2.Effect of obesity and age on the blood pressure.

BACKGROUND: In recent years, the incidence of obesity increased in Koreans. The main cause suggested is the diet style of Korean changed to that of Western. It has been proved that obesity is a risk factor or an aggravating factor of hypertension and diabetes mellitus, cardiovascular disease, etc. Primary physician has been interested in the correlation of blood pressure with relative body weight and aging. The purpose of this study was to focus on the influence of age and relative body weight on blood pressure. METHODS: Subjects were collected from 2,068 adults who had received Adult Health Examination. That subjects were divided according to age, relative body weight and then the relation of blood pressure with age according to relative body weight and relation with relative body weight according to age were investigated. RESULTS: Systolic blood pressure increased significantly according to age in all male and female three weight groups and then increased significantly according to relative body weight in all male and female three age groups except male seventh decade group. Diastolic blood pressure increased significantly according to age in all male and female three weight groups except male obesity group and then increased significantly according to the relative body weight in all male and female three age groups except male and female seventh decade groups. Systolic blood pressure has higher correlation with age than relative body weight and diastolic blood pressure has higher correlation with relative body weight in male. In female, systolic and diastolic blood pressure have higher correlations with age. CONCLUSIONS: It can be suggested that blood pressure increase significantly according to aging process and relative body weight. These two factors have a little different effects to systolic and diastolic blood pressure according to sex. Relative body weight has more effect to diastolic blood pressure than systolic blood pressure in male.

3.The Effect of Platelet-Rich Plasma on Allograft Transplantation after Curettage in Benign Bone Tumor.

PURPOSE: This study was performed to evaluate the efficiency of Platelet-rich plasma (PRP) for acceleration of bone healing process on allograft transplantation after curettage in benign bone tumor. MATERIALS AND METHODS: From December 2007 to February 2009, twenty-one patients who had benign bone tumor and underwent allograft transplantation after curettage were evaluated. Mean follow-up period was 14.6 months (range, 12-26 months). We compared with 13 cases of PRP group and 8 cases of non-PRP group in terms of size of lesion, bone resorption, amount of applied PRP and complications. The mean age at surgery was 23.6 years (range, 4-73 years). The most common diagnosis was simple bone cyst (7) followed by enchondroma (4), giant cell tumor (3), undifferentiated benign bone tumor (3) and so on. RESULTS: The mean size of lesion was 33.5 cm3 (range, 2.3-181.9 cm3) (29.4 cm3 in PRP group and 40.2 cm3 in non-PRP group). The mean volume of injected PRP was 7.4 cc (range, 3-12 cc). Bone union started at 3.0 months (range, 1.5-5.8 months) in PRP group and 5.3 months (range, 4-8 months) in non-PRP group. Three cases for each group were excluded due to recurrence and pathologic fracture. One patient had febrile episode 3 weeks later after surgery which subsided with antibiotics. CONCLUSION: The PRP could accelerate bone union in allograft transplantation after curettage of benign bone tumor. Furthermore, we expect that PRP can accelerate bone union in fracture or non-union.

4.Changes in HO-1, HSP70 and iNOS Expressions in the Rat Liver after Remote Ischemic Preconditioning.

Ischemic preconditioning (IP), short pre-treatment sublethal ischemia, induces a state of protection against subsequent prolonged ischemia-reperfusion. The purpose of this study was to investigate the expression of HO-1, HSP70, and iNOS proteins in the liver subjected to the courses of reperfusion after repetitive cycles of remote IP in the rat. Using thirty five week-old rats, the remote preconditioning was undertaken by vascular clamp occlusion of blood flow to one hindlimb, with 3 and 10 cycles of 5 minutes occlusion followed by 5 minutes reperfusion. The liver was removed 0, 3, 6, 24, and 72 hours of reperfusion after remote IP and assayed by immunohistochemical staining and Western blotting analyses for anti-HO-1, anti-HSP70, and anti-iNOS antibodies. The expression of HO-1 in rat liver increased at 72 hours of reperfusion groups after 3 and 10 cycles of remote IP, compared with normal control groups. The expression of HSP70 in rat liver increased at 6 hours of reperfusion groups after 3 cycles of remote IP, compared with normal control groups. The expression of HSP70 in rat liver increased at 0 hour of reperfusion groups after 10 cycles of remote IP, compared with normal control groups and decreased at 24 and 72 hours of reperfusion groups. The expression of iNOS in rat liver increased at 24 hours of reperfusion groups, but decreased at 72 hours of reperfusion groups after 3 and 10 cycles of remote IP, compared with normal control groups. In summary, these results showed that at early phase of reperfusion after remote IP, HSP70 expression was increased in rat liver. However, at 72 hrs of reperfusion after remote IP, HO-1 expression was increased and iNOS expression was decreased in rat liver.

5.Meta Analysis for the Relation between Korean Dietary Factors and Stomach Cancer.

BACKGROUND: The prevalence of stomach cancer had decreased in developed countries, but still remains high in Korea. There was an agreement that dietary factors were the most important factors in gastric carcinogenesis. We undertook a meta-analysis to verify the relation between Korean food items and stomach cancer. METHODS: We searched all studies compatible for meta-analysis by computer and by manual and found two studies. We analyzed 14 dietary factors presented in the two studies by Peto's method. We calculated summary odds ratios and test for homogeneity. RESULTS: Among the 14 dietary factors, salt preference (odds ratio 1.72 [95%CI 1.29-2.30] P<0.001), soybean paste stew (odds ratio 4.19 [95%CI 2.99-5.89] P<0.001), hot pepper soybean paste stew (odds ratio 1.96 [95%CI 1.45-2.64] P<0.001), Kimchi (odds ratio 2.05 [95%CI 1.43-2.94] P<0.001), meat/fish stew (odds ratio 1.58 [95%CI 1.17-2.13] P=0.003), Bulgogi/broiled fish (odds ratio 1.84 [95%CI 1.37-2.49] P<0.001), salted side dishes (odds ratio 1.96 [95%CI 1.47-2.61] P<0.001), and chemical seasoning (odds ratio 2.55 95%CI 1.62-4.02 P<0.001) were risk factors to gastric cancer. But mung bean pancake (odds ratio 0.2 [95%CI 0.14-0.31] P<0.001) Dubu (odds ratio 0.59 [95%CI 0.43-0.83] P=0.002), and garlic (odds ratio 0.45 [95%CI 0.32-0.63] P<0.001) were preventable factors against gastric cancer. Red pepper (odds ratio 1.21 [95%CI 0.91-1.63] P=0.19) and boiled rice (odds ratio 1.39 [95%CI 0.87-2.2] P=0.16) were risk factors to gastric cancer, and sesame oil preference (odds ratio 0.68 [95%CI 0.46-1.01] P=0.06) was a preventable factor against gastric cancer, but these results were not statistically significant.

6.Blood Glucose Measurement with Dextrostix / Reflectance Meter in Operating Room.

The Dextrostix/Reflectance Meter (GLUCOSCOT GT-4310) for measuring blood glucose con centration was studied. We compared 75 blood glucose values measured by GLUCOSCOT with serum glucose values by laboratory glucose oxidase method. And their correlations were excellent (r=0.96961). This study proved that this portable monitor is valuable in providing anesthesiologists with a rapid and precise method for monitoring blood glucose concentrations in operating room. Perioperative blood glucose concentrations were measured by GLUCOSCOT in gynecologic patients anesthetized with Thalamonal (group 1), halothane (group 2) or enflurane (group 3). Blood glucose values increased significantly after intubation and more increased after start of surgery with time, but these changes were not significant clinically (<120 mg/dl). Changes of blood glucose, mean blood pressure and pulse rate were not different statistically from each other groups.

7.Change of the Self-care Agency and Quality of Life after Craniotomy among Patients with Brain tumor according to time

PURPOSE: The purpose of this study was to identify the self-care agency and quality of life (QOL) among patients with brain tumors after craniotomy. METHODS: A sample of 96 patients was recruited from a university hospital in Seoul. Data were collected through a structured questionnaire from August 2017 to December 2018. Data were analyzed with descriptive statistics, t-tests, ANOVA, Pearson correlations and multiple regression analyses with SPSS 24.0, IBM. RESULTS: The mean scores of the 1st and 2nd stages in the self-care agency increased from 4.75 to 5.15, and in the quality of life rose from 67.94 to 78.60. The score of the second stage was higher than the first in all sub-scales of the self-care agency and the quality. Physical function was the only sub-scale of the self-care agency that affected quality of life in both stages (1st: β=.71, p<.001, 2nd : β=.77, p<.001). CONCLUSION: Based on the result, to improve QOL with brain tumor after craniotomy, it is important to assess physical function of patients and provide appropriate interventions.

8.Respiratory Distress Syndrome in Premature Twins.

OBJECTIVE: The purpose of this study is to see whether the incidence of RDS (respiratory distress syndrome) in premature twin is higher than in premature singleton, and to see the clinical difference between the groups. METHODS: Seventy seven premature twins with respiratory distress syndrome who admitted to the department of Pediatrics, Catholic University Medical Center and Chonnam University Hospital for 5 years from Jan. 1998 to Dec. 2002 were enrolled. They were classified with group A in which both of twins have RDS (A1=25, A2=25), group B1 (B1=14) in which first twin has the RDS and group B2 (B2=13) in which second twin has the RDS. Incidence rate of RDS, birth order, gender, delivery mode, gestational week, birth weight, Apgar score, use of surfactant, and mortality rate between groups were also compared. RESULTS: Premature twins have higher incidence of RDS than premature singletons. The RDS is more often than in female. The average gestational week was 30.5 in A, 32.6 in B1, 32.1 in B2, and the average birth weight (gm) was 1,470 (A1), 1,464 (A2), 1,803 (B1), 1,687 (B2). Gender, delivery mode, Apgar score at 1 minute and 5 minute, use of surfactant, and mortality rate and morbidity showed no significant difference. CONCLUSION: Premature twins are more prevalent to RDS than premature singletons. The second twin don't likely to have the RDS than the first baby, but the special care is required because their mortality rate and morbidity were more or less higher.

9.A Trial of Radiesse(R) for Facial Soft Tissue Augmentation.

Ideal charateristics of soft tissue fillers include longevity, biocompatibility, low adverse event profile and a reasonable cost. Radiesse(R) have many advantage as the desirable properties of facial filler. Authors introduce Radiesse(R) was recently introduced as a facial filler agent. Radiesse(R) was injected to 81 male and female patients in the study, using 27G fine needle. The age range of the patients was 22 to 48 years. The injected areas were nasal root, tip, nasolabial folds and jaw. We measured patient satisfaction scale during the period of follow up 2 to 6 months. The scale of satisfaction was good to excellent in 69 cases of 81 cases and fair in 9 cases of 81 cases. Patients satisfaction was poor in only 3 cases. Patients were observed no systemic effects but minimal adverse events which are leakage, nodule and subclinical inflammation. Radiesse(R) was highly effective, long lasting material for facial soft tissue augmentation. If a precise injection technique were applied, Radiesse(R) can be a very good option for real volumetric facial soft tissue augmentation.

10.Clinical Significance of Acute Kidney Injury in Patients with Colistin Treatment.

PURPOSE: Colistin (colistimethate sodium) became available for clinical use in 1959 and was used until the early 1980s to treat infections caused by Gram-negative rods. It was abandoned during the last two decades mainly due to its significant nephrotoxicity. However, the emergence of multidrug-resistant (MDR) bacteria such as Pseudomonas aeruginosa and Acinetobacter baumanii has resulted in significantly increased use of intravenous colistin. This study was designed to investigate the incidence and risk factors of acute kidney injury (AKI) associated with intravenous colistin (colistimethate sodium) treatment. METHODS: We retrospectively collected the data from patients who were admitted to Chung-Ang University Hospital and treated with colistin from May 2007 to June 2009. Among these, we excluded the patients with baseline glomerular filtration rate (GFR) less than 15 ml/min/1.73m2. AKI was defined as an increase of creatinine more than 150% from the baseline, according to RIFLE criteria. RESULTS: A total of 92 patients met the inclusion criteria and were included in the analysis. AKI occurred in 43 (47%) of the 92 patients. The cumulative doses (2.51+/-1.89 vs. 1.75+/-1.35 g, p=0.032) of colistin were significantly greater in the AKI group than in the normal kidney function (NKF) group. Serum creatinine level showed a significant increase in the AKI group, from day 3 (1.6+/-1.1 vs. 0.9+/-0.5 mg/dL, p=0.001) to day 90 (2.1+/-1.9 vs. 0.7+/-0.2 mg/dL, p=0.033). Furthermore, the occurrence of AKI at day 3 was a significant predictor of shorter survival (Log rank test p=0.031). CONCLUSION: AKI was a relatively common side effect of colistin. The cumulative dose was critical, rather than the daily dose or duration of treatment. Early acute kidney injury may predict shorter cumulative survival in patients undergoing colistin treatment.
