2.A Study of Genetic Inheritance of Bromidrosis.

Bromidrosis is a disorder characterized by rancid body odor which influences a patient's social life and mental health. The therapeutic modalities and the mechanism of bromidrosis have been carefully studied, however, there have been few reports about the genetic inheritance of bromidrosis. We investigated the family history of 42 patients who were operated on for bromidrosis and followed up to the third generation in 10 cases. The results were as follows: Results of investigation which were followed up the second generation. The fathers of five patients and the mothers of 11 patients had bromidrosis in 18 male patients. The fathers of six patients and the mothers of 12 patients had bromidrosis in 24 female patients. Thirty-four patient (81.0%) among a total of 42 have a single parent with bromidrosis. Result of investigation which were followed up to the third generation Bromidrosis was occurred in 17 of 42 patients (40.5%) in the second generation, and 18 of 27 patients (66.7%) in the third generation. In one case, a father transmitted bromidrosis to his three sons, and as a result, X-linked inheritance could be ruled out Bromidrosis was not skipped in every generation of all families. We on conclude that bromidrosis is an autosomal dominant inherited disorder.

4.Progressive Dynamic Equinovarus Deformity in Hereditary Spastic Paraplegia: A Case Report.

In neurogenic equinovarus deformity, surgical intervention such as tendon transfer or osteotomy can be expected to improve symptoms. However, in rare cases of hereditary spastic paraplegia, the deformity and paralysis gradually progress. So limited operation and early post-operative rehabilitation are preferred to aggressive operation. We would like to report our clinical experience with one case of hereditary spastic paraplegia patient with reference review. A 40 year-old male, given tendon transfer of ankle and foot and tendo achilles lengthening 10 years ago, complained about aggravated spastic paraplegia which resulted in dynamic equinovarus and limited walking ability since his operation. Family history showed limited walking ability of his father with gradually progressing spastic paralysis and he was diagnosed as hereditary spastic paraplegia type I. We had performed a limited operation such as tendo achilles and tibialis posterior lengthening to induce plantigrade standing and walking with crutch. As a result, the patient was able to maintain a stabilized standing posture and walk after the operation. Hereditary spastic paraplegia presents with a progressive paralysis which limits rehabilitation after tendon transfer, and the symptoms can be aggravated. Therefore, considering potential hereditary neurogenic disorders in paients with equinovarus deformity and performing limited operative procedures seem to be important.

5.In Thyroid Cancer Patients, Is Preoperative FNAB-C Reliable for Prediction of Lateral Cervical LN Metastasis?.

PURPOSE: The goal of this study was to evaluate the diagnostic accuracy of preoperative fine needle aspiration biopsy cytology (FNAB-C) in predicting lateral lymph node metastasis in papillary thyroid cancer patients. METHODS: A total of 592 patients who underwent thyroid cancer surgery and intra-operative lateral cervical LN frozen section or RND, from January 2002 to December 2011, were evaluated retrospectively. Among them, 228 cases had suspicious findings in FNAB-C of lateral nodes. We reviewed their radiological and pathological reports. RESULTS: Intra-operative frozen section examination was performed in 540 cases and RND was performed in 314 cases. This study included 534 women (83.4%) and 106 men (16.6%). Patients' ranged in age from 9 to 83 years (mean, 45.65 years). FNAB-C was performed in 228 cases. The sensitivity and specificity of FNAB-C was 71.5% and 78.6%, respectively; 35.9% of cases had a false negative result. The combination of FNAB-C and intra-operative frozen section test sensitivity and specificity was 87.2% and 93.6%, respectively. CONCLUSION: The results for sensitivity in FNAB-C actually appear low, and false negative results were very high. In papillary thyroid cancer in patients with FNAB-C, even if the result is negative, if lymph node metastasis is suspected based on radiologic evidence, frozen section examination should be performed for determination of metastasis.

6.Factors Influencing Clinical Competence in Nursing Students.

PURPOSE: This study was done to explore clinical competence and factors related to competence in nursing students. METHODS: Data were collected using structured questionnaires from 210 students in three different schools located in D, B and P metropolitan cities, Korea. Data were using PASW (SPSS) 18.0 program, and included t-test, one-way ANOVA, Pearson correlations coefficients, and multiple regression analyses. RESULTS: There were positive correlations between clinical competence and ego-resilience (r=.40. p<.001), critical thinking dispositions (r=.58, p<.001), and self-leadership (r=.14, p=.043). Variables such as critical thinking dispositions, ego-resilience, satisfaction with clinical practice education, health status and academic performance explained 48% of variance in clinical competence. CONCLUSION: According to the above results, a specific education program for clinical competence of student nurses is needed to increase critical thinking dispositions, ego-resilience, satisfaction with clinical practice education, health status and academic performance.

7.Factors Influencing Sexual Autonomy in College Students.

PURPOSE: This study was done to examine sexual autonomy of college students. METHOD: Between October 2012 and November 2012, data related to self-efficacy, health knowledge, attitude to sex, and sexual autonomy orientation, were collected by questionnaire from 232 students. The data analysis was performed using PASW (SPSS) 18.0 program, and included t-test, one-way ANOVA, Pearson correlation coefficients, and multiple stepwise liner regression analysis. RESULTS: The results showed a positive correlation between self-efficacy and sexual autonomy (r=.56, p<.001), sexual health knowledge (r=.50, p<.001), and attitude to sex (r=.29, p=.001). These 4 variables accounted for 48.7% of the variance in sexual autonomy among college students. CONCLUSION: The results of this study indicate that there is a need to develop mental health nursing strategies for college students in order to provide effective counseling on sexuality and sexual health matters.

8.Predictors of Intention to Quit Smoking in Elderly Smokers following a Stroke.

PURPOSE: This study was done to identify factors based on the theory of planned behavior that predict smoking cessation in elderly smokers following a stroke. METHOD: Between March and May 2013, data related to intention, attitude toward the behavior, subjective norm, and perceived behavioral control, were collected through questionnaire from 223 elders who smoked and who had a stroke. Data analysis was performed using PASW (SPSS) 18.0 program, and included t-test, one-way ANOVA, Pearson correlation coefficients, and multiple stepwise liner regression analysis. RESULTS: Results showed a positive correlation between intention and attitude toward the behavior (r=.494, p<.001), subjective norm (r=.390, p<.001), and perceived behavioral control (r=.189, p=.005). These variables such as perceived behavior control, subjective norm, number of cigarettes per day, and usefulness of quitting smoking explained 50% of intention to quit smoking. CONCLUSION: The results of this study indicate that there is a need to develop smoking cessation nursing strategies for elderly smokers following a stroke in order to provide effective counseling on subjective norm and perceived behavioral control.

9.Wrist Arthrodesis using Radial Sliding Graft in the Treatment of Wrist Tuberculosis: Report of 4 Cases

Four patients with advanced tuberculosis of the wrist joint were treated with curettage & radial sliding graft to obtain radiometacarpal fusion. After each follow-up, their results were complete relief of pain with eradication of focus and solid union. This graft affords the stability of cortical bone combined with the osteogenesis of cancellous or trabecular bone. It furthermore affords a shape that conforms to the desirable dorsiflexion of the wrist after fusion.

10.Infection Control Nurses’ Burnout Experiences in Hospitals during the COVID-19 Pandemic

Purpose:
This study was conducted to gain a comprehensive and in-depth understanding of infection control nurses’ burnout experiences in hospitals during the COVID-19 pandemic.
Methods:
We recruited 11 infection control nurses (ICNs) who worked in hospitals in Korea through purposive sampling. Data collected through one-to-one, in-depth interviews were transcribed and analyzed using qualitative content analysis.
Results:
Infection control nurses’ burnout experiences were categorized into five themes and 11 sub-themes. The themes were as follows: “challenges faced while playing a pivotal role in infectious disease management,” “conflict of interest prevalent inside and outside,” “physical and mental collapse,” “a long road to achieving stability in the infection control unit,” and “source of strength to endure.”
Conclusion
In light of the need to better prepare for future outbreaks of emerging infectious diseases, such as COVID-19, the findings of this study highlight the need for strategic approaches, such as developing programs to provide psychological and social support for infection control nurses, as well as establishing a well-designed system of nursing care for infectious diseases to alleviate their burnout.
