1.Factors Affecting Neurologic Outcome in Asphyxiated Term Baby.

PURPOSE: Asphyxia is the most common cause of neurologic sequelae in perinatal period. We hoped to help other clinicians by investigating factors affecting neurologic outcome in asphyxiated term babas. METHODS: A clinical study of factors affecting neurologic outcome was made on 120 patients, who were asphyxiated term baby. They admitted to neonatal intensive care unit of Chungnam National University Hospital from January, 1995 to December, 1996. RESULTS: There was no significant correlation in neurologic outcome and gestational age, delivery type, sex, birth weight, one minute Apgar score. Five minute Apgar score had influence on neurologic outcome. Presence of acidemia had influence on neurologic outcome. Mental status and seizure influenced on neurologic outcome. Poor neurologic outcome was prone to come in the presence of prolonged duration of abnormal mental function or a repetitive seizure not controlled immediately. There was poor neurologic outcome in the patients who have abnormal findings on EEG, CT, and neurosonography. In addition, there was poor neurologic outcome in congenital heart disease patients. All of above had statistical significance. CONCLUSION: Now, we can obtain much benefit by monitoring clinical course such as five minute Apgar score, mental change, and seizure. And, we can obtain much benefit by monitoring acidemia and perform EEG, CT and neurosonography.

2.Comparison of the Clinical Effects after Finasteride Treatment for Benign Prostatic Hyperplasia according to the Transition Zone Volume and Transition Zone Index.

PURPOSE: Recent studies suggest that the symptomatic improvement in benign prostatic hyperplasia significantly related with transition zone volume (TZV). The purpose of this study was to determine the clinical significance of TZV and transition zone index (TZI) in changes of prostate volume (PV) and clinical parameters following finasteride therapy. MATERIALS AND METHODS: 140 patients over 50 years of age with symptomatic benign prostatic hyperplasia were treated with finasteride (5mg/d) for 12 months and underwent transrectal ultrasound evaluation of PV and TZV prior to initiating therapy and after 12 months. Patients were grouped according to the results of PV (> OR =40ml or <40ml), TZI (> OR =0.45 or <0.45) and PSA level (> OR =2.5 or <2.5). The responders was determined as improvement in peak flow rate more than 3mL/sec. RESULTS: PV decreased by 14.11% in patients with TZI less than 0.45, while the decrease was 19.25% for men with TZI greater than 0.45 (p<0.01). In addition, PV was significantly decreased by 16.72% in patients with PV less than 40cc and TZI greater than 0.45 (p<0.01). PV decreased by 17.37% in patients with PSA less than 2.5, while the decrease was 18.92% in men with PSA greater than 2.5. In responders, only TZI was significantly different among PSA, PV and TZI (p<0.05). CONCLUSIONS: Treatment effect of finasteride on symptomatic benign prostatic hyperplasia patients was increased in proportion to enlarged PV, increased TZI, increased PSA. TZI was a useful proxy for predicting clinical outcomes in initiating finasteride therapy on benign prostatic hyperplasia.

3.Statistical Evaluation of Lineage Markers in Individual Identification.

Mitochondrial DNA (mt DNA) and the non-recombining region of the Y chromosome are passed down, unaltered, from generation to generation, matrilineally and patrilineally, respectively. Therefore, the Y-chromosome DNA and mtDNA are known as lineage markers, and they play important roles in studies based on human migration and evolutionary history. Y-chromosome DNA is used in forensic analysis to identify individuals involved in cases of sexual assault. In this paper, we review the methods of statistical evaluation of lineage markers used in forensic identification. We also review the combined approach of autosomal and lineage marker evidence.

4.The Effect of Self-Management Competence on Social Anxiety in Aviation College Students

Purpose:
This study is a descriptive research study that analyzed the effect of selfmanagement competence on social anxiety in aviation-related college students.
Methods:
Data were collected from a total of 90 students, including 40 from the Department of Aviation Operations and 50 from the Department of Aircraft Maintenance Engineering.
Results:
The self-management competence of aviation college students was found to be 3.55 points, and social anxiety was rated at 2.02 points. The study categorized students into groups based on their self-management competence levels, revealing that the group with high self-management competence had the lowest social anxiety at 1.65 points. The middle group showed social anxiety at 1.99 points, and the group with low self-management competence exhibited the highest social anxiety at 2.51 points.The study identified a negative correlation between self-management competence and social anxiety, indicating that as self-management competence increased, social anxiety decreased. Examining sub-elements of self-management competence and their correlation with social anxiety, the study found that self-emotion awareness and social anxiety exhibited a positive correlation. Conversely, self-control, confidence, selfexpression, perception of reality, independence, and social anxiety showed a negative correlation. Multiple regression analysis revealed that self-control had a negative effect on social anxiety, suggesting that strengthening self-management competence.
Conclusion
In terms of self-control, strengthening self-management competence could reduce social anxiety among aviation college students.
5.An Exploratory Study on Human Factor Analysis of Medical Accidents Using the SHELL (Software, Hardware, Environment, Liveware) Model

The SHELL (Software, Hardware, Environment, Liveware) model is one of the models for determining the cause of aviation accidents. The human factors of medical accidents are analyzed through the SHELL model. This study is a case study that analyzed a total of three cases, including two medical accident court precedents and one large fire at a medical institution. In the human factor analysis of cases 1 and 2, the main factors were that liveware (human) violated the duty of care and did not follow the guidelines of the institution. The large fire incident in the hospital in Case 3 was caused by a lack of awareness of the safety of managers, the installation of illegal facilities and environments, lack of medical staff’s response and training, and the condition of patients who were mostly elderly. The liveware at the center of the SHELL model is the most important human factor, and the environment, facilities, and equipment surrounding liveware must be supplemented to prevent future medical accidents.
6.The Pulmonary Hemodynamic Effects of Nitric Oxide Inhalation on Hypoxic Pulmonary Vasoconstriction.

BACKGROUND: Nitric Oxide (NO) has been discovered to be an important endothelium-derived relaxing factor. The exogenous inhaled NO may diffuse from the alveoli to pulmonary vascular smooth muscle and produce pulmonary vasodilation, but any NO that diffuses into blood will be inactivated before it can produce systemic effects. To examine the effects of NO on pulmonary and systemic hemodynamics, NO was inhaled by experimental dogs in an attempt to reduce the increase in pulmonary artery pressure (PAP) and pulmonary vascular resistance (PVR) induced by hypoxia in dogs. METHODS: Eight mongrel dogs were studied while inhaling 1)50% O2 (baseline), 2)12% O2 in N2 (hypoxia), 3)followed by the same hypoxic gas mixture of O2 and N2 containing 20, 40 and 80 ppm of NO, respectively. RESULTS: Breathing at FIO2 0.12 nearly doubled the pulmonary vascular resistance from 173 56dyn sec cm-5 to 407 139dyn sec cm-5 and significantly increased the mean pulmonary artery pressure from 16 3mmHg to 22 4mmHg. After adding 20~80 ppm NO to the inspired gas while maintaining the FIO2 at 0.12, the mean pulmonary artery pressure decreased (p<0.05) to the level when breathing oxygen at FIO2 0.5 while the PaO2 and PaCO2 were unchanged. The pulmonary vascular resistance decreased significantly and the right ventricular stroke work index returned to a level similar to breathing at FIO2 0.5 by addition of NO into the breathing circuit. Pulmonary hypertension resumed within 3~5 minutes of ceasing NO inhalation. In none of our studies did inhaling NO produce systemic hypotension and elevate methemoglobin levels. CONCLUSIONS: Inhalation of 20~80 ppm NO selectively induced pulmonary vasodilation and reversed hypoxic pulmonary vasoconstriction without causing systemic vasodilation and bronchodilation. Methemoglobin and NO2 were within normal limit during the study.

8.A Case of Corneal Opacity in X-linked Ichthyosis Patient.

X-linked ichthyosis is a hereditary dermatosis characterized by large dark and thick scaly skin of trunk, extremities, scalp and neck. Rare manifested ocular signs include scales on lid and lashes, corneal opacity, lens opacity, and peripheral retinal granular hyperpigmentation. The authors experienced a case of diffuse punctate corneal opacities observed in the deep stroma or pre- Descemet's layer which developed in a 12-year old male patient who visited our clinic complaining symtoms of itching and dark thick scales on trunk, extremities, and scalp since at birth a.nd diagnosed as X-linked ichthyosis by clinical and histologic features.

9.Evaluation of Asthma-related Websites.

OBJECTIVE: The purpose of this study was to analyze current status of the web sites dealing with asthma in the Internet, and to evaluate their technical aspects and contents. METHODS: The evaluation tool consisted of four areas(construction, operation, accessibility, and content) with twenty-six items. Four sites including the contents related with symptoms of asthma, consultation of health professionals, and treatment and nursing for asthma were selected to be analyzed. RESULTS: The results of the analysis were as follows: 1) there was no web site dealing with asthma managed by public institutions 2) the only 5% of the top 100 sites were found to be related with the asthma by exploring the top five search engines in Korea and 3) the evaluation scores four web sites were 6-11 out of 18 in the category of construction, 10-21 out of 24 in operation, 5-10 out of 15 in accessibility, and 8-18 out of 21 in content. CONCLUSION: In conclusion, it will be possible to prevent asthma at first through web sites, which will be one of the most beneficial aspects of web site, so the system to evaluate web sites should be developed to provide high-quality information about asthma.

10.A Study on Impact of Nursing Work Environment, Interpersonal Skills, and Grit on Field Adaptation of Nurses Who Have Experienced Department Transfers

Purpose:
: This study aims to identify the factors influencing field adaptation among nurses who have undergone department transfers, by examining the levels of and the relationships between their nursing work environment, interpersonal skills, grit, and field adaptation.
Methods:
: The study involved 137 nurses who had experienced at least one department transfer within the last three years across two hospitals. Data analysis was performed using SPSS/WIN 29.0, involving descriptive statistics, means, standard deviations, independent t-tests, one-way ANOVA, Scheffé post-hoc tests, Pearson correlation coefficients, and multiple regression analysis (enter method).
Results:
: Factors affecting field adaptation included the nursing work environment (β=.37, p <.001), interpersonal skills (β=.19, p =.021) and grit (β=.18, p =.025), with a total explanatory power of 31% (F=12.16, p <.001).
Conclusion
: This study contributes insights into effective nursing personnel management, offers foundational data for managing department transfers, and aims to improve the overall quality of nursing care.
