Purpura fulminans is a potentially disabling and life-threatening disorder characterized by acute onset of progressive cutaneous hemorrhage and necrosis on distal extremities, and disseminated intravascular coagulopathy. We experienced a case of purpura fulminans due to Streptococcus pneumoniae. A 42-year-old women presented with skin petechiae, ecchymosis and gangrene on distal extremities with laboratory evidence of DIC. The latex agglutination test with CSF was positive at Streptococcus pneumoniae. To our knowledge, this is the first report of purpura fulminans caused by Streptococcus pneumoniae in Korea.

5.A Study on Anxiety-Depression in College Freshmen.

The authors studied anxiety-depression using New Anxiety-Depression Scale (NADS) in the subjects of 2,564 male and 1,252 female college freshmen of Yeungnam University. The authors collected the reports of NADS during the periods from January to February 1987, and applied ANOVA and t-test on anxiety depression scores in order to compare them between various psychosocial factors and sexes. The results are as follows: There was significant difference in the mean average of total scores between male and female students: male students scored 33.41±8.76 and female students scored 35.34±9.02 (P<0.001). The anxiety-depression scores relating to the items of sleep disturbance, apprehension, facial flushing and fatigue were relatively scored higher in both groups. One hundred and thirty nine male students (5.4%) showed seriously higher degree of anxiety-depression scores of 50 or more, while eighty eight female students (7.0%) showed the same scores. There was a strong tendency toward higher anxiety-depression scores in the students who were dissatisfied with their home atmosphere, colleges, departments, familiarity of parents and those who had pessimistic views of self image in the past, presents and future in both groups (P<0.001).

6.MRI Measurement of the Intercondylar Notch and Correlation to Anterior Cruciate Ligament Injuries.

Notch stenosis had been thought to be related with anterior cruciate ligament (ACL) injury. The purpose of this study is to evaluate the possible relationship between notch stenosis and ACL injury. We measured the notch seen on the axial section in MRI at popliteal groove. We have retrospectively analyzed 116 cases of knee MRI. All cases were divided into three groups ; Group I were fifty six normal knee. Group II were thirty knee with contact ACL injuries. Group III were thirty knee with non contact ACL injuries. The result were as follows; 1. Statistically significant difference was found in the notch width index (NWI) between group I and group III but no significant differences was found in the NWI between group I and group II. 2. Statistically significant correlation to non-contact ACL injuries was found in the NWI at both anterior and posterior outlet of the notch. 3. An unique shape of the notch was found in the majority of group III.There seemed to be an obvious relationship between notch stenosis and non-contact ACL injuries.

7.A Case of Bullous Pemphigoid after Administration of Erdafitinib

Bullous pemphigoid (BP) is an acquired autoimmune disease characterized by subepidermal vesicles and bullae. It is now well-accepted that BP could be associated with therapeutic drugs. Immunotherapy can induce BP; however, few reports of tyrosine kinase inhibitor of fibroblast growth factor receptor-induced BP exist in the literature. A 67-year-old male presented with pruritic erythematous variable-sized patches with bullae on entire body. The patient was diagnosed with left renal cancer and treated by erdafitinib for clinical trial purposes. Bullae formation began at the start of the 11th month of erdafitinib administration. Histopathologic examination of the skin lesion showed subepidermal blister with eosinophils and other inflammatory cells in epidermis and dermis. Direct immunofluorescence showed a linear pattern of immunoglobulin G and C3 deposition along the basement membrane zone. The results of ELISA with recombinant purified BP180 and BP230 were positive. Consequently, the patient was diagnosed with erdafitinib-related BP.
8.The Prevalence of Glanucoma in Korean Adults.

Early detection of glaucoma by mass screening is the most practical approach for prevention of blindness. An epidemIOlogical study gives the most essential information on prevalence an distribution of the disease in relation to diverse background factors. But, before now, an epidemiological study for glaucoma has not been carried out in Korea. Our glaucoma survey was conducted in the health examination center of Yongdong Severance hospital during 1990 and 1991. The total numbers of subjects examined was 4424 persons aged 30 years or older. A mainstay of the screening consisted of applanation tonometry and fundus photography with a non-mydriatic camera, followed by automated perimetry as a recall examination. Overall prevalences of glaucoma obtained were primary open angle glaucoma 0.23% (10 patients), low tension glaucoma 2.04% (108 patients), and ocular hypertension 0.23% (10 patients). The mean lOP obtained with applanation tonometry was 14.04 +/- 2.99 mmHg for men and 14,07 +/- 2.67 mmHg for women. In additon, our data have pointed out that the lOP in different ages tends to be constant, whereas prevalences of glaucoma and low tension glaucoma increase with aging.

9.Evaluating the Degree of Conformity of Papillary Carcinoma and Follicular Carcinoma to the Reported Ultrasonographic Findings of Malignant Thyroid Tumor.

OBJECTIVE: We wanted to evaluate the degree of conformity of papillary carcinoma and follicular carcinoma to the reported ultrasonographic findings of malignant thyroid tumor. MATERIALS AND METHODS: Between January 2003 and December 2004, fine needle aspiration biopsy was performed in 1,036 patients with palpable and non-palpable thyroid lesions. We retrospectively reviewed the ultrasonographic findings of patients with papillary carcinomas (n = 127) and follicular carcinomas (n = 23) that were proven by operation or fine needle aspiration biopsy. We analyzed the ultrasonographic findings of these nodules based on the reported ultrasonographic findings of malignant thyroid tumor: hypoechogenicity, a taller than wide orientation, a microlobulated or irregular margin, a thick hypoechoic rim (halo sign), microcalcification and cystic change. RESULTS: The echogenicity was hypoechoic in 72.4% (92/127) of the papillary carcinomas, but it was isoechoic in 65.2% (15/23) of the follicular carcinomas (p < 0.001). The nodule shape was tall or round in 74.1% of the papillary carcinomas, but it was flat in 72.7% of the follicular carcinomas (p < 0.001). The tumor margin was microlobulated or irregular in 92.9% of the papillary carcinomas and in 60.9% of the follicular carcinomas (p < 0.001). A hypoechoic rim was seen in 26% of the papillary carcinomas (thin rim: 13.4%, thick rim: 12.6%) and in 86.6% of the follicular carcinomas (thin rim: 39.1%, thick rim: 47.8%, p < 0.001). Microcalcifications were demonstrated in 33.9% of the papillary carcinomas and in none of the cases of follicular carcinoma (p < 0.001). A solid mass without cystic change were seen in 98.4% of the papillary carcinomas and in 82.6% of the follicular carcinomas (p < 0.001). CONCLUSION: The previously reported ultrasonography findings of malignant thyroid tumor are in conformity with most of the papillary carcinomas, but not with follicular carcinomas. The current ultrasonographic features for thyroid malignancy should be cautiously applied as the indication for needle aspiration biopsy so that follicular carcinomas are not missed by too narrow and strict biopsy criteria.

Congenital M llerian anomalies of uterus are so infrequently encountered and endometriosis, primary infertility, hematometra, and urinary tract anomalies are claimed to be common complaints in women with unicornuate uterus. We report one case of rudimentary horn associated with agenesis of right kidney and pelvic endometriosis which is presented with brief review of literature about anomaly of the female genital tract.
