Objective To identify the different causes of atrial fibrillation (AF) in Chinese by coronary artery angiography,and to compare the episode of stroke in different patients.Method A total of 782 adult patients diagnosed with AF were identified from Anzhen Hospital over a 8-year period.There were 273 patients with rheumatic valvular AF and 509 patients with nonvalvular AF.The results of electrocardiogram,echocardiography and coronary artery angiography were retrospectiwely analyzed to find out the proportion of causes and to compare the episode of stroke in patients with rheumatic valvular AF with those having nonvalvular AF.Results The patients with rheumatic valvular atrial were significantly younger than of coronary heart disease (52.40?5.03 years old vs 64.30?3.25 years old,P
2.Effect of family care on nursing intervention of patients with myocardial infarction in recovery period

Objective To investigate the effect of family care on nursing intervention of patients with myocardial infarction in recovery period.Methods Totals of 66 patients who had their first acute myocardial infarction were chosen for this study.APGAR,SAQ and SDS were used to investigate before their discharge.According to their APGAR score,patients were divided into the better family care group (Group A) with 35 cases whose score was no less than 7 and the worse family care group (Group B) with 31 cases whose score was less than 7.Both groups received nursing intervention for early recovery period after discharge for 10 to 12weeks,and then SAQ and SDS were used again.Two groups' scores of SAQ and SDS before and after nursing intervention were compared.Results Compared with discharge,SAQ score of both groups 12 weeks later improved.The score of body activity limitation,satisfaction toward treatment and knowledge of disease was respectively (66.00 ± 3.65),(78.78 ± 4.60),(65.25 ± 5.55) in Group A and (63.10 ± 3.69),(67.75 ±6.08),(59.94 ± 5.88) in Group B,and the differences were statistically significant (t =4.244,9.166,5.838 ;respectively ; P < 0.05).There was no statistically significant difference of SDS score between two groups on discharge (P > 0.05).SDS score 12 weeks after discharge was (0.39 ± 0.04) in Group A and (0.42 ± 0.07)in Group B,and the difference was statistically significant (t =-3.277,P < 0.01).Conclusions Nursing intervention for patients with myocardial infarction in recovery period can help improve their quality of life.Good family care can further enhance the effect the nursing intervention,and improve patients' adverse metal status combined with nursing intervention.
3.Clinical characterization of hospitalized children infected with human metapneumovirus and the virus genetic characters

Objective To investigate the clinical manifestation of hMPV in infants and young chil-dren presented with acute respiratory tract infection and to identify the molecular character. Methods Na-sopharyngeal aspirates were taken from 310 hospitalized pediatric patients from February to May in 2006, March to April in 2008, and September 2008 to February 2009, and the N gene fragments of hMPV were de-tected by nested PCR amplification. Phylogenetic analysis of 17 strains hMPV N genes was performed. The clinical materials of patients were collected and analyzed. All hMPV-positive samples were examined by multi-PCR for other respiratory viruses. Results Of 310 pediatric patients, 20 (6.5%) were positive for hMPV. The median age of hMPV infected children was 15.0 months(from 16 days to 9 years old), 90% (18/20)of the cases were under 2 years, and 60% were male. Phylogenetic analysis of 17 N gene fragments showed that 11 hMPV strains were A2b subtype. 20 hMPV-positive children were subjected to pneumonia, accounting for 7.1% (20/282) among all pneumonia subjects in this study. The common clinical manifesta-tions of hMPV infected patients were cough, wheezing, shortness of breath and fever. 35% (7/20) needed intensive care, 15% (3/20) were given oxygen therapy. The median length of hospital stay was (11.9 ±4.8) d. No significant seasonal distribution of hMPV was displayed. Two patients were coinfected with ade-novirus and rhinovirus respectively. Conclusion hMPV was an important respiratory pathogen in young children subjected to pneumonia in Tianjin. Three subtypes(A2a/A2b, B1, B2) were prevalent in Tianjin, and A2b was the predominant subtype. No significant difference of clinical characters was observed between A and B type hMPV infected patients.

Safflower seeds (Carthamus tinctorius L. seeds, CTS) and dandelions (Taraxacum coreanum, TC) are widely used to treat bone- or inflammation-related diseases in Oriental countries. This study investigated the protective effect of the CTS–TC combination on scopolamine (Sco)-induced memory deficits through inflammatory response and cholinergic function. Moreover, marker components such as serotonin, N-(p-coumaroyl) serotonin, N-feruloylserotonin, chlorogenic acid, and chicoric acid in the CTS–TC combination were analyzed for their potential benefits on memory function.MATERIALS/METHODS: Water extracts of CTS, TC, and the CTS–TC combination at various ratios (4:1, 1:1, and 1:4) (100 mg/kg) were orally administered to mice for 14 days. Sco (1 mg/kg) was intraperitoneally injected into the mice before each behavioral test. T-maze and novel object recognition tests were conducted to monitor behavioral changes after the treatment.Western blotting was performed to detect protein expression. In addition, the presence of 5 biomarkers, serotonin, N-(p-coumaroyl) serotonin, N-feruloylserotonin, chlorogenic acid, and chicoric acid, was analyzed using high-performance liquid chromatography (HPLC).
RESULTS:
Behavioral tests showed that the CTS–TC combination enhanced memory function in Sco-injected mice. Inflammation-related proteins (inducible nitric oxide synthase, cyclooxygenase-2, and glial fibrillary acidic protein) were downregulated after treatment with the CTS–TC combination. The acetylcholinesterase protein expression was also downregulated.HPLC analysis revealed that N-feruloylserotonin and chicoric acid were the predominant components, followed by N-(p-coumaroyl) serotonin, chlorogenic acid, and serotonin.
CONCLUSION
These findings suggest that the CTS–TC combination protects against Sco-induced memory deficits by inhibiting inflammatory responses and cholinergic dysfunction. 8.A preliminary study on the role of anti-atrial fibrillation pacemaker in the prevention of atrial tachyarrhythmia.

OBJECTIVETo investigate the effect of anti-atrial fibrillation of Philos DDDR pacemaker on atrial tachyarrhythmia.
METHODSThirty-eight patients with sick sinus syndrome and paroxysmal atrial fibrillation (AF) were implanted with Philos DDDR pacemaker. After implantation, auto-Mode-Switch (AMS) function was switched "on" and AF preventive algorithms were "off" in all cases. The number of AMS, atrial premature beats, heart rate and the percentage of atrial and ventricular pacing were recorded by pacemaker diagnostic function for one-month after procedure. AF preventive algorithms function with "middle" (approx 8 bpm) was then switched on and the same parameters as above from the database of pacemaker diagnostic function were collected for additional one month.
RESULTSThe symptoms of dizziness, dyspnoea, and palpitation in the majority of patients were dramatically improved regardless of whether the AF preventive algorithms function was switched "on" or "off" after pacemaker implantation. There were no significant clinical changes in most patients when AF preventive algorithms were "on". However, 5 cases (13.2%) had palpitations and short of breath. These symptoms were relieved by changing the algorithms from "middle to slight (approx 4 bpm)". When AF preventive algorithms were switched on, atrial premature beats were reduced significantly (P < 0.05) with a dramatic increase in atrial pacing percentage and heart rate (P < 0.05). However, there was no significant difference in AMS (P > 0.05) between the two groups of AF preventive algorithms function switching "on" and "of", indicating that atrial tachyarrhythmias were not inhibited by anti-atrial fibrillation pacemaker.
CONCLUSIONThis study suggested that atrial fibrillation and atrial tachycardia were not reduced by implantation of an anti-atrial fibrillation Philos DDDR pacemaker, although atrial premature beats decreased significantly with increasing atrial pacing percentage when AF preventive algorithms were in "middle" and "slight".

9.Successful pregnancy and birth after intrauterine insemination using caput epididymal sperm by percutaneous aspiration.

AIMTo manage male infertility with obstructive azoospermia by means of percutaneous epididymal sperm aspiration (PESA) and intrauterine insemination (IUI).
METHODSNinety azoospermic patients with congenital bilateral absence of the vas deferens (BAVD, n=58) or bilateral caudal epididymal obstruction (BCEO, n=32) requesting for fine needle aspiration (FNA), PESA and IUI were recruited. The obstruction was diagnosed by vasography and determination of the fructose, carnitine and alpha-glucosidase levels in the seminal fluid.
RESULTSThe mean sperm motility, density, abnormal sperm and total sperm count of the caput epdidymis were 16 %+/-22 %, (12+/-31) x 10(6)/mL, 55 %+/-36 % and (16+/-14) x 10(6), respectively. In the 90 couples, a total of 74 PESA procedures and 66 cycles of IUI were performed. Three pregnancies resulted, including one twin pregnancy giving birth to two healthy boys, one single pregnancy with a healthy girl and another single pregnancy aborted at week 6 of conception. The pregnancy rate per IUI cycle was 4.5 %.
CONCLUSIONThe birth of normal, healthy infants by IUI using PESA indicates that the caput epididymal sperm possess fertilization capacity. The PESA-IUI programme is a practical and economical procedure for the management of patients with obstructive azoospermia.

Objective To study the derect action and part possible mechanism of sitagliptin ( SIT ) on the delay or stop of the progress of type 2 diabetic nephropathy (DN).Methods The rats were randomly divided into normal group ( NC group ) , diabetic nephropathy group ( DN group) and sitagliptin treatment group (SIT group).The type 2 diabetes mellitus rats were induced by a high fat diet and repeated low dose strep-tozocin injections.At the end of the 12th week in treatment, Masson stai-ning was used to observe the renal fibrosis .RT-PCR was used to detect the mRNA levels of Toll-like-receptor 2 (TLR2), TLR4, nuclear fac-tor-κB ( NF-κB ) .Immunohistochemical method was used to detect the content of TLR2 and TLR4.Western blotting was used to detect the content of NF-κB.Results Compared with NC group , the degree of renal fibrosis, and the expressions of TLR2, TLR4, NF -κB of DN group significantly increased ( P<0.01 ) .However , compared with DN group, the degree of renal fibrosis , and the expressions of TLR2, TLR4, NF-κB of SIT -treated rats significantly decreased ( P <0.05 ) . Conclusion SIT can prevent the renal fibrosis by down -regulating the expression of TLR2, TLR4/NF-κB pathway.
