1.Corelationship between proton pump inhibitors and hospital-acquired pneumonia in abdominal surgical patients

Objective To investigate the corelationship between proton pump inhibitors (PPI) and the incidence of hospital-acquired pneumonia (HAP) in abdominal surgical patients. Methods A total of 283 cases admitted from 2010 to 2014 were divided into PPI group (n = 326) and HRA group (n = 92). In PPI group, patients were divided into omeprazole subgroup (subgroup A, n = 146), lansoprazole subgroup (subgroup B, n = 102) and pantoprazole subgroup (subgroup C, n = 78) by the types of PPI. In HRA group, patients were divided into famotidine subgroup (subgroup D, n = 58) and ranitidine subgroup (subgroup E, n=34) by the types of histamine-2 receptor antagonist (H2RA). The incidence of stress ulcer bleeding (SUB) and HAP were calculated, while mechanical ventilation period (MV), ICU stay and 28-day-mortality were compared. Results There were lower incidences of SUB and HAP, shorter MV and ICU stay and lower 28-day-mortality in PPI group than those in HRA group (P < 0.05). In three subgroups of PPI group, there were no significant difference on incidence rate of SUB and HAP, as well as MV, ICU stay and 28-day-mortality. Conclusion The incidence rates of SUB and HAP are lower by using PPI than H2RA in abdominal surgical patients, as well as MV, ICU stay and 28-day-mortality. Different types of PPI have no effect on the incidence rate of HAP in abdominal surgical patients.
2.Effect and safety of controlled-release dinoprostone on the induced labor of later pregnancy

Objective To study the effect and safety of controlled-release dinoprostone on the induced labor of late pregnancy . Methods 60 cases of late pregnancy puerpera were treated in our hospital from February 2014 to September 2015, and divided into the control group and the experimental group with reference to the stochastic indicator method, 30 cases in each group. The control group were given induced labor by oxytocin, and the experimental group were given induced labor by controlled-release dinoprostone, Changes of Bishop score, effects of promoting cervical mature, induced labor situation, and adverse reaction were compared between two groups. Results After medication, the cervical Bishop score of the group (8.35±0.93) points were significantly higher than (6.64±0.72) of the control group (P<0.05). The total effective rate 96.67%(29/30) of promoting cervical mature in the experimental group was significantly higher than 80.00% (24/30) of the control group (P<0.05). Medicine to clinical time (14.89±1.57)h and total labor time (6.94±1.52) h of the experimental group were significantly better than (21.48±2.35) h and (9.30±1.31) h of the control group (P<0.05). The rate of delivery by vagina of the experimental group 83.33% (25/30) was significantly higher than 60.00% (18/30) of the control group (P<0.05). There was no different on adverse reaction rate between two groups. Conclusion There was good effects of Controlled-release dinoprostone on the induced labor of later pregnancy, with high rate of delivery by vagina and better safety.
3.Study on HPLC fingerprint of Congrong Zonggan capsule.

HPLC fingerprint of Congrong Zonggan capsule was established in order to provide basis for quality evaluation. With acteoside as the reference, HPLC was adopted for fingerprint analysis on Congrong Zonggan capsule. The chromatographic conditions wereas follows. Waters C18 column (4.6 mm x 150 mm, 5 μm) was used, with methylalcohol-0.1% formic acid as the mobile phase for gradient elution at the flow rate of 1.0 mL x min(-1). The detection wavelength was 330 nm, and the column temperature was 30 °C. This method was highly accurate and reproducible. All of the 13 components in tested samples reached the baseline resolved peak, and 15 batches of finished products showed the similarity of above 0.95. The method was accurate and feasible and could be used as a simple and effective method to evaluate the quality of the traditional Chinese medicines.

4.The relation between positive fluid balance and the prognosis in severe sepsis patients with acute kidney injury

Objective To investigate the relation between positive fluid balance and the prognosis in severe sepsis patients with acute kidney injury (AKI).Methods A retrospective analysis of clinical data of 90 patients in our department was carried out.According to the final outcome,patients were divided into survival group (n =26) and non-survival group (n =64),in which the relation between patients' positive fluid balance and the prognosis was evaluated.Results (1) There was no statistically significant difference in the age and severity between survival group and non-survival group of patients with severe sepsis and consequent AKI,but the patients in non-survival group had greater volume overload.Compared with the survival group,higher mean fluid balance [(1 112.12±546.85) mLvs.(644.69±474.93) mL,P=0.00],and less urine output [(1 224.07 ± 708.79) mL vs.(2 032.36 ± 723.53) mL,P =0.00) in non-survival group.(2) There was no significant difference in mortality between early and late continuous renal replacement therapy (CRRT) during ICU care.However,the average daily fluid load in late CRRT patients was significantly greater than that in early CRRT patients [(1178.81 ±397.03) mLvs.(287.22 ± ± 433.53) mL,P =0.00] and the lung oxygenation index in late CRRT patients was significantly worse thanthat in early CRRT patients [(211.22±42.56) vs.(169.46±57.40),P=0.04] (3) The relevant variables to 28-day mortality in AKI patients with severe sepsis included CRRT treatment,oxygenation index and the average daily fluid balance > 500 mL.Among them,fluid balance > 500mL was an independent risk factor for AKI patients with severe sepsis.Their prognosis was worse if they had greater positive fluid balance.CRRT was the protective factor which could affect the prognosis of patients with severe sepsis complicated by AKI.Conclusions Patients with severe sepsis complicated by AKI has a high mortality.Persistent fluid overload can lead to increased mortality in patients with severe sepsis complicated by AKI.Early CRRT can reduce fluid retention in patients with renal failure and improve oxygenation index.
5.The effect of direct hemoperfusion with neutral resin on patients with septic shock caused by gram-negative bacteria infection

Objective To evaluate clinical effect and safety of direct hemoperfusion with neutral resin (NS-DHP) on patients with septic shock caused by Gram-negative bacteria infection.Methods A total of 42 patients were enrolled in the study and randomly ( random number) divided into two groups.Patients of control group ( n =24) received sepsis bundle therapy,and patients of group D ( n =18 ) were treated with NS-DHP in addition to sepsis bundle therapy.HA330 hemoperfusion device were used in each patient of group D.The procedure of hemoparfusion lasted 2.5 hours and carried out trice a 24 hours.Clinical data including APCHE Ⅱ score,PO2/FiO2 (OI),mean arterial pressure (MAP),dopamine usage (DA),plasma level of endotoxin (ET),C-reactive protein (CRP),TNF-α,IL-6 and IL-10 were recorded during the treatment.Results Patients well tolerated NS-DHP without any complication in group D.All patients in both two groups did not receive long-term renal replacement therapy.At 24 h,48 h and 72 h after the initiation of treatment,APACHE Ⅱ score,O1,MAP,DA,ET,CRP,TNF-α and IL-6 improved obviously both groups (P <0.05),but there was no significant ditterence in serum levels of IL-10 in both groups.In the group D,APCHE Ⅱ score,OI,MAP,DA,CRP,TNF-α and IL-6 were improved more obviously than those in the group C (P <0.05) ).There was no significant difference in plasma levels of ET in both groups during the treatment.Conclusions NS-DHP can improve APACHE Ⅱ score,PO2/FiO2 and MAP in patients with septic shock caused by gram-negative bacteria infection and reduce the levels of CRP,TNF-α and IL-6,but has no effect on the levels of ET and IL-10 as well as on 28-day mortality and ICU stay.
6.Chemical constituents of Fructus Aurantii and Fructus Aurantii Immaturus by HPLC-ESI-MS

Objective To analyze the chemical components of Fructus Aurantii (FA) and Fructus Aurantii Immaturus (FAI).Methods HPLC-ESI-MS with Surveg mass spectrometer was used in the study.Chromatographic column: Symmetry Shield TM RP_ 18 (150 mm?3.9 mm, 5 ?m) (Waters, Milford, MA, USA); mobile phase: (A) water (0.6% HAc, pH=2.5), (B) methanol. Gradient elutions: 20%- 40% B (0-48 min); 40% B (48-54 min); 40%-55% B (54-60 min); 55%-95% B (60-75 min); 95% B (75-85 min); 95%-20% B (85-90 min).Flow rate and wavelength were 0.7 mL/min and 283 nm at room temperature, respectively.Results Four kinds of flavonoids were identified as naringin, neohesperidin, naringenin, and hesperidin, synephrine was also identified in FA and FAI. Furthermore, the contents of them were determined individually.The results showed that the chemical constituents in FA and FAI were the same but the contents were different.Conclusion HPLC-ESI-MS method can be efficiently used to study FA and FAI.
8.Study on Anti-inflammatory and Analgesic Activities of Volatile Oil from Piper Puberulum in Guizhou

Objective:To study the anti-inflammatory and analgesic effects of the volatile oil from Piper puberulum ( Benth. ) Max-im. in Guizhou. Methods:Fifty KM mice were randomly divided into five groups (10 ones in each), the model control group (sa-line), low concentration volatile oil group (0. 125 ml/100 ml), medium concentration volatile oil group (0. 25 ml/100ml), high con-centration volatile oil group (0. 5 ml/100 ml) and aspirin group (3 mg·ml-1). Mouse ear swelling induced by xylene and skin capil-lary permeability were used to evaluate anti-inflammatory activity of the volatile oil from Piper puberulum ( Benth. ) Maxim. in Guizhou. Acetic acid-induced abdominal eonstrietion test and hot-plate test were applied to evaluate anti-analgesic activity of the vola-tile oil from Piper puberulum ( Benth. ) Maxim. In Guizhou. Results:Compared with that in the model group, the mouse ear swelling in the medium concentration volatile oil group, high concentration volatile oil group and aspirin group was significantly reduced ( P<0. 05). The skin capillary permeability in the high concentration volatile oil group and aspirin group was significantly reduced (P<0. 05 or P<0. 01). Compared with that in the model group, the writhing times in the medium concentration volatile oil group, high concentration volatile oil group and aspirin group were significantly reduced (P<0. 05 or P<0. 01). The pain threshold in the high concentration volatile oil group was effectively reduced (P<0. 05 or P<0. 01). Conclusion: The volatile oil from Piper puberulum ( Benth. ) Maxim. in Guizhou has promising anti-inflammatory and analgesic activities.
9.Analysis of the function of the UBE gene based on bioinformatics

[Abstract ] Objective Bioinformatics provides a lot of valuable information for online prediction of new genes.In this study, we predicted the biological function of ubiquitin-conjugating enzyme 2S ( UBE2S) based on bioinformatics. Methods The UBE2S gene was screened and cloned from the cDNA library of human HepG2 cells.The relationship of the structure and function of UBE2S was explored based on the full-length cDNA library.MEGA5.05, CLUSTALW2 and SWISS-MODEL were used to study the phylogeny, conservation, and 3D structure of UBE2S. Results The UBE2S gene encoded a polypeptide of 241 residues with a predicted molec-ular weight of 23 770 and an isoelectric point of 8.81.The UBE2S protein contained no transmembrane locus and the probabilities of their functions of growth factors, cation channel and structural protein were 8.904, 0.313, and 0.291.The analysis of BLASTp showed that the isolated UBE2S had a 90-97%identity with the other species. Conclusion Analysis of the structure and function of the UBE2S protein can not only provide more information about its gene family but also pave the way for further experimental studies on the molecular mechanism of the consequent hepatocellular carcinoma.
10.Effect of xuebijing injection on inflammatory response and cellular immune function in patients with severe sepsis

ObjectiveTo evaluate the effect of xuebijing injection on inflammatory response and cellular immune function in patients with severe sepsis.MethodsSixty-two patients with severe sepsis from September 2008 to August 2009 were divided into treatment group(30 patients) and control group (32patients) by random digits table.All the patients received sepsis-bundle therapy and patients in treatment group added xuebijing injection therapy with 100 ml,twice a day,for 7 days.The levels of serum tumor necrosis factor-alpha(TNF-α ),interleukin(IL)-6,IL-10 and C-reactive protein (CRP),peripheral blood T lymphocyte CD4+、CD8+ 、CD4+/CD8+,the expression of human leucocyte antigen (HLA)-DR on CD14+peripheral blood mononuclear cell (PBMC) were detected before and aftertreatment.ResultsThere was no significant difference in the levels of serum TNF-o,IL-6,IL-10,CRP,peripheral blood Tlymphocyte CD4+、CD8+、CD4+/CD8+ and the expression of HLA-DR on CD14+ PBMC before treatment between two groups(P > 0.05).After treatment,compared with those in control group,the levels of serum TNF- α,IL-6,IL-10 and CRP in treatment group were significantly decreased [ ( 64.4 ± 13.5) ng/L vs.(96.1 ± 22.1 ) ng/L,( 153.8 ± 23.8 ) ng/L vs.(180.1 ± 21.7) ng/L,(73.8 ± 13.8) ng/L vs.(101.1 ± 11.7) ng/L,(53.7 ± 18.8) mg/L vs.(91.3 ± 32.8)mg/L,P <0.05],while peripheral blood T lymphocyte CD4+/CD8+ and the expression of HLA-DR on CD14+PBMC were significantly increased [ 0.311 ± 0.021 vs.0.424 ± 0.035,0.201 ± 0.017 vs.0.238 ± 0.038,1.78 ±0.21 vs.1.56 ±0.18,(38.4 ± 11.5)% vs.(18.1 ± 12.1)%,P<0.05].ConclusionXuebijing injection can reduce the inflammatory response and ameliorate immune disorder in patients with severe sepsis.
