3.Clinical Study of Nutritional Intervention in 575 Cases Patients with Chronic Obstructive Pulmonary Disease and Malnutrition Risks

Objective To exPlore the efficacy of nutritional interVention in Patients with chronic obstructiVe Pulmonary disease (COPD) and malnutrition risks. Methods From Jan. ,2008 to Dec. ,2012,829 COPD Patients with NRS2002 score≥3 in Qinzhou Second PeoPle's HosPital were enrolled in this study. Patients were randomized into control grouP (254 cases) and treatment grouP (575 cases) by random numerical table of SPSS 13. 0 statistic software. Patients without contraindication to enteral nutrition were giVen enteral nutrition suPPort,while those with contraindication to enteral nutrition were giVen Parenteral nutrition suPPort. Patients in the treatment grouP receiVed intensiVe suPPort with fortified nutrition,whereas Patients in the control grouP receiVed routine nutrition treatment. All other treatment methods were the same between the two grouPs. TelePhone follow_uP lasted for 3 years in both grouPs after discharge. Patients in the treatment grouP with NRS2002 score≥3 were giVen guidance on nutrition food intake. No nutrition guide was giVen to the control grouP. Times of acute attack,times and duration of mechanical Ventilation,mortality rate,and NRS2002 score three years after the treatment were comPared between the two grouPs. Data were analyzed by multi_factor Logistic regression analysis to understand the nutritional factors of COPD Patients affecting their mortality rate. Results After 3 years of follow_uP,times of acute attack,times and duration of mechanical Ventilation were lower in the treatment grouP than in the control grouP. Mortality rate was significantly lower in the treatment grouP (0. 696%) than the control grouP (4. 724%). After treatment,NRS2002 score was PositiVely correlated with mortality rate of COPD Patients with malnutrition risks. Conclusion For the COPD Patients with malnutrition risks, actiVe nutritional interVention can imProVe their nutrition status ( lower NRS2002 score) ,increase the number of resPiratory muscles to alleViate anoxia,enhance cellular immune function, and thus imProVe their Prognosis.
5.Multi-disciplinary collaboration in management of acute kidney injury caused by crush syndrome after earthquake

On May 12,2008,a disastrous earthquake scaled 8.0 Richter hit Wenchuan,Sichuan province in China.Treating the acute kidney injury caused by crush syndrome in survivals of the earthquake has been a big challenge to the nephrologists.In this paper,we shared our experiences on the multi-disciplinary collaboration in management of acute kidney injury caused by crush syndrome.In addition to surgical therapy for crush injury and compartment syndrome and the renal replacement therapy for acute renal injury and its related complications,the early multi-disciplinary collaboration including rehabilitation,mental health care,infection control and ICU also contributed greatly to the successful treatment of the victims of the earthquake.

7.Study on acting mechanism of extracts from ginseng, notoginseng and chuanxiong for delaying the aging of endothelial cells induced by angiotensin II.

OBJECTIVETo explore the effects of extracts from ginseng, notoginseng and chuanxiong (Ext) on the angiotensin II induced aging of human umbilical endothelial cells (HUVECs) in vitro.
METHODSCultured HUVECs were divided into 5 groups, the blank control group (A), the model group (B), and the three intervening groups (C, D and E). Except those in Group A, all cells were induced into aging model cells by Ang II in a final concentration of 10(-6) mol/L, and to the three intervening groups, corresponding treatments with low dose Ext (C), high dose Ext (D) and valsartan (E) were accessed. Changes of aging in HUVECs were observed by SA-beta-gal staining; cell cycle was analyzed by flow cytometry; contents of reactive oxygen species (ROS) in cells was measured by laser confocal microscopy; levels of nitric oxide (NO) and anti-superoxide anion in culture medium were examined by nitrate reductase method; and the protein expression of NAD (P) H oxidase p47phox, as well as the angiotensin type 1 and 2 receptor (AT1 R, AT2R) were detected by Western blot.
RESULTSCompared with Group A, in Group B, the positive beta-gal stained HUVECs increased and stagnated at G0-G1 phase, with the fluorescence intensity of ROS evidently enhanced; in the culture medium content of NO and anti-superoxide anion were lowered, and protein expression of p47phox and AT1 R up-regulated. While these aging figures were improved in Group C and D. After intervention with high or low dose of Ext, beta-gal stained HUVECs decreased showing less cells in G0-G1 phase and more cells in G2-M phase, the fluorescence intensity of ROS reduced, contents of NO and anti-superoxide anion increased, and the protein expression of p47phox and AT1 R down-regulated.
CONCLUSIONExt could delay the aging of HUVECs induced by angiotensin II, it is possibly by way of down-regulating the expression of NAD (P) H oxidase subunit-p47phox through AT1 R, and further reducing the superoxide anion production.

8.Determination of triterpene delta-amyrone in Sedum sarmentosum.

OBJECTIVETo establish the quantitative analysis method of triterpene delta-amyrone in Sedum sarmentosum by HPLC-ELSD.
METHODThe Zorbax Sil Column was used and the mobile phase was dichloromethane-methanol (99:1). The flow rate was set at 0.5 mL x min(-1), the temperature of drift tube was set at 100 degrees C and the nebulizer nitrogen flow rate was 3.0 L x min(-1).
RESULTThe linear range of sigma-amyrone was 2.12-10.6 microg. The average recovery was 100.5%.
CONCLUSIONThe results showed that the method was reliable, simple and precise. It would be beneficial for the study of the quality control of S. sarmentosum.

9.Molecular authentication of Sailonggu and its resource distribution in Qinghai-Tibet Plateau.

To provide accurate information on geographic distribution of crude drug Sailonggu in the plateau, we identified zokor species (Eospalax spp.) in Qinghai-Tibet Plateau using molecular methods. Based on the mitochondrial cytochrome B (cytb) gene sequences, we then extracted haplotypes from these sequences and reconstructed phylogenetic trees for the haplotypes using both maximum likelihood (ML) and Bayesian inference (BI) methods. Based on the trees, the species of each sample were determined. Five hundred and three samples from 35 populations were sequenced and their whole cytb sequences (1140 bp) were obtained. From these sequences 150 haplotypes were detected, in which, 126 were Eospalax baileyi, 20 were E. cansus, and 4 were E. smithi of the 35 populations, 28 were E. baileyi type, 5 were E. cansus type, and the remaining 2 were mixed of E. baileyi + E. cansus (DT2) and E. baileyi + E. smithi (ZN3). The results showed that, the regions around the Qinghai lake and near the upper stream of Yellow River started at Guide could be viewed as the producing area of authentic Sailonggu, and also, the cytb gene is a powerful molecular marker to determine the species of zokors as well as for the authentication of geographic distribution of Sailonggu.
