Objective:To observe the analgesic effect of acupuncture and to explore its central analgesic mechanism in rheumatoid arthritis rabbits.Methods:A total of 60 flap-eared white rabbits were randomly assigned into a normal control group (n=6),a model group (n=6),a body-acupuncture group (n=24),and a buccal acupuncture group (n=24).The later 2 groups were further randomly assigned into 0,0.5,1,and 2 h subgroups,with 6 cases in each group.The rheumatoid arthritis model was established by induction of eggalbumin.In the body acupuncture group,bilateral Xiyan and Zusanli were punctured for 15 s while in the buccal acupuncture group,acupuncture was applied to Xi for 15 s,with the needle retaining for 30 min.The pain threshold was detected with PL-200,taking struggle movements of rabbits as a measurement index,response latency from irradiation to struggling movements as the rabbit's pain threshold.The contents of β-endorplhin (β-EP) and cholecystokinin-8 (CCK-8) in cerebrospinal fluid were examined by radioimmunoassay.Results:Compared with the control group,pain threshold and CCK-8 levels decreased significantly (P<0.01) and the concentration of β-EP significantly increased (P<0.05) in the model group.The pain threshold in the body-acupuncture group and the buccal acupuncture group at 0 and 1 h (P<0.05 or P<0.01) increased significantly,while the β-EP and CCK-8 contents in the bodyacupuncture group and the buccal acupuncture group were significantly higher than those in the model group (P<0.01 or P<0.05).Both β-EP and CCK-8 contents in the buccal acupuncture group at 0 h were significantly higher than those in the body-acupuncture group (P<0.05).Conclusion:The analgesic effect of buccal acupuncture is superior to that of body-acupuncture.Both buccal acupuncture and body-acupuncture can effectively raise the pain threshold in acute arthritis rabbits,which is closely associated with their effects in the up-regulation of β-EP and CCK-8 contents in cerebrospinal fluid.
4.Obstructive sleep apnea hypopnea syndrome:risk factors for perioperative

Objective To analyze the emergency-event in patients with obstructive sleep apnea-hypopnea syndrome( OSAHS), and to discuss the risk factors and preliminary strategies for prevention of emergency-event. Method A total of 257 OSAHS patients in Zhejiang Provincial Peoples Hospital, were enrolled from January 2000 to December 2009 for the retrospective study. Demographics of patients include age, sex,height, weight, related diseases, AHI and LSaO2 before operation , the way of anesthesia and operation,when and how the emergency-events happened, the way to cure and the results. Risk facts of emergency-event were analyzed by using univariate analysis and multiple logistic regression analysis. Results Of the 257patients, the incidents of emergency-event was 7.3%. The independent risk factors of emergency-event were general anesthesia, multiple level surgery in one time, BMI and AHI. Performing CPAP treatment before operation had a beneficial effect to the avoidance of emergency-event. Conclusions Emergency-event are more common in severe OSAHS patients, especially when they were operated under general anesthesia and multiple level surgery in one time. Performing CPAP treatment before operation had a beneficial effect to the avoidance of emergency-event.
5.Comparison of clinical effects between the outside and under temporal muscle method in frontal-temporal cranioplasty by digital forming titanium mesh

ObjectiveTo compare the clinical effects of the outside and under temporal muscle titanium cranioplasty in mass frontal-temporal skull defect.MethodsClinical data of forty-two frontal-temporal skull defect pa tients who had cranioplasty with digital forming titanium nets were analyzed retrospectively.Two groups were divided according to the surgical method.Twenty-two cases underwent outside temporal and twenty cases under went temporal muscle titanium mesh.Compared with two groups of surgery condition( operation time,blood loss,titanium nail dos age) and postoperative complications( wound infection,subcutaneous effusion,epilepsy,intracranial hematoma,chew limited,facial paralysis) and the satisfaction rate of appearance discharged after one month.ResultsThe operation time of outside temporal muscle group was obviously less than under temporal muscle group( t =2.42,P < 0.05 ),but the under temporal muscle group patients were more satisfied with the postoperative appearance ( x2 =36.31,P <0.05 ).There was no obvious difference of the postoperative complication between the two groups ( x2 =1.80,P >0.05 ).ConclusionBoth the outside and under temporal muscle method had its advantages and disadvantages.Operation methods selection should comprehensively and individually according to the specific condition of patients,surgi cal doctor's clinical experience.
6.Effect of postoperative immune-enhancing enteral nutrition on outcomes after hepatectomy in patients with hepatocellular carcinoma

Objective To study the effects of postoperative immune-enhancing enteral nutrition (IEEN)on the liver function and postoperative infectious complications after hepatectomy in patients with hepatocellular carcinoma.Methods Between January 2010 and December 2011,96 patients undergoing hepatectomy for hepatocellular carcinoma were randomized in a double-blind manner to receive postoperative enteral nutrition with IEEN (IEEN group,n =48) or standard enteral nutrition (control group,n =48) for 5 days,respectively.Calorie and nitrogen of the same quantity were provided for both groups from the first day after surgery.The liver function tests and routine blood tests were performed.The postoperative hospital stay and postoperative infectious complications were also compared between these two groups.Results The serum albumin,prealbumin,alanine aminotransferase,and total bilirubin levels had no significant differences between the two groups (all P > 0.05).On the 6th day after operation,the level of total lymphocyte count was significantly higher in the IEEN group than that in the control group [(2106 ± 1057) vs.(1648 ± 1079) / μL,P =0.038).The postoperative hospital stay was (10.3 ±5.6) days in the IEEN group and (13.4 ±6.7) days in the control group (P =0.016).Postoperative infectious complications were observed in 8 (16.7%) patients in the IEEN group and 17 (35.4%) patients in the control group (P =0.036).Conclusion Early postoperative IEEN can improve the clinical outcomes of liver cancer patients after hepatectomy.
7.A Randomized Controlled Trial of Warming and Unblocking Acupuncture Treatment for Scapulocostal Syndrome

Objective To investigate the clinical efficacy of warming and unblocking acupuncture in treating scapulocostal syndrome.Method Eighty patients with scapulocostal syndrome meeting the inclusion criteria were allocated by simple randomization to warming and unblocking acupuncture and conventional acupuncturegroups, 40 cases each.The warming and unblocking acupuncture group received warming and unblocking acupuncture at points Tianzong(SI11), Jianjing(GB21)and Dazhui(GV14)on the affected side and affected chest Huatuo jiaji(Ex-B2)points and uniform reinforcing-reducing acupuncture at points Quchi(LI11), Hegu(LI4)and Waiguan(TE5).The conventional acupuncture group received uniform reinforcing-reducing acupuncture at the same points as those in the warming and unblocking acupuncture group after arrival of qi.Both groupswere treated once every other day, 10 days as a course, for a total of one course.Pain severity was assessed using the Pain Visual Analogue Scale(VAS) in the two groups of patients before and after treatment.Theclinical therapeutic effects were evaluated by follow-up at one month after the completion of treatment.Result The total efficacy rate was 95.0% (38/40) in the warming andunblocking acupuncture group and 80.0% (32/40) in the conventional acupuncture group; there was a statistically significant difference (P<0.05).The PainVisual Analogue Scale(VAS) score was significantly lower in the two groups of patients after treatment than before treatment; there was a statistically significant difference (P<0.05).The VAS score was decreased more in the warming and unblocking acupuncture group after treatment and at the follow-up one month after the completion of treatment (P<0.05).Conclusion Warming and unblocking acupuncture has clinically a better therapeutic effect onscapulocostal syndrome.
8.Study on the Change of Nerve Function and Erythrocyte Immune Indexes of Patients with Severe Craniocerebral injury During the Perioperative Period

Objective To study and observe the change of nerve function and erythrocyte immune indexes of patients with severe craniocerebral injury during the perioperative period.Methods Forty patients with severe craniocerebral injury treated with surgery in our hospital from July 2013 to August 2015 were selected as the observation group and 40 healthy persons with the same ages in the same period were selected as the control group.The nerve function related indexes and erythrocyte immune indexes of observation group before and at first,third,fifth,seventh,tenth and fourteenth day after the surgery and those of control group were compared.Results The serum nerve function related indexes of observation group before and at first,third,fifth,seventh,tenth and fourteenth day after the surgery were higher than those of control group.The erythrocyte immune indexes of observation group before and at first,third,fifth,seventh and tenth day after the surgery were worse than those of control group.The erythrocyte immune indexes at fifth and seventh day after the surgery were worse than those before and at first,third,third,tenth and fourteenth day after the surgery.The differences of comparison indexes were all significant (P<0.05).Conclusion The change of nerve function and erythrocyte immune indexes of patients with severe craniocerebral injury during the perioperative period are obvious.Those indexes should be paid with enough monitoring and intervention.

10.Construction and expression of a prokaryotic vector of recombinant human adiponectin global domain.

OBJECTIVETo construct and express the recombinant human adiponectin (gAd) global domain.
METHODSgAd complementary DNA (cDNA) was obtained from human fat tissue by RT-PCR. The PCR product was cloned into the vector pMD18-T and the prokaryotic expression vector pET32a(+). The recombinant vector was identified by digestion with double restriction endonucleases SalI and EcoRI, PCR and sequence analysis. The recombinant plasmid containing gAd gene was transformed into E. coli BL21 (DE3), and the expression of the fusion protein His-gAd was induced by IPTG.
RESULTSThe gAd cDNA of 412 bp was obtained from the total RNA of the fat tissue and verified by sequence analysis.
CONCLUSIONThe recombinant plasmid could stably express the 34-kD fusion protein His-gAd in the engineered bacteria in the form of inclusion bodies.
