2.Effect of different fluid resuscitation on kidney function in pediatric patients with sepsis

Objective To prospectively study the effects of different fluid resuscitation on kidney function in pediatric patients with sepsis.Methods One hundred and eighteen pediatric patients with sepsis shock who needed fluid resuscitation in Intensive Care Unit,Zhengzhou Children′s Hospital from June 2012 to June 2015 were selected, and they were randomly divided into group A,B and C by random number in the table.The patients received fluid resus-citation of hydroxyethyl starch,50 g/L albumin and 9 g/L saline.After that,neutrophil gelatinase associated lipocalin (NGAL),α-1 microglobulin,urea nitrogen,creatinine,urine volume,the incidence of acute kidney injury (AKI),and the renal replacement therapy rate accepted by the patients were recorded.Then all the data were analyzed with SPSS 19.0 software.Results Serum levels of NGAL and α-1 microglobulin in group A [(103.50 ±1.23)μg/L,(3.32 ± 0.20)mg/L]were higher than those in group B [(93.54 ±1.42 )μg/L,(2.71 ±0.18 )mg/L]and group C [(79.57 ±1.54)μg/L,(2.08 ±0.14)mg/L],which were statistically different(all P <0.05 );serum levels of NGAL and α-1 microglobulin in group B were higher than those in group C,which were statistically different(all P <0.05).Serum levels of urea nitrogen and creatinine in group A[(5.23 ±0.34)mmol/L,(112.00 ±7.12)μmol/L] were higher than those in group B [(3.56 ±0.31)mmol/L,(77.25 ±5.34)μmol/L]and group C[(2.95 ±0.42) mmol /L,(72.12 ±6.34)μmol/L],which were statistically different (all P <0.05);serum levels of urea nitrogen and creatinine in group B were higher than those in group C,which were not statistically different(P >0.05).The urine volume in group A [(0.50 ±0.12)mL/(kg·h)]was less than that in group B[(0.90 ±0.23)mL/(kg·h)]and group C[(1.30 ±0.14)mL/(kg·h)],which was statistically different(all P <0.05);the urine volume in group B was less than that in group C,which was not statistically different(P >0.05).In three groups,there were 13 cases (34.2%)with AKI in group A,9 cases(23.1%)with AKI in group B,and 8 cases(19.5%)with AKI in group C, the incidence of AKI in group C was less than that in group B and group C,which was statistically different(χ2 =12.74,10.43;all P <0.05).All the groups all had children accepted renal replacement therapy,there were 8 cases (21.1%)in group A,7 cases(17.9%)in group B,and 6 cases (14.6%)in group C,and the cases who accepted renal replacement therapy rate in group C were less than those in group A and group B,which was statistically different(χ2 =11.36,8.73;all P <0.05).Conclusions The adverse effect of 9 g/L saline on kidney function is less than hydrox-yethyl starch and 50 g/L albumin,and hydroxyethyl starch has more adverse effects on kidney.Thus,9 g/L saline is a better option for fluid resuscitation in pediatric sepsis patients.

4.Clinical Observation of Milrinone Injection in the Treatment of Hypertensive Heart Disease Complicated with Heart Failure

OBJECTIVE:To observe therapeutic efficacy and safety of Milrinone injection in the treatment of hypertensive heart disease complicated with heart failure. METHODS:A total of 120 patients with hypertensive heart disease complicated with chronic heart failure were randomized into control group and observation group according to random number table,with 60 cases in each group. Control group received routine therapy,including blood pressure controlling,anti-heart failure and other symptomatic treatment;observation group was additionally given Milrinone injection 0.3 mg/kg,continuous pump,qd. A treatment course lasted for 5 d. Both groups received one course of treatment each month for consecutive 6 months. Both groups were followed up for one year. 6 min walking distance(6MWD),BNP,LVEF,CI,LVEDD,heart rate,sitting systolic blood pressure(SiSBP)and sitting diastolic blood pressure(SiDBP),re-hospitalization rate and cardiovascular mortality were observed in 2 groups before treatment, after 3,6 months and 1 year of treatment;therapeutic efficacy of heart function and the incidence of ADR were compared between 2 groups. RESULTS:Before treatment,there was no statistical significance in 6MWD,BNP,LVEF,CI,heart rate,SiSBP and SiDBP between 2 groups;after 3 months,6 months and 1 year of treatment,6MWD,LVEF and CI of 2 groups were increased significantly,while BNP,LVEDD,resting heart rate,SiSBP and SiDBP were decreased significantly;the observation group was significantly better than the control group,with statistical significance(P<0.05). Total response rate of heart function was 90.00%in observation group,which was significantly higher than 63.33% in control group,with statistical significance(P<0.05). After 1 year of follow-up,re-hospitalization rate of observation group was significantly lower than that of control group,with statistical sig-nificance (P<0.05);there was no statistical significance in cardiovascular mortality (P>0.05). There were no statistical signifi-cance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Milrinone injection shows good efficacy in the treatment of hypertensive heart disease complicated with heart failure,can significantly reduce BNP and re-hospitalization rate as well as improve prognosis with good safety.

People departure from the treatment, prevention and health care purposes, intermittently or continuously excessive use of drugs with dependence, will cause serious mental and physical damage, while serious social harm.Opioids are the most commonly abused drug, abusers by taking an excess number of pills orally or by crushing the pills, followed by smoking, snorting, or injecting the new altered formulation to get euphoric.The escalating abuse of opioids has recently spawned the development of novel drug formulations resistant to various methods of tampering and misuse.This article discusses available opioids that include abuse-deterrent mechanisms as well as such agents currently in development.

Recent studies have found that peptide therapies tar-geting specific epitopes can avoid nonspecific immune suppres-sion induced by traditional medicines for the treatment of autoim-mune diseases, and have shown great therapeutic effect in ani-mal models of autoimmune diseases and clinical trials. The pa-per summaries the research progress and trends of peptide drugs for the treatment of autoimmune diseases from candidate peptide sources and their suppression mechanisms, which can provide a theoretical basis for the in-depth understanding of immune toler-ance and allow for discovery of new treatment for autoimmune diseases.
7.Comparison of the properties of three denture base materials treated and untreated by hydraulic pressure cooker

Objective:To compare the properties of three kinds of denture base materials treated and untreated by hydraulic pressure cooker. Methods: The impact strength, flexural strength, flexural elastic modulus and color difference roughness of three denture base materials, Kunshan Rijin(KR),GC UNIFAST Trad(GC), Shanghai Zhangjiang(SZ), were measured in vitro. All results were statistically analyzed with SPSS software to have ANOVA and Tamhane's T2 tests, α=0.05. Results: Comparison of the same materials between the treated materials and the untreated materials by hydraulic pressure cooker: there were significant statistical differences of impact strength for all the materials. There were significant statistical differences of color difference for KR and SZ. Comparison of the three materials: impact strength, GC>SZ>KR;flexural strength, KR>SZ>GC;flexural elastic modulus, KR>SZ>GC;roughness, GC>KR>SZ. Conclusion: The properties of denture base materials treated by hydraulic pressure cooker are good.
8.Cultivation and Identification of Dendritic Cells from Mouse Bone Marrow in Vitro

Objective To establish a method of cultivation of dendritic cells (DC) from mouse bone marrow in vitro and identify their phenotype and function. Methods Under aseptic condition, bone marrow cells were extracted from the tibia and femur bones of BALB/c mice. Bone marrow cells were cultured with recombinant mouse granulocyte-macrophage colony-stimulating factor ( rmGM-CSF) in vitro. The expansion and morphological changes of DC were observed with light inverted microscope. Phenotype was identified with flow cytometry and biological function was studied with antigen phagocytosis test. Results A large number of immature and mature DC with typical dendritic morphological characteristics could be generated from murine bone marrow. Immature DC, which had high expression in CD11c and low expression in CD40, MHC-II and CD86, could phagocytize antigen. Mature DC, which could be induced from immature DC by lipopolysaccharides, had high expression in CD11c, CD40, CD86 and MHC-II molecules. Conclusion Immature and mature DC can be generated from mouse bone marrow cells through cytokine induction in vitro and be used for further study associated with DC.

This paper reviews the recent advances in recombinant expression and purification of defensins, including the choice of host cells, vectors and expression strategies in prokaryotic and eukaryotic cell ex- pression systems, as well as the status of purification processes. By summarizing the problems existed in the production and clinical applications of defensins, the authors here also pointed out the research directions for defensins, and conceived the prospects for its exploitation in the future.
10.Clinical research of IA regimen as induction chemotherapy for acute myeloid leukemia

Objective To explore the clinical efficacy and toxicity of idarubicin (IDA)-based IA regimen as induction chemotherapy in treating acute myeloid leukemia (AML) within different risk groups.Methods Data collected in 60 patients initially diagnosed and treated with one course of IA regimen were studied retrospectively.Results Efficacy were different for patients treatment with different prognosis after one course of IA chemotherapy,CR rates achieved in patients with favorable,intermediate and unfavorable prognosis were 83.3 % (10/12),73.1% (19/26),59.1% (13/22) respectively,PR rates were 16.7 % (2/12),19.2% (5/26),13.6% (3/22) while ORrateswere 100% (12/12),92.3 % (24/26),72.7 % (15/22)respectively.After 2 years of follow-up,relapse free survival (RFS) rates were 100 % for patients with good prognosis,53.8 % for those with moderate prognosis,and 45.5 % for people with poor prognosis.The outcome showed that there were significant differences among these three group (P =0.042).Myelosuppression and infections due to neutropenia were the main adverse effects,and severe non-hematologic toxicities were not observed.Conclusion IA regimen as induction chemotherapy may be an efficacious and safe solution and achieve a higher CR rate and 2 years RFS rate for patients diagnosed as AML with good prognosis.Hematopoietic stem cell transplantation should be considered as soon as possible for patients with moderate or poor prognosis.
