Though high stone free rates were reported with ESWL monotherapy or anatrophic nephrolithotomy, percutaneous nephrolithotomy with or without ESWL appears to be the procedure of choice for most staghorn calculi. A total of 41 patients with staghorn calculi underwent percutaneous nephrolithotomy(PNL) alone or primary percutaneous debulking followed by extracorporeal shock wave lithotripsy(ESWL) of residual stone fragments. For the objective analysis of the results of PNL stone removal rate was calculated with stone surface area which was determined by computer analyzer. For staghorn calculi smaller than 2000 mm2(38/41), a mean stone removal rate of about 90% was achieved in the PNL. When stone surface area exceeded 2000 mm2(3,41), the mean stone removal rate was 78.8%. The stone removal rates stratified by stone surface area were not significantly different. The stone free rate was 31.7% in the PNL only but 75.6% in the PNL with or without ESWL. The higher mean stone removal rate and stone free rate was achieved in the kidneys with non-dilated collecting system than those with hydronephrosis, but it was not statistically significant.

PURPOSE: The purpose of this study was to identify the relationship between response to violence, resilience and burnout and to investigate the factors that affect burnout in emergency room nurses. METHODS: Data from 237 nurses in 15 emergency rooms were collected using a structured questionnaire. Data were analyzed using SPSS 23.0, and the analyses included descriptive statistics, t-test, Mann-Whitney test, ANOVA, Kruskall Wallis test, Pearson correlation, and hierarchical multiple regression. RESULTS: Burnout was positively correlated with response to violence and negatively correlated with resilience in emergency room nurses. Response to violence (β=.466, p<.001), resilience (β=−.308, p<.001), and religion (β=−.131, p=.011) were significant predictors of burnout, and they explained 39.9% of emergency room nurses' burnout. CONCLUSION: Response to violence and resilience were identified as significant factors affecting burnout in emergency room nurses. Therefore, it is necessary to develop strategies to cope effectively with violence and to develop programs that can strengthen resilience.

Objectives: This study aims to investigate the effect of determinants on the number of remaining teeth in Korean adults using a machine learning-based decision tree analysis. Methods: The study used data from the Korea National Health and Nutrition Examination Survey (KNHANES) VII (2016-2018) and a decision-tree analysis to explain the causes for the number of remaining teeth in adults. The determinants for the study are sex, age, house income, education level, diabetes, BMI, smoking, alcohol drinking, tooth brushing per day, and periodontitis. Results: Age had the most significant effect on the number of remaining teeth, followed by house income. Conclusions This research is meaningful as it provides a systematic index in the number of remaining teeth in Korean adults based on a combination of numerous variables. These variables have already been validated against the results of previous studies that have attempted to elucidate new variables affecting the number of remaining teeth.

Paragonimiasis was one of the most common causes of cavitary lung lesions until the 1960s, but now it has become a very rare disease in Korea. A 16-year-old boy presented with hemoptysis and several days of cough. His plain chest radiograph showed nodular opacity with a cavity in the left upper lung region. Although his symptoms disappeared after taking antituberculous drugs, his plain chest radiograph and computed tomography images after completion of therapy showed a new nodule with a cavity in the left lung field. Through video-assisted thoracoscopic wedge resection, Paragonimus eggs were found in the lung tissues.

Spinal neurenteric cyst results from the persistence of an abnormal communication between endodermal and neuroectodermal layer. It is a rare malformation that lead to spinal cord compression. The patient is a 22-month-old boy with sudden, progressive left side motor weakness. Preoperative magnetic resonance image revealed intradural extramedullary cyst in C4-C5 level that compressed the spinal cord on the left side. After the resection of the cyst, motor weakness of the left side was partially improved, and his motor power was completely recovered after rehabilitation. Histologic examination revealed a ciliated columnar epithelial neurenteric cyst. The pre- and postoperative clinical features of the case of neurenteric cyst is described with the review of literature.

PURPOSE: To report clinical and laboratory findings of toxic anterior segment syndrome (TASS) in seven patients following cataract surgery with intraocular lens (IOL) implantation. METHODS: The medical records of seven patients who underwent cataract surgery associated with postoperative decreased visual acuity, ocular pain, anterior chamber inflammation and corneal edema between Feb 2007 and Nov 2009 were retrospectively reviewed. RESULTS: All patients were over 60 years of age, four patients had diabetes and four patients had cardiovascular disease. Five patients had received hydrophilic IOL, and six patients underwent surgery later in order. All seven patients presented with increased anterior segment inflammation, acute decreased visual acuity, and severe corneal edema an average of 10.4 days (range 1 to 15 days) after surgery. Treatment of the seven patients included intensive topical and oral steroids, and improvement was noted in all patients. CONCLUSIONS: The incidence of TASS after cataract surgery was 0.8%, and was significantly higher in cases of hydrophilic IOL insertion (5 of 284 cases, 1.76%) compared to cases of hydrophobic IOL insertion (2 of 581 cases, 0.34%) (p = 0.04). Five of the seven cases presented with TASS at postoperative day 14. Inflammation improved in all patients with steroid treatment.

PURPOSE: Duchenne/Becker muscular dystrophy(DMD/BMD) is an X-linked recessive disorder caused by mutations of dystrophin genes. The purpose of the present study is to determine the frequency and the patterns of dystrophin gene deletions and to investigate the correlation of genotypes and phenotypes. METHODS: There were included a total of 89 children(88 boys and 1 girl) diagnosed as DMD/BMD by immunohistochemistry and/or genetic analysis from 1999 to 2003 at Seoul National University Children's Hospital. We analyzed the genomic DNA by multiplex PCR using a 26 dystrophin exon primer set. Direct sequencing was performed on 23 exons(in which point mutations were detected in other previous reports) in 22 patients without deletions. Phenotype and genotype relationship analysis was performed on the basis of retrospective clinical reviews. RESULTS: The frequency of dysmorphin gene deletions was 54%(32/59), which is lower than that of European and American data. Exon deletions were detected in 59 cases and the deletion "hot spots" were exon 44-54 constituting 80% of all deletions. In 6 cases without detectable deletions, 6 point mutaions(3 nonsense mutations and 3 nucleotide variants) were detected. The patients whose deletions were in the central parts or the patients with multiple exon deletions tended to show earlier symptom onsets and more rapid progressions of weakness but there were no statistical significances. CONCLUSION: Since deletions in dystrophin genes were detected in about 50% of the patients, studies on dystrophin protein expressions using muscle biopsy samples must be done for correct diagnosis.
