Objective To investigate the effects of different positions of the patients mechanically ventilated under general anesthesia on respiratory mechanics during one or both lung ventilation before thoracotomy.Methods Twelve ASA Ⅰ or Ⅱ patients of both sexes (8 males, 4 females) aged 33-63 years weighing 62-85 kg scheduled for general thoracic surgery were studied. The preoperative lung function of the patients was normal. The patients were premedicated with intramuscular pethidine, promethazine and atropine. Anesthesia was induced with fentanyl, droperidol, midazolam, propofol and vecuronium. Left-sided double-lumen tube was inserted and correct placement was confirmed using a fiberoptic bronchoscope inserted through the bronchial lumen. Anesthesia was maintained with propofol and vecuronium infusion. The magnitude of neuro-muscular blockade was monitored by stimulation of radial nerve with TOF pulses. T4/T1 was maintained at 0 during study. The patients were placed in supine, left and right lateral position. In each position both lungs, left and right lung were ventilated for 10 min respectively. The non-ventilated bronchial lumen was open to the air during one-lung ventilation. The ventilatory parameters were kept unchanged during the study (VT = 8-10 ml?kg-1 , RR = 12 bpm, I:E = 1:2). Pressure-volume loop (P-V loop), peak airway pressure (Ppeak), airway resistance (Raw), total compliance (CT) (lung and thorax) and PETCO2 were measured and recorded at the end of each 10 min ventilation using S/5 monitor ( Datex-Ohmeda Finland) . Turbulence of immune function begotten by infect-agents attack may occur in the individuals with Eales′ disease due to the specific hereditary diathesis of HLA, which may cause the occurrence and development of Eales disease.
4.Ilizarov external fixation combined with vacuum sealing drainage for the repair of limb fractures with osteofascial compartment syndrome

BACKGROUND:Osteofascial compartment syndrome is usual y due to application of external fixation for fracture treatment. Vaseline gauze is used to cover and compress the wound. Frequent dressing change induces infection and is harmful to wound drainage and tissue granulation, final y causing prolonged time of hospitalization and bad results. <br> OBJECTIVE:To investigate the clinical efficacy of Ilizarov external fixation combined with vacuum sealing drainage treatment for limb fractures with osteofascial compartment syndrome. <br> METHODS:A total of 32 patients with limb fractures with osteofascial compartment syndrome were randomly divided into two groups. Al patients adopted the same fasciotomy and drug treatment. In treatment group, the fracture was fixed with Ilizarov external fixation and the wound was covered with the vacuum sealing drainage dressing. In control group, the fractures were treated by unilateral external fixation and the wound was covered with the vaseline gauze. The fracture healing, clinical effects, hospitalization duration and costs between two groups were compared. <br> RESULTS AND CONCLUSION:In the treatment group, there were 10 cases of healing wel , 4 cases of muscle contracture and 2 cases of amputation. In the control group, there were 7 cases of healing wel , 6 cases of muscle contracture and 3 cases of amputation. No significant difference in clinical outcome measures was detected between the two groups. Fracture healing rate and costs were higher in the treatment group than in the control group (P<0.05), but hospitalization duration, skin grafts and the incidence of infection were lower in the treatment group than in the control group (P<0.05). Results suggested that on the basis of timely fasciotomy and decompression, Ilizarov external fixation combined with vacuum sealing drainage treatment for limb fractures with osteofascial compartment syndrome has a high limb salvage rate, which can effectively treat fractures, elevate the healing rate of fractures, reduce infection rate and skin grafting rate, shorten hospital stays. However, the hospitalization expense is higher.
5.Epidemiological characteristics and therapeutic strategies of hepatitis C virus genotype 6 in Guangxi area

Objective To investigate the epidemiological characteristics and therapeutic strategies of patients infected with hepatitis C virus (HCV)genotype 6 in Guangxi area.Methods Serum samples were collected from 150 patients with serologic HCV RNA positive in Guangxi, China. Reverse transcription nested polymerase chain reaction (PCR)was employed to amplify HCV NS5B fragments and the DNA products were sequenced.The sequences obtained were compared with the sequences deposited in GenBank to construct a phylogenetic tree.Among the patients who accomplished 48-week treatment of interferon plus ribavirin and 24-week follow-up after stopping medication,10 cases were infected with genotype 6a and 28 cases with genotype 1 HCV.The virological responses were evaluated at week 4,week 12,week 24 of treatment and week 24 after the end of the treatment.Results Among all recruited 150 cases,21 (14.0%)cases were HCV genotype 6 including two subtypes 6a (n = 20 )and 6d (n = 1 ). Genotype 6 HCV mainly affected intravenous drug users, especially with age of ≤ 40 years old. Phylogenetic tree showed that there was very close evolutionary distance between HCV 6 strains of Guangxi and Hongkong,China strains (Y12083,DQ 480515)and Vietnam strain (EU246930).All of 10 HCV genotype 6a patients who completed 48 weeks of antiviral therapy achieved sustained virological response (SVR).The rate of SVR was higher than that of genotype 1 patients,but without statistically different significance (10/10 vs 75 .0%,P >0.05).Conclusion HCV genotype 6 in Guangxi area mainly affects young intravenous drug users with age of ≤ 40 years old,which has high homology with Hongkong,China and Vietnam standard strains.Patients with HCV 6 genotype infection treated with interferon plus ribavirin for 48 weeks usually achieve favorable SVR.

AIM: To study the interaction effects of Astragalus polysaccharide(APS) and total glucosides of paeony(TGP) with various dosage on the platelet aggregation. METHODS: Platelet aggregation was estimated by the turbidimetric method. The best interaction effects were determined by uniform design method. RESULTS: APS showed apparent anti-platelet effects and better combination with TGP,and the ratio of 121 was the best one. CONCLUSION: APS concerted with TGP inhibits platelet aggregation.
7.Effect of L-arginine and L-arginine decarboxylase antibodies on pain threshold and analgesic effect of morphine

AIMTo further elucidate the role of agmatine on the pharmacological effects of opioids. METHODSThe effect of L-arginine and L-arginine decarboxylase(L-ADC) antibodies on pain threshold, morphine ntinociception and tolerance were investigated in mouse acetic acid writhing test, mouse radiant heat tail flick test and mouse hot plate test. RESULTSIn mouse acetic acid writhing test, intracerebroventricular injection of L-arginine dose-ependently inhibited the writhing of mice compared with saline control. L-arginine did not influence the tail flick latency itself in mouse radiant heat tail flick test, but enhanced antinociceptive effect of morphine in a dose-dependent manner. The possible maximal analgesia percentage of morphine 2.5 mg*kg-1 was increased from 23% to 71%. Furthermore, L-arginine inhibited acute tolerance induced by morphine 100 mg*kg-1in mouse radiant heat tail flick test. The effect of L-arginine as mentioned above could be antagonized by idazoxan (3 mg*kg-1, ip), which is a selective antagonist of imidazoline receptors. L-ADC specific antibodies inhibited morphine antinociception and promoted the development of tolerance to morphine in mouse radiant heat tail flick test and 55℃ hot plate test. CONCLUSIONL-Arginine and L-ADC play important roles in the formation of pain threshold, morphine antinociception and tolerance.
8.Effect of xuebijing injection on inflammatory response and cellular immune function in patients with severe sepsis

ObjectiveTo evaluate the effect of xuebijing injection on inflammatory response and cellular immune function in patients with severe sepsis.MethodsSixty-two patients with severe sepsis from September 2008 to August 2009 were divided into treatment group(30 patients) and control group (32patients) by random digits table.All the patients received sepsis-bundle therapy and patients in treatment group added xuebijing injection therapy with 100 ml,twice a day,for 7 days.The levels of serum tumor necrosis factor-alpha(TNF-α ),interleukin(IL)-6,IL-10 and C-reactive protein (CRP),peripheral blood T lymphocyte CD4+、CD8+ 、CD4+/CD8+,the expression of human leucocyte antigen (HLA)-DR on CD14+peripheral blood mononuclear cell (PBMC) were detected before and aftertreatment.ResultsThere was no significant difference in the levels of serum TNF-o,IL-6,IL-10,CRP,peripheral blood Tlymphocyte CD4+、CD8+、CD4+/CD8+ and the expression of HLA-DR on CD14+ PBMC before treatment between two groups(P > 0.05).After treatment,compared with those in control group,the levels of serum TNF- α,IL-6,IL-10 and CRP in treatment group were significantly decreased [ ( 64.4 ± 13.5) ng/L vs.(96.1 ± 22.1 ) ng/L,( 153.8 ± 23.8 ) ng/L vs.(180.1 ± 21.7) ng/L,(73.8 ± 13.8) ng/L vs.(101.1 ± 11.7) ng/L,(53.7 ± 18.8) mg/L vs.(91.3 ± 32.8)mg/L,P <0.05],while peripheral blood T lymphocyte CD4+/CD8+ and the expression of HLA-DR on CD14+PBMC were significantly increased [ 0.311 ± 0.021 vs.0.424 ± 0.035,0.201 ± 0.017 vs.0.238 ± 0.038,1.78 ±0.21 vs.1.56 ±0.18,(38.4 ± 11.5)% vs.(18.1 ± 12.1)%,P<0.05].ConclusionXuebijing injection can reduce the inflammatory response and ameliorate immune disorder in patients with severe sepsis.
9.Effects of pretreatment with nerve growth factor-beta on ischemia-reperfusion injury in isolated rat hearts:a comparison with preconditioning

Objective To evaluate the effect of pretreatment with nerve growth factor-beta (NGF-β) on ischemia-reperfusion (I/R) injury in isolated rat hearts by comparing it with NGF-β preconditioning.Methods Pathogen-free male Sprague-Dawley rats,weighing 200-300 g,aged 8-10 weeks,were anesthetized with intraperitoneal 10% chloral hydrate 300 mg/kg.Their hearts were excised and perfused in a Langendorff apparatus with KH solution aerated with 95% O2 and 5% CO2 at 37℃.Twenty-four isolated rat hearts were randomly divided into 3 groups (n =8 each) using a random number table:I/R group,NGF-β pretreatment group (group NGFPt) and NGF-β preconditioning group (group NGFPc).The hearts were perfused with K-H solution for 10 min (equilibration) in each group.In group I/R,the hearts were continuously perfused with K-H solution for 30 min.The hearts were continuously perfused with K-H solution containing NGF 0.1 μg/ml for 20 min before ischemia in group NGF-Pt.The hearts were continuously perfused with K-H solution containing NGF 0.1 μg/ml for 20 min followed by 10 min washout before ischemia in group NGFPc.The perfusion was suspended for 30 min followed by 120 min of reperfusion with K-H solution in each group.HR,left ventricular end-diastolic pressure (LVEDP),left ventricular developed pressure (LVDP) and + dp/dtmax were measured at the end of 10 min equilibration (baseline,T1),immediately before suspension of perfusion (T2),and at 5,30,60 and 120 min of reperfusion (T3-6).The activities of creatine kinase isoenzyme-MB (CK-MB) and lactate dehydrogenase (LDH) in coronary effluent were measured at T1 and T3-6.Myocardial specimens were obtained at T6 for detection of myocardial apoptosis (by TUNEL) and for microscopic examination.Apoptotic index (AI) was calculated.Results Compared with the baseline value at T1,+ dp/dtmax was significantly decreased,and LVEDP and activities of CK-MB and LDH were increased at T3-6 in each group,LVDP and HR were decreased at T3-6 in group I/R,LVDP was decreased at T3,4 in group NGFPt and at T3-6 in group NGFPc,and HR was increased at T2-6 in NGFPt and NGF Pc groups (P < 0.05).Compared with group I/R,LVDP,+ dp/dtmax and HR were significantly increased and LVEDP and activities of CK-MB and LDH and AI were decreased in NGFPt and NGFPc groups (P < 0.05).Compared with group NGFPc,LVDP,+ dp/dtmax and HR were significantly increased,while the LDH activity and AI were decreased (P < 0.05) and no significant changes were found in LVEDP and CK-MB activity in group NGFPt (P > 0.05).The pathologic changes of myocardium were significantly reduced in NGFPt and NGFPc groups as compared with I/R group.Conclusion Pretreatment with 0.1 μg/ml NGF-β attenuates I/R injury in isloated rat hearts,and the efficacy is superior to that of NGF-β preconditioning.
10.Study on antioxidant chemical constituents of Lonicera japonica leaves.

Guided by the antioxidant activity, the EtOAc-soluble and n-butanol-soluble fractions of the 50% methanol extract of Lonicera japonica leaves were isolated and purified by various chromatorgraphic methods, and the structures were identified by spectral analysis and comparison to the data reported in literature. As a result, nine compounds were obtained and identified as 5-O-caffeoylquinicacid (1), chlorogenicacid (2), 4-O-caffeoylquinicacid (3), luteolin-7-O-[α-L-arabinopyranosyl-(1 --> 6)] -β-D-glucopyranoside (4), luteoloside (5), 3,4-di-O-caffeoylquinic acid (6), 3,5-di-O-caffeoylquinic acid (7), 4, 5-di-O-caffeoylquinic acid (8) and luteolin (9). The antioxidant activity of the nine compounds were determined by using DPPH free radical scavenging method, and ascorbic acid was used as a positive control. Their antioxidant activities from high to low were 5 > 9 > 2 > 8 > 7 > 6 > 1 > 3 > 4. Among them, luteoloside (5) had the strongest antioxidant activity with an IC50 of 0.018 18 g x L(-1), and luteolin (IC50 0.023 6 g x L(-1)) and chlorogenicacid (IC50 0.035 17 g x L(-1)) ranks No. 2 and 3. Furthermore, the antioxidant activity of luteoloside and luteolin were stronger than that of ascorbic acid (IC50 0.027 54 g x L(-1)). These results gave a basis for the further study and utilization of L. japonica leaves.
