1.Clinical Study on Ankylosing Spondylitis

Ankylosing spondylitis is a chronic inflammatory disease which most frequently affects the sacro-iliac joints of young men, and also the synovial joints of the whole spine. Bony fusion of these joints and ossification along the longitudinal ligaments lead to total immobility of the vertebrae. Sixty-two cases of ankylosing spondylitis treated at the Severance Hospital from June 1980 to June 1986 were analysed clinically and the followings were obtained. 1. The average age was 38 years and the sex ratio between the male and the female was 58: 4(Male=94%). 2. The most frequent location of pain was low back (32%), and the pain was usually bilateral (81%). 3. The main stiff regions were the hip joint (35%) and the lumbar spine (27%). 4. The most frequent joint affected was the hip (44%), and the extraarticular manifestations were pleuropulmonary disease (29%), gastrointestinal disorder (15%), iritis (10%), etc. 5. The characteristic X-ray findings were sacro-iliac changes (72%), facet joint blurring (53%), and syndesmophyte (37%). 6. Laborstory findings showed increased ESR (85%), positive HLA B-27 (97%), negative rheumatoid factor (94%), and the serum immunoglobulins snd complements were not decreased.

2.The Relationship Among Nurses' Parenting Stress and Turnover Intention.

PURPOSE: The purposes of this study were to understand correlation of parenting stress and turnover intention of nurses, and to provide basic data for effective nursing management. METHODS: Subjects included 287 nurses who had children and worked in hospitals. Data were collected through organized questionnaires from September 1st to 30th 2010. Collected data was analyzed with descriptive statistics, t-test, ANOVA, correlation analysis using PASW statistics 18 program. RESULTS: The average score for parenting stress of the participants was 3.12 out of 5. Nurture stress was statistically different depending on age, job position, shift work, night duty and stages of family development. The average score for turnover intention of the participants was 3.34 out of 5. Turnover intention was statistically different depending on age, number of children and stages of family development. As a result of correlation analysis of parenting stress and turnover intention, turnover intention showed positive correlation with daily stress, stress caused from taking parent role and average parenting stress. CONCLUSION: This study indicates that parenting stress is related with turnover intention, nursing managers should make efforts to decrease parenting stress in order to reduce turnover intention of nurses.

Ossification of the ligamentum flavum (OLF) has been recognised as a definite clinical entity as an ossification of the posterior longitudinal ligament. It has been known that the incidence of OLF is high in Japan and OLF usually occurs in the thoracic and lumbar region. Recently OLF has received considerable attention as a cause of myelopathy. OLF is quite distinct from “hypertrophy” of the ligamentum flvum. We experienced 2 cases of OLF with neurological symptom, which were treated by decompressive laminectomy and removal of the ossified ligamentum flavum. So remarkable symptomatic improvement was obtained.

6.Factors Influencing Parenting Stress in Mothers of Children with Cerebral Palsy.

PURPOSE: This study was aimed at identifying the levels of parenting stress among mothers of children with cerebral palsy and factors influencing parenting stress. METHOD: The research design was cross-sectional survey. Data were collected from 122 mothers of hospitalized children (under 15 years of age) with cerebral palsy at the Y medical center using the questionnaires. The data were analyzed using one way analysis of variance with Scheffe test for post-hoc analysis or t-test, Pearson correlation, and stepwise multiple regression. RESULTS: The mean score of parenting stress was 2.52 (SD=0.79; range: 1~4). The father's participation, mother's parenting hours, self-esteem and social support were significant predictors of parenting stress, significantly accounting for the 33.3% variance (F=16.118, p<.001). CONCLUSION: It is essential for health professionals to consider the aforementioned four factors when developing interventions to reduce parenting stress for mothers of children with cerebral palsy.

7.Giant Intra-articular Osteochondroma of the Knee: A Case Report

The giant intra-articular osteochondroma of the knee is very rare and is probably result of synovial metaplasia of the fibrous capsule or of the adjacent connencive tissue of a joint, and of the fragment of articular cartilage. The gross findings and histology are consistent with the osteochondroma. The authors experienced a case of giant intra-articular osteochondroma of the right knee in a 19 years old man who had history of knee injury when he was 16 years old. The mass was excised and the symptome was relieved.

8.A Study on the Factors related to the Cognitive Impairment of the Elderly in a Rural Area.

To investigate the factors which affecting the cognitive impairment of the 60 or more age group, the authors surveyed for the subjects in some area of Kyungnam Province. 201 studied subjects were tested for cognitive function with mini-mental state examination(MMSE). Information on demographic characteristics and life style has been collected through direct interview. The concentration of Al and Ca of subject's drinking water, which might be related with cognition, was measured by Inductively Coupled Argon Plasma Spectrometer. The main results were summarized as follows. 1. The prevalence rate of cognitive impairment was 18.4% in male and 45.2% in female and this sexual difference was statistically significant(p=0.03). And the uneducated or illiterated showed significantly high prevalence rate of cognitive impairment(p=0.02). 2. In stratified analysis by sex and education year, we can not see significant trend indicating the neurotoxic effects of aluminum and protective effects of calcium to the cognitive function(p>0.05). 3. The correlation between the concentration of aluminum in drinking water and the MMSE score in whole subjects showed weak negative relationship(r=-0.066). But there was no statistical significance(p=0.434).

9.A CLINICAL STUDY ON TMJ ARTHROCENTESIS.

The purpose of this study was to access the effect of TMJ arthrocentesis in locking 8 patients with anterior distal displacement without reduction ( 'closed lock' ). TMJ Arthrocentesis was defined as needle puncture of the joint space and usually combined with lavage. After lavage, the mandible is gently manipulated to evaluate the movement. Arthrocentesis with irrigation has been used successfully with few complications and low morbidity. Postoperative therapy requires repetitive opening and excursive exercises to maintain and improve the function. The mean follow-up period was 17.1 months(range, 8 to 30months). Preoperative and postoperative mean maximal interincisal opening(MIO) was 25.4mm preoperatively and 39.8mm postoperatively. The pain was reduced in 7 out of 8(87.5%) patients with limited mouth opening. The result of this study indicate that TMJ arthrocentesis is an effective means of controlling pain and improving jaw movement in patients with closed lock.

10.Orbital Pseudolymphoma: A case report.

Lymphoid tumors of the orbit are rare, and sometimes it is not possible either clinically or histologically to differentiate between lymphoid tumor and pseudolymphoma. Some authors assert that the degree of cytologic differentiation appears to be the single most important factor for determining the prognosis of patients with orbital lymphoid lesions. However, the cytomorphologic basis is not so helpful to diagnose and classify our case, which shows some discrepancy between pathological findings and clinical and radiological findings. At first we misdiagnosed our case as orbital malignant lymphoma on the basis of cytomorphology and immunohistochemical study. But no responce to local intensive radiotherapy and the follow up study of the patient suggest orbital pseudolymphoma.
