1.Analysis of application efficiencies of two types of intestinal ostomy in the treatment of elderly obstructive sigmoid colon cancer and colorectal cancer

Objectives To study application efficiencies of two types of intestinal ostomy in the treatment of elderly patients with obstructive sigmoid colon cancer and colorectal cancer.Methods 62 elderly patients with obstructive sigmoid colon cancer and rectal cancer from January 2013 to January 2016 in our hospital were chosen.The patients were divided into loop ileostomy group(n=32) and colostomy group(n=30)depending on the type of therapy.And the stage Ⅰ and Ⅱ operative time,hospital stay,postoperative fasting time and postoperative complication incidence rates were compared between the two groups.Results The postoperative fasting time was significantly shorter in loop ileostomy group than in colostomy group during stage Ⅰ surgery(2.5 ± 0.5)d vs.(4.6 ± 0.6)d(t=14.3644,P =0.0000).In stage Ⅱ surgery,the operative time,hospital stay,postoperative fasting time (2.9 ± 0.6)d vs.(4.9 ± 0.4)d,(1.4 ± 0.4)d vs.(2.5 ±± 0.3)d,(6.3± 1.4d)vs.(8.8±1.2)d were significantly lower in the loop ileostomy group than in the colostomy group(t=-15.2762、-10.9714、-7.6601,all P =0.0000).Conclusions Compared with colostomy technique,the loop ileostomy has advantages including shorter operation time,lower incidence of postoperative complications and quicker recovery after surgery for elderly patients with obstructive sigmoid colon cancer and colorectal cancer.
2.Design and synthesis of peptide-drug conjugates and fluorescent probe based on α -conotoxin ArIBV11L,V16D

italic>α-Conotoxin ArIB[V11L,V16D] is currently the most optimal selective inhibitor of α7 nicotinic acetylcholine receptor (nAChR) known. In order to explore chemical modification methods and enrich its application in targeting nAChR, this study utilized the linker to covalently connect camptothecin and 7-amino-4-methylcoumarin to the [2,4] disulfide bond of ArIB[V11L,V16D]. Therefore, two peptide-drug conjugates (PDCs), ArIB[V11L,V16D]-5 and ArIB[V11L,V16D]-6, and one fluorescent-labeled peptide, ArIB[V11L,V16D]-7 were constructed. Cytotoxicity evaluation showed that the IC50 values against non-small cell lung cancer cell line A549 of the two PDCs were respectively 1.3 and 4.1 times of camptothecin, indicating slight reduction in activity at the cellular level which was related to the linker structure. Fluorescence spectrum scanning revealed that the excitation and emission wavelength of the fluorescent-labeled peptide were 340 nm and 403 nm respectively, and the fluorescence features of 7-amino-4-methylcoumarin as a marker were retained without fluorescence quenching. 3.Preliminary application of real-time contrast-enhanced ultrasound in diagnosis of papillary thyroid microcarcinoma

Objective To explore the imaging features of papillary thyroid microcarcinoma(PTMC) with real time contrast-enhanced ultrasound.Methods One hundredand forty-three cases with 149 thyroid nodules(no diffuse lession) were divided into two groups according to the diameter size(group 1,<0.5 cm;group 2,0.5-1.0 cm) and examined by contrast-enhanced ultrasound during preoperation.Pathology was followed up as golden diagnosis criteria.Results Seventy-five benign tumors and 74 PTMC were confirmed by pathology.There were significant differences in echoes homogeneity between benign and malignant tumors in group 2(W =1 029.5,Z =-5.524,P =0.000) but no in group 1(W =933.0,Z =-1.738,P =0.082).And nonhomogeneous enhancement were showed in most PTMC in group 2.But most PTMC showed homogeneous enhancement in group 1.Conclusions Contrast-enhanced ultrasound is valuable in diagnosis of PTMC with the diameter size of 0.5-1.0 cm.
4.Application of the binary regression mode in analyzing the ultrasonographic features of solid thyroid micronodules

Objective To apply the binary Logistic regression model and evaluate the value of grey scale ultrasonography(US),ultrasond elastography(UE) and contrast-enhanced ultrasound(CEUS).Methods US,UE and CEUS were performed in 130 patients with 142 solid thyroid micronodules (the largest diameter less than 10 mm).Pathology was followed up as golden diagnosis criteria.A Logistic model on the basis of ultrasonographic features was obtained.A receiver operating characteristic(ROC) curve was used to assess the performance of Logistic model.Results Four ultrasonic indexes were finally entered into the Logistic regression model including microcalcification,acpect ratio,advanced CEUS mode and elasticity score.The percentage correction of prediction was 92.1% (129/142),the area under the ROC curve was 0.944.Conclusions The multivariate analysis model of binary Logistic regression can describe and analyze the process of differential diagnosis of malignant and benign solid thyroid micronodules by ultrasonography,and select out the valuable indexes for differential diagnosis.
5.Consumption of nutritional supplements among the residents aged eighteen years and below in Tongzhou District

Objective:
To understand the consumption of nutritional supplements among the residents aged eighteen years and below in Tongzhou District of Beijing,so as to provide reference for supplements exposure assessment and health education.
Methods:
A hundred people aged 18 years and below were selected from each of the eight neighborhood committees of Tongzhou Town,Tongzhou District,Beijing. The questionnaire for the Nutritional Supplements Guidelines for Chinese Residents Study was used to collect the rate of taking nutritional supplements,influencing factors,types and frequency.
Results:
The rate of taking nutritional supplements was 44.16%. The rates of taking nutritional supplements in people aged 0-5 years,6-12 years and 13-18 years were 45.56%,42.86% and 41.00%,respectively,without statistically significant difference among them (P>0.05). The rates of taking nutritional supplements in males and females were 44.82% and 43.49%,without statistically significant difference between them (P>0.05). The Results of multivariate logistic regression analysis showed that annual per capita household income of more than 50 000 yuan (OR=1.688,95%CI: 1.171-2.435) and parents taking nutritional supplements (OR=4.104,95%CI: 3.023-5.573) were the promoting factors for the intake of nutritional supplements in people aged 18 years and below. The rates of taking calcium,vitamin D and multivitamin were 26.10%,21.56% and 8.31%,respectively. There were 272 (80.00%) people taking 1-2 kinds of supplements,and 200 (58.82%) people taking them daily.
Conclusions
In Tongzhou District,44.16% of the population aged 18 years and below takes nutritional supplements,mainly calcium and vitamin D. Family income and whether their parents taking or not are determinants.
6.Effect of tetrahydropalmatine on expression of Cav1. 2 in dorsal root ganglion neurons in mice with sciatic nerve chronic constriction injury

Aim To investigate the analgesic effect of tetrahydropalmatine on Cav1 . 2 expression in the dorsal root ganglion ( DRG) of mice with sciatic nerve chronic constriction injury ( CCI ) -induced neuropathic pain. Methods Forty male C57 BL/6 mice were randomly divided into 5 groups ( n =5 ): sham group ( group S) , CCI group ( group C ) and L-THP group ( group L) . Steady mice models of neuropathic pain were es-tablished by inducing CCI of sciatic nerve. According to development of neuropathic pain in mice, L group was divided into induction period, induction with ma-intenance period and long-term low-dose group. The mice were intraperitoneally administered with 45 mg · kg-1 tetrahydropalmatine in induction ( day 0~5 ) , in-duction with maintenance ( day 0~5 , 14~19 ) period of neuropathic pain state. From the instant after opera-tion, 15 mg · kg-1 tetrahydropalmatine was injected into the long-term low-dose group once per day for 19 days. Then, the behavior changes of mice were moni-tored. Moreover, the threshold of mechanical and ther-mal stimuli was tested. In addition, the expression of Cav1 . 2 protein was detected by Western blot and im-munohistochemical staining. Results The lowest ex-pression of Cav1 . 2 was observed in group C and the highest expression level of Cav1 . 2 was found in group S. Cav1. 2 expression was significantly up-regulated in induction period group, induction with maintenance period group and long-term low-dose group ( P<0. 05 , P<0. 01). Compared with group C, high dose of tet-rahydropalmatine in induction period group, induction with maintenance period group and long-term low-dose group showed reduced mechanical allodynia and ther-mal hyperalgesia induced by nerve injury ( P <0. 05 , P<0. 01). Meanwhile, high dose of tetrahydropalma-tine significantly relieved the mechanical allodynia in induction period group, induction with maintenance period group and thermal hyperalgesia in maintenance period group (P<0. 05). However, there was no ob-vious effect on mechanical allodynia and thermal hyper-algesia induced by nerve injury ( P >0. 05 ) in long-term low-dose group. Conclusions High dose of tet-rahydropalmatine in induction period group, induction with maintenance period group and low-dose among the whole experiment process obviously relieves the neuro-pathic pain induced by nerve injury. The analgesic effect of tetrahydropalmatine on neuropathic pain may be due to the increased expression of Cav1 . 2 protein in DRG neurons.

The industrialization chains and their products, which were formed from the process of the production of medicinal materials-prepared drug in pieces and deep processed product of Chinese material medica (CMM) resources, have generated large benefits of social and economic. However, The large of herb-medicine castoff of "non-medicinal parts" and "rejected materials" produced inevitably during the process of Chinese medicinal resources produce and process, and the residues, waste water and waste gas were produced during the manufactured and deep processed product of CMM. These lead to the waste of resources and environmental pollution. Our previous researches had proposed the "three utilization strategies" and "three types of resources models" of herb-medicine castoff according to the different physicochemical property of resources constitutes, resources potential and utility value of herb-medicine castoff. This article focus on the conversion efficiency of resources model and analysis the ways, technologies, practices, and application in herb-medicine cast off of the conversion efficiency of resources model based on the recycling economy theory of resources and thoughts of resources chemistry of CMM. These data may be promote and resolve the key problems limited the industrialization of Chinese material medica for long time and promote the realization of herb-medicine castoff resources utilization.

8.Effects of beta glucan in highland barley on blood glucose and serum lipid in high fat-induced C57 mouse.

OBJECTIVEStudy the effects of β-glucan in highland barley on blood glucose and serum lipid in high fat diet induced C57 mouse.
METHODSUsing table of random number, 40 male C57BL/6 mice were randomly divided into 4 groups (10 mice in each group) by weight: high dosage group (4% β-glucan and high fat diet), low dosage group (2% β-glucan and high fat diet), high fat diet group and normal control group. Food-intake and body weight of C57 mouse were observed. Glucose tolerance tests and examinations of fasting blood glucose were performed at the end of 11 weeks of intervention. Mice were sacrificed after 12 wk of treatment, and serum specimens were obtained to test relevant biochemical indicators.
RESULTSAfter 12 weeks raise, among high dosage group, low dosage group, high fat diet group and normal control group, the weight was (32.8 ± 1.5), (40.4 ± 1.9), (40.7 ± 2.1) and (33.5 ± 1.3) g, respectively (F = 55.26, P < 0.05); average food intake was (3.48 ± 0.56), (3.69 ± 0.76), (3.66 ± 0.81) and (3.54 ± 0.61) g/d respectively (F = 0.26, P > 0.05); fasting blood-glucose was (5.29 ± 1.59), (6.13 ± 1.75), (7.63 ± 1.09) and (4.24 ± 0.98) mmol/L respectively (F = 9.54, P < 0.01); serum insulin level was (1.97 ± 0.10), (2.44 ± 0.24), (3.02 ± 0.36) and (1.48 ± 0.28) ng/ml respectively (F = 47.58, P < 0.01); the area under blood glucose concentration curve was (25.81 ± 1.44), (30.42 ± 2.01), (35.17 ± 1.20) and (21.03 ± 1.24) mmol×L(-1)×h(-1), respectively (F = 64.98, P < 0.05); insulin resistance index was (9.84 ± 3.78), (13.69 ± 4.48), (21.54 ± 3.27) and (5.81 ± 1.59) respectively (F = 30.18, P < 0.01); serum total cholesterol (TC) level was (4.05 ± 0.88), (4.30 ± 0.48), (4.73 ± 0.66) and (3.37 ± 0.40) mmol/L respectively (F = 6.70, P < 0.01); serum triglyceride (TG) level was (0.90 ± 0.09), (0.98 ± 0.09), (1.05 ± 0.06) and (0.76 ± 0.26) mmol/L respectively (F = 6.75, P < 0.01); serum high-density lipoprotein cholesterol (HDL-C) level was (2.91 ± 0.59), (3.34 ± 0.46), (4.89 ± 0.42) and (3.24 ± 0.37) mmol/L respectively (F = 31.73, P < 0.01); serum low-density lipoprotein cholesterol (LDL-C) level was (0.25 ± 0.15), (0.42 ± 0.19), (0.72 ± 0.12) and (0.32 ± 0.11) mmol/L, respectively (F = 17.27, P < 0.01); free fatty acids (FFA) level was (1.06 ± 0.03), (1.05 ± 0.05), (1.18 ± 0.32) and (1.04 ± 0.02) mmol/L, respectively (F = 1.36, P > 0.05); HDL-C/LDL-C was (13.77 ± 5.51), (9.11 ± 3.53), (7.04 ± 1.65) and (11.21 ± 3.31), respectively (F = 5.24, P < 0.01).
CONCLUSIONThe β-glucan in highland barley reduced the serum glucose and serum lipid, as well as insulin resistance and the risk of arterial sclerosis in high-fat induced C57 mouse.

The objects of research on the resources chemistry of Chinese medicinal materials (RCCMM) are promotion of efficient production, rational utilization and improving quality of CMM and natural products. The development of TCM cause depends on the efficient utilization and sustainable development of CMM, hinges on the technologies and methods for using and discovering medicinal biological resources, stand or fall on the extension of industy chains, detailed utilizaion of resource chemical components by multi-way, multi-level. All of these may help to the recycling utilization and sound development of RCMM. In this article, five respects were discussed to the RCCMM researches and resources recycling utilization ways and goals and tasks. First, based on the principle of resource scarcity, discovering or replacing CMM resources, protecting the rare or endangered species or resources. Second, based on the multifunctionality of CMM, realizing the value-added and value compensation, and promoting the utilization efficiency through systermatic and detailed exploitation and utilization. Third, based on the resource conservation and environment-friendly, reducing raw material consumption, lowering cost, promoting recycling utilization and elevating utilization efficiency. Fourth, based on the stratege of turning harm into good, using the invasive alien biological resources by multi-ways and enriching the medicial resources. Fifth, based on the method of structure modification of chemical components, exploring and enhancing the utility value of resouces chemical substances. These data should provide references and attention for improving the utilization efficiency, promoting the development of recycling economy, and changing the mode of economic growth of agriculture and industry of CMM fundamentally.

10.Prognostic value of preoperative CA199 in advanced colorectal cancer patients with normal carcinoembryonic antigen level

Objective:To investigate whether increased levels of preoperative carcinoembryonic antigen (CEA) and CA199 were associated with the mortality of patients with advanced colorectal cancer and to determine whether CA199 can be used to discriminate patients with normal preoperative CEA level from good to poor prognosis. Methods:A total of 314 patients with advanced colorectal cancer cases who underwent primary tumor resection were collected from 2014 to 2016 at Peking University Cancer Hospital and Institute. Preoperative CEA and CA199 serum levels were examined using electrochemiluminescence immunoassay. The association of CEA and CA199 with clinicopathologic features and their possible prognostic values were analyzed. Results:In the tested patients, 5.4%of whom had increased CA199 level but not up-regulated CEA level, whereas 28.3%only had increased CEA level, and 21.3%had both CEA and CA199 levels increased. Kaplan-Meier survival curves indicated that patients with elevated preoperative CEA level had worse disease-free survival (DFS) and overall survival (OS) than those with normal CEA (P<0.001 and P<0.001, respectively). Meanwhile, patients with elevated CA199 level had worse DFS and OS than those with normal CA199 (P<0.001 and P<0.001, respectively). Preoperative CA199 level could be used in discriminating patients with normal CEA from good to poor prognosis (P=0.012). Multivariate analysis revealed that elevated CA199 level was an independent prognostic factor for OS in patients with advanced colorectal cancer (HR=2.025, 95%CI=1.331-3.082, P=0.001). Conclusion:Combined detection of preoperative CEA and CA199 can be used in evaluating the prognosis of patients with advanced colorectal cancer. Preoperative serum CA199 level can be used in evaluating the prognosis of patients with colorectal cancer without an increase in CEA level. Patients with increased CA199 level had worse 5-year survival than those with increased CEA level.
