4.Foramen of Morgagni Hernia in Infant.

Morgagni hernia is an uncommon finding in adult and rarer still in infants and children, adn they say that Morgagni hernia occur about one in every 300 diaphragmatic hernias diagnosed. This paper presents a 16 months old Korean male infant with symptomatic Morgagni's hernia in whom the diagnosis was established during the course of treatment of upper respiratory tract infection and who were operated successfully. This well nourished patient was admitted with the chief complaints of mid dyspnea, coughing and restlessness for 2 days. During treatment and investigation we detected a peristaltic sound in the right side lung field and confirmed it as Morgagni hernia by chest X-ray and barium swallowing. We performed corrective surgery through a midline supra-umbilical incision. As opened the peritoneum, transverse colon was gushed out, which suspected as herniated intestinal loop. After well mobilization of visible bowel, hernia sac was notified from the anterior portion or right crus of diaphragm. The hernia was reduced very easily. The patient made an uninterrupted recovery and on the 8th post-op. day, he went to home with symptom-free state. We made a brief literature review.

7.Effects of CAPD on Cardiac Function in Patients with End-Stage Renal Disease: in Comparison with Hemodialysis.

To investigate long term effects of CAPD on the left ventricular function in end-stage renal disease patients, M-mode echocardiographic studies and measurement o fsystolic time intervals were performed in 20 CAPD cases, 28 hemodialysis cases and 29 uremic controls. Compared to the uremic control grup, the patients on CAPD treatment revealed significant improvement of ventricular contractility and reduction of volume. On the other hand in hemodialysis group, even though there was improvement of ventricular contractility, volume control was not adequate. In the systolic time interval measurement, it is postulated that increase of PEP/LVET ratio in CAPD group probably results from reduction of volume(preload) rather than from deranged ventricular function.

8.The Neurilemmoma of the Penis.

Neurilemmoma is the most frequent solitary tumor of the peripheral nerves. It may occur anywhere in the body, but their occurrence in the penis is rare and unusual. Treatment of this lesion is simple surgical excision. We report a case of neurilemmoma of the penis in a 27 year-old male and review the literature.

9.Blood P50 Calculated from a Single Measurement of pH, PO2 and SO2 under General Anesthesia.

BACKGROUND: The P50 which is the oxygen tension corresponding to 50% saturation at pH 7.40, PCO2 40 mmHg and temperature 37oC is an accepted and useful reference to characterize the oxygen affinity for hemoglobin. This study evaluated the accuracy of P50 calculated from a single measurement of pH, PO2 and SO2 compared with linear regression and sigmoid Emax model in patients undergoing general anesthesia. This study also compared P50 of just after anesthetic induction to P50 of 90 min later. METHODS: This study was conducted in 10 patients undergoing total abdominal hysterectomy. Just after induction of anesthesia, 20G catheter was indwelled at cephalic vein of forearm and venous return was occluded with a tourniquet. After 4 min, 6 blood samples were obtained every one minute. After 90 min, 6 blood samples were obtained with same method. All blood samples were analyzed with blood gas analyzer within 30 min after withdrawal. P50 was calculated from three methods of calculation, linear regression and sigmoid Emax model. The difference between the actural P50 and calculated P50 was evaluated. RESULTS: P50's calculated from three methods were 25.8+/-1.1, 26.1+/-1.1, 26.0+/-1.2 mmHg respectively. P50's 90min after anesthetic induction (26.1+/-1.3, 26.3+/-1.2, 26.2+/-1.2 mmHg) were significantly greater than P50's just after anesthetic induction (25.5+/-0.8, 25.8+/-1.0, 25.9+/-1.1 mmHg)(P<0.05). 10.The True Deep Femoral Artery Aneurysm: A Case Report.

A 55-year-old man with a palpable pulsatile mass and pain in his left thigh was presented to us. He had no history of trauma in his left leg, interventions, operation, or medical diseases, including cardiac valve disease, endocarditis, and systemic infection. The size of the aneurysm was 10 cm×7 cm with a mural thrombus in ultrasonography and multidetector computer tomography. There was no evidence of other aneurysms or occlusive lesions in the other arteries. The aneurysm was resected without a vascular reconstruction of the deep femoral artery. The patient’s symptom improved rapidly. The patient had an uneventful postoperative recovery without complications. We report a case of true deep femoral artery aneurysm, which was successfully treated with resection of an aneurysm without a vascular reconstruction.
