1.Effects of miniscrew implant anchorage on adjacent roots and periodontal tissues
Chinese Journal of Tissue Engineering Research 2014;(38):6085-6089
BACKGROUND:Implanted site and direction of miniscrew implants, post-implantation movement of implants, contact between tooth and implant after tooth movement al can cause root damage.
OBJECTIVE:To investigate the changes of the root surface, periodontal tissue and osteoprotegerin expression in the periodontal tissue at periods of time after miniscrew implant loading.
METHODTwo beagle dogs were selected. For each dog, miniscrews were implanted into the labial alveolar bone near the roots of the maxil ary second, third, and fourth premolars and the maxil ary second, third, and fourth premolars and first molar. Fourteen miniscrew implants were in one dog, including six in the maxil ary and eight in the mandible;two miniscrews of the maxil ary acted as control group with no loading, and the rest miniscrews were as experimental group under orthodontics force. Two weeks after miniscrew implantation, 150 g force was loaded between two miniscrews with nickel-titanium closed-coil springs. The dogs were sacrificed after miniscrews loading 4 and 8 weeks respectively, the teeth were cut completely with alveolar bone tissue and decalcified to make the histological specimen. The histological changes of the root and periodontal tissue were observed by hematoxylin-eosin staining and expression of osteoprotegrin in the periodontal ligament was also inspected by using immunohistochemistry.
RESULTS AND CONCLUSION:When the miniscrews were adjacent to the roots, absorption lacuna occurred in the alveolar bone;after horizontal force was loaded opposite to the roots, alveolar resorption became much more animate. When the miniscrews contacted with the pericementum, some areas of the cement absorbed seriously and even reached the dentin;when sustaining pressure from the miniscrews, the whole layer of the cement absorbed and the dentin was exposed and began to be absorbed. The osteoprotegrin in the periodontal ligament of control group expressed sharply and its expression was notable at 8 weeks;while the osteoprotegrin of experimental group weakly expressed at 8 weeks, and there was significant divergence between control group and experimental group at 8 weeks (P<0.01). The roots and periodontal tissue were al damaged variously after miniscrew implantation adjacent to the roots. When horizontal orthodontic force was loaded opposite to the roots, the expression of osteoprotegrin was influenced slightly;as the loading period prolonged, pressure dramatical y restrained the expression of osteoprotegrin. When discovering the miniscrew implant adjacent to roors, the miniscrew implant anchorage should not be used to load orthodontical force opposite to the roots but ought to be removed in order to leave the root repair autonomously and avoid further damage.

5. Diminished dose nedaplatin combined with low-dose cisplatin as first-line therapy for advanced esophageal carcinoma: A randomized clinical trial
Tumor 2008;28(5):446-449
Objective: To investigate the safety and efficacy of the combination of diminished dose of nedaplatin (NDP) and low dose of cisplatin (PDD) for advanced esophageal carcinoma. Methods: The patients who had no indications for surgery or radiotherapy were recruited in our study. They were divided into the three groups randomly. Group A were given NDP (25 mg/m2, iv) and PDD (15 mg/m2, iv) on day 1 and day 8 and continuously infused with 5-FU (300 mg/m2) for 24 h on days 1-5 and days 8-12. Group B were given NDP (40 mg/m2, iv) on days 1-2 and continuously infused with 5-FU (500 mg/m2, iv) on days 1-5. Group C were administered PDD (40 mg/m2, iv) on days 1-2 and infused with 5-FU (500 mg/m2, iv) on days 1-5. All the patients were given folic acid tablet 60 mg/d following infusion of 5-FU. The therapeutic regimens were repeated every 22 days (one cycle). The effect was evaluated after two cycles. Results: The total response ratio (complete response and partial response) and median remission time were 60.00% and 5.5 months for group A. respectively; 54.54% and 5.0 months for group B, respectively; 41.18% and 3.0 months for group C, respectively. The difference was not significant between group A and group B (P>0.05). The clinical efficacy in groups A and B was significantly different compared with group C (P<0.05). The main toxicities included leucopenia and thrombocytopenia. The incidence rate of III to IV grade leucopenia was 14.29%, 27.27%, and 8.82% in groups A, B, and C, respectively; and that of thrombocytopenia was 11.43%, 39.39%, and 5.88%, respectively. The difference was significant (P<0.05). Conclusion: Diminished dose of NDP combined with low dose of cisplatin has definite effects on advanced esophageal carcinoma with less hematological toxicity.

Department of rheumatology and immunology is a compulsory training department for the standardization training of the residents. However, it has been noted that the residents often ignore the importance of rheumatology. They are usually short of communication skill with the patients and theoretical knowledge. They often have high error rate in the clinical work and training their clini-cal skills and thinking ability is very difficult in the actual work. All of these problems may decrease the training quality and make the training a mere formality. Therefore, in order to carefully implement resident standardization training, a set of specific training programs has been developed, involving in theory of knowledge, practice standardization and comprehensive quality training. With these training programs, the quality and efficiency of the residents' training has greatly improved.

Objective To investigate the factors related to outcome (recovery of motor, activities of daily living, and balance) of stroke patients. Methods 67 stroke patients with hemiplegia were evaluated with the Ueda Classfication of upper and lower limbs function, Functional Independence Measure (FIM), Berg balance scale (BBS) before and after treatment. The scores of all the assessment were as the dependent variable respectively, and regression analyzed with the factors possiblely related to the outcome. Results The positive factors for lower limbs function included rehabilitation intervention, time of rehabilitation, and hypercholesterolemia; while the negative factors were body mass index (BMI), and damage of superficial sensibility. The positive factors for upper limbs function were rehabilitation intervention and time of rehabilitation; while the negative factors were BMI, dysphagia, damage of superficial sensibility, and frequency of attack. The positive factors for hand function were rehabilitation interventions and interval from diagnosis to rehabilitation, while the negative factors were the muscle tension, incontinence, and frequency of attack. The positive factors for FIM were the rehabilitation intervention, while the negative factors were BMI, incontinence, hypercholesterolemia, gender and residence status. The positive factors for BBS were the rehabilitation intervention, while the negative factors were BMI, frequency of attack, and hyperglycemia. Conclusion Rehabilitation promotes the recovery of stroke patients, but the BMI may block it.

Objectives To evaluate the effect of N-Acetylcysteine on the airway inflammation and remodeling of obese asthma mice with high-fat diets. Methods Forty female C57BL/6J mice were randomly divided into 4 groups, asthma group (A), obese asthma group (B), treatment group (C) and normal control group (D). Group A were sensitized and challenged by ovalbum (OVA) and normal diets. Group B were sensitized and challenged as group A but fed with high-fat diets, while group C were sensitized, challenged and fed as group B, but administrated N-Acetylcysteine 200 mg/kg. d from the third week after challenge. The cells in BALF were counted and classified after staining, IL-6 and 8-iso-PGF2α(8-iso-PGF2α) in lung tissues were detected by ELISA. WAt, WAm, Pbm, as the remodeling indices, measured in lung pathological section. All parameters were compared among 4 groups. Results In comparison with group D, the leukocytes and EOS in BALF, WAt/Pbm, WAm/Pbm in lung section were increased as well as IL-6 and 8-iso-PGF2α in lung tissue elevated in group A and group B, while the maximum changes were observed in group B (P <0. 05). After NAC treatment, the IL-6, 8-iso-PGF2α and WAt/ Pbm were decreased significantly (P <0. 05). Pearson correlation analysis showed that WAt/Pbm and IL-6 were in positive correlation with 8-iso-PGF2α (r =0. 817, 0. 737, P <0. 01). Conclusions N-Acetylcysteine can alleviate the airway inflammation and remodel reaction of asthma by a substantial inhibition of the oxidative stress reaction in lung.
