nurses.Conciusions Nurses’knowledge level and uncertain attitude to HIV/AIDS could not met the need of higher AIDS epedimic in the 3 areas.especially in Sichuan.Delivering training course for nurses about HIV/AIDS in these areas is very urgent.

5.Expressions and clinicopathological significances of BMI-1 and PADI4 in esophageal squamous cell car-cinoma

Objective To evaluate the expression levels of peptidylarginine deiminase 4 (PADI4)and B-cells pecific Moloney leukemia virus insert site-1 (BMI-1 )in esophageal squamous cell carcinoma (ESCC) tissues and pericarcinous tissues.To explore the function and clinical significance in the development of ESCC and their association.Methods The expression levels of PADI4 and BMI-1 were measured by immunohisto-chemistry,Western blotting and quantitative real time PCR in ESCC tissues and pericarcinous tissues from 86 patients.The relationships between the expressions of PADI4 and BMI-1 and the clinicopathologic characte-ristics were analyzed.Results The immunohistochemistry showed that the expressions of PADI4 and BMI-1 in ESCC tissues (68.6% and 73.3%)were significantly higher than those in pericarcinous tissues (37.2% and 30.2%,χ2 =1 7.01 1 ,P =0.000;χ2 =31 .876,P =0.000).Western blotting indicated that the levels of PADI4 and BMI-1 were higher than those in pericarcinous tissues (0.91 9 ±0.098 vs.0.71 8 ±0.1 03,t =2.462,P =0.021 ;0.975 ±0.074 vs.0.71 7 ±0.071 ,t =2.640,P =0.01 4).The expressions of BMI-1 and PADI4 mRNA in ESCC tissues were higher than those in pericarcinous tissues,but the differences were not sta-tistically significant (0.091 ±0.005 vs.0.038 ±0.002,t =1 .701 ,P =0.1 01 ;0.1 1 4 ±0.075 vs.0.048 ± 0.003,t =1 .499,P =0.1 46)by the quantitative real time PCR.The expression of PADI4 was correlated with lymph node metastasis (χ2 =5.771 ,P =0.01 6),depth of invasion (χ2 =6.672,P =0.01 0)and clinical stage (χ2 =5.771 ,P =0.01 6).The BMI-1 gene expression had a correlation with lymph node metastasis (χ2 =7.1 76,P =0.007),the differentiation degree (χ2 =1 3.787,P =0.001 )and clinical stage (χ2 =7.1 76,P =0.007).In addition,there was a positive correlation between PADI4 and BMI-1 expression in ESCC by immunohistochemistry and quantitative real time PCR (r =0.21 4,P =0.047;r =0.534,P =0.005).Conclusion The expression levels of PADI4 and BMI-1 are significantly higher in ESCC compared to pericarcinous tissues.PADI4 and BMI-1 are positively correlated and may contribute to the diagnosis and prog-nosis of the ESCC.

In order to evaluate the effect and mechanism of the mulberry leaf alkaloid, flavones, and polysaccharide intervention on diabetes, the overall metabolite profiling characteristics for the plasma of diabetic mouse was performed by using an ultra-performance liquid chromatography/electrospray-tandem mass spectrometry (UPLC-ESI-MS). The 8 potential biomarkers were found in diabetic mice plasma based on the data of MS/MS characteristics obtained from the UPLC-OrbitrapMS analysis, which mainly involved in sphingolipids, amino acid metabolic pathway. The principal component analysis showed that the normal group and model group were obviously distinguished and implied that metabolic disturbance was happened in diabetic mice plasma. The extracts of mulberry leaf flavonoids, polysaccharide, alkaloid had exhibited the effects of callback function for diabetic mice through regulating the amino acid metabolism and sphingolipid metabolism.

7.Application of peritoneal dialysis for the treatment of acute renal failure in children underwent cardiac surgery

10.Current understanding of iron overload hazard in patients with myelodysplastic syndrome.

Patients with myelodysplastic syndromes (MDS) become dependent on blood transfusions and develop into transfusional iron overload, which is exacerbated by increased absorption of dietary iron in response to ineffective erythropoiesis. However, it is uncertain whether there is an association among iron accumulation, clinical complications, and decreased likelihood of survival in MDS patients. Thereby our current understanding of the effects of transfusion dependency and iron overload in MDS are discussed. Particular emphasis should be placed on further characterizing the role of redox-active forms of labile iron and oxidative stress in iron overload, decreased life expectancy and increased risk of leukemic transformation in MDS patients with iron overload.
