Leiomyoma is one of the rarest solid tumor of the ovary. Approximately 50 cases have been published to date. However, most reported leiomyoma of the ovaries were small and rarely induced serious symptoms. We report a case of ovarian leiomyoma in 57-year-old woman which has been experienced in our haspital with brief review of literature.

3.May-Thurner Syndrome in Lung Cancer.

May-Thurner syndrome is deep vein thrombosis (DVT) of the iliofemoral vein due to compression of the left common iliac vein (CIV) by the overlying right common iliac artery (CIA). In contrast to the right CIV, which ascends almost vertically to the inferior vena cava (IVC), the left CIV follows a more transverse course. Along this course, it underlies the right CIA, which may compress it against the lumbar spine. A 69-year-old man with squamous cell lung cancer presented with acute onset painful left leg swelling. He had been undergoing chemotherapy with gemcitabine and cisplatin as a 2nd line treatment after concurrent chemoradiation. Physical examination revealed left leg edema with tenderness and warmth. The D-dimer level was elevated and a lower extremity computed tomographic angiogram (CTA) showed a DVT involving the left infrapopliteal vein to the common iliac vein with collapsed junction between the CIV and IVC. Systemic anticoagulation with low molecular weight heparin (LMWH) and an IVC filter insertion was performed to prevent further thrombosis, such as a PTE. After IVC filter placement, mechanical thrombectomy was performed on the left femoral vein and left CIV. A vascular stent was then deployed in the left CIV. Left leg swelling seemed to be improved after heparinization, but he had a 2nd episode one week later. Therefore, he underwent a 2nd mechanical thrombectomy and stent deployment of the left external iliac vein. His leg swelling was gradually relieved. 5.Retromandibular approach for open reduction of the mandibular condyle fracture.

Most popular incisions for open reduction of mandibular condylar fractures are submandibular approach, preauricular approach, Risdon approach, and retromandibular approach etc. These methods are selected according to the location of condylar fracutre, condylar segments displacement, and preference of the surgeon. We performed the open reduction and rigid fixation by means of retromandibular approach for the treatement of mandibular condyle fracture in the 9 patients (8 cases were performed by direct reduction and rigid fixation, and 1 case was performed by Dr. Nam's method). The retromandibular approach has many advantages. This is more easier approach to the fracture site, better visibility and accessibility, less wound exposure, less muscle retraction, and more comfortable to patient because lesser sensory and motor nerve injury.

6.A Case of Exon 7 and 8 Deletion of Survival Motor Neuron Gene in Spinal Muscular Atrophy.

Spinal muscular atrophy(SMA) is a genetic disorder of the motor neurons that cause muscular weakness and muscular atrophy due to anterior horn cell degeneration. Classic spinal muscular atrophy patient is caused by mutation in the chromosome 5(q11.2-q13.3), and the majority of the patient shows homozygous deletion of the telomeric survival motor neuron(SMN) gene in the chromosome 5. Deletion of exon 7 and 8 of the SMN gene and deletion of exon 4 and 5 of the neuronal apoptosis inhibitory protein(NAIP) are typically observed in SMA patients. The SMN protein plays a role in an essential cell metabolism process, the splicing of pre mRNA in the spliceosomes. We report a 7 month old male with SMA. He showed rapidly aggrdvatial muscular weakness and died at 7 months. His DNA analysis proved deletion of exon 7 and 8 of the telomeric copy of the SMN gene.

7.Urine Methyl Hippuric Acid Levels in Acute Pesticide Poisoning: Estimation of Ingested Xylene Volume and Association with Clinical Outcome Parameters.

To determine the relationship between the oral ingestion volume of xylene and methyl hippuric acid (MHA) in urine, we measured MHA in 11 patients whose ingested xylene volume was identified. The best-fit equation between urine MHA and ingested amount of xylene was as follows: y (ingested amount of xylene, mL/kg) = −0.052x² + 0.756x (x = MHA in urine in g/g creatinine). From this equation, we estimated the ingested xylene volume in 194 patients who had ingested pesticide of which the formulation was not available. Our results demonstrated that oxadiazole, dinitroaniline, chloroacetamide, organophosphate, and pyrethroid were xylene-containing pesticide classes, while the paraquat, glyphosate, glufosinate, synthetic auxin, fungicide, neonicotinoid, and carbamate classes were xylene-free pesticides. Sub-group univariate analysis showed a significant association between MHA levels in urine and ventilator necessity in the pyrethroid group. However, this association was not observed in the organophosphate group. Our results suggest that MHA in urine is a surrogate marker for xylene ingestion, and high urine MHA levels may be a risk factor for poor clinical outcome with some pesticide poisoning.

8.Relationship between Depression and Laryngopharyngeal Reflux.

This study investigated the relationship between depression, somatization, anxiety, personality, and laryngopharyngeal reflux (LPR). We prospectively analyzed 231 patients with symptoms with LPR using the laryngopharyngeal reflux symptom index and the reflux finding score. Seventy nine (34.2%) patients were diagnosed with LPR. A significant correlation was detected between the presence of LPR and total scores on the Patient Health Questionnaire-9 (5.6±5.3 vs. 4.0±4.6, p=0.017) and the 7-item Generalized Anxiety Disorder Scale (4.3±4.9 vs. 3.0±4.5, p=0.041). LPR was significantly more frequent in those with depression than in those without (45.6% vs. 27.0%, p=0.004). A multivariate analysis confirmed a significant association between the presence of LPR and depression (odds ratio, 1.068; 95% confidence interval, 1.011–1.128; p=0.019). Our preliminary results suggest that patients with LPR may need to be carefully evaluated for depression.

9.Prenatal Development of Human Lip with Immunohistochemical Study.

BACKGROUND: This study is aimed to elucidate the developmental pattern of human fetal lip by histological and immunohistochemical examinations. METHODS: Totally 231 normal human lip tissues obtained from autopsied fetuses were fixed with 10% buffered formalin, sectioned in cross and longitudinal directions, routinely stained for H&E and performed for immunohistochemistry with antibodies of S-100 protein, proliferating cell nuclear antigen (PCNA), transglutaminase C (TGase-C), metalloproteinase (MMP)-3, MMP-10, tenascin, KL1, K8.12, E-cadherin, tissue inhibitors of matrix metalloproteinase (TIMP)-1, TIMP-2 and total keratin (TK). RESULTS: The lip structure first appeared as an orifice of stomodeum around the 7-8th week of gestation, and a major structure of the midface was observed by the 11-12th week. As the squamous epithelium of the lip became thick and was keratinized, the vermilion border became distinguished in the 15-16th week, and the lip structure was almost completed with the presence of orbicularis oris muscle in the lingual side of vermilion border by the 17-18th week. Immunohistochemically, the vermilion border showed strong reactions for tenascin, E-cadherin and MMP-3 and increased positivity for PCNA, cytokeratins (TK, KL1, K8.12), and TGase-C. CONCLUSIONS: With the above findings we suppose that the cytodifferentiation of vermilion border epithelium plays an important role for the development of human fetal lip.

10.The Effects of Recombinant Bacillus Calmette-Guérin Resistant to Antimicrobial Peptides on Orthotopic Bladder Cancer Mouse Model

Purpose:
Although Mycobacterium bovis Bacillus Calmette-Guérin (BCG) is the most widely used bladder cancer immunotherapy, innate immune responses involving antimicrobial peptides (AMPs) cause BCG failure. Here, we developed genetically modified recombinant BCG (rBCG) strains which escape AMPs and evaluate the efficacy and effects of rBCG.
Materials and Methods:
We constructed rBCG strains expressing Streptococcal inhibitor of complement (Sic), which confers resistance to human α-defensin-1 and cathelicidin, and d-alanyl carrier protein ligase (dltA), which confers resistance to cationic AMPs. Sic and dltA were separately cloned into the pMV306 plasmid and introduced into BCG via electroporation. The efficacy of the Sic and dltA gene electroporation into BCG was evaluated by quantitative real-time polymerase chain reaction (qRT-PCR). The internalization rates and anticancer effects of the rBCG strains containing Sic (rBCG-Sic) and dltA (rBCG-dltA) was evaluated by the orthotopic bladder cancer mouse model.
Results:
The cycle quantification (Cq) values of rBCG-Sic (y=-4.8823x+13.645, R2=0.9996) and rBCG-dltA (y=-5.438x+11.641, R2=0.9995) were inverse correlations to the amount of Sic and dltA genes dose dependently. The mean introduction proportions of Sic and dltA genes into BCG by electroporation were 22.2%, 27.5% and showed constant efficacy. In the orthotopic bladder cancer mouse model, the relative internalization number of rBCG-Sic, and rBCG-dltA into bladder cell in mouse bladder were higher than that of BCG and the tumor volume at rBCG-Sic were lower than at BCG and rBCG-dltA at 11, 14 and 18 days.
Conclusions
Our results showed that constructed rBCG-Sic and rBCG-dltA by electroporation and the rBCG-Sic and rBCG-dltA can effectively escape BCG-stimulated AMPs, and significantly improved immunotherapeutic tools to treat bladder cancer in orthotopic bladder cancer mouse model.
