1.Effect of different fluid resuscitation on kidney function in pediatric patients with sepsis

Objective To prospectively study the effects of different fluid resuscitation on kidney function in pediatric patients with sepsis.Methods One hundred and eighteen pediatric patients with sepsis shock who needed fluid resuscitation in Intensive Care Unit,Zhengzhou Children′s Hospital from June 2012 to June 2015 were selected, and they were randomly divided into group A,B and C by random number in the table.The patients received fluid resus-citation of hydroxyethyl starch,50 g/L albumin and 9 g/L saline.After that,neutrophil gelatinase associated lipocalin (NGAL),α-1 microglobulin,urea nitrogen,creatinine,urine volume,the incidence of acute kidney injury (AKI),and the renal replacement therapy rate accepted by the patients were recorded.Then all the data were analyzed with SPSS 19.0 software.Results Serum levels of NGAL and α-1 microglobulin in group A [(103.50 ±1.23)μg/L,(3.32 ± 0.20)mg/L]were higher than those in group B [(93.54 ±1.42 )μg/L,(2.71 ±0.18 )mg/L]and group C [(79.57 ±1.54)μg/L,(2.08 ±0.14)mg/L],which were statistically different(all P <0.05 );serum levels of NGAL and α-1 microglobulin in group B were higher than those in group C,which were statistically different(all P <0.05).Serum levels of urea nitrogen and creatinine in group A[(5.23 ±0.34)mmol/L,(112.00 ±7.12)μmol/L] were higher than those in group B [(3.56 ±0.31)mmol/L,(77.25 ±5.34)μmol/L]and group C[(2.95 ±0.42) mmol /L,(72.12 ±6.34)μmol/L],which were statistically different (all P <0.05);serum levels of urea nitrogen and creatinine in group B were higher than those in group C,which were not statistically different(P >0.05).The urine volume in group A [(0.50 ±0.12)mL/(kg·h)]was less than that in group B[(0.90 ±0.23)mL/(kg·h)]and group C[(1.30 ±0.14)mL/(kg·h)],which was statistically different(all P <0.05);the urine volume in group B was less than that in group C,which was not statistically different(P >0.05).In three groups,there were 13 cases (34.2%)with AKI in group A,9 cases(23.1%)with AKI in group B,and 8 cases(19.5%)with AKI in group C, the incidence of AKI in group C was less than that in group B and group C,which was statistically different(χ2 =12.74,10.43;all P <0.05).All the groups all had children accepted renal replacement therapy,there were 8 cases (21.1%)in group A,7 cases(17.9%)in group B,and 6 cases (14.6%)in group C,and the cases who accepted renal replacement therapy rate in group C were less than those in group A and group B,which was statistically different(χ2 =11.36,8.73;all P <0.05).Conclusions The adverse effect of 9 g/L saline on kidney function is less than hydrox-yethyl starch and 50 g/L albumin,and hydroxyethyl starch has more adverse effects on kidney.Thus,9 g/L saline is a better option for fluid resuscitation in pediatric sepsis patients.
2.Nutrition Care of Diet - refusal Patients having Difficulties in Adapting to Hospital Diet.

The purpose of this study was to produce proper nutrition and foodservice through positive intervention for diet-refusal patients who has difficulties in adaptation to hospital diet, and to contribute to hospital management with the focus of fast recovery. The survey was conducted using questionnaires between July 18 and August 17, 2000. The subjects were consisted of 24 diet-refusal patients who were hospitalized in St. Mary's Hospital except the patients hospitalized for the purpose of operation or pre-therapy tests. Also chart reviews were conducted. The statistical analysis of data was done by SAS/win 6.11 package to determine descriptive analysis and paired t-test and Pearson's correlation. The summary of study results is as follows : 1. Most of the subjects was receiving chemotherapy. The leading place for offering private food was from patient's home. Subjects worried about delayed recovery from illness due to nutritional problems caused by extreme fasting. The problems were in order of anorexia, nausea and vomiting during therapy process. 52.4% of subjects ate hospital diet after positive intervention. 2. Major reasons of hospital diet-refusal patients' dissatisfation were in order of expeness, dissatisfation, serum total protein levels were significantly lower(p<.05). 3. Serum Alb(p<.05), Hb(p<.05), Hct(p<.05) and TLC(p<.001) levels showed significantly negative correlations with fasting duration. With these results, it is concluded that a major cause of malnutrition among patients is to last fasting.

3.Inhibitory Activity Screening of Endophytic Fungi from Polygala tenuifolia Willd

Eighty-five endophytic fungal strains were isolated from the roots、stems and leaves of Polygala tenuifolia Willd, among which, fifty-two from natural plants and thirty-three from cultivated ones. Sev-enty-six strains were classified as twenty-three fungal genera. The inhibitory activity screening to fourteen microbe were conducted research. The results showed that some endophytic fungi had remarkble inhibitory activities to Bacillus subtilis, Shigella sonnei, Escherichia coli, Candida albicans, Fusarium kyrushuense and they were all belonged to Fusarium, Alternaria, Aphanocladium respectively. All of the endophytic fungi isolated from Polygala tenuifolia showed no inhibitory activities to Staphyloccocus aureus, Salmonel-lae enteritis, Bibrio parahemolyticus.

5.Clinical research of IA regimen as induction chemotherapy for acute myeloid leukemia

Objective To explore the clinical efficacy and toxicity of idarubicin (IDA)-based IA regimen as induction chemotherapy in treating acute myeloid leukemia (AML) within different risk groups.Methods Data collected in 60 patients initially diagnosed and treated with one course of IA regimen were studied retrospectively.Results Efficacy were different for patients treatment with different prognosis after one course of IA chemotherapy,CR rates achieved in patients with favorable,intermediate and unfavorable prognosis were 83.3 % (10/12),73.1% (19/26),59.1% (13/22) respectively,PR rates were 16.7 % (2/12),19.2% (5/26),13.6% (3/22) while ORrateswere 100% (12/12),92.3 % (24/26),72.7 % (15/22)respectively.After 2 years of follow-up,relapse free survival (RFS) rates were 100 % for patients with good prognosis,53.8 % for those with moderate prognosis,and 45.5 % for people with poor prognosis.The outcome showed that there were significant differences among these three group (P =0.042).Myelosuppression and infections due to neutropenia were the main adverse effects,and severe non-hematologic toxicities were not observed.Conclusion IA regimen as induction chemotherapy may be an efficacious and safe solution and achieve a higher CR rate and 2 years RFS rate for patients diagnosed as AML with good prognosis.Hematopoietic stem cell transplantation should be considered as soon as possible for patients with moderate or poor prognosis.
6.Application of pulse indicator continuous cardiac output monitoring in the fluid management of children with acute respiratory distress syndrome

Objective To explore the application value of pulse indicator continuous cardiac output (PiCCO) monitoring in the fluid management of children with acute respiratory distress syndrome (ARDS).Methods Thirty-two children with ARDS admitted to Pediatric Intensive Care Unit(PICU) of Zhengzhou Children's Hospital,from April 2013 to April 2016,were divided into intervention group (15 cases) and control group (17 cases) by adopting random number table method.Fluid management of intervention group by PiCCO,control group by central venous pressure,the 2 groups' oxygenation index (OI),acute lung injury score,mechanical ventilation time and 28 days mortality were statistically compared.The categorical data were analyzed by using SPSS 11.0 software,and the t test was used for the measurement data.The categorical data and mortality comparison were analyzed by adopting x2 test.The difference was statistically significant at P < 0.05.Results After 3 days of mechanical ventilation,the changes of OI in the intervention group were significantly higher than those in the control group [(175.0 ±-43.7) mmHg vs.(143.0 ± 42.8) mmHg (1 mmHg =0.133 kPa),t =2.090 0,P < 0.05].The intervention group was significantly shorter than the control group [(10.45 ± 3.12) d vs.(12.63 ± 2.87) d,t =2.058 7,P < 0.05].There was no significant difference between 2 groups in acute lung injury score,PICU length of stay and 28 days mortality (all P > 0.05).Conclusions PiCCO monitoring and guidance in the fluid management of pediatric ARDS can improve oxygenation after 3 days,reduce mechanical ventilation time,but can not significantly reduce the 28-day mortality.
7.Effect of Traditional Chinese Drug Combined with Training of Musculus Quadriceps Fexoris on Knee Osteoarthritis

Objective To observe the effect of traditional Chinese drug combined with training of musculus quadriceps fexoris on knee osteoarthritis(KOA).Methods70 out-patient clinic KOA patients were divided randomly into the Chinese drug group and control group with 35 cases in each group.The Chinese drug group was treated with Shentongzhuyu medicinal broth PO bid;the control group was treated with Sulindac 0.2 g PO bid.Two groups were combined with the training of musculus quadriceps fexoris,having 10 times per course and 3 courses in total with a 3 days interval between two courses.The therapeutic effect was evaluated with footplate pressure gait analysis and modified JOA marks.ResultsAfter treatment,the effect of the Chinese drug group was superior to the control group(P<0.05),especially 12 weeks post treatment.There maximum weight loading,time integral and weight loading intergral of affected limb of the Chinese drug group significantly improved after treatment(P<0.01),but for the control group,only weight loading intergral improved(P<0.05).ConclusionTraditional Chinese drug combined with training of musculus quadriceps fexoris has better curative effect on the pain and functional disturbance of KOA.
8.Clinical research on the treatment of dacryocystitis combined with canalicular obstruction by dacryocystorhinostomy with canalicular intubation

AlM: To investigate the clinical efficacy of treatment of dacryocystitis combined with canalicular obstruction by endonasal endoscopic dacryocystorhinostomy with canalicular intubation.
METHODS: A total of 27 cases of dacryocystitis combined with canalicular obstruction were treated by endonasal endoscopic dacryocystorhinostomy with canalicular intubation. Canalicular obstruction was treated by laser under dacryoendoscopy and antegrade intubation.
RESULTS: For 27 cases, operations were successful, with no complications. All patients were followed up for 6mo, 25 were cured, 2 were effective with no failed. The cure rate was 93%.
CONCLUSlON:Treatment of dacryocystitis combined with canalicular obstruction by endonasal endoscopic dacryocystorhinostomy with canalicular intubation has clear field, minimal invasion, quick recovery, exact effect and less recurrence, so it is worthy of promotion.

10.Application of a Silicone Sheet in Negative-Pressure Wound Therapy to Treat an Abdominal Wall Defect after Necrotizing Fasciitis.

Necrotizing fasciitis (NF) is an aggressive soft-tissue infection involving the deep fascia and is characterized by extensive deterioration of the surrounding tissue. Immediate diagnosis and intensive treatment, including debridement and systemic antibiotics, represent the most important factors influencing the survival of NF patients. In this report, we present a case of NF in the abdomen due to an infection caused by a perforated small bowel after abdominal liposuction. It was successfully treated using negative-pressure wound therapy, in which a silicone sheet functioned as a barrier between the sponge and internal organs to protect the small bowel.
