2.A Case of Glycogen Storage Disease Type lb.

Glycogen storage disease(GSD) type lis due to defect of glycose-6-phosphatase at the membrane of the endoplasmic reticulum in liver. Clinical presentations of GSD 1 are massive hepatomegaly without splenomegaly, failure to thrive, bleeding tendency, hypoglycemia, fasting ketosis and hyperlipidemia. The appearance of patient is short and fat with particularly fat cheeks. Mental development is usually normal. It was diagnosed by liver biopsy and cofirmend as GSD type lby enzyme analysis. 3.Obstruction of Ventilator Exhalational Valve due to High Moisture Content in compressed Air - A case report.

An air-compressor system takes atmospheric air, and filters, pressurises and dries it, before delivering it to the pipeline as a medical air. Because the air supplied by the air compressor may be used to wentilate patients or to drive surgical tools, its quality and purity must be carefully controlled. Atmospheric air contains a variable amount of water vapour but always more than is specified for medical air. Water vapour must be removed from medical air since it can damage surgical tools and some ventilators and can condense in low-lying parts of the pipeline system. Reeently we have had experience of pneumothorax by pulmonary barotrauma associated with obstruction of the ventilator exhalational valve. As a result of investigating the possible causes, we concluded the cause as high moisture content in the compressed air due to the malfunctioning air drier and filters. Therefore we submit this case report along with discussion and literature reviews.

4.Generation of Mature Dendritic Cells from Peripheral Blood.

PURPOSE: Dendritic cells (DCs) are the most potent antigen presenting cells and should be differentiated to mature form to induce primary T cell response. In this study, we intended to generate mature DCs from peripheral blood mononuclear cells (PBMCs), so that develope the basis for immunotherapy using DCs. METHODS: PBMCs were isolated from 25 mL of normal adults' peripheral blood and evenly distributed in 5 wells of a 6-well plate. Nonadherent cells were gently aspirated after 2 hour-incubation under humidified 5% CO2 at 37degrees C. Adherent monocytes were cultured in 3 mL of 10% fetal bovine serum plus RPMI-1640 media containing granulocyte/macrophage-colony stimulating factor (GM-CSF) 200 ng/mL and interleukin (IL)-4 20 ng/mL. To assess the effect of tumor necrosis factor (TNF)-alpha and interferon (IFN)-alpha on DC maturation, either or both were added on day 4 of culture. Cells were harvested on day 4 and 7 to calculate the cell counts, CD83 /HLA-DR cells, and CD86 /HLA-DR cells. RESULTS: On day 4, large amounts of DCs were observed. CD83 /HLA-DR cells and CD86 / HLA-DR cells were 11.6% and 16.6% of total cells counted and yields were 1.3% and 2.0%, respectively. On day 7, DCs were more frequently observed in all instances and purity ranged from 24.0% to 31.0% as a mean value. The final yields of matue DCs were 2.9~3.4% of PBMCs inoculated. Adding TNF-alpha plus IFN-alpha led to the best yield. But, IFN-alpha alone did not increase the mature DCs compared to the control. CONCLUSION: We successfully cultured large quantities of mature DCs from PBMCs using GM-CSF and IL-4. IFN-alpha seems to have a synergistic effect when added with TNF-alpha, but further studies are required to prove the clinical significance.

Among the social factors, family factor which affects the primary social function can be the most fundamental. We investigated the effects of father's health behavior on the health behavior of their children during adolescence. METHODS: From June to August 1999, 300 adolescent students from middle schools and high schools of areas including four cities (Seoul, Incheon, Anyang, Gunpo) and their fathers were questioned about general characteristics and health behavior such as smoking, alcohol drinking, exercise, duration of sleep, body weight, height and eating habits for breakfast ('Alameda 7'). The relationship between father's health behavior and their adolescents' health behavior was analyzed by chi-squared test. RESULTS: Among 300 adolescents and their fathers, 211 pairs responded appropriately to the questionnaires. Adolescent's smoking was not associated with father's smoking. In drinking there was no significant association between father and adolescent, but in drinking amount, father's high drinking amount was significantly associated in all students, especially in females and high school students. Regular diet, exercise, sleep did not show significant association between father and adolescent. The proportion of obesity in adolescents whose fathers were obese was significantly greater compared to those whose fathers were not. CONCLUSION: High drinking amount and obesity had a statistical significance of association between fathers and adolescents. When we consider adolescents' health behavior, their fathers' health behavior must be considered.

8.Clinical Comparison of Posterolateral Fusion with Posterior Lumbar Interbody Fusion.

OBJECTIVE: The purpose of this study is to compare the outcomes of two methods for stabilization and fusion: Postero-Lateral Fusion (PLF, pedicle screw fixation with bone graft) and Posterior Lumbar Interbody Fusion (PLIF, cage insertion) for spinal stenosis and recurred disc herniation except degenerative spondylolisthesis. METHODS: Seventy one patients who underwent PLF (n=36) or PLIF (n=35) between 1997 and 2001 were evaluated prospectively. These two groups were compared for the change of interbody space, the range of segmental angle, the angle of lumbar motion, and clinical outcomes by Prolo scale. RESULTS: The mean follow-up period was 32.6 months. The PLIF group showed statistically significant increase of the interbody space after surgery. However, the difference in the change of interbody space between two groups was insignificant (P value= 0.05). The range of segmental angle was better in the PLIF group, but the difference in the change of segmental angle was not statistically significant (P value=0.017). Angle of lumbar motion was similar in the two groups. Changes of Prolo economic scale were not statistically significant (P value=0.193). The PLIF group showed statistically significant improvement in Prolo functional scale (P value=0.003). In Prolo economic and functional scale, there were statistically significant relationships between follow-up duration (P value<0.001), change of interbody space (P value<0.001), and range of segmental angle (P value<0.001). CONCLUSION: Results of this study indicate that PLIF is superior to PLF in interbody space augmentation and clinical outcomes by Prolo functional scale. Analysis of clinical outcomes showed significant relationships among various factors (fusion type, follow-up duration, change of interbody space, and range of segmental angle). Therefore, the authors recommend instrumented PLIF to offer better clinical outcomes in patients who needed instrumented lumbar fusion for spinal stenosis and recurred disc herniation.

9.Asystole during microvascular decompression in case of trigeminal neuralgia: A case report.

Manipulation of the sensory branches of the trigeminal nerve is known to cause autonomic changes, such as bradycardia or asystole, known as the trigemino-cardiac reflex. In this case, the patient underwent microvascular decompression due to trigeminal neuralgia and developed sudden bradycardia, followed by abrupt asystole with a concurrent fall in the systolic blood pressure. There was spontaneous return of cardiac rhythm and blood pressure, but two more episodes of sinus bradycardia occurred during the surgery.

Mediastinal fibrosis is pathologically characterized by chronic inflammation and fibrosis of mediastinal soft tissue. Mediastinal fibrosis is local expression of a family of systemic fibrosing syndroms. This can result in compression of adjacent mediastinal structures. Idiopathic fibrosing syndromes include retroperitoneal fibrosis, sclerosing cholangitis of the orbit and fibrosis of the thyroid gland(Riedel's struma). The cause of these disorders is obscure, in some instance there is an underlying malignancy, infection, history of drug ingestion, or trauma with retoperitoneal bleeding. Treatment of mediastinal fibrosis depends on structures involved by the fibrotic process. The disease is self limited in most case or improved by steroids uses. We experienced a case of idopathic solerosing mediastinitis with orbital fibrous dysplasia of unknown cause, which was confirmed by open lung biopsy, so reported it with a review of literature.
