2.Clinical Observation on Bladder Rupture.

A clinical observation was made on 26 cases of bladder rupture (16 patient had intraperitoneal rupture and 10 patient had extraperitoneal rupture) in the Department of Urology, Catholic Medical College from Jan. 1972 to Feb. 1981. The following results were obtained: 1. The bladder rupture was 26 cases (12.0%) of 217 urogenital injuries. 2. The 26 patient comprised 20 males and 6 female cases. Nine patients were belong to the age group of 31 to 40 years old, 8 Cases were 41 to 50 years old and 5 cases were 21 to 30 years old. 3. Traffic accidents were responsible for the injury in 10 patients, direct blow in 6 patients, industrial accidents in 5 patients and spontaneous bladder ruptures were seen in 2 patients. 4. The major clinical manifestations of bladder rupture were hematuria (20 patients, 76.9%) and abdominal pain and distension(12 patients, 46.2%). 5. Of the 26 patients, 15 had associated fractured pelvis, 7 had fractured extremity, 6 had urethral rupture, 3 had intestinal injury and 3 had skull fracture. 6. Diagnosis was made by retrograde cystography, laparatomy and cystoscopy. Twenty cases (76.9%) was diagnosed by retrograde cystography. 7. Four patients with intraperitoneal rupture and 6 patients with extraperitoneal rupture were treated by operative repair Of the defect with suprapubic cystostomy and drainage of the perivesical space. Twelve patients with intraperitoneal rupture and 2 patients with extraperitoneal rupture were treated by operative repair of the defect with indwelling urethral catheter and drainage of the perivesical space. The remaining 2 patients of extraperitoneal bladder rupture were treated by indwelling urethral catheter for7 to 10 days. 8. There was a 7.7% mortality rate.

3.Two Cases of FSGS Maintaining Renal Function by Long Term Cyclosporine Treatment.

Cyclosporine can cause remission of 60% in steroid resistant FSGS, but its responses are variable. Now we report two cases of steroid resistant FSGS who are maintaining remission using cyclosporine continuous therapy. The first patient had been failed several times of steroid therapy, had edema, azotemia and severe proteinuria. We used steroid pulse therapy then maintenance dose of oral cyclosporine to reduce proteinuria for more than 6 years. He has been received cyclosporine therapy up to now and maintaining normal renal function. The second patient had severe azotemia who needed hemodialysis but after cyclosporine therapy, he recovered his renal funciton. The findings of renal biopsies in one patient after 6 years of cyclosporine therapy revealed that there was no improvement of sclerosing glomeruli, then we guess that maintenance therapy of cyclosporine might need for lifelong period.

4.Accidentally Diagnosed Myotonic Dystrophy after Cholecystectomy.

Myotonic dystrophy is the most common systemic disease causing myotonia. We report the case of respiratory failure in a patient with myotonic dystrophy after laparoscopic cholecystectomy. We carried out neurologic testing, electromyography and DNA testing in this patient and electromyography in the family of the patient. Through electromyography and DNA testing, this patient was diagnosed with myotonic dystrophy type I. Myotonic dystrophy is characterized by gradual decline of muscle tone and myotonia. It is important that this disease be excluded through preoperative history taking, physical examination and family history taking.

5.Radical Cystectomy with Pelvic Lymphadenectomy in Bladder Tumor: Review of 14 Cases.

A clinical observation was made on 14 patients of bladder tumor who were treated by radical cystectomy with pelvic lymphadenectomy in the Department of Urology, Catholic Medical College during the period from March, 1984 to July, 1986. Metastases to pelvic lymph node were proved histologically in 3 cases among 14 cases. Operative pathologic staging demonstrated an understanding error of 36 per cent, an overstaging error of 28 per cent and an accuracy of 36 per cent. The addition of a meticulous pelvic lymph node dissection did not increase the mortality or morbidity compared to simpler procedures. Results of this study support the early aggressive use of radical cystectomy with pelvic lymphadenectomy and urinary diversion in the management of patients with bladder cancer.

6.Comparison of survival between radical nephrectomy with and without extensive lyphadenectomy in renal cell racinoma.

From 1980 to 1988, 101 patients underwent radical nephrectomy for renal cell carcinoma. Among them, extensive lymphadenectomy was done in 47 patients, and not in the rest of patients. According to cellular grade, tumor size. cell type and stage, survival rates of the two groups were compared. Survival rate of the renal cell carcinoma was decreased in high grade, large size and non-clear cell type, and so they correlated with prognosis. No difference of survival rate was found between extensive lymphadenectomized patients and non-extensive lymphadenectomized patients according to these prognostic factors. Also, no difference or survival rate `in each stage was found between above two groups. An extensive lymphadenectomy is unlikely to increase survival, but we have difficulty in compare the two groups because cases of this study is limited and nodal metastasis can not be confirmed in non-extensive lymphadenectomized patients. Therefore, more randomized controlled study must be performed for accurate result.

8.Ultrastructural Study on the Renal Corpuscle after Removal of Ureteral Obstruction in Rabbit.

This study was designed to clarify morphological changes in the renal corpuscles of the experimental rabbit kidney after unilateral ureteral obstruction and release of Unilateral ureteral obstruction through a light and electro microscopy. It was further aimed at obtaining data, esp. of electron microscopy, concerning the morphological repair after relieving ureteral obstruction. A total of 25 white rabbits, weighing 2.5kg, were used. 25 rabbits were divided into three groups-normal control, unilateral ureteral obstruction, release of unilateral obstruction. The normal! control group consisted of 5 rabbits. The remaining 20 rabbits were completely ligated in the left ureter with silk threads. Ten obstructed kidneys were studied by light and electron microscopy, five of them two weeks after obstruction, and the remaining five four weeks after obstruction. A total of ten postobstructed kidneys, five of which were reoperated by Mackinnon method for the relief of ureteral obstruction two weeks after obstruction and the remaining five four weeks after obstruction. They were studied for light and electron microscopy 15 days after relieving ureteral obstruction. Specimens of renal cortex from experimental animals were fixed in 10% neutral formalin or Bouin's solution, embedded in paraffin wax, sectioned at a thickness of 5 um, and stained with hematoxylin-eosin, periodic acid-schiff reaction or Masson's trichome for light microscopy. For electron microscopy, the tissues were fixed in a mixture of 2% paraformaldehyde and 2.5ao glutaraldehyde (phosphate buffer, pH 7.2) prior to fixation in 1% osmium tetroxide (phosphate buffer, pH 7.2) and then embedded in Epon 812. The sections were cut with LKB- III ultramicrotome and contrasted with uranyl acetate and lead citrate and examined with electron microscopy JEM 100B. The results of this study were as follows: 1. The abnormal morphology of the glomerulus in kidneys obstructed for four weeks showed: Slight glomerular congestion, capillary dilatation, no changes in endothelium, podocyte and microstructures. 2. Deformity of Bowman's capsular epithelium such as irregular basal invaginations facing with the basal lamina was observed in the two week obstructed kidneys, and severe degenerative changes such as desquamated capsular epithelial cells in the Bowman's space and long basal cytoplasmic processes embedded in the basal lamina in the four week obstructed kidneys. 3. Among the postobstructed kidney, the two week obstructed kidneys were restored to near normal on the 15th day, but in the four week obstructed kidney, Bowman's capsular epithelium showed partial recovery. 4. The results indicate that glomeruli in the renal corpuscle were preserved until four week weeks after ureteral obstruction but Bowman's epithelium showed change in the 2nd week after obstruction and severe degenerative changes of Bowman's capsular epithelium were noticed in the 4th week after ureteral obstruction. The morphological changes were totally restored in the group where ureteral obstruction was relieved after two weeks, but in the group which was relieved of ureteral obstruction after four weeks the morphological repairs were prolonged until after 15 days.

9.INFLUENCE OF TOOTH SURFACE ROUGHNESS AND TYPE OF CEMENT ON RETENTION OF COMPLETE CAST CROWNS.

Bond strength of luting cements to dentin is a critical consideration for success of complete cast crowns. This study was performed to evaluate the relationship between surface characteristics of teeth prepared for complete cast crowns and retention of cemented restorations. Eighty artificial crowns were cast for standardized complete crown tooth preparations accomplished with the use of a special device on recently extracted human teeth. Coarse diamond(#102R, Shofu) and superfine finishing diamond(#SF102R, Shofu) burs of similar shape were used. Crowns in each group were randomly subdivided into four subgroups of 10 for luting cements selected for this study: zinc phosphate cement (FLECK'S), polycarboxylate cement (Poly-F), reinforced glass ionomer cement (Fuji PLUS), and adhesive resin cement (Panavia 21). Retention was evaluated by measuring the tensile load required to dislodge the artificial crown from tooth preparations with an Instron testing machine, and analysed by one-way ANOVA and Student's t-test. The obtained results were as follows ; 1. When tooth preparation was done with coarse diamond bur, retentive force was diminished in order of Panavia 21, Fuji PLUS, FLECK'S, and Poly-F. Retentive forces showed the significant difference between Fuji PLUS group and FLECK'S group(p<0.001). 2. When tooth preparation was done with superfine diamond bur, retentive force was diminished in order of Fuji PLUS, Panavia 21, FLECK'S and Poly-F. Retentive forces showed the significant difference between Panavia 21 group and FLECK'S group(p<0.001). 3. Retentive force in coarse tooth surfaces was significantly higher than that in superfine tooth surface with all luting cements(p<0.001), and cement residues were almost retained with in the cast crown in all groups.

10.The Results of Miniplate Fixation for the Fractures of Metacarpal and Phalangeal Bones of the Hand

Fracture healing in the hand is not an isolated goal. The functional end result is of paramount importance. When the correct techniques are used, miniplate fixation for metacarpal and phalangeal fractures of the hand provides rigid internal fixation, stabilizes the skeleton, preserves the gliding tissue, and therefore allowing immediate active range of motion exercise as well as minimizing joint and tendon complications. The authors have reviewed 39 patients, 52 cases of metacarpal and phalangeal fractures of the hand which were treated with miniplate in the department of orthopaedic surgery, Chang Ang Gil hospital from Sep. 1989 to Feb. 1993. The following results were obtained. 1. Miniplate fixation, in terms of fracture reduction and maintenance, allows on accurate anatomic reduction with rigid fixation which in turn affords on early mobilization without loss of reduction alignment. 2. The roentgenographic union was obtained within 15.1 weeks in average. The mean total acitve motion at the fractured finger was 230°. The complication rate was 11.5%. 3. In the treatment of metacarpal and phalangeal fractures of the hand, especially fractures of metacarpal bones, fixation with miniplate is considered to be an useful method in terms of getting fracture union and good range of motion with relative low complication rates.
