3.Diagnosis and treatment of male pseudohermaphroditism (report of 21 cases)

Objective To report and summarize the improvement of diagnosis and treatment of male pseudohermaphroditism. Methods Among the 21 cases (mean age of 13 years,range from 6 to 31 years),19 were socially female sex and 2 were male sex.Visual clinical examination indicated abnormal external genitalia,lumps on both sides of inguina or labia majora pudendi.Genetic examination showed that the sex chromosome karyotypes of all cases were 46,XY.Before operation diagnoses were established in 20 cases by blood biochemistry,B-ultrasound and CT examination with misdiagnosis in only 1 case. Results All the 21 cases received operation.Female features were retained in 19 cases and male features in 2.Of them 15 cases were followed up for 3 to 8 years.Of the 15 cases 14 were treated by hormone replacement (oral stilbestrol)with obvious improvement of secondary sexual character.Of the 14 cases 11 had normal prepuberal physical development.The appearances of the external genitalia were satisfactory.One patient who maintained male sex had obvious growth of breasts during puberty. Conclusions Early diagnosis and treatment are very important to the therapeutic effects and the life quality of the patients.It is suggested that operation of maintaining female sex can be preferred as treatment of choice for male pseudohermaphroditism.
4.Study on the time selection of pregnancy and delivery in women with systemic lupus erythematosus

Objective To analyze the appropriate time selection of pregnancy and delivery in women with systemic lupus erythematosus (SLE ). Methods Twenty-nine pregnancies in women with SLE in our hospital from 1998 to 2003 were retrospectively analyzed regarding the selection of appropriate time of pregnancy and delivery. Results All patients did not take any cytotoxic medicine for at least 6 months before pregnancy. Twenty-three conceptions occurred when SLE was inactive for at least 1 year. Two conceptions occurred when SLE was active without doctors' agreement SLE was diagnosed during pregnancy in the remaining 4 cases. The condition of all patients fluctuated and the gestational time at delivery ranged from 30 to 38 weeks after we modified the doses of glucocorticoid (prednisone). Among totally 29 living neonates, eight were premature neonates, three were FGR and one had serious congenital heart disease. Two neonates died of complications in early stage of neonatal period. None of the 29 neonates from all patients had neonatal lupus. Conclusion Pregnancy safety will be improved obviously if the condition of SLE is controlled and the patients are given reasonable doses of glucocorticoid and intensive monitoring. If pharmacotherapy does not work well and the condition threatens the safety of mother and fetus, or the fetus has matured, termination of pregnancy should be done on time, which reduces maternal complications and improves the perinatal mortality rate. The gestational time should be 34 to 38 weeks.
5.Antioxidative Activity and Anti-melanogenic Effect of the Extract from the Leaves of Robinia Pseudo-acacia L.

BACKGROUND: Plant extracts have been widely used as important therapeutic drugs for many centuries all over the world. There have been many reports that natural products have various kinds of biological activities such as anti-allergy, anti-inflammatory and antimicrobial activities. Recently, the screening for the efficacy and safety of natural products has been extensively performed. OBJECTIVE: This study was carried out to find a beneficial plant extract possessing excellent antioxidative and anti-melanogenic activities. We have found that the leaf of Robinia pseudo-acacia L. has active substances which are involved in those activities. METHODS: To confirm the antioxidative activity of the extract obtained from the leaves of Robinia pseudo-acacia L., scavenging ability of the extract on DPPH free radicals and its inhibitory effects on lipid autoxidation and peroxidation were investigated. In addition, inhibitory effects of the extract on mushroom tyrosinase as well as melanin biosynthesis in cultured B16 melanoma cells were evaluated. RESULTS: The acacia extract showed not only powerful antioxidative activity but also antimelanogenic acitivity as strong as that of arbutin which is a well known inhibitor of melanogenesis. CONCLUSION: These resulis suggest that the extract from the leaves of Robinia pseudo-acacia L. could be used as a 4ghtening and antioxidative agent for the skin.

6.Agmatine inhibits formalin-induced protein kinase C gamma,phosphorylated cAMP response element binding protein,c-Fos and c-Jun up-regulation in the spinal dorsal horn

Aim To investigate the changes in the expression of protein kinase C gamma (PKC?), phosphorylated cAMP response element binding protein(pCREB)and immediate-early gene(c-fos and c-jun) in the spinal cord in formalin-induced inflammatory pain and study the effect of agmatine on the changes of PKC? activation, phosphorylation of CREB and expression of c-fos and c-jun.Methods Rats were decapitated at 10, 20 min or 2 h after intraplantar injection of 50 ?l 5% formalin and L_4, 5 spinal cords were dissected. Immunohistochemistry and western blotting analyses were used to observe the expression of PKC?, pCREB, c-fos and c-jun in the spinal dorsal horn and the effect of agmatine on the changes of their expression. Results Unilateral peripheral inflammation induced PKC? activation and CREB phosphorylation bilaterally while c-fos and c-jun expression ipsilaterally in rat spinal cord. PKC activity increased in membrane fractions with unchanged levels in the cytosolic fractions. Pretreatment intraperitoneally with 160 mg?kg-1 agmatine 15 min before inflammation significantly inhibited the activation of PKC? in the membrane fraction, suppressed the phosphorylation of CREB and the expression of c-fos and c-jun. Conclusion The mechanism of the analgesic effect of agmatine may be associated with inhibiting PKC? activation in the plasma membrane, CREB phosphorylation, c-fos and c-jun up-regulation which play roles in the hyperalgesia with peripheral inflammation.
7.Inhibitory Activity Screening of Endophytic Fungi from Polygala tenuifolia Willd

Eighty-five endophytic fungal strains were isolated from the roots、stems and leaves of Polygala tenuifolia Willd, among which, fifty-two from natural plants and thirty-three from cultivated ones. Sev-enty-six strains were classified as twenty-three fungal genera. The inhibitory activity screening to fourteen microbe were conducted research. The results showed that some endophytic fungi had remarkble inhibitory activities to Bacillus subtilis, Shigella sonnei, Escherichia coli, Candida albicans, Fusarium kyrushuense and they were all belonged to Fusarium, Alternaria, Aphanocladium respectively. All of the endophytic fungi isolated from Polygala tenuifolia showed no inhibitory activities to Staphyloccocus aureus, Salmonel-lae enteritis, Bibrio parahemolyticus.
8.Presentation Patterns and Surgical Management of the Complications of Thread Rhinoplasty

Objectives:
. Nonsurgical rhinoplasty using threads has gained popularity in recent years. While the benefits of this procedure have been emphasized, possible complications and their management are not well-known. This study aimed to present the surgical management and results of the complications of thread rhinoplasty.
Methods:
. We retrospectively reviewed the medical records of seven patients who underwent revision rhinoplasty due to the complications of thread rhinoplasty from January 2018 to May 2021. The presentation of complications, detailed surgical procedures, and outcomes of revision rhinoplasty were reviewed.
Results:
. Visible or extruded threads at the tip were the most common complication, followed by dorsum irregularity. All the threads were unabsorbed and intact in shape, even several years after insertion. Thread removal necessitated careful tissue dissection, resulting in the loss of tip support and dorsal irregularity. To restore the tip support and camouflage the dorsum shape, an autologous tissue graft was needed.
Conclusion
. Removal of threads at the tip and dorsum was accompanied by structural weakening and partial tissue loss, which required tip support restoration and dorsum camouflage.
9.Study on microsatemte instability of p14,p15,p16 gene in gastric carcinoma

Objective To detect the microsatellite instability (MSI) of D9S166,D9S171,D9S941, D9S942 and IFNA located at chromosome 9p21 in gastric carcinoma with high-grade intraepithelial neoplasia and normal tissue,and investigate the correlation of p14,p15,p16 gene and the gastric carcinogenesis. Objective To explore the value of color doppler ultrasonography by bed side in the early diagnosis of HIE in full term neonates.Methods The changes of cerebral parenchymal and cerebral arterial blood stream parameter on 35 cases of neonates clinically diagnosed HIE of mild and moderate degree and 40 cases of normal newborns on the 24,48 and 72 hours after birth were observed by color doppler ultrasonography by bed side.Results 1.The cerebral parenchyma was even echo in normal newborns,but it was maldistributed and reinforced in mild asphyxia neonates and it was more serious in moderate degree.The echo of cerebral parenchyma in mild degree was near normal in 48 hours after birth,while the echo of cerebral parenchyma in moderate degree was still maldistributed and reinforced in 48 and 72 hours after birth.2.There was obvious changes in the cerebral arterial blood stream parameter and hemodynamics of the asphyxia newborns compared with normals.The systolic peak velocity(Vs)and end diastolic velocity(Vd)of the cerebral arteries in mild and moderate degree were obviously lower than that of control group in 24,48 hours after birth(Pa0.05).3.Resistance index(RI)of the cerebral arteries in mild and moderate degree were higher than that of control group in 24,48 hours after birth(Pa0.05).Conclusion Color doppler ultrasonography by bed side is a convenient,noninvasive method for diagnosing HIE.
